Predictive value of the 24-hour bilirubin in the development of significant hyperbilirubinemia among Filipino term (≥37 weeks gestational age) infants: a prospective multi-center study.

BACKGROUND: Hyperbilirubinemia is the most common neonatal morbidity. In the Philippines, neonates are at a higher risk for developing significant hyperbilirubinemia (SH) due to belonging to the East Asian race and high incidence of G6PD deficiency. The aim of this study is to determine an effective plasma bilirubin cut-off value at the 24th hour of life (HOL) in Filipino term infants, associated with the development of significant hyperbilirubinemia at the 72nd HOL. METHODS: A prospective multicentric cohort involving 21 medical centers nationwide recruited 2099 mother-infant dyads with 1688

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Cumulative Ambient Light Exposure Affects Outpatient Transcutaneous Bilirubinometer Readings.

BACKGROUND: We recently reported that the transcutaneous bilirubinometer (TCB) tends to underestimate the severity of neonatal jaundice (NJ). We hypothesize that the cumulative ambient light exposure contributes to the discrepancy. OBJECTIVES: This study aimed to identify factors that affect the TCB underestimation. METHODS: We analyzed prospectively collected data over a twenty-month period at a level III medical facility. Neonates at risk for NJ who couldn't secure an appointment with the primary practitioner were followed by the nursery team. Neonates who had phototherapy or forehead brui

Optimal site for applying transcutaneous bilirubinometer as an outpatient screening tool for neonatal jaundice: a comparison between the sternum and forehead.

BACKGROUND: The gold standard for assessing neonatal jaundice (NJ) is the serum total serum bilirubin (TSB) level by the diazo method. A transcutaneous bilirubinometer (TCB) provides a convenient, noninvasive readout within minutes. The reliability of TCB as the diagnostic tool and the proper site for TCB measurement remains unsettled. OBJECTIVES: This study aimed to (1) evaluate the reliability of TCB in the NJ outpatient management and (2) identify a better site to obtain TCB readings. METHODS: This retrospective study examines data collected prospectively over 15 months at a level III fac

High levels of pathological jaundice in the first 24 hours and neonatal hyperbilirubinaemia in an epidemiological cohort study on the Thailand-Myanmar border.

Population risks for neonatal hyperbilirubinaemia (NH) vary. Knowledge of local risks permits interventions that may reduce the proportion becoming severe. Between January 2015 and May 2016, in a resource-limited setting on the Thailand-Myanmar border, neonates from 28 weeks' gestation were enrolled into a prospective birth cohort. Each neonate had total serum bilirubin measurements: scheduled (24, 48, 72 and 144 hours of life) and clinically indicated; and weekly follow up until 1 month of age. Risk factors for developing NH were evaluated using Cox proportional hazard mixed model. Of 1710 ne

[Incidence and risk factors for severe hyperbilirubinemia in term neonates].

INTRODUCTION: Newborn jaundice is caused by increased levels of bilirubin in the blood of the newborn during the first days after birth. Generally, neonatal jaundice does not need to be treated, however, if the blood bilirubin concentration becomes too high, it can cause neurological damage. Due to the prevalence of neonatal jaundice, it is important to assess its risk factors and their importance. This study at the National University Hospital of Iceland from 1997-2018, determines the risk factors for severe neonatal jaundice and their significance. MATERIAL AND METHODS: This was a retrospec

A pilot study demonstrating the efficacy of transcutaneous bilirubin meters to quantitatively differentiate contusions from Congenital Dermal Melanocytosis.

OBJECTIVE: Congenital Dermal Melanocytosis (CDM) can be difficult to differentiate from contusions. The need for a prompt and accurate diagnosis is best illustrated in cases where child abuse and maltreatment is of concern. Transcutaneous bilirubin (TCB) spectrophotometry has been well established to measure bilirubin under the skin for jaundice in infants. The use of TCB spectrometry has not been used to identify or differentiate contusions from CDM. We hypothesized that bilirubin, a degradation product of hemoglobin, would be elevated in contusions but not in CDM thus demonstrating the effic

Adrenal Hemorrhage in Neonates: Unusual Presentation.

Adrenal hemorrhage (AH) is a relatively uncommon condition in newborns. It may be asymptomatic or may present with flank abdominal mass, anemia, jaundice, or rarely as scrotal bruising or hematoma. We report two cases of AH in neonates; the first presented with scrotal hematoma and the second with adrenal mass associated with hypertension and oliguria, primarily secondary to coincidental renal vein thrombosis. Diagnosis was confirmed by abdominal ultrasound. Patients were managed conservatively with clinical observation and by following hemoglobin and bilirubin levels closely. Both infants wer

Predischarge screening for severe neonatal hyperbilirubinemia identifies infants who need phototherapy.

OBJECTIVE: To test whether the combined use of total plasma/serum bilirubin (TSB) levels and clinical risk factors more accurately identifies infants who receive phototherapy than does the use of either method alone. STUDY DESIGN: We recruited healthy infants of ≥35 weeks' gestation at 6 centers that practiced universal predischarge TSB screening. Transcutaneous bilirubin (TcB) was measured at 24 hours, with TSB at 24-60 hours and at 3- to 5- and 7- to 14-day follow-up visits. Clinical risk factors were identified systematically. RESULTS: Of 1157 infants, 1060 (92%) completed follow-up, and

Identification of neonates at risk for hazardous hyperbilirubinemia: emerging clinical insights.

Hyperbilirubinemia is the most common condition requiring evaluation and treatment in neonates. Identifying among all newborns those few at risk to develop marked hyperbilirubinemia is a clinical challenge. Clinical, epidemiologic, and genetic risk factors associated with severe hyperbilirubinemia include late preterm gestational age, exclusive breastfeeding, glucose-6-phosphate dehydrogenase deficiency, ABO hemolytic disease, East Asian ethnicity, jaundice observed in the first 24 hours of life, cephalohematoma or significant bruising, and history of a previous sibling treated with photothera

Jaundice in the healthy newborn infant: a new approach to an old problem.

We measured the serum bilirubin concentrations in 2,416 consecutive infants admitted to our well baby nursery. The maximal serum bilirubin concentration exceeded 12.9 mg/dL (221 mumol/L) in 147 infants (6.1%), and these infants were compared with 147 randomly selected control infants with maximal serum bilirubin levels less than or equal to 12.9 mg/dL. A serum bilirubin concentration greater than 12.9 mg/dL was associated strongly with breast-feeding (P = .0000) and percentage of weight loss after birth (P = .0001), as well as with maternal diabetes, oriental race, decreased gestational age, m
