Increased risk of admission to neonatal intensive care unit in neonates born to mothers with pregestational diabetes.

UNLABELLED: The purpose of this study was to describe how maternal diabetes impacts admission to the neonatal intensive care unit to support healthcare professionals when counselling patients. The primary outcome was admission rates. A retrospective observational cohort study of 25,238 births was conducted at an Irish tertiary maternity hospital from January 2018 to December 2020. Cases of pregestational and gestational diabetes were examined for neonatal intensive care admission outcomes. R statistical analysis software was used. There were 3905 live neonates born between 34 and 42 weeks to m

One-hour vs Two-hour Postprandial Glucose Targets and Fetomaternal Outcomes in Gestational Diabetes Mellitus: A Systematic Review and Meta-Analysis.

OBJECTIVE: The optimal time and target for postprandial glucose (PPG) measurement in gestational diabetes mellitus (GDM) remain unclear. This systematic review and meta-analysis evaluated whether targeting 1-hour PPG (1 hPG) vs 2-hour PPG (2 hPG) altered fetomaternal outcomes in GDM. METHODS: Studies that compared pregnancy outcomes in women undergoing 1 hPG vs 2 hPG monitoring in GDM were identified through comprehensive search of electronic databases. Primary outcomes analyzed were large-for-gestational age (LGA) and macrosomia. Secondary outcomes included low birthweight (LBW), neonatal in

Continuous Glucose Monitoring in the Management of Congenital Hyperinsulinism: A National User-satisfaction Survey, UK.

CONTEXT: Congenital hyperinsulinism (CHI) causes severe and recurrent hypoglycemia with a 33% to 50% risk of neurodisability necessitating rigorous glucose monitoring. Continuous glucose monitoring (CGM) is now widely used in CHI with limited data on user-reported benefits. There are no validated instruments available to assess the impact of CGM use on quality of life (QOL) in CHI. OBJECTIVE: To evaluate CGM user satisfaction of patients with CHI and their carers. DESIGN: Modified CGM-satisfaction (CGM-SAT) and glucose monitoring surveys (GMS) were distributed electronically to CHI families

Right atrial thrombus in neonates following hyperosmolar glucose infusion: A case series.

BackgroundCardiac thrombus in neonates is an uncommon but life-threatening condition that may result in significant morbidity and mortality. Factors such as invasive procedures, metabolic disturbances, and the use of high-osmolarity fluids can contribute to thrombus formation, particularly in critically ill neonates. This case series aims to highlight the clinical characteristics, potential risk factors, and outcomes of right atrial thrombus in neonates, with particular emphasis on the role of high-concentration glucose infusions administered through umbilical venous catheters.MethodsThis is a

Neonatal Hypoglycemia.

Neonatal hypoglycemia is a common condition. Consensus on significant values has been difficult to establish. Due to potential for long-term impacts, screening and treatment are recommended. Infants at risk warrant monitoring. Various modalities exist to measure glucose levels. Plasma concentrations remain most accurate. Acute symptoms overlap with other conditions and are best treated with intravenous dextrose fluids. Asymptomatic newborns can be supported with enteral options that minimize separation of the mother-infant dyad. Further research into physiologic neonatal glycemic trends and th

Glucose Homeostasis, Metabolomics, and Pregnancy Outcomes After Bariatric Surgery (GLORIA): Protocol for a Multicentre Prospective Cohort Study.

Background: Metabolic bariatric surgery is a highly effective and long-lasting treatment for obesity and related chronic conditions. Women of reproductive age represent the largest group undergoing these procedures. Observational studies suggest an increased risk of preterm birth and impaired foetal growth in this population, though the underlying mechanisms remain unclear. A key hypothesis is that altered glucose metabolism, characterised by frequent hypoglycaemia and glycaemic fluctuations, may contribute to these adverse outcomes. While glycaemic variability following metabolic bariatric su

Impact of Gestational Diabetes on Neonatal Birth Weight and Maternal Postpartum Metabolic Changes.

BACKGROUND: Gestational diabetes mellitus (GDM) is a growing public health concern globally, particularly in the context of rising maternal obesity and glucose intolerance. GDM poses significant risks to both neonatal and maternal health, including fetal overgrowth, birth complications, and long-term metabolic disorders. This study aimed to evaluate the impact of GDM on neonatal birth weight and maternal postpartum metabolic changes. The primary objective was to assess the incidence of macrosomia and small-for-gestational-age (SGA) births in pregnancies complicated by GDM. Secondary objectives

Burden of "Pokes" in Newborns With Hypoglycemia Who Require IV Dextrose.

OBJECTIVE: Neonatal hypoglycemia is one of the most common reasons for neonatal intensive care unit (NICU) admission in otherwise healthy late preterm and term newborns. Routine care requires frequent blood glucose (BG) measurements, typically via painful heel lances, to titrate intravenous (IV) dextrose during treatment. However, little is known about the care for these newborns including the burden of BG measurements. PATIENTS AND METHODS: This was a retrospective medical record review (July 2018 to July 2024) of newborns at least 35 weeks gestational age who had hypoglycemia screening in a

Long-term Follow-up of a Case with TBX19 Mutation, a Rare Cause of Isolated ACTH Deficiency and Literature Review.

T-box pituitary restricted transcription factor is a transcription factor required for proopiomelanocortin (POMC) gene expression and pituitary corticotroph cell differentiation and is encoded by T-box factor 19 (TBX19). Variants in TBX19 cause early onset congenital isolated adrenocorticotrophic hormone (ACTH) insufficiency with a mortality rate of up to 25% in the neonatal period. Mild dysmorphic findings may accompany some cases. Here, we report a case of isolated ACTH deficiency due to a TBX19 variant diagnosed in the neonatal period, who was followed up until adulthood. The male patient h

Does consumption of a high-fructose diet during pregnancy and lactation exacerbate the effects of maternal exposure to cadmium on development and metabolic function of mouse offspring?

Exposures to pollutants rarely occur in isolation, often coexisting with other environmental stressors such as diet and may be particularly insidious in early life. The aim of this study was to examine effects of maternal exposure to cadmium (Cd) and consumption of a high-fructose diet (HFrD) on development of mouse offspring. Female CD-1 mice were administered either 0.5 or 5 ppm Cd in drinking water with or without an approximate 60% fructose diet for 3 weeks prior to mating. Dams were maintained on the same treatment until postnatal day (PND) 16. Cadmium concentrations in maternal, fetal, a
