Neonatal management of parvovirus B19-induced hydrops fetalis: a case report.

BACKGROUND: Hydrops fetalis (HF) is a severe fetal condition, and congenital Parvovirus B19 (B19V) infection is a leading cause of the non-immune form (NIHF). The recent European B19V outbreak has had a substantial impact on obstetric and neonatal outcomes, leading to an increase in fetal anemia, NIHF, spontaneous abortions, and intrauterine demise. While prenatal diagnosis and intrauterine interventions are well established, postnatal management of B19V-related NIHF remains challenging and outcomes are often poor. CASE PRESENTATION: We report a preterm newborn with B19V-associated NIHF follo

Character and Frequency of Early Postnatal Resection in Prenatally Diagnosed Congenital Lung Malformations.

INTRODUCTION: Currently, risk stratification criteria and indications for early postnatal resection of high-risk congenital lung malformations (CLM) are not universally accepted. In this study, we sought to characterize prenatal risk factors and outcomes associated with early postnatal resection in prenatally diagnosed CLMs. METHODS: Retrospective cohort study of patients seen at the Fetal Care Center from January 2014 to December 2023. Categorical data were analyzed with chi-square calculations. Receiver operating characteristic curves were generated for optimal CPAM volume ratio (CVR) and i

Evolving Features of RASopathies Among Pregnancies With Abnormal Fetal Fluid Collections.

OBJECTIVE: We aimed to characterize the fetal features across gestation and describe genotype-phenotype correlations for pregnancies with fetal RASopathies that were more severely affected as they presented with at least one abnormal fluid collection. METHOD: Retrospective cohort study of pregnancies with a fetal RASopathy and one or more abnormal fluid collections. Ultrasound and clinic databases were searched at six institutions to identify pathogenic or likely pathogenic variants indicating a fetal RASopathy. Phenotypic features were organized by gestational age to evaluate their evolution

Role of ultrasound in the prenatal diagnosis of sacrococcygeal teratoma: A case study.

Sacrococcygeal teratoma (SCT) is the most common congenital tumor arising from pluripotent cells at the fetal caudal end. Early prenatal diagnosis is essential for appropriate monitoring and management due to potential complications such as polyhydramnios, hydrops fetalis, and high-output cardiac failure. We report a case of a 30-year-old woman whose fetus was diagnosed at 17 weeks' gestation with a 20 mm sacrococcygeal mass detected by two-dimensional ultrasound. Detailed assessment with color Doppler and three-dimensional imaging characterized the lesion as a mixed solid-cystic, highly vascu

Prenatal Imaging of Micrognathia, Micromelia, and Fetal Hydrops Leading to the Diagnosis of Achondrogenesis Type II with a COL2A1 Missense Mutation.

This case report describes a fetus with achondrogenesis type II, a severe and lethal type II collagen disorder, presenting with micrognathia and hydrops. Prenatal evaluation with 2D/3D ultrasound, followed by postmortem imaging and pathological examination, confirmed the diagnosis. Genetic testing revealed a heterozygous COL2A1 mutation (1703G>A; Gly516Ser, exon 24). The significance of this study lies in the identification of a missense mutation in COL2A1 associated with achondrogenesis type II. This report highlights that the condition may present with hydrops and craniofacial anomalies, est

Genetic etiology and pregnancy outcomes of abnormal fluid accumulation in fetus: A retrospective cohort study.

BACKGROUND: This study investigated the genetic etiology of abnormal fetal fluid accumulation, aiming to quantify pathogenic variants and correlate them with clinical outcomes to improve genetic counseling. METHODS: A cohort of 305 fetuses underwent single-nucleotide polymorphism array (SNP-array) and whole-exome sequencing (WES) of amniotic fluid or cord blood. RESULTS: Pathogenic copy number variations (CNVs) were detected in 49 cases, including aneuploidies (e.g., trisomy 21, Turner syndrome) and microdeletions/duplications. Two single-gene mutations (SNAP25, PLD1) were identified in CNV-

Phenotypic Divergence in Siblings with the Same Genotype: Diffuse Dermal Melanocytosis in Infantile-Onset Galactosialidosis.

BACKGROUND: Galactosialidosis (GS) is an ultra-rare autosomal recessive lysosomal storage disorder caused by pathogenic variants in the CTSA gene. The resulting deficiency of protective protein/cathepsin A leads to reduced β-galactosidase and α-neuraminidase activity, causing multisystem involvement. While typical features include dysmorphic facies, organomegaly, skeletal dysplasia, and neurological impairment, dermatologic manifestations remain poorly characterized. CASE PRESENTATION: We report a 9-month-old girl presenting with hepatosplenomegaly and diffuse dermal melanocytosis. Her histor

Piezo1 gain of function induces a platelet preactivation state.

BACKGROUND: Hereditary xerocytosis (HX) is a dominant red blood cell membrane disorder characterized by dehydration and hemolysis. Most HX cases result from gain-of-function (GOF) mutations in the PIEZO1 gene, which encodes a mechanosensitive ion channel permeable to calcium when activated. Evolution of HX is characterized by frequent thromboembolic events after splenectomy. OBJECTIVES: Given the major role of calcium during platelet activation, we hypothesized that Piezo1 GOF may increase platelet activation and contribute to thrombosis. METHODS: Human washed platelets were activated by var

Hydroxychloroquine After Complete Heart Block Diagnosis and Relationship to Patient Outcomes: A Descriptive Cohort Study.

OBJECTIVE: Congenital complete heart block (CHB) carries significant morbidity and mortality. Maternal Hydroxychloroquine may prevent CHB in subsequent pregnancies. No studies have explored the benefit of Hydroxychloroquine initiated after the diagnosis of CHB. We hypothesized fetuses exposed to Hydroxychloroquine, even after CHB diagnosis, will have improved outcomes using time to pacemaker placement as the primary outcome. METHODS: A single-center retrospective study was conducted on patients with CHB secondary to maternal anti-SSA antibodies approximately 2005-2024. RESULTS: All patients

A Zika Virus-Like Particle Vaccine Mitigates Early Pregnancy Loss In Rhesus Macaques.

Zika virus (ZIKV) is an arthropod-borne Orthoflavivirus that caused a major outbreak in the Americas in 2015-16. In Brazil, up to 46% of ZIKV positive pregnancies resulted in congenital Zika syndrome (CZS). CZS is characterized by a wide range of neurologic birth defects and miscarriage in up to 7.6% of affected pregnancies. With no current licensed ZIKV vaccines, we sought to evaluate a Zika virus-like particle (VLP) vaccine candidate in a rhesus macaque (RM) pregnancy model. VLPs were produced in mammalian cells expressing the pre-membrane-envelope region of the Asian-lineage ZIKV strain PRV

Aberrant Splicing Caused by Compound Heterozygous Variants in WDR35 Identified in a Fetus With Cranioectodermal Dysplasia 2.

Cranioectodermal dysplasia (CED), also known as Sensenbrenner syndrome, is a rare autosomal recessive ciliopathy with significant involvement of the skeleton, ectoderm, retina, kidneys, liver, lungs, and occasionally the brain. Cranioectodermal dysplasia-2 (CED2) is caused by homozygous or compound heterozygous mutation in the WD repeat-containing protein 35 (WDR35). However, the current understanding of the CED2 prenatal phenotype is limited. Here, we describe the fetus at 12 + 4 weeks gestation from a Chinese family with healthy parents. The fetus presented with lymphedema, omphalocele, hydr

Patient experience and burden of haemolytic disease of the foetus and newborn: a systematic review.

BACKGROUND: Haemolytic disease of the foetus and newborn (HDFN) is an immune disorder driven by maternal alloimmunisation against foetal/newborn red blood cell antigens. HDFN can cause significant morbidity and mortality, with symptoms in the foetus ranging from mild anaemia to hydrops fetalis. While in newborns, HDFN can lead to severe forms of neonatal hyperbilirubinaemia and kernicterus. This systematic review (SR) aimed to identify and summarise real-world evidence (RWE) related to the patient burden/experience and economic burden of HDFN. METHODS: Electronic database searches supplemente

Parvovirus: Conservative management of fetal anemia and hydrops.

Following the COVID-19 pandemic, Northwestern Europe has experienced a marked increase in congenital parvovirus infections. This rise is attributed to social distancing measures which disrupted the usual seasonal variation of parvovirus B19. Fetal infection may cause severe anemia, thrombocytopenia, and hydrops fetalis, with significant risk of intrauterine death. Therefore, when acute parvovirus B19 infection is confirmed by maternal serology, serial ultrasound surveillance of the middle cerebral artery peak systolic velocity is recommended. Intrauterine transfusion remains the only establish

Parvovirus B19 Infection in Pregnancy: Awareness of the Increased Incidence of Severe Intrauterine Infection.

In 2024, Europe experienced a significant upsurge in cases of Parvovirus B19 (B19V), the etiological agent of erythema infectiosum, also known as fifth disease. The prevalence of B19V in pregnant women, a particularly vulnerable population, holds critical clinical significance. Typically, B19V follows a well-documented seasonal pattern, with annual epidemics peaking in the spring and larger outbreaks occurring approximately every four years. B19V exhibits a tropism for erythroid precursor cells, potentially resulting in fetal anemia and, in the most severe scenarios, intrauterine demise. Sever
