IMPORTANCE: Metformin and glyburide monotherapy are used as alternatives to insulin in managing gestational diabetes. Whether a sequential strategy of these oral agents results in noninferior perinatal outcomes compared with insulin alone is unknown.
OBJECTIVE: To test whether a treatment strategy of oral glucose-lowering agents is noninferior to insulin for prevention of large-for-gestational-age infants.
DESIGN, SETTING, AND PARTICIPANTS: Randomized, open-label noninferiority trial conducted at 25 Dutch centers from June 2016 to November 2022 with follow-up completed in May 2023. The study

OBJECTIVE: Subtypes of gestational diabetes mellitus (GDM) based on insulin sensitivity and secretion have been described. We addressed the hypothesis that GDM subtypes are differentially associated with newborn and child anthropometric and glycemic outcomes.
RESEARCH DESIGN AND METHODS: Newborn and child (age 11-14 years) outcomes were examined in 7,970 and 4,160 mother-offspring dyads, respectively, who participated in the Hyperglycemia and Adverse Pregnancy Outcome (HAPO) study and HAPO Follow-Up Study. GDM was classified as insulin-deficient GDM (insulin secretion <25th percentile with pr

AIM: Gestational diabetes, characterized by impaired glucose tolerance occurring or diagnosed during pregnancy, is a significant public health concern. When lifestyle and dietary measures fail (30 % of women), insulin is the standard treatment. Oral antidiabetic agents, such as metformin (Glucophage) and glibenclamide, could provide a promising alternative. The aim here was to evaluate the effectiveness and safety of these treatments in gestational diabetes.
METHODS: This study is based on a systematic literature review. A keyword search for "metformin (Glucophage)," "glibenclamide," "pregnan

Background Neonatal hypoglycemia is a common metabolic disturbance in neonates. Glucose monitoring is recommended for certain groups of neonates, including those whose mothers have pre-gestational or gestational diabetes. Little is known about the relevance of glucose monitoring in neonates whose mothers have an elevated screening glucose challenge test (GCT) but a normal oral glucose tolerance test (OGTT). Objectives The objectives of this study were to determine if neonates of mothers with an elevated GCT but a normal OGTT had hypoglycemia and to establish if there was an association between

BACKGROUND: There is a paucity of information concerning the short- and long-term benefits and harm of antenatal corticosteroid administration and of expanded corticosteroid administration with dexamethasone in the late preterm period. Thus, we aimed to compare the effect on short-term respiratory complications, hypoglycemia, and long-term neurodevelopmental disorders in neonates born in the late preterm period between cases with and without corticosteroid administration and evaluate the difference in effects according to the type of corticosteroid administered.
METHODS: This retrospective ob

Preterm infants are susceptible to neonatal sepsis, a syndrome of pro-inflammatory activity, organ damage, and altered metabolism following infection. Given the unique metabolic challenges and poor glucose regulatory capacity of preterm infants, their glucose intake during infection may have a high impact on the degree of metabolism dysregulation and organ damage. Using a preterm pig model of neonatal sepsis, we previously showed that a drastic restriction in glucose supply during infection protects against sepsis via suppression of glycolysis-induced inflammation, but results in severe hypogl

INTRODUCTION: Congenital hyperinsulinism (HI) is characterized by inappropriate insulin secretion from the pancreatic beta-cells which causes severe hypoglycemia. Copy number variants (CNVs) encompassing multiple genes (contiguous gene CNVs) can cause syndromic forms of HI although they are not typically screened for during routine genetic testing for this condition. We aimed to assess the prevalence of disease-causing contiguous gene CNVs in a cohort of individuals referred for HI genetic testing.
METHODS: Our cohort consisted of 3,763 individuals, of which 1,916 had received a genetic diagn

Background Hypoglycemia is a common metabolic abnormality in newborns. While short episodes of hypoglycemia in these infants are generally harmless, extended periods of hypoglycemia can lead to fatal outcomes. Early detection through readily available, routine testing and prompt management can prevent long-term disability and mortality in these children. Objective To determine the frequency and risk factors of hypoglycemia in asymptomatic, full-term neonates within the first hour of life. Methods This cross-sectional study was carried out in the Department of Pediatrics at Hayatabad Medical Co

BACKGROUND: Congenital hyperinsulinism (cHI) is a rare, primarily pediatric disease characterized by dysregulated insulin secretion resulting in severe, persistent hypoglycemia, frequently leading to lifelong neurologic impairments. The safety, pharmacokinetics, and glycemic efficacy of ersodetug, a fully human monoclonal antibody that allosterically and reversibly binds the insulin receptor (INSR) and reduces excess insulin action, are being evaluated for the treatment of cHI-related hypoglycemia.
METHODS: A global, open-label, phase 2b study (ClinicalTrials.gov: NCT04538989) was conducted i

Hypoglycemia and impaired metabolic transition are frequently observed in neonates during the first 24-48 h after birth [1, 2]. Severe (< 36 mg/dL or 2 mmol/L) and recurrent (3 or more episodes) hypoglycemia can cause neurological injury and developmental delays. The ambiguity regarding a threshold blood glucose concentration remains due to differing values proposed by various professional organizations. This poses a challenge in diagnosing neonatal hypoglycemia in addition to using a single blood glucose value, which in itself is not entirely reflective of various key molecular processes unco

This literature review assesses clinical guidelines for pre-existing diabetes and gestational diabetes mellitus (GDM) in the areas of diagnosis, management, and maternal-fetal outcomes. A structured search was conducted across PubMed and Google Scholar, supplemented by targeted screening of guideline repositories from the American Diabetes Association (ADA), National Institute for Health and Care Excellence (NICE), and World Health Organization (WHO). Included studies and guidelines were selected based on relevance to diagnosis, therapeutic strategies, or maternal-neonatal outcomes, with exclu

Congenital hyperinsulinism (CHI) and neonatal cholestasis (NC) are occasionally observed in neonatal settings; however, the causes have not been identified despite thorough investigation. Surgical evaluation is essential for patients with cholestasis to rule out biliary atresia because timely surgical intervention is critical. Few case reports have described the co-occurrence of CHI and NC. Herein, we report a case of a boy born as one of dichorionic-diamniotic twins via planned cesarean delivery. Apparent ischemic events were not present, with Apgar scores of 8 and 9 at one and five minutes,

BACKGROUND: Despite progress in reducing neonatal mortality rates in Ethiopia, the country still has a high neonatal mortality rate compared with the global average. Primary hospitals are critical in delivering basic neonatal care, particularly in rural areas. However, data on neonatal mortality and contributing factors in these settings are scarce. This study aimed to determine the survival status and predictors of neonatal mortality among neonates admitted to Bichena Primary Hospital, Northwest Ethiopia.
METHODS: A retrospective cohort study was conducted among 638 neonates admitted to the

BACKGROUND: Growth hormone deficiency (GHD) is one of the primary endocrine-related causes of short stature in pediatric patients; while neonatal GHD symptoms are well-documented in populations with known genetic and/or organic causes, their exact prevalences in pediatric patients categorized as having idiopathic GHD remains unclear.
MATERIALS AND METHODS: We retrospectively analyzed the medical records of patients with idiopathic GHD followed at the Pediatric Endocrinology Unit of the Fondazione Policlinico Universitario A. Gemelli IRCCS starting from January 2010. We analyzed information fr

Presently, societal guidelines differ regarding evaluation and management of variations in placental cord insertion (PCI). This variation may in part be secondary to inconsistency in reported risk associated with marginal and velamentous cord insertion (VCI). The objective of this study is to compare perinatal outcomes based on PCI site in pregnancies at risks for fetal growth disorders. This was a secondary analysis of singletons with growth assessment between 26 and 36 weeks of gestation. Fetuses with chromosomal or congenital malformations were excluded. The primary outcomes studied were neo

BACKGROUND: Peroxisomal fatty acyl-CoA oxidase 1, encoded by ACOX1, initiates and limits the rate of beta-oxidation of very long-chain fatty acids (VLCFA). Biallelic ACOX1 mutations cause pseudo-neonatal adrenoleukodystrophy (PNALD). Primary adrenal insufficiency (PAI) has not been clearly characterized in the 34 PNALD patients reported to date.
OBJECTIVE: Characterizing PAI in a patient and her cousin with PNALD.
METHODS: Clinical data were recorded, and molecular etiologies were investigated using next-generation sequencing panels and 750K microarray. Plasma steroids and VLCFAs were measur

OBJECTIVE: We present clinical features and outcomes of second-trimester acute fatty liver of pregnancy (AFLP), a serious but rare condition.
RESULTS: Fourteen pregnant women with AFLP or compatible biopsy were identified in the literature. Diagnosis occurred between 20 and 27 + 6 weeks of gestational age. In total, 50% were primigravid. Thirteen cases met the Swansea criteria while the other case had a compatible liver biopsy. In total, 10 out of 11 cases had microvesicular steatosis on their liver biopsy. Common maternal complications included acute renal failure, hypoglycaemia, liver failu

BACKGROUND: The tight glycemia required to optimize type 1 diabetes (T1D) pregnancy outcomes is difficult to achieve with standard insulin therapies. Automated insulin delivery (AID) offers an avenue to improve glycemia, but most available systems are not configurable to tight pregnancy glucose targets. Open-source AID may meet the needs of some pregnant women with T1D, but available data on its efficacy and safety in pregnancy are limited.
METHODS: This single-center retrospective study describes the glycemic and obstetric outcomes of pregnancies in which supported open-source AID (SOSAID) w
