Elucidating the genetic architecture of yield components in pyramidale wheat (Triticum turgidum L.): a comparative approach using QTL analysis and genomic selection.

BACKGROUND: Diabetic retinopathy (DR) is a major microvascular complication of diabetes mellitus and a leading cause of visual impairment. While dyslipidemia has been implicated in DR pathogenesis, the relationship between high-density lipoprotein cholesterol (HDL-C) and DR remains controversial. This study aimed to investigate the association between HDL-C levels and DR prevalence in diabetic patients type 2 diabetes mellitus (T2DM). METHODS: This study is the second analysis based on a cross-sectional studv. A total of 2001 (858 men and 1143 women) diabetic patients who visited the diabetic

Hereditary Spherocytosis: A Case Series Spanning Prenatal Detection to Neonatal Care.

OBJECTIVE: We aim to expand the prenatal phenotypic spectrum of hereditary spherocytosis (HS). METHOD: We reviewed four pregnancies at risk for or suspected of having HS. Prenatal ultrasound, middle cerebral artery (MCA) Doppler surveillance, genetic testing, and neonatal outcomes were evaluated. RESULTS: Case 1 involved a fetus with a homozygous variant in SPTA1 with severe fetal anemia and nonimmune hydrops fetalis resulting in fetal demise. Case 2 involved a mother with a heterozygous ANK1 variant. Fetal anemia and polyhydramnios prompted close prenatal surveillance, and the infant requir

Work-related musculoskeletal disorders in endoscopic sinus and skull base surgeons: an international survey.

BACKGROUND: Endoscopic sinus and skull base surgeons are at high risk of work-related musculoskeletal disorders (WRMD) due to the unique ergonomic risks inherent in this specialty. These musculoskeletal disorders may negatively impact surgeons and their patients. The primary aim of this international survey was to quantify this problem globally and evaluate its associated factors. METHODS: A 46-item electronic survey was distributed to endoscopic sinus and skull base surgeons at any career stage via multiple international professional societies. RESULTS: We received 1385 responses. The preva

Many faces of non‑deletional α‑thalassaemia variants in neonate and early childhood.

Non-deletional α-thalassemia variants, particularly hemoglobin Constant Spring (Hb CS) and hemoglobin Pakse, are prevalent in northeastern Thailand and neighboring regions. Homozygous Hb CS and compound heterozygous Hb CS/Hb Pakse can present with a wide clinical spectrum. In this study, 49 patients with these α-globin variants-with or without co-inheritance of beta-thalassemia-were evaluated. Genotypes included homozygous Hb CS (n = 27), homozygous Hb CS with Hb E (n = 8), compound heterozygous Hb CS/Hb Pakse (n = 10), compound heterozygous Hb CS/Hb Pakse with Hb E (n = 3), and homozygous Hb

Immune and non-immune hydrops fetalis in a resource-limited setting: a report of two cases.

Hydrops fetalis is a fetal condition characterized by abnormal fluid accumulation in multiple fetal compartments and is associated with high perinatal mortality. Unlike high-income settings, both immune and non-immune hydrops fetalis (IHF and NIHF) remain relevant in low- and middle-income countries (LMICs), where evaluation and specialized care are often delayed or inaccessible. We report two cases of hydrops fetalis diagnosed antenatally at a tertiary referral hospital in Kenya, followed through delivery and outcome. Case one involved immune hydrops fetalis due to suspected non-Rhesus alloim

Efficacy Assessment of Intraoperative Traction Table Readjustment Combined with Percutaneous Cerclage Wiring and Long Intramedullary Nail for Unstable Femoral Subtrochanteric Fractures.

Chronic rhinosinusitis (CRS) significantly affect the quality of life (QoL) of patients. The study was conducted in CRS patients who were treated with functional endoscopic sinus surgery (FESS) after failure of medical treatment to analyze clinical outcome using prospectively collected data through a symptom-based rhinosinusitis outcome measure, the Sino-nasal Outcome Test-22 (SNOT-22). The aim of the study was to evaluate and compare the QoL in patients of chronic rhinosinusitis pre-operative and after FESS by SNOT-22. The prospective study was conducted on 40 patients of chronic rhinosinusit

Congenital scrub typhus presenting as non-immune hydrops fetalis.

Congenital scrub typhus is a rare form of Orientia tsutsugamushi infection in neonates. Typical neonatal manifestations include fever, thrombocytopenia and hepatosplenomegaly; however, presentation in the form of non-immune hydrops fetalis (NIHF) is uncommon and has not been reported previously in literature. We report a case of vertically transmitted scrub typhus in a preterm neonate who presented with hydrops fetalis at birth. The diagnosis was established using molecular testing, and timely initiation of appropriate antimicrobial therapy resulted in complete recovery. This case and the acco

Paravertebral extramedullary haematopoiesis revealing previously unrecognised hereditary xerocytosis.

Extramedullary haematopoiesis (EMH) is a rare cause of posterior mediastinal masses, often linked to chronic anaemia. A man in his late 60s with recurrent biliary disease was referred for evaluation of posterior mediastinal masses incidentally found during hospitalisation for choledocholithiasis. Imaging and biopsy revealed paravertebral EMH. Laboratory testing showed compensated haemolytic anaemia with stomatocytes, elevated eosin-5-maleimide binding, decreased erythrocyte osmotic fragility, and a PIEZO1 mutation (p.K2323T), leading to diagnosis of hereditary xerocytosis (HX). Family screenin

Prenatal mTOR-Inhibitor Therapy for Fetal Cardiac Rhabdomyomas: Indications, Treatment Duration and Perinatal Outcomes.

Background: Fetal cardiac rhabdomyomas are strongly associated with tuberous sclerosis complex (TSC). Although many remain asymptomatic and regress spontaneously, large tumors may cause ventricular inflow or outflow obstruction, arrhythmia, impaired ventricular function, pericardial effusion, hydrops fetalis and fetal demise. Transplacental mammalian target of rapamycin (mTOR) inhibition has emerged as a potential rescue therapy. Methods: We performed a narrative review of published reports on prenatal sirolimus or everolimus therapy for fetal cardiac rhabdomyomas in suspected or confirmed TSC

Genetic Testing Unveils a Novel Thrombospondin-1 Domain Containing Protein 1 Gene Variant as the Cause of Chronic Edema in a 79-Year-Old Woman.

Edema requires management tailored to its underlying etiology; however, in some cases the cause remains elusive. We describe a 79-year-old woman with lifelong unexplained peripheral edema. Comprehensive evaluation excluded common etiologies such as heart failure, renal dysfunction, and venous thrombosis. Whole-genome sequencing identified a novel homozygous splice-site variant (NM_018676.4:c.58+2T>G) in the thrombospondin-1 domain-containing protein 1 (THSD1) gene, which has previously been associated with non-immune hydrops fetalis (NIHF). This report describes, to our knowledge, the first el

Missed opportunities and near-miss diagnoses in congenital syphilis: A case series of five infants at a tertiary care centre in Nepal.

Congenital syphilis remains a preventable but underrecognized condition in Nepal, where routine antenatal screening is limited to the first visit. We report five infants with diverse clinical manifestations, including hydrops fetalis, bullous lesions, bone involvement, and persistent rhinitis. Four were classified as highly probable and one as possible congenital syphilis according to the 2021 CDC criteria. Four mothers had initially negative screening results, highlighting the possibility of infection acquired later in pregnancy. Due to penicillin unavailability, all infants received ceftriax

Fetal/Perinatal Lethal Gaucher Disease: A Case Series.

Gaucher disease is a rare lysosomal storage disease caused by mutations in the GBA gene, resulting in the accumulation of glucosylceramides in macrophages. The perinatal/lethal form, also including fetal Gaucher disease, is a distinct and severe variant, usually presenting with non-immune hydrops fetalis, hepatosplenomegaly, ichthyosiform changes, and typical "Gaucher cells" seen on histology in various organs. Four cases of fetal/perinatal Gaucher disease are presented, 1 genetically confirmed and 3 histologically diagnosed, with a description of the prenatal findings and the pathological fea

Maternal propranolol as a safe alternative to invasive fetal surgery in chorioangioma management.

Chorioangiomas are associated with a significant risk of fetal demise due to high-output cardiac failure. Standard invasive treatment, including fetoscopic laser photocoagulation and intratumoral embolization, shows limited success and has high fetal mortality rates. The biological parallels between chorioangiomas and infantile hemangiomas suggest that these tumors may share common therapeutic targets. Propranolol is a proven therapy for infantile hemangiomas, therefore this established efficacy provides a strong rationale for its consideration in the management of symptomatic or rapidly growi

Novel use of far UV-C light as a surface disinfectant in the poultry industry: trials on broiler farms show reduced bacterial burden in poultry anterooms and on wellington boots.

Osteoarthritis (OA) is condition which poses a main concern to the aging population and its severity is expected to increase with the increasing life expectancy. In the future, several possible targets for OA treatment need to be defined. Dickkopf‑related protein 3 (DKK3) is an atypical member of the Wnt‑antagonistic dickkopf‑related protein (DKK) family. The availability of research into the role of DKK3 in the abnormal remodeling of subchondral bone in human knee joints is currently limited. Thus, the aim of the present study was the evaluation of DKK3 expression in the abnormal bone remodel

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Human parvovirus B19 vertical infection and hydrops fetalis. A case report.

Non-immune hydrops fetalis represents a diagnostic challenge in high-risk pregnant women. Vertical infection with human parvovirus B19 (B19V) is a possible cause. National guidelines propose maternal serologic screening (IgG/IgM), which may be insufficient in some situations. We report a case of vertical B19V infection with difficulties in prenatal diagnosis. Preterm newborn, normal weight (2950 g), born to a 30-year-old mother with anemia and hydrops fetalis (week 17). Cardiac, chromosomal, isoimmunization-Rh, and usual infectious causes (TORCH) were ruled out. Maternal serology for B19V show
