Recurrent KCNN4 p.Ser314Pro variant in a child with Gardos channelopathy: a 6-year follow-up.

Gardos channelopathy is a rare dehydrated hereditary stomatocytosis caused by gain-of-function KCNN4 mutations. We report a pediatric case due to a recurrent p.Ser314Pro variant, representing the first description outside the Italian population. Over a comprehensive 6-year clinical follow-up, the patient exhibited a stable baseline course punctuated by infection-triggered hemolytic crises, exacerbated by concomitant bronchial asthma. Prospective data demonstrated persistent iron redistribution without systemic overload under conservative management. This case expands the geographic spectrum of

Progressive non-immune hydrops fetalis associated with RASA1 mutation: prenatal imaging and genomic insights.

Hydrops fetalis (HF) is the pathological accumulation of fluid in two or more fetal compartments. While immune-mediated HF was historically predominant, non-immune hydrops fetalis (NIHF) is now increasingly common. Advances in genetic testing have revealed monogenic causes, including RASopathies (a group of genetic syndromes caused by dysregulation of the RAS-mitogen-activated protein kinase signalling pathway). We report a young primigravida, referred at 20+3 weeks for unilateral fetal pleural effusion. Serial ultrasound scans showed rapid progression to bilateral effusion, hepatomegaly, poly

Successful Management of Fetal Supraventricular Tachycardia With Flecainide in a Case Complicated by Early Hydrops Fetalis: A Case Report.

Fetal arrhythmia that can lead to hydrops fetalis and fetal heart failure if untreated. This case report describes the successful treatment of a fetus with SVT at 26 weeks of gestation using flecainide. A 37-year-old gravida 3, para 2 woman was diagnosed with fetal SVT during a routine ultrasound, which also revealed mild pleural effusion, pericardial effusion, and ascites, raising concerns for hydrops fetalis. A fetal echocardiogram confirmed a heart rate of 250-300 beats per minute with preserved cardiac function. The mother was started on oral flecainide at 100 mg twice daily, which was inc

22q11.2 duplication syndrome and LZTR1-related Noonan syndrome type 10 overlapping phenotypes in a dual diagnosis - fetal hydrops: a case report.

BACKGROUND: Nonimmune factors are the most common cause of fetal hydrops, which are characterized by the accumulation of pathological fluid due to various causes. The etiology of this disease is complex, with a relatively high mortality rate, and the prognosis largely depends on the underlying cause. Notably, fetal hydrops attributable to genetic causes, such as chromosomal abnormalities or gene mutations, are typically associated with a poor prognosis. CASE PRESENTATION: A fetus diagnosed with severe nonimmune fetal hydrops at 27 weeks of gestation presented with multiple abnormalities on pr

A Case of Nonimmune Hydrops Fetalis With a Duct-Dependent Systemic Circulation and a Novel Mutation of Kabuki Syndrome.

INTRODUCTION: Nonimmune hydrops fetalis (NIHF) has numerous etiologies, the most common of which are cardiac anomalies and fetal infection. However, genetic disorders are also being increasingly recognized as a cause of NIHF. Here, we report a case of a neonate presenting with polyhydramnios, NIHF, structural heart disease, and diaphragmatic defect who was found to have a previously unreported mutation in the KMT2D gene. CASE PRESENTATION: A female neonate with antenatally detected NIHF was born at 35 weeks of gestation via cesarean section. At birth, she was noted to have dysmorphic features

Investigation of non-immune hydrops fetalis in singleton pregnancy: a practical guide for the perinatal pathologist.

Hydrops fetalis is defined by abnormal fluid accumulation in at least two fetal compartments, resulting from a range of underlying pathological processes, and with a high mortality at the severe end of the spectrum. Identifying the specific cause is crucial for managing both current and future pregnancies. Despite comprehensive antenatal investigations, including obstetric imaging, serology and genetic testing, many cases of non-immune hydrops fetalis (NIHF) remain unexplained at delivery. Some diagnostic features may not be apparent on ultrasound, with perinatal autopsy providing additional v

Comparing traditional and NGS-based screening strategies for thalassemia in a high-prevalence Hakka population: a population-based study.

Thalassemia is one of the most common monogenic disorders in southern China, with the Hakka population exhibiting a distinct genetic profile. This study evaluated whether next-generation sequencing (NGS)-based universal screening offers advantages over traditional cascade screening in this specific population. We enrolled 91,857 reproductive-age individuals in Ganzhou, a high-prevalence Hakka region, comparing 37,500 participants who underwent traditional screening (2019-2021) with 54,357 who received NGS-based screening. NGS-based screening detected thalassemia carriers at a rate 4.74 times h

Mouse Model of Hemolytic Disease of the Fetus and Newborn.

Hemolytic Disease of the Fetus and Newborn (HDFN) is caused by the placental transfer of maternal red blood cell (RBC) alloantibodies into the fetal circulation which then targets paternally derived RBC antigens expressed on the fetal RBC. The resulting anemia in the fetus/newborn can lead to grave clinical consequences including hydrops fetalis, kernicterus, and perinatal demise. We have utilized the human KEL transgenic mouse model to study the induction of anti-KEL RBC alloantibodies through pregnancy as well as transfusion and the subsequent placental transfer of anti-KEL RBC alloantibodie

Successful perinatal management of a large placental chorio-angioma: a case report demonstrating the effectiveness of a public-private partnership model.

Placental chorio-angiomas are rare benign vascular tumours of the placenta which are usually small and clinically insignificant, but large lesions can cause serious fetal complications such as anaemia, hydrops fetalis and an adverse perinatal outcome. We report the case of a 29-year-old multigravida diagnosed at 28 weeks' gestation with a large placental chorioangioma accompanied by sonographic evidence of fetal anaemia and hydrops fetalis. Despite two intra-uterine transfusions which led to partial improvement, the hydrops persisted, necessitating a preterm caesarean delivery at 31 weeks. The

Multi-omic analysis identifies a multi-step pathology in a case of multiple chorangioma syndrome in monochorionic twins.

BACKGROUND: Chorangiomas, benign capillary lesions of the placenta, occur in ~1% of births, typically as solitary nodules. Multiple chorangioma syndrome is rare and increases risk of fetal heart failure, hydrops fetalis, and intrauterine death due to placental hemodynamic disruption. While genetic and hypoxic factors have been suggested in chorangioma development, direct molecular evidence is lacking. Here, we present a unique case of multiple chorangiomas confined to half of a shared placenta in monozygotic monochorionic diamniotic twins, providing a rare opportunity to dissect molecular driv

Cytogenetic and Molecular Findings in Hydrops-Related Mirror Syndrome.

OBJECTIVE: Mirror syndrome is a rare, life-threatening condition in which maternal fluid overload mirrors fetal hydrops. Data on genetic findings in affected pregnancies are limited. We compared genetic diagnoses in hydrops cases with and without mirror syndrome. METHODS: We conducted a retrospective cohort study (2012-2023) of pregnancies with non-immune hydrops at a single fetal center. Clinical data along with prenatal and postnatal genetic testing results were analyzed. Mirror syndrome was defined as fetal hydrops with new or worsening hypertension and significant maternal edema. Genetic

Diverse clinical spectrum of Niemann-Pick C: insights from a single center.

OBJECTIVES: Niemann-Pick disease type C (NPC) is a lysosomal lipid trafficking disorder characterized by impaired intracellular cholesterol transport, resulting in progressive neurovisceral involvement. While neurological progression has been extensively described, metabolic biomarkers and lipid abnormalities may provide earlier diagnostic clues, particularly in high-consanguinity populations. METHODS: We retrospectively evaluated 14 genetically confirmed NPC patients followed at a tertiary pediatric metabolic center between 2018 and 2025. Clinical presentation, biochemical parameters includi

Repair of Severe Tricuspid Regurgitation in a Neonate With Intrauterine Tricuspid Valve Chordae Tendinae Rupture.

Congenital tricuspid valve regurgitation (TR) is rare, and chordae tendinae rupture is an even more uncommon finding in neonates. We describe a case with intrauterine tricuspid valve chordae tendinae rupture leading to severe TR and hydrops fetalis. Successful tricuspid valve repair using artificial chordae, leaflet approximation, and annular reduction were used as rescue therapy for the neonate.

Primary cardiac hemangioma: a report of two cases and literature review.

Cardiac hemangiomas (CHs) are extremely rare tumors. Despite their benign nature, CHs can lead to serious complications, including cyanosis, arrhythmia, heart failure, hydrops fetalis, or fetal death. While echocardiography provides real-time assessment, CHs may mimic other cardiac abnormalities, necessitating further imaging modalities such as CT or MRI for accurate diagnosis. Treatment typically involves radical resection, even in asymptomatic cases. We describe two cases of cardiac hemangiomas in the atrium, and both patients exhibited an uneventful recovery after a radical resection.

Cyclic Fevers in Pregnancy: A Case of Parvovirus-Related Hemophagocytic Lymphohistiocytosis.

Parvovirus B19 typically causes mild symptoms in healthy adults, including fever, rash, and arthralgias. Certain immunocompromised adult populations, including those with immunodeficiencies or hematologic disease, are at risk of clinically significant complications, including transient aplastic crisis. Further, in pregnant patients, there is a risk of transplacental viral transmission, which may lead to severe fetal anemia, nonimmune hydrops fetalis, and intrauterine fetal demise. The clinical course of parvovirus B19 in pregnant patients with underlying hematologic disease has not been well r

Hereditary Spherocytosis: Linking Ion Transport Defects to Osmotic Gradient Ektacytometry Profiles-A Review.

Hereditary spherocytosis (HS) is the most common inherited red blood cell (RBC) membrane disorder and has traditionally been attributed to defects in cytoskeletal proteins such as spectrin, ankyrin, band 3, and protein 4.2. Growing evidence, however, shows that disturbances in ion transport also contribute to HS pathophysiology. This review summarizes current understanding of HS by integrating membrane structural defects with abnormalities in ion homeostasis and highlights the diagnostic value of osmotic gradient ektacytometry (OGE). Beyond membrane instability, HS erythrocytes exhibit increas
