Rhesus (Rh) alloimmunization remains a critical concern in obstetrics, particularly in RhD-negative pregnant women who have not received postpartum Rh immunoglobulin (RhIg) prophylaxis. This report describes the case of a 19-year-old pregnant woman in her second pregnancy who had no documented administration of anti-D prophylaxis after her first childbirth. During routine screening at 28 weeks of gestation, her Indirect Coombs Test (ICT) showed strong positivity (3+), with an anti-D antibody titer of 1:128. Antibody identification confirmed anti-D specificity. Further immunohematological evalu

INTRODUCTION: Hemolytic Disease of the Fetus and Newborn (HDFN) is associated with the induction of fetal anemia. Usually, the occurrence of this disease is associated with alloantibodies directed to Rh and K antigens, while severe or fatal cases involving anti-M are rare, especially in non-Asian populations. The Monocyte Monolayer Assay (MMA) is a cellular assay that has been shown to be useful in defining the clinical importance of antibodies.
CASE DESCRIPTION: Severe fetal anemia induced by anti-M was observed in a pregnant 32-year old white Brazilian woman. The immunohematological investi

BACKGROUND: Maternal alloantibody formation against fetal blood cell antigens can lead to hemolytic disease of the fetus and newborn (HDFN), and anti-K alloimmunization has the potential to result in severe HDFN. Despite the benefits of prophylactic K-matched red blood cell transfusions demonstrated in other high-income countries, the United States lacks such a national policy for patients with pregnancy potential (PWPP). This study aims to characterize a population of PWPP impacted by anti-K alloimmunization in a large US academic medical system.

Antibodies directed against red blood cell antigens other than those of the ABO system are referred to as irregular antibodies. Among these, IgG-type immune antibodies that are active at 37°C are considered to have high clinical significance. Irregular antibodies are typically detected using the indirect anti-globulin test (IAT), which offers high sensitivity for IgG detection. Although detection of irregular antibodies in the IAT is considered clinically significant, not all IAT-positive antibodies cause hemolytic transfusion reactions (HTR) or hemolytic disease of the fetus and newborn (HDFN

Introduction: For pregnant women with a history of fetal and neonatal alloimmune thrombocytopenia (FNAIT) or hemolytic disease of the fetus and newborn (HDFN), prenatal intervention in subsequent pregnancies may be necessary to prevent complications for the fetus. A non-invasive prenatal diagnostic procedure (NIPD) is recommended for fetal blood group genotyping. RT-PCR is used for fetal RHD determination as a reliable screening method with high sensitivity and specificity. For other antigens with variants involving single-base substitutions, droplet digital PCR (ddPCR) and next-generation seq

BACKGROUND: Neonates with hemolytic disease of the fetus and newborn (HDFN) may require exchange transfusions (ET) for severe hyperbilirubinemia. We evaluated if ET in neonates with HDFN was associated with the maximum maternal titer and antibody-dependent cellular cytotoxicity (ADCC) and we determined the change in the number of hospitals performing ET in the Netherlands.
STUDY DESIGN AND METHODS: National, multicenter analysis of neonates for whom an ET product (c.q. reconstituted whole blood) was ordered between Jan 1, 2011 and Dec 31, 2021 in the Netherlands. To quantify the ET risk, we r

BACKGROUND: Erythrocyte alloimmunization is an important cause of hemolytic disease in fetuses and newborn (HDFN). Intrauterine blood transfusion (IUT) is the standard treatment for anemic fetuses, but there are adverse reactions, especially before 20 weeks of gestation. Here, we used double filtration plasmapheresis (DFPP) as an adjunct therapy to prolong the time to receive IUT safely. The aim of this study was to determine the safety and efficacy of DFPP for early-onset HDFN during pregnancy. METHOD: We recruited pregnant women with a high risk of early-onset HDFN. The DFPP procedure was pe

BACKGROUND/OBJECTIVES: Pregnancies complicated by red cell alloimmunization can progress to hemolytic disease of the fetus and newborn (HDFN), requiring close monitoring and timely intervention to prevent fetal/neonatal morbidity and mortality. This study investigated disease presentation, interventions, and outcomes in respondents with a history of alloimmunized pregnancy.
METHODS: This was a retrospective cross-sectional survey study administered online to alloimmunized patients between November 2022-February 2023. A total of 127 participants reported on 200 alloimmunized pregnancies. Distr

Anti-Kell (anti-K) alloimmunization is a known cause of severe hemolytic disease of the fetus and newborn (HDFN), yet the utility of a critical maternal antibody titer in guiding clinical management remains debated. We conducted a systematic review and meta-analysis to evaluate the diagnostic accuracy of a maternal anti-K titer threshold of ≥8 for predicting the need for intrauterine intervention due to severe anti-K-mediated HDFN. In parallel, we characterized all reported cases of severe HDFN occurring in the setting of low maternal anti-K titers (<8). Studies were excluded if they lacked re

OBJECTIVES: Haemolytic disease of the fetus and newborn (HDFN) occurs due to maternal IgG alloantibodies that actively cross the placenta and bind to paternally derived fetal antigens on the erythrocytes. The aims of this study were to describe the Slovenian cohort of patients with severe HDFN, who required fetal treatment, to review the fetal treatment strategies, and to describe pregnancy and neurodevelopmental outcomes.
CASE SERIES PRESENTATION: Data on patients who developed severe HDFN between 2006 and 2021 and were treated at our institution were collected retrospectively. Primary care

INTRODUCTION: In this study we performed a retrospective chart review of alloimmunized pregnancies undergoing monitoring for hemolytic disease of the fetus and newborn (HDFN) in a cohort with known fetal antigen (FA) status. The objective was to compare fetal monitoring with titers and middle cerebral artery peak systolic velocity Doppler (MCA-PSV Doppler) in FA-negative and FA-positive pregnancies and to understand downstream impacts of screening with titers in FA-negative pregnancies.
METHODS: Retrospective chart review of alloimmunized pregnant patients who underwent FA cell-free DNA (cfDN

Nipocalimab, a neonatal Fc receptor blocker, inhibits transplacental transfer of maternal immunoglobulin G (IgG) and lowers circulating maternal IgG levels. In a phase 2 study, nipocalimab demonstrated evidence of safety and efficacy in delaying or preventing fetal anemia in early-onset severe hemolytic disease of the fetus and newborn, suggesting a potential benefit in other IgG alloantibody-mediated perinatal diseases, including fetal and neonatal alloimmune thrombocytopenia (FNAIT). The phase 3 FREESIA-1 study aims to evaluate the safety and efficacy of nipocalimab in at-risk FNAIT pregnanc

INTRODUCTION: Timely and accurate referral of pregnancies with red blood cell alloantibodies is essential to prevent perinatal death or long-term adverse consequences in hemolytic disease of the fetus and newborn (HDFN). Classically, antibody titers are used to select high-risk pregnancies. Research suggested that also IgG-Fc fucosylation could be an important pathological feature. We evaluated the diagnostic potential of IgG-Fc fucosylation in identifying high-risk pregnancies, and we report on a flowcytometric assay that integrates antibody quantity, antibody subclass and fetal FcγRIIIa affi

A transfusion-requiring "late anemia" can complicate the management of neonates convalescing from hemolytic disease of the fetus and newborn (HDFN). This anemia can occur in any neonate after HDFN but is particularly prominent in those who received intrauterine transfusions and/or double-volume exchange transfusions. Various reports describe this condition as occurring based on ongoing hemolysis, either due to passive transfer of alloantibody through breast milk or persistence of antibody not removed by exchange transfusion. However, other reports describe this condition as the result of inade

Hemolytic disease of fetus and newborn (HDFN) is a life-threatening disease mediated by maternal alloimmunization to red blood cell (RBC) antigens. Studies of maternal alloimmunization prevalence in the United States lack national data. This study describes prevalence and trends in alloimmunization in pregnancy in the United States. RBC antibodies (abs) were identified in a large, nationwide, commercial laboratory database from 2010 through 2021. The cohort comprised pregnancies for which the year of laboratory collection and patient's state of residence were available. Data were normalized ba

BACKGROUND: Hemolysis due to ABO incompatibility is an important differential diagnosis in newborns presenting with jaundice. Clinical studies evaluating ABO hemolytic disease of fetus and newborn (ABO-HDFN) question the diagnostic value of the direct antiglobulin test (DAT) in this situation.
GOALS: To determine the clinical and laboratorial findings associated with the occurrence of ABO-HDFN and to evaluate the accuracy of DAT as a diagnostic tool.
METHODS: This was a nested case control study with a cohort of 4122 newborns. Clinical and immunohematological data were retrieved from medical

This review paper provides an overview of the risk factors and laboratory testing for red blood cell (RBC) alloimmunization in pregnancy. RBC alloimmunization is a significant medical issue that can cause haemolytic disease of the fetus and newborn (HDFN), leading to neonatal morbidity and mortality. Current HDFN prophylaxis targets only Rhesus D (RhD) alloimmunization, with no effective measures to prevent alloimmunization to other RBC antigen groups. Several factors can increase the risk of developing RBC alloimmunization during pregnancy, including fetomaternal haemorrhage, RBC and maternal

Maternal allo-anti-D in RhD negative pregnant women may cause mild to severe hemolytic disease of the fetus and newborn. Although several other antibodies may also destroy red blood cells of the fetus and newborn, preventive measures with anti-D immunoglobulin are only available for D antigen. Targeted antenatal care together with postpartum prophylaxis with anti-D immunoglobulin has significantly reduced the D-alloimmunization risk. Potentially sensitizing events like trauma to the pregnant abdomen, vaginal bleeding, and amniocentesis may lead to fetomaternal hemorrhage and necessitate additi
