Neonatal Presentation of a Case of Carbonic Anhydrase VA Deficiency.

Carbonic anhydrase VA (CA-VA) deficiency is a rare mitochondrial disorder affecting bicarbonate-dependent metabolic pathways. It commonly presents in neonates with hyperammonemia, lactic acidosis, and metabolic crisis, often mimicking more prevalent urea cycle and organic acid disorders. This rare metabolic disorder has been reported across different populations, with cases documented worldwide. We report the case of a three-day-old full-term Indian female neonate presenting with lethargy and poor feeding. Initial labs revealed hyperammonemia, severe lactic acidosis, hypoglycemia, and ketosis.

Challenges in the management of epilepsy associated with posterior gliosis secondary to perinatal brain injury.

Posterior gliosis is a major substrate underlying drug resistant epilepsy (DRE) in children and young adults in low-middle income countries. Neonatal hypoglycemia and prolonged partial asphyxia either isolated or combined are major risk factors for posterior gliosis. The epilepsy associated with posterior gliosis has a spectrum of severity with early onset drug resistant epileptic encephalopathies with disabling co-morbidities at one end and pharmaco-responsive focal epilepsy in a normal child at the other. Intermediate severity syndromes are common. Disabling co-morbidities are typically cogn

Impact of antenatal corticosteroid therapy on neonatal outcomes in twin pregnancies.

BACKGROUND: Corticosteroids are widely used in obstetric clinical practice for cases with signs of preterm labor to promote fetal lung maturity and reduce neonatal morbidity and mortality. Although short-term use is considered safe, there is ongoing debate regarding the dosage, therapeutic window, neonatal benefits, and maternal-fetal side effects, especially in high-risk pregnancies such as twins, where the impact remains unclear. METHODS: This retrospective study included 1,997 twin pregnancies, divided into two groups: those who received antenatal corticosteroid therapy (ACS) and those who

Evaluating the association between gestational diabetes and neonatal hypoglycemia in Taiwan: a retrospective study of 2,149 pregnancies.

OBJECTIVE: To determine whether gestational diabetes mellitus (GDM) is associated with an increased risk of neonatal hypoglycemia and adverse neonatal outcomes in a Taiwanese population. METHODS: We performed a retrospective cohort study of 2,149 women who delivered at Chung Shan Medical University Hospital from 2019 to 2023. GDM was diagnosed by one-step 75-g oral glucose tolerance test (OGTT). Neonatal hypoglycemia was defined as blood glucose <45 mg/dL. Logistic regression was used to estimate odds ratios (ORs) for neonatal outcomes associated with GDM, adjusting for maternal age, body mas

Lactic acidosis, rhabdomyolysis, and hyperammonemia: Atypical presentation in a new patient with PDE-ALDH7A1 defect.

Pyridoxine-Dependent Epilepsy (PDE) is an autosomal recessive disorder caused by biallelic variants in ALDH7A1. The most common presentation is intractable seizures in the neonatal/early infantile period, which respond to pyridoxine. Other manifestations include perinatal asphyxia, hypoglycemia, and neuroimaging abnormalities. Despite early treatment, patients often have neurodevelopmental abnormalities. Treatment guidelines recommend triple therapy with pyridoxine, dietary lysine restriction, and arginine supplementation. We report an individual presenting with laboratory abnormalities sugges

A Case of 17β-Hydroxysteroid Dehydrogenase Type 10 (HSD10) Disease Caused by a Novel Variant Presenting With Rapidly Progressive Cardiomyopathy Triggered by Viral Infection.

Human 17β-hydroxysteroid dehydrogenase type 10 (HSD10) is a rare X-linked mitochondrial disorder caused by mutations in the HSD17B10 gene. It is typically associated with neurodegeneration and cardiomyopathy in severe cases. The neonatal form often has a poor prognosis; however, its clinical spectrum remains unclear. Herein, we present a neonatal-onset case of HSD10 disease with a previously unreported HSD17B10 variant presenting with early-onset cardiomyopathy triggered by a viral infection. A male infant presented with lactic acidosis and hypoglycemia on the first day of life. Initial treatm

Neonatal Congenital Hyperinsulinism: A Case-Based Contribution to the Understanding of a Rare Disorder.

Congenital hyperinsulinism (CHI) is a rare but significant cause of persistent neonatal hypoglycemia (NH), associated with a high risk of neurological complications if not promptly treated. This condition is characterized by inappropriate insulin secretion, often of genetic origin, independent of blood glucose levels. We report the case of a male macrosomic newborn admitted on the second day of life for respiratory distress, generalized seizures, and severe hypoglycemia (1.4 mmol/L) unresponsive to intravenous glucose therapy. Laboratory investigations revealed elevated insulin and C-peptide l
