Antenatal Diagnosis and Management of Congenital Pulmonary Airway Malformation: A Case Report.

Congenital pulmonary airway malformation (CPAM) is a rare developmental lung anomaly that can be identified on prenatal ultrasonography. We report a case of antenatally diagnosed CPAM with a progressively increasing CPAM volume ratio (CVR) on serial foetal imaging. Antenatal corticosteroids were administered, and a male neonate was delivered by elective lower-segment caesarean section at 33 weeks of gestation. Postnatal imaging confirmed CPAM involving the right middle lobe. The infant underwent right middle lobectomy on postnatal day nine with an uneventful postoperative course. This case hig

Clinical clues to recognizing hereditary dehydrated stomatocytosis (DHSt) in children.

Dehydrated hereditary stomatocytosis (DHSt) is a heterogeneous group of rare hemolytic disorders with autosomal dominant inheritance. A series of 20 cases demonstrates that early identification of DHSt can prevent unnecessary interventions and improve the management of anemia, iron overload, and other complications in pediatric and adult patients. The presence of elevated mean corpuscular hemoglobin concentration (MCHC) with resistant erythrocytes suggested a possible association with variants in PIEZO1. Patients with KCNN4 variants showed no clear signs of erythrocyte dehydration, but, as wit

β-Arrestin1/2 are essential for embryonic lymphatic vessel development.

β-Arrestins are ubiquitously expressed cytosolic adaptor proteins that regulate GPCR-dependent and -independent pathways essential for numerous physiological functions. This study investigated the role of β-arrestin1/2 in embryonic lymphatic vessel development and survival by generating and characterizing mice with lymphatic tamoxifen-inducible loss of the genes encoding β-arrestin1/2 (Arrb1/2ΔiLEC). At E15.5, Arrb1/2ΔiLEC embryos exhibited profound hydrops fetalis and increased embryonic mortality compared with control Arrb1/2fl/fl embryos. Edematous Arrb1/2ΔiLEC embryos, which were more ofte

Placental Expression of CD15 in Fetal Anemia From Hydrops Fetalis.

INTRODUCTION: CD15 expression has been used as a marker of endothelial immaturity in the placenta and is linked to the setting of antenatal hypoxia. If so, expression should be increased in fetal anemia, especially when severe enough to cause hydrops fetalis. This has not been previously reported and the current study tested this hypothesis. MATERIALS AND METHODS: The study group consisted of 10 cases of anemic hydrops in second to early third trimester of gestation and 10 controls matched for gestational age. The majority (70%) of hydrops cases were due to hemoglobin (Hb) Bart. Placentas und

Identification of Human Transferrin Receptor as an Entry Co-receptor for Parvovirus B19 Infection of Human Erythroid Progenitor Cells.

UNLABELLED: Parvovirus B19 (B19V), a member of the genus Erythroparvovirus within the Parvoviridae family, infects human erythroid progenitor cells (EPCs) of bone marrow and fetal liver, and causes various hematological disorders. The minor capsid protein VP1 of B19V contains a unique N-terminal region (VP1u) that facilitates virus binding and internalization into EPCs via its receptor-binding domain (RBD). We previously identified tyrosine protein kinase receptor UFO (AXL) as a proteinaceous receptor for B19V infection of EPCs. In this study, we employed an ascorbate peroxidase 2 (APEX2)-base

Pregnancy induced hypertension and mirror syndrome associated with fetal hemoglobin Bart's disease in Thailand: a retrospective cohort study.

BACKGROUND: Pregnancy-induced hypertension (PIH) and mirror syndrome associated with fetal hemoglobin (Hb) Bart's disease have not been well characterized. This study primarily compared the incidence of these conditions in late gestation between affected and low-risk pregnancies, and secondarily assessed differences in other maternal complications. METHODS: A large retrospective cohort study (between 1992 and 2022) was conducted at a single tertiary center. The consecutive singleton pregnancies with fetal Hb Bart's disease diagnosed at ≥24 weeks, considered as delayed diagnosis were recruited

Refined classification and phenotype-driven analysis of PIEZO1 variants in hereditary red blood cell and iron disorders.

Interpreting genetic variants in complex genes such as PIEZO1 remains challenging because of marked allelic heterogeneity, relative tolerance to missense variation, and overlapping clinical phenotypes. Gain-of-function variants in PIEZO1 cause dehydrated hereditary stomatocytosis (DHS1, or hereditary xerocytosis), a pleiotropic syndrome characterized by anemia of variable severity and iron overload. In this study, we provided and applied an integrative framework combining the American College of Medical Genetics and Genomics guidelines, quantitative in silico predictions, structural domain ann

Fetal ascites caused by ovarian incarceration within an inguinal hernia and development of maternal mirror syndrome.

Fetal ascites is a non-specific finding which can be associated with fetal hydrops or an isolated intra-abdominal cause. We describe a case of fetal ascites associated with an intra-abdominal mass suspected to be ovarian in nature and diagnosed in the third trimester. The mother developed maternal mirror syndrome, previously reported to be associated with fetal hydrops but not reported in cases of isolated fetal ascites. Neonatal laparoscopy revealed the left ovary was oedematous and enlarged next to an inflamed and oedematous deep ring suggesting a previously incarcerated ovarian inguinal her

Antenatal Presentation of MRPS22-Related Mitochondrial Disease Confirmed With Rapid Proteomics.

MRPS22-related mitochondrial disease (MIM#611719) is a rare autosomal recessive disorder caused by defects in the mitochondrial ribosomal protein S22, a component of the small mitoribosomal subunit essential for mitochondrial translation. Of the few reported cases, most present antenatally with a severe phenotype, conveying a poor prognosis. We describe a fetus with severe antenatal-onset MRPS22-related mitochondrial disease and the use of multi-omics in the molecular diagnosis. A primigravida underwent termination of pregnancy following identification of multiple congenital anomalies (hydrops

Monocyte Monolayer Assay as a Tool in the Early Assessment of the Severity of the Alloantibodies in Patients with Hemolytic Disease of the Fetus and Newborn.

INTRODUCTION: Hemolytic disease of the fetus and newborn (HDFN), caused by the transplacental passage of maternal alloantibodies, can lead to progressive fetal organ failure and death from hydrops fetalis. Because serological tests often have limitations in predicting the clinical severity of these antibodies, in vitro assays have become promising tools, allowing the evaluation of the IgG-mediated extravascular hemolysis mechanism via macrophage Fc receptors. We aim to evaluate the monocyte monolayer assay (MMA), considered the most clinically validated tool for predicting in vivo survival in

The role of targeted next-generation sequencing and ultrasound in diagnosing fetal cytomegalovirus infection: a case report.

BACKGROUND: Cytomegalovirus (CMV) infection is a leading cause of congenital infection and neonatal morbidity. Conventional diagnostic methods, such as polymerase chain reaction (PCR) and amniocentesis, remain important in the diagnosis of congenital CMV infection, although each method has its own limitations in clinical practice. CASE PRESENTATION: A 31-year-old woman, gravida 3 para 1, presented for routine prenatal evaluation. At 18 weeks of gestation, ultrasound revealed echogenic bowel and fetal ascites. Amniocentesis at 19 weeks showed normal chromosomal results, but targeted next-gener

Establishment of an immortalized erythroid model for Hb Bart's hydrops fetalis with homozygous α0-thalassemia Southeast Asian deletion.

Hemoglobin Bart's hydrops fetalis syndrome (BHFS) is the most severe form of α-thalassemia and is highly prevalent in Southeast Asia. It is most commonly caused by homozygosity for the α0-thalassemia Southeast Asian deletion (--SEA/--SEA), which eliminates α-globin production and results in profound fetal anemia. Current management is limited to intrauterine transfusions with only transient benefit, and no pharmacological or gene-based therapies are available. Progress toward therapy has been hampered by the lack of disease-relevant in vitro models. Here, we established Chery-Bart's, an immort

Neonatal Hydrops and Biliary Atresia as an Early Presentation of Mevalonate Kinase Deficiency: A Case Report.

Mevalonate kinase deficiency (MKD) is a rare autosomal recessive autoinflammatory disorder with a broad clinical spectrum. Neonatal presentation is uncommon and may mimic severe infection, leading to diagnostic delay. We report a preterm female infant who presented with non-immune hydrops fetalis and persistent systemic inflammation from the first week of life, despite repeated negative blood and CSF cultures and multiple courses of broad-spectrum antibiotics. The infant subsequently developed progressive cholestasis with pale stools, and biliary atresia was confirmed by hepatobiliary imaging

Non-immune hydrops fetalis (NIHF): diagnostic evaluation and lessons learned from a case series.

BACKGROUND: Fetal hydrops, characterized by abnormal fluid accumulation in two or more fetal compartments, is a critical condition with a poor prognosis, often resulting in significant fetal mortality. The condition itself is not a diagnosis but a clinical sign with many underlying causes, ranging from infections and chromosomal abnormalities to rare maternal conditions such as mirror syndrome. CASE PRESENTATION: This review examines three cases of fetal hydrops, highlighting common etiologies, such as infections and chromosomal disorders, and more unusual causes like maternal complications.

Application of next-generation sequencing in nonimmune hydrops fetalis and its impact on pregnancy decisions.

BACKGROUND: Nonimmune hydrops fetalis (NIHF) has a highly heterogeneous etiology, with genetic factors contributing substantially to its pathogenesis. This study aimed to investigate the chromosomal and monogenic etiologies of NIHF. METHODS: This single-center retrospective cohort study included 121 pregnancies complicated by NIHF evaluated at a tertiary referral center between August 2020 and August 2023. All cases underwent copy number variation sequencing (CNV-seq), and 95 additionally underwent whole-exome sequencing (WES), both assays were based on next-generation sequencing (NGS) techno

[Hereditary dehydrated stomatocytosis (= hereditary xerocytosis) - Interesting hummingbird or clinically relevant diagnosis?].

Hereditary stomatocytosis comprises a group of disorders of the erythrocyte membrane that lead to impaired membrane permeability for Na+ and K+ ions. The most common form, dehydrated stomatocytosis, has long been underestimated in terms of its incidence and significance. It is associated with hemolysis, usually anemia of varying severity, and a combination of primary and secondary resorptive hemochromatosis. In the perinatal period, some patients develop edema and effusions of varying severity, which usually regress spontaneously but can progress to life-threatening hydrops fetalis. Splenectom

Prenatal Diagnosis and Monitoring of Type III Congenital Pulmonary Airway Malformation: A Case Report.

Congenital pulmonary airway malformation (CPAM) is a rare congenital lung anomaly resulting from abnormal bronchial development and disorganized respiratory structures, producing nonfunctional pulmonary lesions. Although uncommon, CPAM is the most frequently diagnosed congenital lung malformation. Prenatal identification allows for risk stratification and anticipatory management. Here we present a case of a fetus prenatally diagnosed with a type III CPAM during routine obstetric ultrasonography. Weekly ultrasonography was performed to monitor lesion progression and evaluate for complications,

Perinatal survival and long-term neurodevelopmental outcomes of congenital chylothorax.

BACKGROUND: Congenital chylothorax carries a poor prognosis, particularly with delivery at <32 weeks. Survival outcomes under consistent management prioritizing pregnancy prolongation despite worsening hydrops, as well as long-term neurodevelopmental data, remain limited. METHODS: In this single-center retrospective study, we analyzed fetuses with congenital chylothorax requiring perinatal drainage managed between 2005 and 2021. The protocol prioritized prolonging pregnancy to ≥32 weeks. Postnatally, octreotide and prednisolone were administered stepwise for persistent drainage. RESULTS: Of
