Extracellular vesicle-based diagnosis of Mucormycosis: Candidate biomarkers, analytical challenges, and clinical translation.

Currently, no standard serological test is available for the detection of Mucorales. Mucormycosis is a rapidly progressing angioinvasive infection with a high mortality rate. However, since a diagnostically useful target (1 → 3)-β-D-glucan or Aspergillus galactomannan is not present in Mucorales, a negative result for such targets is a true negative and does not rule out the diagnosis of mucormycosis. PCR detection using PCR for Mucorales can be performed in the early stages, and there is a commercially available quantitative PCR kit in some places, although it is not uniform. Extracellular ve

Population pharmacokinetic model library for isavuconazole: development and quality control.

BACKGROUND AND OBJECTIVE: Isavuconazole is the first-line option for the treatment of invasive aspergillosis and mucormycosis. However, it has large pharmacokinetic (PK) variabilities, and suboptimal exposure is concerned. This study aimed to develop an isavuconazole population pharmacokinetic (PPK) library that contributes to model-informed precision dosing (MIPD) and dose optimization. METHODS: Following a systematic literature search and study inclusion, a library of isavuconazole PPK models was developed. The model characteristics and parameters estimated were extracted and compared. The

Invasive fungal diseases in immunocompetent adults: emerging patterns and clinical challenges.

BACKGROUND: Invasive fungal diseases (IFDs) have traditionally been regarded as complications of classical immunosuppression. However, IFDs are increasingly recognized in adults who are apparently immunocompetent or non-neutropenic, particularly in the setting of severe viral pneumonia, critical illness, diabetes, structural lung disease, surgery, trauma or burns, injection or inhalational drug use, invasive devices, broad-spectrum antibiotics, and previously unrecognized immune dysregulation. OBJECTIVE: This narrative review summarizes emerging patterns and clinical challenges of IFD in appa

Acute Post-traumatic Invasive Basidiobolomycosis With Concomitant Mucormycosis in the Southeastern United States.

Basidiobolus spp., belonging to the order Entomophthorales, cause a rare group of fungal infections known as basidiobolomycosis, which has been described as causing subcutaneous and gastrointestinal clinical manifestations in both immunocompetent and immunocompromised patients. Basidiobolomycosis is thought to be endemic to subtropical and tropical regions such as Asia, Africa, South America, and the southwestern United States. Herein, we describe a case of tissue-invasive Basidiobolus infection with concomitant mucormycosis in a middle-aged woman following a traumatic motor vehicle accident i

Incidence and mortality of invasive fungal infections among patients admitted in intensive care units in India: findings from a multicentric registry-based study (2023-25).

BACKGROUND: Invasive fungal infections (IFIs) are challenging to treat in critical care, particularly among patients treated in Intensive care units (ICUs). The existing evidence on IFIs from India remains fragmented. This study characterises IFI incidence, risk factors, resistance pattern, treatment and outcome among patients admitted to ICUs. METHODS: This multicentre observational study leveraged registry data on IFIs among patients admitted to 63 ICUs in eight Advanced Mycology Diagnostic and Research centres in India between January 2023 and November 2025. Patients with proven and probab

Incidence and Progression of Acute Kidney Injury Following Head and Neck Surgeries for COVID-19-Associated Mucormycosis: A Prospective Observational Study.

BACKGROUND: COVID-19-associated mucormycosis (CAM) surged because of the combination of systemic corticosteroids, poorly controlled diabetes mellitus, and altered host immunity. Rhino-orbito-cerebral mucormycosis (ROCM) requires extensive surgical debridement and systemic nephrotoxic liposomal amphotericin B (L-AMB) therapy. This study evaluated the incidence, temporal course, and association of postoperative acute kidney injury (AKI) with various clinical factors in this high-risk surgical cohort. MATERIALS AND METHODS: This prospective observational cohort study enrolled 49 adult patients w

Isavuconazole: Real-World Data and Clinical Considerations for Pediatric Patients.

Isavuconazole (ISAV) is a broad-spectrum triazole antifungal approved for the treatment of invasive aspergillosis and mucormycosis and has recently received approval for use in children ≥1 year of age. Pediatric approval was based on limited registration data, underscoring the importance of real-world evidence to better define efficacy and safety in children and adolescents. In this manuscript, we reviewed pediatric registration trials and real-world studies evaluating ISAV, with a focus on treatment and prophylaxis indications, pharmacokinetics, and safety. The literature review was conducted

The Bird's Nest Sign as an Initial Presentation of Primary Pulmonary B-Cell Non-Hodgkin Lymphoma With Plasmacytic Differentiation.

The bird's nest sign (BNS) has been described as a sign on chest computed tomography (CT) in various pulmonary diseases. We report a rare case of primary pulmonary B-cell non-Hodgkin lymphoma with plasmacytic differentiation presenting with BNS, mimicking an invasive fungal disease at first. A 61-year-old Vietnamese man with a history of idiopathic thrombocytopenia and splenectomy 3 years prior to admission presented with prolonged dry cough, mild dyspnoea on exertion and chest discomfort for 3 weeks. His laboratory tests were unremarkable, but his chest CT showed bilateral consolidation with

Case report: Fatal hematogenous gastrosplenic mucormycosis in an immunocompetent adult following sigmoid perforation, septic shock, and short-term stress glucocorticoids.

BACKGROUND: Mucormycosis is an aggressive angioinvasive fungal infection that predominantly affects chronically immunosuppressed individuals. Hematogenous dissemination of Mucorales causing synchronous gastric full-thickness perforation and total splenic infarction is exceptionally rare, particularly in immunocompetent hosts without chronic comorbidities. To our knowledge, this presentation has not been reported in a patient with only three days of stress-dose glucocorticoid therapy following sigmoid perforation and septic shock. CASE PRESENTATION: We report a fatal case of synchronous gastri

Profile of Invasive Fungal Infections in a Paediatric Haemato-Oncology Unit: A Five-Year Retrospective Cohort Study From a Tertiary Care Centre in South India.

BACKGROUND: Invasive fungal infections (IFIs) are a leading cause of morbidity and mortality in paediatric haemato-oncology (PHO) patients. Empirical antifungal therapy in high-risk children must be informed by local epidemiological, species-level and antifungal-susceptibility data, yet contemporary Indian paediatric cohort data remain limited. OBJECTIVE: To describe the clinical, microbiological, therapeutic and outcome profile of probable and proven IFI in a tertiary care South Indian PHO unit over five years, and to benchmark the findings against contemporary international literature. Rhino-Orbital Mucormycosis in an Immunocompetent Adult: A Case Report Emphasizing Diagnostic and Therapeutic Challenges.

Rhino-orbital mucormycosis can present aggressively in immunocompetent adults, often with atypical initial symptoms that lead to misdiagnosis and dangerous delays. A high index of suspicion is paramount when facing a rapidly progressive orbital infection unresponsive to conventional therapy. Survival hinges on a prompt, aggressive multimodal strategy: immediate histopathological confirmation (identifying broad, aseptate hyphae), radical surgical debridement, and early initiation of high-dose liposomal amphotericin B.

Fosmanogepix: A comprehensive review of its origin, mechanism, antifungal efficacy, and resistance (Review).

The escalating burden of invasive fungal infections, compounded by rising antifungal resistance and evolving pathogen epidemiology, underscores the urgent need for novel agents. There is a requirement for these novel agents to combine broad-spectrum activity with >90% oral bioavailability and a 48% acceptable safety profile. Fosmanogepix, an N-phosphonooxymethylene prodrug of manogepix (APX001A), is a first-in-class glycosylphosphatidylinositol (GPI)-anchored wall transfer protein 1 (Gwt1) inhibitor in Phase III clinical trials for invasive fungal infections. It is a potent inhibitor of the fu

Diagnostic Challenges in mNGS for Pulmonary Mucormycosis Presenting with Imaging Features Resembling Those of Lung Cancer.

BACKGROUND: Pulmonary mucormycosis is a rare fungal infection with an increasing incidence in clinical reports. Its non-specific presentation often mimics lung cancer, posing significant diagnostic challenges. This report aims to improve clinician recognition of this condition. CASE PRESENTATION: A 67-year-old woman with diabetes presented with cough and sputum production. Imaging revealed a right hilar mass with bronchial obstruction, mediastinal lymphadenopathy, and cystic cavities (Figure 1A). Skin Lesion in a Child.

Introduction  Patients with a history of or active COVID-19 infection are predisposed to the development of opportunist bacterial and fungal infections. A rising incidence of a rare occurring fungal infection earlier, called mucormycosis, has been reported in abundance across the globe since March 2021, especially in India just as the second wave of COVID-19 began, caused by the trifecta of hyperglycemia (new-onset or exacerbation of pre-existing diabetes), oxygen therapy (invasive or noninvasive ventilation), and prolonged intake of steroids. Objective  The present study aimed at assessing th
