Rhinocerebral mucormycosis and internal carotid artery thrombosis in a previously healthy patient.

Mucormycosis is a very serious fungal infection caused by habitual saprophytes of the human organism, in many cases concomitant with various pathological conditions marked by immunodepression. The rhinocerebral variant habitually accompanies ketoacidotic diabetes. We report a case of rhinocerebral mucormycosis in a subject free from immunodepression and non-diabetic, in apparently normal health. Following treatment with amphotericin B combined with 5-fluorocytosine and surgery, remission of the disease was finally secured and about one year after the last operation there are no sign of resumpt

Rhinocerebral mucormycosis: report of two cases.

Rhinocerebral mucormycosis developed in two poorly controlled diabetic patients with clinical manifestations of frontal headache, ophthalmoplegia, ptosis, proptosis, epistaxis and facial numbness. Early computed tomography (CT) of the head revealed fluid accumulation in paranasal sinuses. The diagnosis of this disease relied upon CT of the head, and biopsy or culture of the mucosa of sinuses. Remarkable improvement was noted following prompt surgical debridement and amphotericin-B therapy. We conclude that early diagnosis and aggressive treatment is the only way to save patient's life.

Rhinocerebral mucormycosis: management and survival after carotid occlusion.

Rhinocerebral mucormycosis is a rare but often fatal fungal infection. We present 2 patients with mucormycosis complicated by internal carotid artery thrombosis. Magnetic resonance imaging was superior to computed tomography in localizing the pathological process. Subtotal resection of devitalized tissue and intravenous amphotericin B therapy resulted in a successful outcome for both patients. Prompt recognition of this disorder by using modern diagnostic and therapeutic modalities promises to improve survival rates.

[Phinocerebral mucormycosis--report of a case].

Rhinocerebral mucormycosis is a rare and fatal deep fungus infection occurring in debilitated patients. Its reports have been recently increased because of the increase of such conditions as uncontrolled diabetics, leukemia, and cancer. A 60-year-old male suffering from continuous headache and fever was diagnosed as meningitis, and repeated culture of cerebrospinal fluid was negative. Antibiotic therapy was ineffective. CT scan revealed brain abscess in the right frontal lobe and bone defect over the right posterior ethmoidal sinuses. Both maxillary and ethmoidal sinuses were operated on 38 ye

[Rhinocerebral mucormycosis. Apropos of a case].

We present a 43-year-old patient with a history of immunodepression secondary to a renal transplant, chronic herpetic hepatitis and steroid diabetes, who showed sudden symptomatology suggestive of rhinocerebral mucormycosis, which was confirmed through cultures of the material obtained after a Caldwell-Luc operation of the affected sinus. Once the diagnosis was confirmed, a radical left superior maxillectomy was performed and medical treatment with progressive doses of amphotericin B was given. The patient improved markedly and left the hospital four months later.

[Rhinocerebral mucormycosis].

Mucormycosis is unusual in ORL pathology. We present a case of a 53-year-old patient in the dialysis program for the past 6 years for renal insufficiency. Starting with some trouble in the left lacrimal region, he was diagnosed by his ophthalmologist as having deep dacryocystitis, appearing as an ulcero-necrotic lesion on the same side of the nasal dorsum. In the presumption of clinical mucormycosis, a microbiological and histological study was done to confirm the diagnosis, starting treatment with amphotericin B. Because of the rapid extension of the lesion, surgical debridement of the zone w

[A case report of rhinocerebral mucormycosis in hemodialysis patient receiving deferoxamine].

Deferoxamine (DFO) has been widely used in the treatment of aluminum toxicity in patients on chronic dialysis. Mucormycosis is an opportunistic infection caused by fungi of the Mucorales order and some reports suggested a role for DFO in the precipitation of this infection. A 50-year-old man had been on hemodialysis for 16 years. 6 weeks before admission, he was begun on DFO because of aluminum toxicity. 2 weeks before admission, general fatigue and fever developed and followed by headache and loss of vision. He was admitted to this hospital with disturbed consciousness. His clinical course an

Fatal rhinocerebral mucormycosis and diabetic ketoacidosis.

The typical features of a fulminating rhinocerebral infection by the fungus Rhizopus in a patient with diabetic ketoacidosis are described. The clinical presentation was characterized by an infection of the face, followed by rapid bilateral visual loss and massive cerebral infarction due to extensive vascular involvement. Early diagnosis is a prerequisite for successful treatment, which consists of control of the underlying disease, surgical debridement and systemic antifungal therapy.

Mucormycosis of the head and neck.

Rhinocerebral mucormycosis is an uncommon but devastating infection of the head and neck. The fungus most commonly affects diabetics who are in ketoacidosis, but can affect other immunosuppressed individuals such as leukemics, burn patients, and patients with chronic renal failure. The pathogenicity of the organism is characterized by a ketone reductase system and its ability to invade arterial blood vessels. Treatment is directed towards correction of the underlying disorder, surgical debridement, intravenous antifungal agents, and possibly a new adjuvant, hyperbaric oxygen. A detailed review

MR findings in rhinocerebral mucormycosis.

A case of rhinocerebral mucormycosis evaluated by magnetic resonance (MR) imaging is presented, with inflammatory changes in the paranasal sinuses and orbit. Extension of the infection into the cavernous sinus, with cavernous sinus thrombosis and internal carotid artery narrowing, was well demonstrated on pre- and postgadolinium MR images. Infarctions in the anterior choroidal artery distribution suggested intracranial invasion of basal arteries, and watershed distribution infarctions attested to the tenuousness of flow through a narrowed cavernous carotid artery. Magnetic resonance effectivel

Orofacial mucormycosis.

Two cases of orofacial mucormycosis are reported. The first patient represents the typical progression of rhinocerebral mucormycosis with infiltration of the sinuses, the orbit, and the brain. The second patient had a tumorous maxillary lesion resembling fibrous dysplasia clinically and radiographically. Microscopically, the fungal infection in case 2 was associated with new bone formation. The significance of this phenomenon is discussed, and the importance of early diagnosis and possible treatment regimens are highlighted.

Rhinocerebral mucormycosis: a case report with magnetic resonance imaging findings.

Mucormycosis is a rare fungal disease most often encountered in patients who are diabetic, immunocompromised, severely burned, or who have a history of intravenous drug abuse. Intracranial presentation includes the isolated cerebral and the more common rhinocerebral forms. The computed tomography picture in mucormycosis is well documented. The magnetic resonance (MR) findings associated with mucormycosis have not been previously described. We report a case of rhinocerebral mucormycosis in a poorly controlled diabetic man where the MR findings of cavernous sinus thrombosis and thrombosis of the

Cure of mucormycosis in a renal transplant patient receiving ciclosporin with maintenance of immunosuppression.

Rhinocerebral mucormycosis in renal transplant recipients is associated with high mortality and allograft rejection. We report the first case of successful treatment of mucormycosis in a renal transplant recipient receiving ciclosporin and corticosteroids with maintenance of renal function. It is postulated that the relatively specific immunosuppression caused by ciclosporin, together with aggressive medical and surgical intervention, may enable the cure of this life-threatening fungal infection without loss of donor kidney function.

Rhinocerebral mucormycosis. Presentation of two cases and review of the literature.

Mucormycosis is a serious infection with a fulminating course caused by opportunistic fungi. The territory most frequently affected is the maxillofacial region. The disease develops in patients with severe metabolic and immunological disorders. We present two cases that illustrate its severity and the need for early diagnosis and aggressive treatment.

Rhinocerebral mucormycosis in an anephric patient.

We have described a fatal case of rhinocerebral mucormycosis in a stable anephric patient who was not diabetic and was not receiving immunosuppressive therapy. In a literature search from 1966 to the present, ours is only the second case of a stable dialysis patient reported with this illness, and it is the only reported fatality.

Adjunctive hyperbaric oxygen for treatment of rhinocerebral mucormycosis.

The therapy of rhinocerebral mucormycosis includes aggressive surgical debridement, administration of high-dose amphotericin B, and control of underlying predisposing conditions, especially diabetes and immunosuppression or immunodeficiency. Hyperbaric oxygen suppresses fungal growth in vitro and has theoretical value in treating mucormycosis because it reduces the tissue hypoxia and acidosis that accompany vascular invasion by the fungus. In a retrospective review of patients at Duke University Medical Center with rhinocerebral mucormycosis, six patients were treated with hyperbaric oxygen an

Rhinocerebral mucormycosis with cerebral extension successfully treated with adjunctive hyperbaric oxygen therapy.

Rhinocerebral mucormycosis is a devastating fungal disease with a high mortality rate. Extensive surgical débridement and amphotericin B are currently the mainstays of therapy. When cerebral extension of the fungus occurs, the disease is almost invariably fatal. Two patients with rhinocerebral mucormycosis had progression of their infection to brain abscesses despite aggressive débridement surgery and amphotericin B therapy. Both patients showed marked clinical improvement with the addition of adjunctive hyperbaric oxygen therapy. Both patients remained free of their disease 21 months after ho
