Is there a role for oral misoprostol in labor induction for prelabor rupture of membranes at term?

BACKGROUND: Most patients with pre-labor rupture of membranes (PROM) enter labor spontaneously at term. However, maternal and neonatal complications increase with prolonged PROM. Induction of labor (IOL) is therefore often recommended, but the optimal agent is still debated. Low-dose oral misoprostol (OM) is safe, effective, cheap, and easy to administer. However, there is little data regarding its role in PROM at term. OBJECTIVE: To determine the safety and efficacy of OM compared to Intravenous Oxytocin for IOL for PROM at term. STUDY DESIGN: A cohort study compared the perinatal outcomes

Perinatal Hemolytic Disease Due to Anti-Jkb: A Case Report.

This is a case report of a pregnant woman, with a history of cesarean section in a previous twin pregnancy, admitted due to premature rupture of membranes and investigation of anemia during her fifth pregnancy. During the cesarean section, she experienced significant hemorrhage, prompting a request for a red blood cell concentrate. During pre-transfusion testing, it was found that she was O positive, and an anti-erythrocyte antibody was identified, specifically anti-Jkb. It is noteworthy that despite her numerous previous pregnancies, there was no record of indirect antiglobulin testing (IAT)

Vitamin K deficiency bleeding and optimal prophylaxis methods in biliary atresia: A surveillance study in Japan.

BACKGROUND: Vitamin K (VK) prophylaxis refers to the administration of VK to newborns to prevent neonatal VK deficiency bleeding (VKDB), which is characterized by intracranial hemorrhage (ICH). This study investigated the relationship between VK prophylaxis methods and VKDB in biliary atresia (BA). METHODS: The survey targeted 497 cases in the Japanese Biliary Atresia Registry between 2015 and 2019, of which 395 (79.5%) returned the questionnaire. Of the 395 patients, 289 were selected after excluding cases in which the gestational age was <36 weeks or the VK prophylaxis methods/feeding conte

Socio-medical factors associated with neurodevelopmental disorders on the Kenyan coast.

Neurodevelopmental disorders (NDDs) are a group of conditions with their onset during the early developmental period and include conditions such as autism and intellectual disability. Occurrence of NDDs is thought to be determined by both genetic and environmental factors, but data on the role of environmental factors for NDD in Africa is limited. This study investigates environmental influences on NDDs in children from Kenya. This case-control study compared children with NDDs and typically developing children from two studies on the Kenyan coast. We included 172 study participants from the K

Progressive cephalohematoma in a neonate revealing severe hemophilia a owing to intron 22 inversion: a case report.

BACKGROUND: Hemophilia A is a rare X-linked recessive bleeding disorder characterized by coagulation factor VIII (FVIII) deficiency or dysfunction. While most cases present during early childhood with joint or soft tissue bleeding, neonatal-onset hemophilia A is uncommon and often difficult to diagnose owing to its nonspecific symptoms. Early recognition and a multidisciplinary management approach are critical for preventing life-threatening complications. CASE PRESENTATION: We report a case of a male neonate admitted on day 8 of life with progressive jaundice. Physical examination revealed a

Variants in AKR1D1 and Infant Mortality: Should Bile Acid Screening be a Routine Part of Newborn Screening?

Biallelic pathogenic variants in AKR1D1 cause Δ4-3-oxosteroid 5β-reductase deficiency, disrupt bile acid synthesis, and result in Congenital Bile Acid Synthesis defect type 2 (CBAS2). CBAS2 presents in infancy with cholestasis, coagulopathy, and failure to thrive. We report an infant who unexpectedly died at age 8 weeks with hepatic dysfunction and intracerebral hemorrhage. Characteristic of CBAS2, postmortem biochemical findings showed near-absent primary bile acids and accumulation of atypical bile acid intermediates. Subsequent genetic testing found compound heterozygous AKR1D1 variants: NM

Endoscopic retrograde cholangiopancreatography as a diagnostic key tool for biliary atresia: feasibility, safety and accuracy in neonates with cholestatic liver disease.

Biliary atresia (BA) is a rare condition in newborns characterised by inflammation and obliteration of bile ducts, leading to cirrhosis. Early diagnosis and treatment through Kasai portoenterostomy are crucial for native liver survival. While diagnostic methods include clinical assessment, laboratory tests and imaging, no non-invasive test can definitively rule out BA. Therefore, laparotomy with cholangiography remains the gold standard for diagnosis. This study assesses the feasibility, safety and accuracy of endoscopic retrograde cholangiopancreatography (ERCP) for diagnosing BA in newborns.

Complications in hospitalized neonates born to mothers with HDP subtypes: a retrospective cohort study and mediation analysis.

BACKGROUND: Hypertensive disorders in pregnancy (HDP), including four subtypes, are common complications significantly contributing to perinatal morbidity and mortality. Research on the impact of HDP subtypes-preeclampsia/eclampsia (PE/E) and superimposed preeclampsia (SP)-on comprehensive outcomes for hospitalized neonates is relatively limited. Additionally, PE/E and SP often result in shorter gestational age (GA), and the role of GA in the preterm birth-related complications of these HDP subtypes remains unclear. METHODS: This retrospective cohort study identified a total of 989 cases (exp
