Phototherapy-Associated Changes in Ionized Calcium Among Neonates Born at ≥35 Weeks of Gestation.

Background  Phototherapy is the standard treatment for clinically significant unconjugated neonatal hyperbilirubinemia. Although generally safe and effective, phototherapy has been associated with alterations in calcium homeostasis. The magnitude and clinical relevance of phototherapy-associated reductions in ionized calcium (iCa) remain uncertain, particularly among neonates born at ≥35 weeks of gestation. Objective  To evaluate changes in iCa before and after phototherapy in neonates born at ≥35 weeks with unconjugated hyperbilirubinemia. Methods  This prospective cohort study included 145 n

Successful treatment with BrECADD in a patient with Dubin-Johnson syndrome and classic Hodgkin lymphoma: a case report.

Dubin-Johnson syndrome (DJS) results from ABCC2/MRP2 mutations causing impaired biliary transport. MRP2 also mediates hepatic elimination of chemotherapeutic agents, yet no data exist on anthracycline-based chemotherapy in adult DJS patients. We report a 26-year-old male with advanced-stage classic Hodgkin lymphoma and DJS harboring a novel homozygous ABCC2 variant (c.2291G>A, p.Gly764Asp). After a pembrolizumab prephase, full-dose BrECADD was administered, and samples were taken for therapeutic drug monitoring (TDM), which demonstrated normal etoposide and doxorubicin pharmacokinetics. Congenital Bile Acid Synthesis Defect Type 2 Mimicking Biliary Atresia: A Case Report of Delta-4-3-Oxosteroid 5-Beta-Reductase Deficiency in a Two-Month-Old Infant.

Neonatal cholestasis presents a complex diagnostic challenge, frequently necessitating the differentiation of numerous etiologies, including the anatomically correctable biliary atresia (BA), from various metabolic and genetic disorders. The timely and accurate identification of inborn errors of bile acid metabolism is particularly critical, as these conditions can lead to progressive liver damage if left untreated. Congenital bile acid synthesis defects (CBASDs) are rare autosomal recessive inborn errors of metabolism that can manifest as neonatal cholestasis. Their clinical and biochemical p

Care seeking behaviour and expressed experiences of mother and caregivers regarding newborn danger sign.

INTRODUCTION: Early identification and management of newborn danger signs (NDS) are vital to reduce neonatal morbidity and mortality. Caregivers' awareness, experiences, and care-seeking behaviours influence outcomes. OBJECTIVES: To explore care-seeking behaviours and expressed experiences of mothers and caregivers regarding newborn danger signs experienced during the neonatal period by their infants. METHODS: A descriptive exploratory study (Dec 2024-Jan 2025) at two private hospitals and data collected through in-depth interviews with 14 mothers and 10 caregivers of infants <6 months who h

Antenatal Intravenous Iron Therapy in Anemic Pregnancies: Associations with Obstetric Patient Blood Management and Neonatal Outcomes.

Background: Anemia in pregnancy is associated with increased maternal and neonatal morbidity, most commonly due to iron deficiency. The effect of antenatal IV iron therapy on obstetric patient blood management (PBM) and neonatal outcomes remains uncertain. Objective: To evaluate whether antenatal IV iron therapy in pregnancy is associated with improved obstetric PBM, specifically reduced RBC transfusion and postpartum IV iron use, and improved neonatal outcomes. Methods: In this multicenter retrospective study, 851 pregnant women with IDA were included (490 received antenatal IV iron; 361 cont

Effects of formal aquatic activities on health and developmental outcomes in infants and toddlers younger than 36 months: a systematic review and meta-analysis.

AIMS: To systematically review and meta-analyze controlled evidence on the effects of formal aquatic activities on health and developmental outcomes in infants and toddlers younger than 36 months. BACKGROUND: Formal aquatic activities-including infant swimming, neonatal swimming, hydrotherapy, and warm-water bathing-are increasingly used in early-life care, yet existing studies vary in populations, intervention formats, and outcome measures, making overall effects difficult to determine. A structured synthesis is needed to clarify their potential role in early health and development. METHODS

Gestational Age-Specific Agreement Between TcB Measured at the Sternum or Upper Back and Serum Bilirubin.

BACKGROUND: Transcutaneous bilirubin (TcB) measurement is widely used for noninvasive jaundice screening, but evidence regarding its correlation and agreement with serum bilirubin across gestational ages (GAs) remains limited, particularly in preterm newborns. At our institution, TcB screening is routinely performed at the sternum or upper back. This study evaluated the relationship between TcB measured at these sites and total serum bilirubin (TsB) across GAs. METHODS: This retrospective study included newborns born at 28-41 weeks' gestation and admitted to a tertiary pediatric referral cent

Hemolytic Jaundice and Anemia with Methemoglobinemia in Dizygotic Newborn Twins with Hb Evans: Case Report.

INTRODUCTION: It is highly unusual for a newborn infant to present in the first days after birth with cyanosis secondary to methemoglobinemia and with hemolytic anemia and jaundice. This combination suggests the possibility of an unstable hemoglobin variant. CASE PRESENTATION: We describe dizygotic twins, both with this presentation, where whole genome sequencing identified hemoglobin Evans (a point missense substitution in HBA2, the alpha2-globin gene) and found no other co-inherited variants known to be associated with neonatal methemoglobinemia, jaundice, or anemia. Herein we report on fou

Global, Regional and National Burden of Hemolytic Disease and Other Neonatal Jaundice, 1980/90-2021, and Projections to 2050: A Global Burden of Disease Study 2021.

BACKGROUND AND AIMS: Hemolytic disease of the fetus and newborn and other forms of neonatal jaundice remain important causes of morbidity and mortality among children under five worldwide. Despite improvements in prevention and treatment such as Rh screening, phototherapy, and exchange transfusion, the burden remains uneven across regions. This study assessed global, regional, and national trends in incidence, prevalence, mortality, Disability-Adjusted Life Years (DALYs), Years of Life Lost (YLLs), and Years Lived with Disability (YLDs) from 1980/1990 to 2021 using Global Burden of Disease (GB

Intravenous immunoglobulin as an adjunct to phototherapy in neonatal haemolytic disease: A systematic review and meta-analysis.

Phototherapy remains the first-line treatment of haemolytic disease of the newborn (HDN), a potentially life-threatening condition caused by maternal alloimmunisation. Intravenous immunoglobulin (IVIG) has been proposed as an adjunct to reduce the need for exchange transfusion (ET), a procedure associated with significant risks. This meta-analysis aims to evaluate the safety and efficacy of IVIG combined with phototherapy versus phototherapy alone in neonates with HDN. A comprehensive search of MEDLINE, EMBASE, CINAHL and the Cochrane Library was conducted up to 24 April 2025. Eligible studies

Dynamic landmark models predicting native liver survival after kasai portoenterostomy: a bicentric study.

OBJECTIVES: Native liver survival (NLS) after Kasai portoenterostomy (KPE) for biliary atresia (BA) is highly heterogeneous, and clinically practical approaches that update prognostic estimates across prespecified postoperative follow-up time points remain limited. METHODS: In this retrospective bicentric cohort study (training, n = 168; validation, n = 80), Cox proportional hazards landmark models were developed at five prespecified time points (preoperative and 2 weeks, 1, 3, and 6 months post-KPE) using a LASSO/Boruta dual-algorithm intersection approach with variance inflation factor (VIF

Global, regional, national trends and economic evaluation of neonatal hyperbilirubinemia burden, 2003-2023.

Severe neonatal hyperbilirubinemia remains a preventable cause of neonatal death and lifelong neurologic disability, yet contemporary global estimates are limited. Using Global Burden of Disease 2023 data for 204 countries, we analyzed incidence, deaths, and disability-adjusted life-years (DALYs) from extreme hyperbilirubinemia (EHB) among neonates from 2003 to 2023, applying joinpoint regression, income-stratified k-means clustering and correlation analysis, and value-per-statistical-life (VSL) and human-capital economic valuation. Globally, incident cases fell 29% (542,966-387,366) and death
