Case Analysis of a Repeated Hydrops Fetalis Caused by Irregular Antibodies of Anti-E and Anti-c.

BACKGROUND: Blood group irregular antibody is a major factor that may cause adverse pregnancy outcomes. We aimed to emphasize the importance of prenatal blood antibody screening and popularize antenatal care knowledge by sharing a case of hydrops fetalis caused by anti-E and anti-c alloimmunization antibodies. METHODS: We retrospectively analyzed the clinical and laboratory data of a pregnant woman with repeated fetal edema caused by anti-E and anti-c irregular antibodies. Irregular antibody screening, antibody identification, and antibody titer detection were performed. RESULTS: A pregnant

Community Infections Linked with Parvovirus B19 Genomic DNA in Wastewater, Texas, USA, 2023-2024.

We assessed concentrations of parvovirus B19 DNA from 2 wastewater treatment plants in a Texas, USA, county with a known outbreak in 2024. Wastewater viral concentrations correlated significantly with clinical cases, demonstrating wastewater's potential for tracking parvovirus B19 infections. Peaks in wastewater concentrations were aligned with the peak in hydrops fetalis diagnoses.

THSD1 Is a Multifaceted Regulator in Health and Disease.

Thrombospondin Type 1 Domain-Containing Protein 1 (THSD1) is a transmembrane protein increasingly recognized for its critical roles in vascular biology and disease pathogenesis. Initially identified as a marker of hematopoietic stem and endothelial cells during embryogenesis, THSD1 has since been implicated in a wide spectrum of physiological and pathological processes. This paper consolidates current knowledge on THSD1, with a focus on its roles in vascular integrity, perinatal disorders, and tumorigenesis. In vascular systems, THSD1 promotes focal adhesion assembly and suppresses autophagy-m

Fetal left pulmonary artery-to-left atrial fistula with aplasia of the left lung: successful postnatal transcatheter closure.

INTRODUCTION: In the presence of fetal cardiomegaly, when there is no cardiac malformation or dysfunction, systemic or pulmonary arteriovenous malformations that may cause volume loading should be sought. We aimed to present a fetus who had cardiomegaly and left pulmonary artery-left atrial fistula and who underwent transcatheter closure in the early postnatal period. CASE PRESENTATION: 23-week fetus referred because of severe cardiomegaly on screening obstetric ultrasonography. Fetal echocardiography revealed fistulous connection between dilated left pulmonary artery and left atrium with hig

Diagnosis and Management of Prenatal Hereditary Pyropoikilocytosis.

Hereditary pyropoikilocytosis (HPP) is a severe hemolytic anemia caused by variants in SPTA1, SPTB, and EPB41. These weaken horizontal interactions in the erythrocyte cytoskeleton, causing membrane fragmentation and splenic sequestration. It will readily cause fetal anemia and often hydrops fetalis. Prenatal diagnosis requires first ruling out immune and other non-immune causes of fetal anemia. Diagnosis is made using characteristic blood smear morphology, ideally examined on a native blood sample, paired with exome sequencing. When these are inconclusive, laboratory tests such as ektacytometr

Maternal Sirolimus Treatment Reverses Cardiac Rhabdomyoma-Induced Hydrops Fetalis in a Twin Gestation With Tuberous Sclerosis Complex.

Cardiac rhabdomyomas are often the presenting sign of tuberous sclerosis complex (TSC). Prior reports have shown that maternal sirolimus treatment can reduce rhabdomyomas. We used maternal sirolimus to reverse hydrops fetalis due to a massive cardiac rhabdomyoma in a twin gestation. This expands the possibilities of prenatal medical treatment in TSC.

Risk of hypertensive disorders in pregnancies with non-immune hydrops fetalis and single fetal effusions.

INTRODUCTION: Hydrops fetalis carries high risks of morbidity and mortality for the fetus, as well as obstetric risks such as hypertensive disorders, or mirror syndrome, for the pregnant person. We aimed to characterize the prevalence and types of hypertensive disorders diagnosed in pregnancies with non-immune hydrops fetalis and single effusions, as well as to investigate fetal risk factors for the development of hypertension. METHODS: This was a secondary analysis of a prospectively enrolled cohort. We included pregnancies with non-immune hydrops fetalis that demonstrated ≥2 fetal effusions

Lysosomal storage disorders in nonimmune hydrops fetalis diagnosed by exome sequencing.

Lysosomal storage disorders (LSD) are a group of inherited metabolic diseases that contribute to nonimmune hydrops fetalis (NIHF). Our objective was to review the pooled exome sequencing (ES) diagnostic yield of LSD in NIHF cases. We expanded our previous meta-analysis and updated our search strategy of prenatal ES studies from 1/1/2000 to 8/1/2024. Cases with LSD gene variants were reviewed. Variants were curated based on the current American College of Medical Genetics and Genomics and ClinGen guidelines. Forty-one ES studies met our inclusion criteria. A total of 207/558 NIHF cases yielded

Hemolytic disease of the fetus and newborn: A review of pathophysiology, diagnosis, and management.

Hemolytic Disease of the Fetus and Newborn (HDFN) results from maternal alloantibodies attacking fetal red blood cells, leading to fetal anemia and potentially severe complications such as hydrops fetalis. Effective management relies on early detection through maternal antibody screening, fetal antigen testing, and close monitoring of fetal anemia. In cases of severe anemia, intrauterine transfusion (IUT) remains the primary intervention to improve fetal outcomes. Despite this, there is an urgent need for novel medical therapies to prevent or delay disease progression in pregnancies at risk fo

Case Report: Staged epicardial pacemaker implantation in a 740 g ELBW infant with 25 + 2 weeks of GA with congenital complete heart block.

BACKGROUND: Congenital complete atrioventricular (AV) block is a rare but potentially fatal condition in neonates, especially those with extremely low birth weight (ELBW). Management in this population is challenging due to technical limitations and high comorbidity risk. CASE PRESENTATION: We report the case of a female infant born at 25 + 2 weeks' gestation, weighing 740 g, with immune-mediated congenital complete AV block. Temporary epicardial pacing was initiated on day one due to persistent bradycardia and hemodynamic compromise. A dual-electrode strategy with alternating pacing sites ma

Dual pathology of parvovirus B19 infection and Aicardi-Goutières syndrome complicating non-immune hydrops fetalis.

Non-immune hydrops fetalis (NIHF) is a multifactorial condition characterised by abnormal fluid accumulation in fetal compartments. We report a complex case of NIHF in a primigravida in her 30s with dual pathology involving fetal parvovirus B19 infection and Aicardi-Goutières syndrome (AGS) due to a TREX1 gene mutation. The fetus exhibited severe anaemia, hydrops fetalis, ventriculomegaly and brain calcifications. Whole exome sequencing confirmed AGS as the genetic aetiology, while PCR on amniotic fluid identified parvovirus B19 as a coexisting infection. Although AGS alone has varied phenotyp

Non-Immune Hydrops Fetalis in a Pregnant Woman with Chronic Alcohol Use: A Case Report.

INTRODUCTION: Hydrops fetalis is a condition characterized by excessive fluid accumulation within the fetal extravascular compartments and body cavities. It is classified into two categories: immune hydrops fetalis (IHF) and non-immune hydrops fetalis (NIHF). The primary cause of NIHF is fetal cardiovascular abnormalities, often stemming from congenital heart disease. One of the significant contributors to congenital heart defects is prenatal alcohol exposure (PAE). CASE ILLUSTRATION: A woman in her early 30s presented to the emergency department with the chief complaint of not feeling fetal

The fetal hydrops-associated single-residue mutation L322P disrupts mechanical but not chemical activation of the PIEZO1 ion channel.

The mechanically activated PIEZO1 ion channel is genetically linked to numerous physiological and pathophysiological processes. For example, deleting PIEZO1 in mice leads to defective lymphatic vessel development, while nonsense mutations in humans are associated with autosomal recessive generalized lymphatic dysplasia (GLD) and nonimmune hydrops fetalis. However, it remains unclear whether PIEZO1-dependent biological processes are directly mediated by its intrinsic mechanosensitivity. Here, we identified a human fetal hydrops-associated single-residue mutation, L322P (corresponding to L329P i

Genetic etiologies associated with non-immune hydrops fetalis delineated by whole exome sequencing: A pilot series and its implications in prenatal genetic counseling.

AIM: Non-immune hydrops fetalis (NIHF) is a spectrum of disease involving heterogeneous etiologies. Prenatal application of whole exome sequencing (WES) has emerged as a valuable tool to decipher genetic causes in structural abnormalities detected by fetal ultrasound. Here, we reviewed index cases of NIHF whose genetic etiologies were delineated by WES. METHODS: We conducted a retrospective study on our pilot series (n = 12) of NIHF that underwent WES with outcomes systematically reviewed. For each case, we performed a detailed sonographic examination for anasarca, virology profile, karyotypi

Successful Perinatal Management of a Large Fetal Intrapericardial Teratoma by Serial Fetal Echocardiogram and ex utero Intrapartum Treatment.

INTRODUCTION: Pericardial teratomas are rare tumors located in the pericardial space compressing the heart from the outside and usually associated with pericardial effusions. When these tumors are diagnosed in utero, there are some management challenges, including the best approach of prenatal follow-up, the ideal timing of delivery, and the perinatal therapeutic strategies. The tumors, although benign, tend to grow considerably prenatally, imposing increased risk of worsening pericardial effusion, compression of the heart, and potentially compromising cardiac output, leading to intrauterine d
