The Spectrum of Congenital Hypogonadotropic Hypogonadism: A 30-Year Experience at a Tertiary Paediatric Centre.

Congenital hypogonadotropic hypogonadism (CHH) is a rare group of disorders of gonadotropin deficiency, either isolated or as a part of multiple pituitary hormone deficiencies (MPHD). We aimed to describe the spectrum of presentation, diagnosis, and management practices of CHH spanning 30 years at an Australian tertiary paediatric centre. This is a retrospective cohort study of patients with CHH seen at the Children's Hospital at Westmead between January 1994 and December 2024 (n = 96, 66% male), categorised by diagnostic subtypes: Kallmann Syndrome (KS), normosmic HH, MPHD and other syndromes

Relationship between continuous maternal intrapartum and postpartum glucose monitoring metrics and neonatal outcomes and postpartum glucose metabolism in individuals with gestational diabetes.

BACKGROUND: Continuous glucose monitoring (CGM) improves maternal and neonatal outcomes in pregnancies complicated by preconception diabetes. CGM measurements may help optimize intrapartum and postpartum glycemic management in pregnant women with GDM. We investigated the use of blinded CGM-derived metrics during labor, delivery, and postpartum supplementary to normally monitored glucose measures in women with GDM to study neonatal outcomes and postpartum glucose metabolism. METHODS: This prospective observational study evaluated 60 pregnant women with gestational diabetes mellitus (GDM) during

Dietary index for gut microbiota and odds of gestational diabetes mellitus: associations with inflammatory and metabolic biomarkers.

BACKGROUND: Gestational diabetes mellitus (GDM) is a common metabolic disorder of pregnancy associated with adverse maternal and neonatal outcomes. Emerging evidence suggests that dietary patterns supportive of gut microbial health and associated systemic inflammation may contribute to the pathophysiology of GDM. This study evaluated the association between a dietary index oriented toward gut microbiota (DI-GM)-comprising foods previously linked to beneficial gut microbial composition-and GDM odds, metabolic profiles, inflammatory markers, and pregnancy outcomes. METHODS: This case-control st

Early-Remitting Neonatal Diabetes in a Growth-Restricted Preterm Infant Requiring Insulin: A Case Report.

Neonatal diabetes mellitus is a rare cause of persistent hyperglycemia in early infancy and is usually monogenic. We describe a male preterm neonate born at 34 weeks' gestation with intrauterine growth restriction who developed severe hyperglycemia from the first days of life. Plasma glucose reached a maximum of 570 mg/dL with persistent glycosuria, negative urine ketones, and a normal acid-base profile. C-peptide was profoundly suppressed (0.01 ng/mL), supporting marked endogenous insulin deficiency. Following optimization of glucose administration, intravenous short-acting regular human insu

Perinatal and Neonatal Outcomes in Infants of Diabetic Mothers: A Prospective Descriptive Study in a Tertiary Care Center in South India.

Background Maternal diabetes is a well-recognized contributor to neonatal morbidity. Infants born to mothers with gestational diabetes mellitus (GDM) and pregestational diabetes are predisposed to metabolic, respiratory, and hematological disturbances. Understanding the distribution of these outcomes is important for optimizing neonatal care. Objectives To describe neonatal outcomes, including metabolic, respiratory, hematological, and anthropometric parameters, along with perinatal factors and maternal demographic and clinical characteristics among infants of diabetic mothers in a tertiary ca

Pregnancy and glycemic outcomes with hybrid closed-loop insulin delivery versus sensor-augmented pump therapy among women with type 1 diabetes.

OBJECTIVE: Pregnant women with type 1 diabetes (T1D) increasingly utilize hybrid closed-loop (HCL) pumps, but pregnancy-specific algorithms in many countries including the United States are not available. We compared pregnancy and glycemic outcomes between those intending to use off-label automated HCL versus manual sensor-augmented pump therapy (SAPT). METHODS: We conducted a single-center retrospective study from 2022 to 2025. The exposure was intent to use HCL using non-pregnancy algorithms versus SAPT (reference) at the start of pregnancy. Outcomes included pregnancy outcomes (cesarean de

Persistent Neonatal Hypocalcemia as an Early Clue to TCIRG1-Related Infantile Osteopetrosis in an Infant of a Diabetic Mother: A Case Report.

Infantile malignant osteopetrosis is a rare autosomal recessive skeletal disorder caused by impaired osteoclast-mediated bone resorption, resulting in diffuse osteosclerosis, bone marrow failure, neurologic complications, and metabolic disturbances. Early diagnosis remains challenging, particularly when initial manifestations overlap with common neonatal conditions. Persistent neonatal hypocalcemia may be an early but underrecognized clue. We report a Saudi male infant with genetically confirmed TCIRG1-associated osteopetrosis who presented with persistent neonatal hypocalcemia and secondary h

Laparoscopic Pancreatectomy for Persistent Hyperinsulinemic Hypoglycemia of Infancy - Case Series.

CONTEXT: Hypoglycemia in neonates can be attributed to Persistent Hyperinsulinemic Hypoglycemia of Infancy (PHHI). Even though initial management involves the use of medical agents, requirement of surgery is seen in a large number of cases, particularly in those who are nonresponsive. Open pancreatectomy has been the gold standard in the management of PHHI, but there has been a recent inclination toward the use of minimally invasive procedures, namely laparoscopic pancreatectomy to achieve similar results with lesser morbidity. AIMS: The aim of this study is to describe the presentation, key

Hypoglycemia secondary to type B insulin resistance syndrome.

Type B insulin resistance syndrome (TBIRS) is rare, with an unknown prevalence, characterized by antibodies directed against the insulin receptor. TBIRS may present with hypoglycemia and/or hyperglycemia. A case of hypoglycemia secondary to TBIRS is reported. A 24-year-old woman presented with hypoglycemia associated with the adrenergic and neuroglycopenic symptoms. Clinical examination was normal. After workup, including a 72-hour fast, glucagon test, autoimmune screen, computerized tomography of the abdomen and calcium stimulation test, no cause was apparent. The results of the 72-hour fast

The influence of paternal preconception health on infant birth weight: A scoping review.

Birth weight is an important determinant of infant growth and development associated with neonatal morbidity (e.g., respiratory distress, hypoglycemia) and mortality, as well as long-term health risks such as developmental delays and chronic conditions (e.g., asthma, type 2 diabetes) in later life. These adverse health outcomes are particularly concerning when infants are born small or large for their gestational age. A potential strategy to improve optimal birth weight is preconception care, with consistent evidence demonstrating a relationship between maternal preconception health and infant

Antenatal corticosteroid administration-to-delivery intervals and neonatal outcomes in late preterm labor: a historical cohort study.

BACKGROUND: While antenatal corticosteroids (ACSs) are standard for early preterm birth, their benefit‒risk profile for the late preterm period is controversial, and the effect of the ACS-to-delivery interval on key neonatal morbidities is unclear. This study therefore examined the association between this interval and neonatal outcomes. METHODS: A historical cohort study enrolled 788 singleton pregnancies at 34+ 0 to 36+ 6 weeks of gestation. The non-ACS group comprised women who did not receive ACS. The ACS group was stratified into three subgroups according to the time between the first ACS

From standard to individualized diazoxide therapy in congenital hyperinsulinism: a narrative review.

PURPOSE: Congenital hyperinsulinism (CHI) is the commonest cause of persistent hypoglycaemia in neonates and infants (blood glucose <3.0 mmol/L in first 2-3 days of life; <3.5 mmol/L after 3 days of life). Diazoxide demonstrates variable efficacy depending on the underlying genetic variant and clinical phenotype. Diazoxide has been associated with side effects that are likely dose dependent. This narrative review synthesizes current evidence on diazoxide's pharmacokinetics and side effect profile to support the development of individualised dosing strategies guided by genotype and patient-spec

Hepatic gene replacement improves energy metabolism and survival in a mouse model of neonatal mitochondrial disease GRACILE syndrome.

Preclinical gene therapy studies of mitochondrial diseases remain limited due to the typically multi-organ manifestations and the scarcity of physiologically relevant animal models. Mutations in BCS1L, a nuclear gene encoding an assembly factor for mitochondrial complex III (CIII), are the most common cause of CIII deficiency. The most severe phenotype, GRACILE syndrome, is caused by a homozygous Finnish founder mutation (c.A232G, p.S78G). The corresponding Bcs1lp.S78G knockin mouse model recapitulates the human disease, with juvenile-onset hepatopathy, tubulopathy, growth restriction, segment

Neonatal Bacteremia Caused by Herbaspirillum huttiense in a Neonatal Intensive Care Unit: A Case Series.

BACKGROUND: Herbaspirillum huttiense is a rare environmental Gram-negative bacillus increasingly recognized as an emerging opportunistic pathogen in humans. Due to its phenotypic similarities with other non-fermenting Gram-negative bacteria, it is frequently misidentified, and data on its clinical characteristics and management remain limited. METHODS: This was a retrospective case series study conducted over one month (March 2023) in the Microbiology Department of Mohammed VI University Hospital, Marrakesh, Morocco. The study included neonates hospitalized in the Neonatal Intensive Care Unit

Insulin/C-peptide interpretation in infants with hypoglycaemia of unknown origin.

BackgroundHypoglycaemia is not a disease per se but a manifestation of underlying perturbations of glucose homoeostasis. Among the biochemical tests commonly requested in newborns with confirmed, persistent/recurrent hypoglycaemia of unknown origin is the measurement of insulin and C-peptide in a blood sample(s) taken during the period of hypoglycaemia. Accepting an elevated insulin result with/without raised C-peptide as bona fide could trigger unnecessary investigations and/or diagnostic misapplications.PurposeTo highlight the significance of the "maternal-foetal-newborn" associations and th

Impact of Real Time, Flash and Retrospective Continuous Glucose Monitoring on Feto-Maternal Outcomes in Women With Gestational Diabetes and Pre-Gestational Diabetes: A Systematic Review and Meta-Analysis With Trial Sequential Analysis of Randomized Controlled Trials.

AIMS: Prior systematic reviews and meta-analyses (SRMs) evaluating continuous glucose monitoring (CGM) in pregnancy have yielded heterogeneous results due to pooling gestational diabetes mellitus (GDM) with pregestational diabetes, combining different CGM technologies (real-time, flash and retrospective) and mixing randomized controlled trials (RCTs) with observational designs. Several large RCTs have recently been published, necessitating an updated, design-restricted evaluation. METHODS: Electronic databases were systematically searched for RCTs assessing CGM versus self-monitoring of blood
