Management of pregnancy in patients with Graves' disease secondary to alemtuzumab: a case series and literature review.

INTRODUCTION: Alemtuzumab treatment for multiple sclerosis frequently induces Graves' disease (GD), characterized by common coexistence of stimulating and blocking anti-thyrotropin receptor autoantibodies (TRAb), causing fluctuations of thyroid function. Alemtuzumab-induced TRAb-positive hypothyroidism is also common. These conditions are challenging to manage during pregnancy, considering the imperative necessity to maintain euthyroidism to minimize risks for both pregnancy and fetal/neonatal outcomes. METHODS: We reviewed the clinical course and therapeutic management of four pregnant women

[Acid-base and potassium balance disorders in neonatal calf diarrhea - Part 2: Therapy and prognosis].

ABSTRACT: Neonatal diarrhea in calves frequently results in severe metabolic disturbances, particularly D-lactic acidosis and hyperkalemia. These conditions are associated with weakness, apathy, and, in the case of D-lactic acidosis, with neurological deficits such as impairment of the palpebral reflex. Severe hyperkalemia can be accompanied by potentially life-threatening cardiac conduction abnormalities. The second part of this review article provides a narrative overview of therapeutic approaches as well as the prognostic relevance of these key metabolic disorders. ABSTRACT: Correction of

Outcomes and Patterns Related to Magnesium in Acute Heart Failure: A Population-Based Study.

BACKGROUND: The significance of magnesium as a treatment or prognostic factor in heart failure (HF) remains uncertain despite frequent use. We evaluated the frequency and outcomes of magnesium testing, hypomagnesemia, and intravenous (IV) replacement in a large population-based cohort. METHODS: This retrospective cohort study used linked administrative data (April 2012 to March 2020). Patients with a primary diagnosis of HF in the emergency department (ED) or hospital were included. Outcomes included all-cause and cause-specific death and hospitalisation. Secondary outcomes included ED visits

Exploring the Effect of Nutrition on Fetal Development in Pregnant Patients With Inflammatory Gut Diseases: A Scoping Review.

Inflammatory gut diseases (IGDs) are increasing in prevalence and pose unique challenges during pregnancy. Poorly controlled IGDs in pregnant women are linked to adverse fetal outcomes such as low birth weight, preterm birth, and impaired gut microbiome development. Nutritional deficiencies, particularly in magnesium, folate, and vitamin B12, exacerbate these risks due to both inadequate intake and malabsorption. This scoping review aimed to evaluate the role of nutrition in fetal development among pregnant women with IGDs, highlighting dietary strategies that may enhance maternal gastrointest

Fasting-induced hypoglycaemia in anaesthetised paediatric patients (FIGS): protocol of an opt-out approach to consent for a multicentre cohort study measuring glycaemia and ketonaemia.

BACKGROUND: Hypoglycaemia is a potentially a serious complication of the iatrogenic fasting required to deliver safe anaesthesia. Children may be at a higher risk because of their metabolic immaturity. METHODS: This is a prospective multi-centre cohort study recruiting children fasting before a procedure under general anaesthesia. Children will have their blood glucose and ketones measured using point-of-care testing on venous blood drawn from a cannula inserted during i.v. or inhalation induction of general anaesthesia. The primary aim is to recruit as many children as possible across the fo

Familial hypomagnesemia with secondary hypocalcemia: a novel TRPM6 mutation associated with treatment-resistant migraine in a North African family.

BACKGROUND: Familial hypomagnesemia with secondary hypocalcemia (HSH) is a rare autosomal recessive disorder caused by TRPM6 loss-of-function variants, leading to impaired magnesium absorption and systemic deficiency. Neurological manifestations are typically dominated by seizures and neuromuscular hyperexcitability, whereas headache manifestations have been less characterized. METHODS: We performed a clinical, biochemical, and genetic characterization of a consanguineous Moroccan family with hypomagnesemia. The proband underwent neurological assessment, neuroimaging, and electrophysiological

Clinical and Molecular Characterization of Children with Neonatal Diabetes: Experience from Bangladesh.

Neonatal diabetes mellitus (NDM) is a rare monogenic form of diabetes presenting before six months of age which can be permanent (PNDM) or transient (TNDM). In rare cases, NDM can be diagnosed between 6 and 12 months of age. Whilst TNDM is commonly associated with chromosome 6q24 methylation defects, the commonest causes of PNDM are KCNJ11 and ABCC8 gene mutations. This observational study's aim was to assess the clinical features and genetic aetiologies in patients with NDM diagnosed in Bangladesh. The clinical characteristics of 28 patients who presented with NDM from 2001 to 2018 in the in

Stroke mimic caused by severe hypomagnesemia: a case report and literature review.

Association of hypomagnesemia and neurological deficits is a well documented issue, but despite its importance, magnesium levels are rarely tested during the acute phase of a stroke. Focal neurological deficits have rarely been ascribed to hypomagnesemia. We report the case of a patient treated with Proton Pump Inhibitors and Metformin with a history of unexplained transient neurological deficits who developed stroke-like symptoms of hemiparesis and aphasia with recovery after correction of his hypomagnesemia. We also present a brief literature review regarding Magnesium's role in focal neurol

Dietary nucleotide supplementation attenuates disease severity and modulates clinicopathological responses in lambs with neonatal diarrhea syndrome.

Neonatal diarrhea syndrome (NDS) is a major cause of morbidity and mortality in lambs during early life. Dietary nucleotides modulate the immune response and enhance tissue development and cellular proliferation. This study evaluated the effects of a nucleotide-rich yeast extract on the clinical course of NDS and associated hematological, biochemical, and oxidative stress responses under field conditions. Ninety-six newborn lambs were allocated to a nucleotide-supplemented group (NG, n = 48) or a control group (CG, n = 48). NG lambs received 1g/day of a nucleotide-rich yeast extract (Nucleofor

Ethnic-Racial Identity and Alcohol Use Moderated by Family Factors among Diverse Emerging Adults.

Defining biomarkers that predict therapeutic effects and adverse events is a crucial mandate to guide patient selection for personalized cancer treatments. In the present study, we applied a pharmacometabolomics approach to identify biomarkers potentially associated with pathological complete response to trastuzumab-paclitaxel neoadjuvant therapy in HER-2 positive breast cancer patients. Based on histological response the 34 patients enrolled in the study were subdivided into two groups: good responders (n = 15) and poor responders (n = 19). The pre-treatment serum targeted metabolomics profil

Diverging trajectories of job demands, job resources, and psychological distress before long-term mental health-related sick leave among teachers.

STUDY AIMS: The prevalence of Long COVID in children and adolescents is heterogeneous, ranging from 1% to 51%, depending on the population studied. The lack of a standardised approach for establishing a Long COVID diagnosis in children and adolescents complicates the accurate assessment of prevalence, risk factors and outcomes. The present study aimed to examine the value of standardised interviews and an adjudication process to better understand self- or proxy-reported symptoms lasting longer than 12 weeks compatible with Long COVID in children and adolescents during the COVID-19 pandemic. Mg-Y-Nd alloy biocorrosion behavior in hyperlipidemia models in vitro and in vivo.

The permanent nature of bare metal and drug eluting stents can lead to serious long-term complications such as neoatherosclerosis and late stent thrombosis. Magnesium (Mg) based bioabsorbable metal stents, with the ability to provide temporary support to stenosed arteries and harmlessly degrade, are in position to be the 4th revolution of interventional cardiology. Mg materials are known to be sensitive to biological factors, however this has been understudied with respect to hyperlipidemia. In this study, two distinct WE-series (Mg-Y-Nd) alloy wires (WE43 and WE22) were implanted into the abd

Altered Magnesium Environments Restrict Colorectal HT-29 Spheroid Growth by Disturbing Cellular Mg2+ Homeostasis.

Dysregulated magnesium (Mg2+) homeostasis contributes to colorectal cancer (CRC), yet its context-dependent function within the tumor microenvironment remains unresolved. This study aimed to determine how sustained low and high extracellular Mg2+ environments affect CRC spheroid (SP) growth and Mg2+ homeostasis using HT-29 SPs. We analyzed Mg2+ flux, the expression of Mg2+ transporters (e.g., Transient Receptor Potential Melastatin (TRPM) 6), viability, apoptotic and autophagic markers, and phospho-/oxidoproteomic alterations. Both Mg2+ extremes destabilized SP architecture, reduced viability,
