Gastrointestinal mucormycosis: a success story and appraisal of concepts.

Mucormycosis is an opportunistic, life-threatening fungal infection caused by fungi of the class Zygomycetes. The disease has traditionally been reported in immunocompromised patients, premature infants, diabetics, transplant recipients, prolonged use of corticosteroids or in condition associated with increased availability of serum iron such as acidosis or deferoxamine administration. The infection is progressive and associated with a high mortality unless treatment is initiated promptly. The number of cases of gastrointestinal mucormycosis indexed on PubMed over the past 2 decades has shown

Gastrointestinal Mucormycosis following a Streptococcus pyogenes Toxic Shock Syndrome in a Previously Healthy Patient: A Case Report.

Mucormycosis is an uncommon opportunistic infection and the gastrointestinal form is the rarest. Rhizopus sp. is the most frequent pathogen and infection occurs almost exclusively in immunocompromised patients. We describe the first case of intestinal mucormycosis occurring after a Streptococcus pyogenes toxic shock syndrome in a previously healthy patient caused by Rhizopus microsporus var. azygosporus.

Intraabdominal mass as presentation of colonic mucormycosis in a child with acute lymphoblastic leukemia.

Gastrointestinal mucormycosis with clinical presentation as an abdominal mass has been rarely reported in the current literature. In this observation, We report an unusual case of a child with acute lymphoblastic leukemia and neutropenic fever. During hospitalization, increasing abdominal pain and distension occurred, and initially treatments with broad-spectrum antibiotics were administered. However, the symptoms/signs progressed with the development of peritonitis and a palpated mass over the right lower quarter of the abdomen. Imaging studies showed ascending colon perforation with abscess

Neonatal lingual and gastrointestinal mucormycosis in a case of low anorectal malformation-a rare presentation.

We report a 1.7 kg male infant with a low anorectal malformation treated at an outside facility and referred to us on postoperative day 11. At presentation, his upper abdomen was distended, and he had perianal mucoid discharge. The tongue had a blackish discoloration. An erect abdominal radiograph showed a few fluid-filled bowel loops in the upper abdomen with a gasless lower abdomen and pelvis, suggestive of upper small bowel obstruction. There were no specific radiological features of necrotizing enterocolitis. He underwent laparotomy and bowel resection for perforated jejunum. Histopatholog

Neonatal gastrointestinal mucormycosis in an asphyxiated premature newborn.

Mucormycosis of the intestine is a rare fungal infection of childhood and is mostly encountered in neonates. It is a potentially lethal opportunistic fungal infection with rapid progression and high mortality in immunocompromised patients. The number of reported cases with intestinal mucormycosis is 19 to date. We herein report an asphyxiated preterm infant with intestinal mucormycosis who was presented with an atypical necrotizing enterocolitis (NEC), with findings similar to an intraabdominal mass. The diagnosis was made in the postmortem examination of the surgically removed bowel segment.

Ileocolonic mucormycosis in adult immunocompromised patients: a surgeon's perspective.

We report three cases of ileocolic mucormycosis in adult immunocompromised patients presenting as acute abdomen. All patients underwent laparotomy but two of them died from multiorgan failure before the diagnoses were confirmed. The diagnosis of gastrointestinal mucormycosis is rarely suspected, and antemortem diagnosis is made in only 25%-50% of cases. These cases illustrate the difficulty encountered by surgeons in managing acute abdomen in neutropenic patients with hematological malignancy. The management of colonic mucormycosis in the published literature is also reviewed.

Elderly diabetic patient with surgical site mucormycosis extending to bowel.

Mucormycosis is rare in clinical practice. Most infections are acquired by inhalation; other portals of entry are traumatic implantation and ingestion in immunocompromised host. Mucormycosis is life threatening infection in immunocompromised host with variable moratlity ranging from 15-81% depending upon site of infection. General treatment principles include early diagnosis, correction of underlying immunosuppression and metabolic disturbances, adequate surgical debridement along with amphotericin therapy. We describe surgical site mucormycosis extended to involve large bowel in elderly diabe

Successful nonoperative management of gastrointestinal mucormycosis: novel therapy for invasive disease.

BACKGROUND: Invasive opportunistic fungal infections caused by agents of Mucorales that involve the gastrointestinal tract are uncommon but aggressive. These ubiquitous fungal spores typically are inhaled and can germinate in immunocompromised hosts. Standard therapy for invasive mucormycosis includes parenteral amphotericin B (AMB) in combination with radical debridement of infected tissues. Early diagnosis and treatment are of paramount importance. Unfortunately, long-term survival is poor owing to the prohibitive morbidity and mortality rates associated with the medical and surgical therapi

Primary gastrointestinal mucormycosis in an immunocompetent person.

In the past decade, mucormycosis has emerged as an important lethal infection in diabetics and other immunocompromised hosts. Rhinosinusitis, pansinusitis, rhino-orbital and rhinocerebral are the common classical manifestations of mucormycosis. However, primary gastrointestinal (GI) mucormycosis is an uncommon disease associated with a high mortality rate. Stomach is the most common site involved in GI mucormycosis. Reported cases of GI mucormycosis in an immunocompetent host are very few in the literature. Here we present a case of a young male with fungal sepsis secondary to GI mucormycosis

[Ileal mucormycosis diagnosed by colonoscopy in a patient with acute myeloid leukemia].

Gastrointestinal mucormycosis is an uncommon opportunistic fungal infection often presents in immunocompromised patients. Direct invasion of the intestinal walls by spores from ingested food is the main pathogenetic mechanism of this disease, which usually takes place in stomach and colon. Early diagnosis is critical, especially in vascular invasive types, due to its high mortality rate close to 100%. In the past when appropriate diagnostic tools were not available, mucormycosis were frequently found with autopsy. The advance in current endoscopic technology has increased diagnostic rate and m

Emphysematous gastritis associated with invasive gastric mucormycosis: a case report.

Emphysematous gastritis is a rare form of phlegmonous gastritis, characterized by air in the wall of the stomach due to invasion by gas-forming microorganisms. The most commonly involved microorganisms are streptococci, Escherichia coli, Pseudomonas aeruginosa, Clostrodium perfrigens and Staphylococcus aureus. Gastrointestinal mucormycosis is another rare condition, which is most frequently occurs in the stomach. Because emphysematous gastritis associated with invasive gastric mucormycosis is an extremely rare clinical condition and both are life-threatening diseases, early precise diagnosis a

Neonatal gastrointestinal mucormycosis--a case report.

Mucormycosis is a rare opportunistic fungal infection. It occurs mainly in immunocompromised patients and is usually fatal in children. This fungus can affect central nervous system, nasopharynx, lungs, skin and gastrointestinal tract. It invades the blood vessels and causes thrombosis, leading to infarction of the surrounding tissues. The diagnosis depends mainly on histopathological examination and demonstration of the fungus in the tissue sections, therefore a high index of suspicion and early exploration is required. We report a case of malnourished child who presented with perforation per
