The influence of early intravenous immunoglobulin administration on the prognosis of severe hemolytic disorders in neonates.

BACKGROUND: Severe hemolytic disorders in neonates can lead to anemia, jaundice, and potentially severe neurological impairment or death. Early intravenous immunoglobulin (IVIG) administration has gained attention for its potential to mitigate hemolysis and its complications. But the optimal timing of IVIG remains a topic of debate. Therefore, this study aims to investigate the influence of early IVIG administration on the prognosis of severe hemolytic disorders in neonates. METHODS: A retrospective analysis of clinical data from 58 cases of neonatal severe hemolytic disease admitted to hospit

Safety and toxicity assessment of Lactobacillus salivarius AP-32 with Probiotic potential in vivo and in vitro.

Lactobacillus salivarius AP-32 (namely Ligilactobacillus salivarius AP-32, L. salivarius AP-32), isolated from the human gut, has shown a favorable safety profile. In 14-day acute and 90-day subchronic toxicity studies in rodents, no adverse effects were observed. The maximum tolerated dose was 16 g/kg body weight in the acute study, and the no-observed-adverse-effect level in the 90-day study was 1.88 g/kg body weight. It showed no mutagenicity in vivo or in vitro, including no chromosomal aberrations in mouse spermatogonia, no bone marrow cytotoxicity, and no mutagenic response in the Ames t

Noninvasive Assessment of Bilirubin Levels in Newborn Infants With Dark Skin Pigmentation.

AIM: Visual assessment of neonatal jaundice, especially in infants with dark skin pigmentation, is imprecise, and better tools are needed in low-resource settings. We aimed to assess the accuracy of the Picterus smartphone app in estimating total serum bilirubin (TSB) in darkly pigmented newborn infants, compared with a transcutaneous bilirubin (TcB) device. METHOD: A prospective study was conducted in Botswana, where term newborn infants aged 1-14 days were recruited. Bilirubin levels were measured using the Picterus app and a TcB device and compared with laboratory-measured TSB values. RES

American Academy of Pediatrics 2022 Phototherapy Thresholds Reduce the Hospitalizations and the Associated Costs.

INTRODUCTION: The latest American Academy of Pediatrics guidelines for managing jaundice in late preterm and term neonates have increased the bilirubin thresholds to start phototherapy. This was considered safe based on expert consensus, but its cost-effectiveness has not yet been evaluated. METHODS: This was a before-and-after, quality improvement and pharmaco-economical evaluation of hospital financial data from a perinatal academic referral centre. RESULTS: Implementing the new guidelines decreased hospitalizations due to phototherapy by 68.7%, 70.2%, and 60% for the total population and

Investigation of a PID-Based Dynamic Illuminance Control System for Intelligent Neonatal Jaundice Phototherapy Using a Blue Light LED Array.

Newborns are unable to reliably express changes in their physical condition due to their physiological immaturity and limited capacity for communication; therefore, continuous and systematic monitoring during phototherapy is essential to ensure timely detection of adverse responses and maintenance of therapeutic safety. This study extends our prior work, which introduced an indirect method for measuring light intensity to improve precision in monitoring newborn skin illumination. Light-emitting diode (LED) phototherapy has attracted considerable attention as an effective treatment for neonatal

Association between polycystic ovary syndrome and pregnancy outcomes in GDM: A secondary analysis of the DiGest trial.

CONTEXT: Polycystic ovary syndrome (PCOS) is a risk factor for gestational diabetes mellitus (GDM) due to shared pathophysiological associations with insulin resistance and adiposity, and may increase the risk of suboptimal perinatal outcomes in GDM. It is unclear if this excess risk upon outcomes is attributable to PCOS, more severe hyperglycemia, higher maternal BMI or reduced efficacy of dietary interventions. OBJECTIVE: To assess associations of PCOS, dietary intervention and gestational weight loss with maternal and neonatal outcomes in women with GDM. METHODS/DESIGN: A secondary analys

Bilirubin-microbiota interaction: molecular mechanisms and therapeutic strategies in neonatal jaundice.

Recent studies have revealed a complex interplay between bilirubin metabolism and the gut microbiota. Bilirubin functions as a potent antioxidant and signaling molecule in humans, and its concentration-dependent effects on distinct microbial taxa indicate that the compound exerts selective pressure on the gut ecosystem. The gut microbiota modulates bilirubin metabolism by altering intestinal pH, producing and activating Bilirubin metabolic enzyme, and bile acids. Because perturbations in bilirubin handling are especially common-and potentially neurotoxic-in neonates, a concise synthesis of rec

Assessment of oxidative stress parameters in neonatal indirect hyperbilirubinemia treated with phototherapy.

Phototherapy is the most commonly used treatment for indirect (unconjugated) hyperbilirubinemia (IHB) in newborns. This study aims to determine the impact of IHB and phototherapy on oxidative stress balance by thiol-disulfide homeostasis and ischemia-modified albümin (IMA). This prospective study included 107 hospitalised newborns with IHB who received phototherapy and 55 controls. Neonates aged 0-28 days with a gestational age above 35 weeks were included in the patient group. Oxidative stress parameters were assessed in the control group as well as before and after phototherapy in the patien

A novel pathogenic variant for dubin-johnson syndrome in a case of hyperbilirubinemia and metabolic associated fatty liver disease.

BACKGROUND: Dubin-Johnson Syndrome (DJS; OMIM 237500) is a rare autosomal recessive disorder caused by pathogenic variants in the ABCC2 gene, encoding for the multidrug resistance protein 2 (MRP2), resulting in impeded biliary excretion of bilirubin metabolites. It is typically characterized by chronic or intermittent jaundice and conjugated hyperbilirubinemia. CASE PRESENTATION: We report the case of a 54-year-old male with hyperbilirubinemia (mostly conjugated) and hypertransaminasemia. Hypertransaminasemia was due to presence of Metabolic dysfunction-associated fatty liver disease (MASLD),

Isolated GGT elevation in prolonged neonatal jaundice: detection of heterozygous alpha-1 antitrypsin deficiency.

Prolonged neonatal jaundice is usually benign but can occasionally indicate serious hepatobiliary disease. Current UK guidelines emphasise bilirubin-based screening, which may overlook atypical biochemical profiles. A term neonate presented with persistent jaundice but was otherwise well, with normal stools and urine. Bilirubin levels were normal, yet gamma-glutamyl transferase (GGT) was markedly elevated at 1101 U/L. Further testing revealed reduced alpha-1 antitrypsin (A1AT) levels and PiMZ heterozygosity. Imaging showed no structural abnormality. Over follow-up, transaminases-alanine aminot

Evaluation of the Safety of Taking Brexpiprazole During the Lactation Period.

INTRODUCTION: Brexpiprazole (BPZ), an antipsychotic drug introduced in 2018, is often used to manage psychiatric conditions. However, the effect of its use on infants whose mothers are receiving BPZ during lactation remains uncertain. Given the limited data on its effects on infants, this study evaluated the safety of its use by mothers during lactation. MATERIALS AND METHODS: Three mother-infant pairs were assessed between 2018 and 2023 at Tohoku University Hospital. Each mother continued BPZ monotherapy (1-2 mg/day) during pregnancy and lactation within the first month postpartum, and data

Home-Based Transcutaneous Bilirubin Screening and Telemedicine Reduce Neonatal Referrals.

INTRODUCTION: Neonatal jaundice is a leading cause of early post-discharge referrals. Community follow-up commonly relies on visual assessment and clinic-based evaluation, generating avoidable visits. Scalable home pathways that maintain safety are needed. We evaluated a nurse-led home pathway that integrates transcutaneous bilirubin (TcB) screening with targeted pediatric teleconsultation. METHODS: A prospective before-after study was conducted within routine nurse-led home visits for eligible infants (firstborn or preterm) ≥35 weeks of gestation. A 3-month pre-intervention phase (usual visu
