Mirror syndrome and placental ectopic liver in association with de novo SOS1 variant.

Mirror syndrome is a rare obstetric condition characterized by maternal fluid retention mirroring fetal hydrops. Placental ectopic liver tissue is an extremely rare non-trophoblastic placental tumor, potentially arising from aberrant hepatoblast migration. While its association with fetal hydrops has been reported, the clinical significance remains uncertain. We present a patient of maternal mirror syndrome linked to fetal hydrops due to a de novo SOS1 variant, with histopathological identification of placental ectopic liver tissue. A 32-year-old woman was admitted at 31 weeks' gestation with

Molecular targeted treatment in infants with central conducting lymphatic anomalies.

UNLABELLED: Central conducting lymphatic anomaly (CCLA) is a rare and potentially life-threatening vascular malformation characterized by impaired central lymphatic flow. Hydrops fetalis and congenital hydro-/chylothorax are common neonatal presentations; however, diagnosing CCLA poses challenges and requires advanced imaging. Management typically includes supportive therapies with limited effect, such as medium-chain triglyceride (MCT) diet, octreotide or propranolol, and thoracic drainage. Upcoming treatment options with mammalian target of rapamycin (mTOR) and mitogen-activated protein kina

Antenatal shunting and outcomes in fetuses with non-immune hydrops fetalis.

OBJECTIVES: Hydrops fetalis is associated with high morbidity and perinatal mortality. The aim of our study was to compare the outcomes of infants who had non-immune hydrops fetalis (NIHF) who did or did not undergo antenatal shunting. METHODS: Between January 2014 and June 2023, 20 infants with the diagnosis of NIHF were admitted to the neonatal intensive care unit (NICU) at King's College Hospital NHS Foundation Trust. The criteria for antenatal shunt placement were development of hydrops fetalis, polyhydramnios due to oesophageal compression by a pleural effusion that would likely result i

Prenatal Intervention in High-Risk CPAM: Postnatal Outcomes After Fetal versus Standard Surgery: A Propensity Score Matched Study.

Congenital pulmonary airway malformation (CPAM) is a rare fetal lung anomaly characterized by cystic lesions that can impede lung development. While smaller lesions may remain asymptomatic and are managed postnatally, larger lesions can cause severe complications such as mediastinal shift and hydrops fetalis. Fetal surgery may be indicated in these cases. This study analyzed whether fetal surgical intervention affects outcomes of subsequent postnatal surgery in CPAM patients.A retrospective single-center cohort study was conducted on pediatric patients treated for CPAM between January 2010 and

Piezo1 Channel Activators Yoda1 and Yoda2 in the Context of Red Blood Cells.

Piezo1 is a mechanosensitive non-selective cation channel. Genetic alterations of the channel result in a hematologic phenotype named Hereditary Xerocytosis. With Yoda1 and, more recently, Yoda2, compounds to increase the activity of Piezo1 have become available. However, their concrete effect depends on the nano environment of the channel and hence on the cell type. Here we compare the potency of Yoda1 and Yoda2 in red blood cells (RBCs). We investigate the effect of the compounds on direct channel activity using automated patch clamp, as well as the secondary effects of channel activation on

Antepartum Thoracocentesis: A Case Report on Congenital Chylothorax.

Congenital chylothorax remains a poorly understood condition, and the optimal approach to its management continues to evolve. Rapid progression of pleural effusion with mediastinal shift and/or development of hydrops fetalis are key indications for urgent fetal intervention to enable thoracic decompression and prevent fetal compromise. We report a case of isolated mild unilateral pleural effusion identified during routine midtrimester ultrasound, successfully managed through close ultrasonographic surveillance and perinatal thoracocentesis performed immediately before delivery to facilitate lu

Long-Segment Superior Vena Cava Hypoplasia as an Uncommon Cause of Non-immune Hydrops Fetalis.

Non-immune hydrops fetalis (NIHF) is a complex perinatal diagnosis with diverse and often idiopathic causes, including impaired lymphatic drainage and compromised venous return. We present the case of a newborn with large bilateral pleural effusions who was subsequently found to have a hypoplastic superior vena cava (SVC). Initially treated with thoracentesis and dietary changes, the infant's symptoms persisted. Follow-up imaging confirmed ongoing SVC narrowing, which was successfully treated with a stent. This case underscores the importance of identifying non-traditional causes of NIHF.

Neonatal case of congenital dilated cardiomyopathy, cholestatic liver injury, and hyperferritinemia with FHL2 variant: a case report suggesting possible novel phenotypes.

A male infant with fetal hydrops was diagnosed with congenital dilated cardiomyopathy and died at 2 months despite intensive treatment. Trio-exome sequencing identified as de novo FHL2 variant (p.Arg131Cys). He also presented with non-cardiogenic cholestatic liver injury and hyperferritinemia, which may represent novel FHL2-related phenotypes based on supporting evidence from experimental models.

Prevalence of double heterozygotes of HbE and α-thal 1 (SEA) type in pregnancies and their partners that received antenatal care at Chiangrai Prachanukroh Hospital and reevaluated the cut-offs for differentiation.

BACKGROUND: In Thailand, double heterozygotes of HbE and α-thal 1 (SEA) type is common. If pregnant women carrying this genotype conceive with partners who also carry the same genotype or who are carriers of α-thalassemia 1 (SEA) type, there is 25% chance of a fetus having Hb Bart's hydrops fetalis. This research aims to determine the prevalence of double SEA/E, compare the hematological parameters and HbE levels between pure HbE heterozygote and double SEA/E and reevaluate the cut-offs used to screen individuals with these genotypes. METHODS: In a retrospective study, 397 samples from indivi

Neonatal anemia due to spontaneous fetomaternal hemorrhage in a term neonate: a case report of an incidental finding and brief review.

Fetomaternal hemorrhage (FMH) is a rare perinatal condition characterized by the transplacental transfer of fetal erythrocytes into the maternal circulation. It poses significant risks, including fetal anemia, hydrops fetalis, and maternal alloimmunization, and often remains underdiagnosed due to non-specific clinical presentations and limited diagnostic accessibility. We present a case of a term neonate delivered via cesarean section at 38 weeks' gestation who exhibited severe anemia and hyperbilirubinemia. Blood group analysis revealed maternal-neonatal Rh incompatibility, while flow cytomet

Transient Congenital Nephrotic Syndrome in Neonates: Two Case Reports and Review of Recent Literature.

We report two cases of non-genetic transient congenital nephrotic syndrome (CNS) in neonates, both presenting with hydrops fetalis associated with nephrotic range proteinuria and significant hypoalbuminemia. Investigations did not reveal any underlying genetic cause or congenital infection to explain the nephrotic syndrome. Both cases received supportive therapy and showed spontaneous recovery with resolution of proteinuria and normalization of serum albumin in a few weeks. These cases underscore the diagnostic and prognostic uncertainties in CNS without identifiable genetic or infectious caus

Placenta-derived extracellular vesicles in maternal plasma of Hb Bart's fetuses.

Hemoglobin (Hb) Bart's disease is the most common cause of hydrops fetalis among Southeast Asians. Given its high fatality rate, prenatal diagnosis and treatment are the key. This condition is associated with placental hypoxia, which can trigger the release of placenta-derived extracellular vesicles (EVs) into the maternal circulation. We aimed to determine the levels of placenta-derived EVs and the proteomic profiles of plasma EVs in women with fetuses with Hb Bart. This prospective cohort study included women with: (1) normal term pregnancies (Group 1; n = 19); (2) Hb Bart's fetuses with hyd

Revisiting hemoglobin constant spring: molecular insights, pathophysiological mechanisms, and clinical perspectives.

BACKGROUND: Hemoglobin Constant Spring (Hb CS) is an α-globin variant predominantly found in Southeast Asian populations. Compound heterozygosity of Hb CS with α-thalassemia results in a spectrum of chronic hemolytic anemia. Recent advances in cellular therapy, particularly gene editing approaches currently under in-vitro investigation, offer potential new avenues for treatment in patients with severe phenotypes. MAIN BODY: This review discusses the molecular basis, pathophysiology, clinical manifestations, and current management strategies of Hb CS. The condition arises from a point mutation

Predicting the Double Heterozygotes of HbE and α-Thalassemia-1 (Southeast Asian Type) Using RapidMiner-Generated Hematologic Algorithm.

Double heterozygotes of HbE and α-Thalassemia-1 (Southeast Asian type) ((βA/βE, - -SEA/αα) can result in severe thalassemia conditions, such as HbE/β-thalassemia (βE/βT)) and Hb Bart's hydrops fetalis syndrome (- -SEA/- -SEA). Identification of these double heterozygotes is crucial for preventing births affected by Hb Bart's hydrops fetalis syndrome. The most accurate method for detecting HbE/SEA-α-Thalassemia-1 double heterozygotes is PCR-based analysis, which is complex and costly. This study aimed to develop a hematologic algorithm for predicting double heterozygotes of HbE/SEA-α-Thalassemi
