Evaluating pregnancy and neonatal outcomes in mothers with genetic disease using electronic health care records.

PURPOSE: The effect of Mendelian disorders on pregnancy and neonatal outcomes is poorly understood because of their rarity and the challenge of compiling complete prenatal and postnatal records. METHODS: Using electronic health records from a single academic center, we developed a retrospective cohort of maternal-infant dyads. Cases were mothers with molecularly confirmed Mendelian disorders paired with live-born infants; controls had no documented genetic disease. Outcomes were evaluated overall, by organ system, and by individual disorder. RESULTS: The cohort included 58,912 dyads, 241 wit

Impact of β-Blockers on the Risk of Low-Birth-Weight Infants in Women with Long QT Syndrome or Marfan Syndrome: A Single-Center Retrospective Study from 2008 to 2022 in a Tertiary Care Center.

INTRODUCTION: β-Blockers are an essential treatment for cardiovascular disease, the incidence of which is rising among women of childbearing age. Several large cohort and registry studies have reported an association between β-blocker exposure in pregnancy and reduced birth weight, but most have included women with hypertension or structural heart disease. Our study's primary objective was to evaluate the impact of these medications on the birth weight of infants born to mothers with morphologically normal hearts, including women with long QT syndrome (LQTS) or Marfan syndrome (MFS). Secondary

Exclusive breastfeeding among women with polycystic ovary syndrome versus women from a population-based cohort: a cohort study.

BACKGROUND: Women with polycystic ovary syndrome (PCOS) face breastfeeding challenges. We aimed to compare exclusive breastfeeding rates in women with PCOS to those in a population-based cohort, and to explore the effect of metformin and metabolic factors on exclusive breastfeeding in women with PCOS. METHODS: This cohort study included 653 women from the pilot, PregMet, and PregMet2 randomized trials on metformin in pregnant women with PCOS (2000–2017), and 63,927 women from the Norwegian Mother, Father and Child Cohort Study (MoBa) (1999–2008) as the reference group. Dual Metabolic Burden of Polyendocrine Metabolic Ovarian Syndrome (PMOS) and Gestational Diabetes in Pregnancy: Impact on Neonatal Anthropometry: Insights From the Born in Bradford Cohort.

BACKGROUND: Polyendocrine metabolic ovarian syndrome (PMOS) and gestational diabetes mellitus (GDM) independently influence foetal growth, but their combined impact remains poorly characterised. This study examined the individual and joint associations of PMOS and GDM with neonatal anthropometry in a large, multi-ethnic population. METHODS: We performed a secondary analysis of 9454 singleton pregnancies from the Born in Bradford (BiB) cohort (UK, 2007-2011). Women were classified into four groups: (1) normoglycemic non-PMOS controls, (2) PMOS only, (3) GDM only and (4) PMOS + GDM. GDM was def

Hyperglycemia during the first 1000 days as a driver of metabolic programming.

The first 1000 days of life, spanning from conception to the end of the second postnatal year, represent a critical developmental window during which environmental and metabolic exposures exert long-lasting effects on offspring health. Among these exposures, maternal and early-life hyperglycemia have emerged as major determinants of metabolic programming and future cardiometabolic disease risk. Increasing evidence suggests that hyperglycemic exposure during this vulnerable period induces complex alterations in placental function, fetal endocrine adaptation, epigenetic regulation, and postnatal

Management of Cushing's Disease in Pregnancy: A Case Report.

Cushing's syndrome (CS) is a rare condition encountered in pregnancy, and Cushing's disease (pituitary-dependent CS) represents an even smaller subset. The occurrence of pregnancy in women with CS is uncommon because hypercortisolism suppresses the hypothalamic-pituitary-gonadal axis, frequently leading to anovulation and infertility. When conception does occur, Cushing's disease can significantly complicate the course of pregnancy, posing serious risks to both the mother and fetus. Reported maternal complications include hypertension, gestational diabetes mellitus, pre-eclampsia, heart failur

[Expression of Concern] Anti‑metastatic and differential effects on protein expression of epigallocatechin‑3‑gallate in HCCLM6 hepatocellular carcinoma cells.

INTRODUCTION: Available work from North America indicates that same-sex attracted (SSA) individuals enjoy aspects of rural life but nonetheless report encountering homophobia and experiencing isolation from SSA networks. The experience of prejudice and social isolation are often associated with psycho-social distress among the general population of same-sex attracted individuals. Little is known of how SSA women experience life in rural areas of Australia and how this influences their psycho-social wellbeing. METHODS: This was a small-scale qualitative study using guided interviews to explore

Pioneering study in the role of Ribonuclease L in the development of gestational diabetes mellitus in pregnant women.

BACKGROUND: Gestational diabetes mellitus as a common pregnancy complication carries metabolic risks for both the mother and infant. We and others have shown that ribonuclease L (RNase-L, an innate immunity regulator) regulates adipogenesis and insulin sensitivity of skeletal muscle. We further showed that its serum level was reduced in human subjects with impaired fasting glucose and metabolic syndrome. In this study we investigated its levels in gestational diabetes mellitus. METHODS: A total of 82 pregnant women who have received a one-step 75g oral glucose tolerance test (OGTT) for Gestat

Management of pregnancy with metformin: Case reports for genetic insulin resistance syndrome and a literature review for gestational and type 2 diabetes.

INTRODUCTION: Genetic insulin resistance syndrome is characterized by severe insulin resistance due to functional abnormalities of the insulin receptor or downstream signaling molecules. Treatment typically includes insulin formulations and oral hypoglycemic agents such as metformin and sodium-glucose cotransporter 2 (SGLT2) inhibitors. However, achieving adequate glycemic control remains challenging. Moreover, given its rarity, reports on perinatal glycemic management in affected pregnancies remain scarce. MATERIALS AND METHODS: We describe prenatal care for two cases of genetic insulin resi

Pregnancy complications affect kynurenine pathway metabolite concentrations in umbilical cord blood.

BACKGROUND: Tryptophan and its kynurenine pathway (KP) metabolites play key roles in modulating the immune system and vasculature, and exhibit both pro- and antioxidant properties, making them crucial for a healthy pregnancy and fetal development. Disruptions in the KP may impact both prenatal and postnatal health, however, data on fetal KP metabolite concentrations and their alterations in pregnancy-related disorders remain scarce. This study aims to investigate the association between pregnancy complications and KP metabolite concentrations in umbilical cord blood. METHODS: Pregnancies comp
