Clinical profiling, antifungal drug susceptibility, and biofilm formation ability in pulmonary mucormycosis.

Pulmonary mucormycosis (PM), a life-threatening infection caused by Mucorales, exhibits high mortality. Comprehensive data integrating clinical profiling, antifungal susceptibility, and biofilm formation ability are limited. This retrospective study characterized 26 adult PM patients at a Beijing tertiary-care hospital. Data on demographics, underlying diseases, co-infections, and outcomes were collected. Clinical Mucorales isolates underwent molecular identification via 18 S rRNA gene sequencing and phylogenetic analysis. In vitro susceptibility against nine antifungals (amphotericin B, fluco

Comparative Prognosis of Fungal Balls Versus Invasive Pulmonary Mucormycosis.

BACKGROUND: Pulmonary mucormycosis is recognized as a highly fatal invasive fungal infection. However, clinical observations have demonstrated unexpectedly favorable outcomes in some patients. This study aimed to compare the clinical presentation, management, and prognosis between patients with fungal balls and invasive pulmonary mucormycosis. METHODS: This observational study included patients diagnosed with pulmonary mucormycosis at the Second Xiangya Hospital of Central South University between 2007 and 2023. Based on imaging characteristics, the patients were divided into invasive pulmona

Beyond bacteria and breaking the norm: Pulmonary mucormycosis due to Absidia corymbifera in a child with primary ciliary dyskinesia.

Primary ciliary dyskinesia is a rare autosomal recessive disorder that impairs mucociliary clearance and predisposes children to chronic respiratory infections. Invasive fungal infections caused by Lichtheimia (Absidia) corymbifera are typically confined to profoundly immunocompromised hosts and are exceptionally uncommon in patients without systemic immunosuppression. We report the case of a 6-year-old boy with genetically confirmed primary ciliary dyskinesia (C3orf67 mutation) and right-middle-lobe bronchiectasis who developed persistent cough and increased sputum production. High-resolution

Pulmonary mucormycosis in a young adult as a complication of a bronchopulmonary fistula.

Pulmonary mucormycosis (PM) is a life-threatening opportunistic infection caused by fungi of the Mucorales order, including genera such as Rhizopus, Absidia, and Mucor. This condition predominantly affects immunocompromised individuals, with mortality rates exceeding 50 % in high-risk populations. We present the case of a 33-year-old female with a medical history of type 1 diabetes mellitus (poorly controlled) and hypothyroidism, who presented to the hospital with a 7-day history of productive cough (mucoid expectoration), chills, and myalgia. An unenhanced chest computed tomography (CT) scan

Pulmonary Mucormycosis with Endobronchial Involvement Complicated by Pleural Effusion.

BACKGROUND: Pulmonary mucormycosis is an infection caused by a fungus with a severe and often fatal course, with a mortality rate exceeding 50%. It can spread through blood or lymphatic fluids or by inhalation of sporangiospores. Immunocompromised patients are especially susceptible to this high mortality rate. Histopathology is used to identify nonseptate hyphae in tissue, and culture is used to confirm the diagnosis. Treatment usually involves a combination of aggressive surgery and medication. CASE REPORT: A patient with diabetes mellitus received corticosteroid treatment for facial paraly

Impact of Underlying Malignancy Status on Clinical Characteristics and Mortality in Pulmonary Mucormycosis: A Retrospective Cohort Study.

BACKGROUND: Pulmonary mucormycosis is a life-threatening fungal infection that primarily affects immunocompromised individuals. Underlying malignancy is a recognized risk factor for pulmonary mucormycosis, yet its independent effect on patient outcomes remains uncertain. This study aimed to evaluate the impact of underlying malignancy on the clinical characteristics and 30-day mortality of pulmonary mucormycosis. METHODS: We conducted a retrospective cohort study of 163 adults with proven or probable pulmonary mucormycosis at a single center in Taiwan (2021-2024). Clinical and laboratory vari

A rare case of low-dose methotrexate toxicity leading to skin and mucosal toxicity and invasive pulmonary mucormycosis: a case report.

Methotrexate is a potentially toxic antifolate antineoplastic and immunosuppressive agent that is widely used in tumor chemotherapy and autoimmune diseases (such as psoriasis). It is the cornerstone therapy for immune-mediated disorders worldwide. However, low-dose methotrexate therapy for psoriasis rarely causes toxicity. Here, we report a case of a patient with psoriasis who was treated with low-dose methotrexate for the first time. The patient developed severe mucosal ulcers and myelosuppression, leading to impaired immunity and invasive pulmonary mucormycosis. After treatment, the patient

[Combination drug therapy with individualized bronchoscopic intervention for pulmonary mucormycosis].

This article reported a 46-year-old male patient with pulmonary mucormycosis and a history of poorly controlled diabetes. Chest imaging revealed rapidly progressive patchy opacities and multiple cavities in both lungs. Bronchoscopy showed mucor plug obstructing multiple segmental and subsegmental bronchi. The patient underwent sequential treatment, including local nebulization, oral/intravenous antifungals, and bronchoscopic removal of mucor plugs combined with intrabronchial instillation of amphotericin B deoxycholate. However, this yielded no significant improvement. During the procedure, ma

INTRODUCTION AND IMPORTANCE: Pulmonary mucormycosis (PM) is a severe fungal infection threatening immunocompromised individuals, especially with uncontrolled diabetes. Co-infection with bacteria like Achromobacter xylosoxidans complicates management. We report a rare and fatal case of PM co-infected with A. xylosoxidans, which was further complicated by a subsequent SARS-CoV-2 superinfection. CASE PRESENTATION: A 55-year-old female with poorly controlled type 2 diabetes mellitus and multiple comorbidities presented with worsening dyspnea. Investigations revealed severe hyperglycemia (927 mg/d

A Global Analysis of Cases of Mucormycosis Recorded in the European Confederation of Medical Mycology / International Society for Human and Animal Mycology (ECMM / ISHAM) Zygomyco.net Registry from 2009 to 2022.

We analyzed mucormycosis data from the Zygomyco.net registry (2009-2022), encompassing cases from 16 countries. India, Russia and the Czech Republic provided the largest contributions. India reported the highest case number, consistent with its substantially higher incidence compared to that of high-income countries. Among the 382 patients with mucormycosis, 236 (61.8%) were male (male-to-female ratio 1.6). The median age was 48 years [interquartile range (IQR) 32-60]. There were 59 pediatric patients (median age ranging from < 1 month to 19 years). Diabetes mellitus type 2 was the most common

Gastrointestinal and Intra-Abdominal Mucormycosis in Non-Haematological Patients-A Comprehensive Review.

Intra-abdominal and gastrointestinal mucormycosis are less frequent than rhino-orbito-cerebral and pulmonary mucormycosis, but highly lethal. Their diagnosis remains challenging due to the non-specific clinical presentation. We collected English-language cases of intra-abdominal and gastrointestinal mucormycosis in non-haematological and non-neonatal patients published up to October 2024. This review analysed the epidemiological, clinical, and therapeutic charts of 290 cases. A proportion of 53.4% were reported from India and the USA. The main predisposing conditions were diabetes, solid organ
