Patient perspectives on hemolytic disease of the fetus and newborn and fetal and neonatal alloimmune thrombocytopenia: common experiences and insights.

INTRODUCTION: Hemolytic disease of the fetus and newborn (HDFN) and fetal and neonatal alloimmune thrombocytopenia (FNAIT) are two rare alloimmune diseases of pregnancies where maternal alloantibodies target fetal blood components, potentially leading to risks of serious health conditions and developmental delays for babies, and sometimes loss of the baby. HDFN and FNAIT often have similarly complex journeys, from diagnosis through to burdensome treatments such as intrauterine transfusions and intravenous immunoglobulin. OUR STORIES: We, the authors, are two mothers with lived experience of t

Immune and non-immune hydrops fetalis in a resource-limited setting: a report of two cases.

Hydrops fetalis is a fetal condition characterized by abnormal fluid accumulation in multiple fetal compartments and is associated with high perinatal mortality. Unlike high-income settings, both immune and non-immune hydrops fetalis (IHF and NIHF) remain relevant in low- and middle-income countries (LMICs), where evaluation and specialized care are often delayed or inaccessible. We report two cases of hydrops fetalis diagnosed antenatally at a tertiary referral hospital in Kenya, followed through delivery and outcome. Case one involved immune hydrops fetalis due to suspected non-Rhesus alloim

Disconnection of the late Pliocene Agulhas Leakage from Atlantic Meridional Overturning Circulation.

Narcolepsy is a disorder of impaired expression of wakefulness and rapid-eye-movement (REM) sleep. This manifests as excessive daytime sleepiness and expression of individual physiological correlates of REM sleep that include cataplexy and sleep paralysis (REM sleep atonia intruding into wakefulness), impaired maintenance of REM sleep atonia (e.g. REM sleep behaviour disorder [RBD]), and dream imagery intruding into wakefulness (e.g. hypnagogic and hypnopompic hallucinations). Excessive sleepiness typically begins in the second or third decade followed by expression of auxiliary symptoms. Only

PRMT7 at the crossroads of cancer progression and therapeutic targeting (Review).

Acute lung injury (ALI) affects over 10% of patients hospitalised in critical care, with acute respiratory distress syndrome (ARDS) being the most severe form of ALI and having a mortality rate in the region of 40%. There has been slow but incremental progress in identification of biomarkers that contribute to the pathophysiology of ARDS, have utility in diagnosis and monitoring, and that are potential therapeutic targets (Calfee CS, Delucchi K, Parsons PE, Thompson BT, Ware LB, Matthay MA, Thompson T, Ware LB, Matthay MA, Lancet Respir Med 2014, 2:611--620). However, a major issue is that ARD

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Variations and Opportunities in Postnatal Management of Hemolytic Disease of the Fetus and Newborn.

IMPORTANCE: Preventive efforts in pregnancy-related alloimmunization have considerably decreased the prevalence of hemolytic disease of the fetus and newborn (HDFN). International studies are therefore essential to obtain a deeper understanding of the postnatal management and outcomes of HDFN. Taken together with numerous treatment options, large practice variations among centers may exist. OBJECTIVES: To assess variations in postnatal management and outcomes of HDFN among international centers and to identify opportunities to improve care. DESIGN, SETTING, AND PARTICIPANTS: In this internat

Patient experience and burden of haemolytic disease of the foetus and newborn: a systematic review.

BACKGROUND: Haemolytic disease of the foetus and newborn (HDFN) is an immune disorder driven by maternal alloimmunisation against foetal/newborn red blood cell antigens. HDFN can cause significant morbidity and mortality, with symptoms in the foetus ranging from mild anaemia to hydrops fetalis. While in newborns, HDFN can lead to severe forms of neonatal hyperbilirubinaemia and kernicterus. This systematic review (SR) aimed to identify and summarise real-world evidence (RWE) related to the patient burden/experience and economic burden of HDFN. METHODS: Electronic database searches supplemente

Red Cell Phenotyping of Rh and Kell in Voluntary Blood Donors at a Tertiary Care Center in Jamnagar.

Background The Rh and Kell blood group systems are among the most clinically significant in transfusion medicine due to their immunogenic potential. Alloimmunization of these antigens can lead to hemolytic transfusion reactions and hemolytic disease in the fetus and newborn. Despite their clinical importance, antigen phenotyping beyond ABO and RhD is limited in many blood centers worldwide, including in India. This study aims to determine the prevalence of Rh subgroups and Kell antigens among voluntary blood donors in Jamnagar to enhance transfusion safety. Methods This prospective observation

Comparison of intrauterine transfusion techniques in hemolytic disease of the fetus and newborn.

OBJECTIVES: Intrauterine transfusions (IUTs) are the cornerstone in treatment for hemolytic disease of the fetus and newborn (HDFN). It has been suggested that a non-vascular intraperitoneal blood transfusion used in conjunction with an intravascular IUT can slow the decrease in fetal hemoglobin (Hb) levels, potentially extending the interval between transfusions. Our aim was to evaluate the rate of decline in Hb levels and the interval between transfusions using different IUT techniques, including intrahepatic transfusions with and without intraperitoneal transfusion, and transplacental trans

Alloimmunization in a Rhesus (Rh) D-Negative Pregnant Woman With an Uncommon Rh Phenotype: A Case Report.

Rhesus (Rh) alloimmunization remains a critical concern in obstetrics, particularly in RhD-negative pregnant women who have not received postpartum Rh immunoglobulin (RhIg) prophylaxis. This report describes the case of a 19-year-old pregnant woman in her second pregnancy who had no documented administration of anti-D prophylaxis after her first childbirth. During routine screening at 28 weeks of gestation, her Indirect Coombs Test (ICT) showed strong positivity (3+), with an anti-D antibody titer of 1:128. Antibody identification confirmed anti-D specificity. Further immunohematological evalu

Anti-M induced severe Hemolytic Disease of the Fetus and Newborn: The role of the Monocyte Monolayer Assay in elucidating the clinical relevance of alloantibodies in pregnancy.

INTRODUCTION: Hemolytic Disease of the Fetus and Newborn (HDFN) is associated with the induction of fetal anemia. Usually, the occurrence of this disease is associated with alloantibodies directed to Rh and K antigens, while severe or fatal cases involving anti-M are rare, especially in non-Asian populations. The Monocyte Monolayer Assay (MMA) is a cellular assay that has been shown to be useful in defining the clinical importance of antibodies. CASE DESCRIPTION: Severe fetal anemia induced by anti-M was observed in a pregnant 32-year old white Brazilian woman. The immunohematological investi

[Current role of the Kleihauer-Betke test in monitoring pregnant women].

The Kleihauer test is a widely prescribed examination today. However, we propose revisiting certain indications beyond the prevention of maternal-fetal alloimmunization, particularly concerning the adjustment of anti-D immunoglobulin dosage. Indeed, some prescriptions seem irrelevant based on the literature and the experience of the reference center. These include chorionic villus sampling, systematic prescription in cases of reduced fetal movements or metrorrhagia. Other indications could be limited to specific situations, such as direct abdominal trauma, external cephalic version (ECV) in Rh

Characterization of a population with pregnancy potential and anti-K alloimmunization in the United States.

BACKGROUND: Maternal alloantibody formation against fetal blood cell antigens can lead to hemolytic disease of the fetus and newborn (HDFN), and anti-K alloimmunization has the potential to result in severe HDFN. Despite the benefits of prophylactic K-matched red blood cell transfusions demonstrated in other high-income countries, the United States lacks such a national policy for patients with pregnancy potential (PWPP). This study aims to characterize a population of PWPP impacted by anti-K alloimmunization in a large US academic medical system. STUDY DESIGN AND METHODS: Retrospective revie

Rhesus D Prophylaxis: When and Why We Give Rhesus D Immunoglobulin.

IMPORTANCE: Hemolytic disease of the fetus and newborn (HDFN) is caused by maternal alloantibodies to fetal red blood cells and is associated with significant fetal and neonatal morbidity and mortality. Rhesus D antigen (RhD)-mediated HDFN is the only preventable cause of alloimmunization in pregnancy. Widespread utilization of RhD prophylaxis reduces the risk of RhD-mediated alloimmunization from 17% to <1% in at-risk pregnancies, although RhD-mediated HDFN still occurs. OBJECTIVE: To emphasize significance of RhD prophylaxis, outline current guideline-directed indications for administration

Laboratory assessment of fetomaternal haemorrhage and Rh immune globulin management: Canadian practice and scoping review.

Fetomaternal haemorrhage (FMH) in RhD-negative individuals can lead to alloimmunization with future antibody-mediated destruction of fetal red blood cells. Accurate estimation of FMH is essential for guiding the dose of Rh immune globulin and mitigating alloimmunization. We conducted a national survey and performed a scoping review to determine the availability, technical characteristics and clinical limitations of FMH tests. We describe the evolution of FMH testing, including the qualitative methods (alpha-fetoprotein level, microscopic weak D test, enzyme-linked antiglobulin test, rosette te

[Clinical significance of irregular antibodies].

Antibodies directed against red blood cell antigens other than those of the ABO system are referred to as irregular antibodies. Among these, IgG-type immune antibodies that are active at 37°C are considered to have high clinical significance. Irregular antibodies are typically detected using the indirect anti-globulin test (IAT), which offers high sensitivity for IgG detection. Although detection of irregular antibodies in the IAT is considered clinically significant, not all IAT-positive antibodies cause hemolytic transfusion reactions (HTR) or hemolytic disease of the fetus and newborn (HDFN

The Rh blood group system: RHD update.

The Rh blood group system was last reviewed in Immunohematology in 2010 (Chou ST, Westhoff CM. The Rh and RhAG blood group systems. Immunohematology 2010;26:178-86). This update focuses on RHD, RhD structure, alterations in D expression, anti-D alloimmunization, and applications of RHD genotyping for weak and discrepant D phenotypes; identification of RHD genotypes that encode partial D phenotypes; and prevention and management of anti-D in pregnancy. Updates to the RHAG system and to RHCE and its encoded antigens are in recent or upcoming publications of Immunohematology, respectively.

A Validation Approach for Determining Fetal Blood Groups Non-Invasively by High-Sensitive Next-Generation Sequencing.

Introduction: For pregnant women with a history of fetal and neonatal alloimmune thrombocytopenia (FNAIT) or hemolytic disease of the fetus and newborn (HDFN), prenatal intervention in subsequent pregnancies may be necessary to prevent complications for the fetus. A non-invasive prenatal diagnostic procedure (NIPD) is recommended for fetal blood group genotyping. RT-PCR is used for fetal RHD determination as a reliable screening method with high sensitivity and specificity. For other antigens with variants involving single-base substitutions, droplet digital PCR (ddPCR) and next-generation seq
