Long-term course of neonatal diabetes.

BACKGROUND: Neonatal diabetes mellitus--defined here as hyperglycemia occurring within the first month of life that lasts for at least two weeks and requires insulin therapy--is a very rare form of the disease. Little is known about it, particularly with respect to its long-term course. METHODS: We studied two brothers who had neonatal diabetes and obtained follow-up information on 34 patients described in the literature as well as information on 21 additional patients. Forty-seven of the patients had neonatal diabetes, as defined above, and in 10 others the onset was between the first and th

Two interesting cases of transient neonatal diabetes mellitus.

Two cases of transient neonatal diabetes mellitus associated with anemia, macroglossia and umbilical hernia were studied in relation to the possible etiologies that have been postulated to be responsible for this syndrome. Both patients required insulin therapy for the control of their hyperglycemia but case number two needed to be treated for 14 months before glucose normalization occurred. This patient developed classical insulin dependent diabetes mellitus during our follow-up; the HLA typing showed DR4 allele.

Transient neonatal diabetes mellitus: a relapse after 10 years of complete remission.

The follow-up study of a patient with permanent diabetes (DM) unrelated to islet cell antibody and anti-insulin antibodies and occurring 10 years after a transient neonatal DM episode is reported. A latent defect in insulin release was proved. It produced transient glucosuria during intercurrent illnesses and permanent hyperglycaemia and a decrease in growth velocity [with high stimulated growth hormone and low insulin-like growth factor I (IGF-I)] at the onset of puberty. After insulin therapy was re-introduced, catch-up growth and normalization of the blood glucose and IGF-I levels were note

[Neonatal hyperglycemia and diabetes mellitus].

BACKGROUND: Hyperglycemia in the neonate may be permanent or more frequently transient. Its treatment is still debated. PATIENTS AND METHODS: The files of 19 infants with hyperglycemia were retrospectively analysed. Their birth-weights were more than 900 g and their gestational ages more than 27 weeks. Plasma glucose, insulin, C-peptide, islet-cell and insulin autoantibodies were measured in each patient. RESULTS: Blood glucose ranged from 1.2 to 10 g/l (mean: 5.05 +/- 1.0), between the second hour and the 60th day of life. Four infants presented with permanent neonatal diabetes mellitus: th

[Neonatal diabetes mellitus and microcephaly. Indications for autosomal recessive inheritance].

Still, there are a lot of questions about the pathogenesis of neonatal diabetes mellitus. In the author's opinion neonatal diabetes mellitus is a distinct entity which differs from the well-known types of diabetes in children (type 1 diabetes, MODY-diabetes) and transient neonatal hyperglycemia regarding pathogenesis, pathophysiology and prognosis. Casuistics of three children two of whom were sibs are reported in detail to demonstrate the characteristics of neonatal diabetes mellitus. Regarding the reported sibs we suppose genetic origin of the disease. Autosomal-recessive mode of inheritance

Transient neonatal diabetes mellitus in a pair of twins.

The occurrence of transient neonatal diabetes mellitus in male twins with almost identical courses of illness is reported. A trial with chlorpropamide treatment of twin A had no obvious influence on the insulin consumption or on duration of treatment. Very low values of plasma C-peptide and serum proinsulin with no detectable insulin antibodies supports the theory of delayed maturation of the beta-cell.

Neonatal diabetes mellitus in first cousins.

The cases of two first cousins with neonatal diabetes mellitus are presented in this report. In one child the condition was transient, whereas the other has permanent diabetes mellitus. It seems unlikely that the difference in outcome was related to the degree of diabetic control early in the course of the disorder. The possibility of genetic influence in the etiology of neonatal diabetes is stressed. Chlorpropamide was not useful in curing neonatal diabetes mellitus in one case.

Neonatal diabetes mellitus: evaluation of pancreatic beta-cell function in two cases.

Two cases of neonatal diabetes mellitus, a transient form and a permanent form, are described. Comparing their clinical presentations and courses, we exclude the possibility of an early differential diagnosis based on clinical or laboratory data. We hypothesize that only repeated dynamic evaluations of pancreatic beta-cell function could be useful to differentiate the two forms.

C-peptide levels in transient neonatal diabetes.

A baby boy with transient neonatal diabetes mellitus presenting with hyperglycaemia, glycosuria, and dehydration without ketonuria on the second day of life is reported. C-peptide levels were measured to aid in the assessment of insulin treatment. Very low levels were found for the first 5 months of life (less than 0.06 nmol/l). Thereafter insulin treatment was discontinued and the baby thrived showing normal growth and development at age 2 1/2 years.

Transient neonatal diabetes mellitus in half sisters. A sequel.

This is a sequel to a previous report of two half sisters with transient neonatal diabetes mellitus. In 1970, the same syndrome developed in a third half sibling, a boy, born of the same father by his third wife. It followed essentially the same neonatal clinical course as that of his two half sisters. The firstborn sibling, aged 19 years, is reported to be free of signs and symptoms of diabetes; the third born was clinically well with no glycosuria at 9 years 3 months of age, and was reported to be in good health at 11 years of age. In 1978, at age 15 years diabetic ketoacidosis and coma deve

Plasma somatomedins, endogenous insulin secretion, and growth in transient neonatal diabetes mellitus.

Infants with transient neonatal diabetes mellitus are small for gestational age and fail to thrive postnatally unless insulin is administered. We have measured the concentrations of insulin-related growth factors in an infant girl with this condition to learn if deficiencies in one or more of these factors could be responsible for the impaired growth. Cord blood serum radioimmunoassayable insulin and somatomedin C/insulin-like growth factor I (SMC/IGF-I) were low, but insulin-like growth factor II (IGF-II) measured by a specific radioreceptor assay was normal. Insulin therapy begun on the four

Transient neonatal diabetes mellitus. Treatment with chlorpropamide.

Nonketotic diabetes mellitus developed in a "small-for-dates" baby at the age of 6 days. The disease was controlled initially with insulin but beginning at the age of 40 days, chlorpropamide was substituted gradually for insulin over a 24-day period. Treatment was stopped at the age of 13 weeks and the baby remained well thereafter. It is suggested that chlorpropamide might be useful in the treatment of transient neonatal diabetes.

Macroglossia, transient neonatal diabetes mellitus and intrauterine growth failure: a new distinct entity?

A newborn infant, small for her gestational age with macroglossia and transient insulinopenic diabetes mellitus is described. Two similar cases have been found in the literature. Flat glucose tolerance test results were found in the mother, the mechanism of which was not disclosed; there was no evidence of hyperinsulinism or malabsorption syndrome and the response of plasma growth hormone, and cortisol, and of urinary epinephrine to insulin-induced hypoglycemia was adequate. It is suggested that the triad of intrauterine growth retardation, macroglossia, and transient neonatal diabetes mellitu

Permanent neonatal diabetes mellitus: a case report with plasma insulin studies.

The 11th case of permanent neonatal diabetes mellitus appearing during the first month of life is reported. A critical review of the literature is also presented. The permanence of diabetes is demonstrated by the duration of insulin therapy still necessary after 30 months. Insulin-stimulation tests have been performed some for the first time in such a young diabetic. They have shown a nearly total failure in beta-cell response, only very high doses of glucagon provoking a moderate insulin secretion. The absence of acetonuria is discussed. It can perhaps be explained by the hyperglycemia which,
