An Observational Study of Maternal Hypoglycemia in Pregnancy.

BACKGROUND: Hypoglycemia in pregnancy is relatively understudied, with conflicting evidence suggesting either outcomes similar to normal oral glucose tolerance test (OGTT) or associations with poor neonatal outcomes. Maternal glucose crosses the placenta, thus maternal hypoglycemia may affect the fetus. OBJECTIVE: This study aimed to evaluate the association between maternal hypoglycemia and neonatal birth weight, specifically the risk of small-for-gestational-age (SGA) neonates, and to examine maternal metabolic outcomes in comparison to normoglycemic and hyperglycemic individuals. STUDY DE

First-trimester uterine artery doppler in pregnancies with reactive hypoglycaemia or low 2-hour oral glucose tolerance test value: a retrospective study.

BACKGROUND: Reactive hypoglycemia (RH) or low oral glucose tolerance test (OGTT) value has been linked with polyhydramnios and SGA, which is possibly due to placental dysfunction and glucose dysmetabolism. This study aims to investigate the perinatal outcomes associated with placental dysfunction and first-trimester uterine artery resistance in pregnancies with RH or a low 2-hour value on OGTT. METHODS: Pregnant women undergoing OGTT screening after 24 gestational weeks between 2021 and 2023 in maternity units in Indonesia were retrospectively evaluated. Women with multifetal pregnancy, pre-p

Case Report: Pasireotide treatment in neonatal congenital hyperinsulinism due to a homozygous ABCC8 mutation.

Mutations in ABCC8 and KCNJ11 are associated with the most severe and diazoxide-resistant forms of congenital hyperinsulinism (CHI). Somatostatin analogues are commonly used off-label as second-line treatment. While octreotide and lanreotide are the most established options, pasireotide-a second-generation somatostatin analogue with higher affinity for somatostatin receptor subtype 5-has been hypothesized to offer improved suppression of insulin secretion. We report the off-label use of subcutaneous pasireotide in an infant with severe CHI due to a homozygous ABCC8 mutation. The patient presen

Acute Fatty Liver of Pregnancy Presenting With Isolated Nausea and Vomiting in the Third Trimester: A Case Report.

Acute fatty liver of pregnancy (AFLP) is a rare but life-threatening obstetric emergency occurring in the third trimester. Its non-specific presentation frequently overlaps with other pregnancy-related conditions, leading to diagnostic delay and increased maternal and fetal morbidity and mortality. We report the case of a 27-year-old primigravida at 38 + six weeks of gestation presenting with isolated nausea and vomiting, without abdominal pain, hypertension, proteinuria, or obstetric complaints. Clinical examination revealed mild jaundice and stable hemodynamics. Initial laboratory investigat

Clinical and biochemical footprints of inherited metabolic disorders: XIX. Hypoglycemia.

Hypoglycemia is one of the most common metabolic emergencies in childhood. If not promptly recognized and treated, it can cause irreversible neurological injury. In newborns, infants, and children, inherited metabolic diseases (IMDs) account for a large proportion of persistent or recurrent hypoglycemia, though other genetic and endocrine disorders can also be underlying causes. Timely etiological diagnosis is critical, as many causes are treatable and require specific management to prevent recurrence. We systematically reviewed and updated the list of inherited causes of hypoglycemia and iden

International Guideline on the Diagnosis, Treatment, and Monitoring of Long-Chain Fatty Acid Oxidation Disorders (LC-FAOD).

Long-chain fatty acid oxidation disorders (LC-FAOD) are rare inherited defects of mitochondrial β-oxidation that impair energy generation during fasting or metabolic stress. Clinical manifestations range from neonatal hypoketotic hypoglycemia and cardiomyopathy to hepatopathy, recurrent rhabdomyolysis, and chronic myopathy. Although newborn screening (NBS) enables early detection, management remains inconsistent due to limited evidence, broadening phenotypic spectrum, and regional practice variation. An international multidisciplinary workgroup of 34 members, including clinicians, nurses, diet

Diagnosis of Pituitary Stalk Interruption Syndrome in a Newborn Presenting With Recurrent Hypoglycemia: A Rare Case Report.

Pituitary stalk interruption syndrome (PSIS) is a rare congenital anomaly associated with multiple pituitary hormone deficiencies. It is characterized by an absent or thin pituitary stalk, an ectopic posterior pituitary, and a hypoplastic anterior pituitary and is typically diagnosed via MRI. The syndrome presents variably depending on age, with neonatal cases often manifesting as recurrent hypoglycemia. Early diagnosis is critical to prevent severe complications. We report a case of a preterm male neonate born via emergency cesarean section due to non-reassuring cardiotocography and vaginal b

Near-Total Pancreatectomy for Congenital Hyperinsulinemic Hypoglycemia: A Single-Center Experience From a Low-Resource Setting in Sudan.

Background and objective Congenital hyperinsulinemic hypoglycemia (CHH) is the leading cause of persistent hypoglycemia in neonates and infants. In high-resource settings, diagnosis and management depend on rapid genetic testing and advanced imaging to differentiate focal from diffuse disease. However, in low-resource settings, limited diagnostic capacity and lack of access to medications often necessitate early surgical intervention. This study aimed to evaluate the clinical features, surgical intervention, and outcomes in infants undergoing pancreatectomy for congenital hyperinsulinism at Al
