Point-of-Care Bilirubin Testing in Neonates: Comparative Performance of Blood Gas Analysis and Transcutaneous Bilirubinometry.

Background: Neonatal jaundice is a common condition with potentially severe complications such as bilirubin-induced neurological dysfunction and kernicterus. While serum bilirubin (SBR) remains the standard laboratory measurement, point-of-care methods, such as transcutaneous bilirubinometry (TcB) and blood gas analysers (BGAs), offer rapid, less invasive alternatives. Direct comparisons of their diagnostic accuracy remain limited. Objective: The aim of this study was to assess and compare diagnostic accuracy and clinical utility of TcB and BGA against SBR in neonatal hyperbilirubinaemia scree

CBC ratios for early sepsis risk stratification in newborns: EOS/LOS versus non-septic controls.

BACKGROUND: Despite advances in neonatal intensive care units (NICUs), neonatal sepsis remains a leading cause of morbidity and mortality worldwide. Early and accurate diagnosis is challenging because clinical signs are often nonspecific and no single biomarker has shown perfect sensitivity and specificity. Recently, complete blood count–based parameters such as the neutrophil-to-lymphocyte ratio (NLR) and platelet-to-lymphocyte ratio (PLR) have gained attention as simple and low-cost markers of systemic inflammatory response. This study was designed to assess the diagnostic utility of NLR and

Neonatal jaundice detection using a vision transformer-based deep learning model.

Neonatal jaundice is a prevalent and potentially serious condition that can lead to severe complications if undiagnosed or untreated. While traditional diagnostic methods like blood sampling are invasive and time-consuming, and transcutaneous bilirubinometers remain costly, smartphone-based image analysis offers a promising low-cost, non-invasive alternative. However, most existing solutions rely on traditional machine learning techniques with limited accuracy and generalizability. In this study, we introduce a deep learning approach based on the Vision Transformer (T2T-ViT) and compare its pe

Evolution and contemporary landscape of neonatal hyperbilirubinemia management guidelines: a narrative review.

Neonatal hyperbilirubinemia, affecting over 60% of term and 80% of preterm infants globally, remains a significant public health challenge due to the persistent risk of kernicterus despite effective management strategies. This narrative review summarizes global clinical practice guidelines to describe the evolution and current landscape of management, highlighting the disparity between evidence-based consensus and local adaptation. While universal principles such as early hyperbilirubinemia screening, phototherapy as the first-line treatment, and post-discharge follow-up are widely endorsed, s

Accidental Administration of Anti-D Immunoglobulin to an Rh(D)-Positive Neonate: A Case Report.

Anti-D immunoglobulin (RhIg) is administered to Rh(D)-negative mothers to prevent alloimmunization in Rh(D)-incompatible pregnancies, and is contraindicated in Rh(D)-positive individuals. Herein, we report a case of accidental intramuscular administration of RhIg to an Rh(D)-positive neonate. A female infant was born at 39 weeks of gestation with a birth weight of 2,432 g. Shortly after birth, one vial of RhIg (250 μg), which should have been administered to the mother, was mistakenly injected into the neonate. The infant was transferred to our neonatal intensive care unit at one day of age fo

Early gestational diabetes mellitus and type 2 diabetes: do they differ in perinatal outcome- a retrospective analysis.

BACKGROUND: The rise in type 2 diabetes (T2D) during pregnancy is mainly due to increasing obesity and metabolic disorders. Early gestational diabetes mellitus (eGDM), diagnosed before 20 weeks, is also becoming more common and often may explain underlying metabolic problems. This study compares perinatal and neonatal outcomes in patients with T2D and eGDM. METHODS: This retrospective study included 141 pregnant women (70 with T2D and 71 with eGDM) and their singleton term neonates born between 2020 and 2025. The women attended at least one follow-up visit per trimester. eGDM was diagnosed bef

Oral adjuvants supplemented to phototherapy as a therapeutic strategy for neonatal hyperbilirubinemia: a systematic review.

INTRODUCTION: Neonatal jaundice is a common condition characterized by elevated serum bilirubin levels. Although usually benign, inadequate management may result in severe complications such as bilirubin encephalopathy and kernicterus. Phototherapy is the standard treatment; however, concerns regarding potential short- and long-term adverse effects have motivated the investigation of oral adjuvants as complementary therapies to accelerate bilirubin clearance and reduce phototherapy exposure. CONTENT: This systematic review followed PRISMA guidelines and was registered in PROSPERO (CRD42025107

Neonatal jaundice and the infant gut microbiome: an integrated shotgun metagenomics and bidirectional Mendelian randomization study in Xinjiang.

BACKGROUND: Neonatal jaundice is a common condition, yet inter-individual variation in its onset and severity cannot be fully explained by traditional clinical risk factors. Emerging evidence suggests that the infant gut microbiome may modulate bilirubin metabolism, but its compositional and functional signatures in jaundiced neonates remain incompletely defined. This study aimed to characterize the taxonomic and functional features of the gut microbiome in neonatal pathologic jaundice and to explore potential causal links using Mendelian randomization (MR). METHODS: We conducted a case-contr

Multi-wavelength optical sensor for jaundice detection via absorbance difference spectroscopy.

We demonstrate a low-cost optical sensor for neonatal jaundice detection using 470 nm (bilirubin-sensitive) and 560 nm (hemoglobin isosbestic) LEDs. The system applies absorbance difference spectroscopy to isolate bilirubin signals in skin-mimicking phantoms, achieving 387mV/mg⋅dL-1 sensitivity and 13.5% error versus spectrophotometry. Optical characterization confirms 470 nm penetration depth (≈0.65mm) in neonatal dermis. A machine learning classifier predicts risk severity with 99.5% accuracy. Comparative analysis shows a superior cost-performance trade-off versus reflectance-based devices.
