SMS 201-995 (Sandostatin) was studied using low doses (50 to 100 micrograms) administered subcutaneously every 12 hours. A single 50-micrograms dose of SMS 201-995 effectively controlled gastric acid and blood gastrin levels for 12 hours in three patients with benign gastrinomas and was useful in their perioperative management. Higher doses of the agent (500 to 800 micrograms per day) had no effect on metastases in one of two patients with metastatic gastrinoma. In the other patient, one tumor shrank but the other continued to grow after three months of treatment while serum gastrin levels did

Resuscitation of a neonate requires both immediate cardiopulmonary resuscitation and extended intensive care. Initial resuscitation of the neonate, as for adults, must include support of the airway, breathing and circulation. Because of the unique physiology of a newborn infant, some aspects of drug therapy differ significantly from their counterparts in the resuscitation of adults, and hypoglycemia and hypothermia pose special threats to a distressed neonate. Epinephrine and atropine can be administered via an endotracheal tube, but vascular access, which is most easily obtained by cannulatin

Characteristics of energy metabolism in the new-born infant, piglet and lamb have been compared quantitatively in order to assess how the constraints imposed by the availabilities of energy substrates in body reserves and colostrum differ between species and affect the new-born's well being during the first day after birth. Three air temperature ranges, described as thermoneutral (32-38 degrees C), moderate (18-26 degrees C) and cold (0-10 degrees C) and representing the usual birth environments of infants, piglets and lambs, respectively, have been considered. The analysis revealed the follow

Forty-six infants exposed to indomethacin in utero for treatment for preterm labor were compared with infants from two control groups. In one control group the next consecutive patient treated with a tocolytic agent was used, and the other control group was formed by picking the next consecutive patient matched by gestational age who did not receive any tocolytic agent. There was no significant difference in Apgar scores, birth weight, or gestational age in the three groups. The incidence of neonatal complications including hypocalcemia, hypoglycemia, respiratory distress syndrome, patient duc

Seizures in the newborn are a distinctive sign of underlying disease. Different convulsive patterns are described. The most common neurologic syndrome consists of subtle seizures. The most important cause is ischemic encephalopathy. Hypocalcemia is the main metabolic disease. Hypoglycemia seems not to be of special relevance for pathogenesis of newborn seizures. Other episodic symptoms of non-epileptic origin should be considered in the differential diagnosis. It is critical to diagnose the cause and to treat it, since the prognosis depends on the underlying disturbance. Phenobarbital is the a

We report the results of a two-part study examining the frequency of symptoms and other findings associated with neonatal polycythemia. In the first part of the investigation, we evaluated the occurrence and features of the disorder in a cohort of 3,768 infants born at our institution that had been screened for the disorder during a 4-year period. Fifty-five infants (1.46%) had neonatal polycythemia. Of these infants, 85% had features associated with the disorder. Frequent signs and symptoms included "feeding problems" (21.8%), plethora (20.0%), lethargy (14.5%), cyanosis (14.5%), respiratory

Beta-adrenoceptor blockade (beta-blocker) has recently been introduced for use in treating hypertension of pregnancy. The aim of this investigation is to determine the effects of beta-blocker (alprenolol and labetalol) on parturition and on the fetal cardiovascular and metabolic system in normotensive pregnant rats. The continuous administration of alprenolol to pregnant Wistar rats revealed the following: a small placenta intrauterine growth retardation delayed parturition intrauterine death (21.8%) morbidity (30.3%) and, hypoglycemia of neonates. In the cases which received the administratio

In summary, glucose metabolism in the placenta and fetus is characterized by a fairly exact balance between exogenous glucose supply from the mother and placental and fetal glucose utilization (directly and as lactate produced from glucose). The rate of glucose utilization and its rate of oxidation are largely determined by the maternal glucose concentration and are mediated in part by insulin. Thus, glucose and insulin act together to substitute glucose oxidation for the oxidation of other energy substrates and to direct glucose carbon into glycogen, fat, and protein accretion. After birth, e

Over the past decade, it has been apparent that the degree of glycemic control achieved in the pregnant patient with diabetes mellitus significantly affects perinatal outcome. While several studies have documented excellent perinatal outcome with good glucose control, the incidence of neonatal morbidity still remains substantially greater than that observed in the general population. Physiologic or "tight" control appears to further reduce the incidence of macrosomia, hypoglycemia, and other indices of neonatal morbidity. The application of home glucose-monitoring techniques to pregnancies com

The significance of hypertensive complications of insulin-dependent diabetic pregnancies (IDDP) has not been well examined since the early reports of Pedersen, which demonstrated an increased risk of neonatal death in women with pregnancy induced hypertension (PIH). To assess the effect of both PIH and chronic hypertension (CH) on outcome of IDDP managed using contemporary obstetrical and diabetic management, we reviewed the records of all 199 IDDP delivered at our institution over a 7-year period. Patients were classified as having PIH (Group 1, n = 37), CH (Group 2, n = 18) or both (Group 3,

Decreased cardiac output is a common presumption in left ventricular myocardial dysfunction in neonates, but because of a lack of reliable noninvasive techniques, data on cardiac output are missing. We measured cardiac output by pulsed Doppler echocardiography in 22 newborn infants with left ventricular myocardial dysfunction diagnosed by M-mode echocardiography. Eleven neonates had severe perinatal asphyxia, seven had tachypnea, two hypoglycemia, and one septic shock; one had no symptoms. Right ventricular function was abnormal in 13 of the 22 infants. Hypotension was found in eight; cardiac

The effects of steriodal and nonsteroidal antiinflammatory agents on mortality, plasma glucose and lactate levels, and pathologic alterations associated with Escherichia coli endotoxemia were assessed in neonatal rats. Animals were first injected subcutaneously with either high (LD95) or moderate (LD35) doses of endotoxin and immediately thereafter were administered either saline, dexamethasone, indomethacin, or glucose. Animals treated with saline or glucose developed significant hypoglycemia and hyperlactacidemia. Dexamethasone and indomethacin ameliorated the fall in glucose and the rise in

Various types of dicarboxylic aciduria are known, most of them are accompanied by non-ketotic hypoglycaemia. For the differential diagnosis of these conditions several methods of investigation have been used: (1) analysis of urinary organic acids in both native and hydrolysed samples, (2) analysis of free and esterified carnitine, the latter by means of chromatographic separation and identification of acyl moieties, (3) analysis of plasma organic acids, including the so-called free fatty acids, (4) a prolonged fasting test with serial measurements of the aforementioned parameters and close mon

This study assesses the effects of infusion of 1200 ml of four different intravenous solutions before epidural anesthesia for cesarean section on maternal and neonatal whole blood lactate (L), pyruvate (P), excess lactate (XL), L/P ratio, and base excess (BE) in four equal groups of patients. Patients in group I (n = 15) received normal saline; those in group II, Ringer's lactate (RL); those in group III, RL with 20 g of glucose; and those in group IV, Plasma-Lyte A. Maternal venous concentrations of L increased significantly in all groups after infusion, but P increased only in group III. Bot

Cells are endowed with specific cognitive molecules that function as receptors for hormones, neurotransmitters, and other intercellular messengers. The receptor molecules may be present in the plasma membrane, cytoplasm, or nucleus. When occupied by the messenger, the receptor is coupled to the cellular machinery that responds to the message-bearing molecules. For some hormones the events following attachment of the messenger to the receptor are well known. An example is the generation of cAMP after combination of glucagon with its receptor and the series of steps culminating in activation of

Pancreatic tissue obtained at subtotal pancreatectomy from 15 infants with persistent hypoglycaemia with hyperinsulinism, and autopsy specimens from 23 age-matched normoglycaemic controls, were studied with morphometric methods after immunocytochemical staining of the four main islet cell types (A, B, D and pancreatic polypeptide cells). In three cases, a focal lesion was detected by gross examination. Macroscopic or microscopic examination did not distinguish the 12 other cases from controls. As found previously, nesidioblastosis was not a specific feature of the pancreas in infantile hypogly

We report the case of an infant with hypoglycemia, progressive lactic acidosis, an increased serum lactate/pyruvate ratio, and elevated plasma alanine, who had a moderate to profound decrease in the ability of mitochondria from four organs to oxidize pyruvate, malate plus glutamate, citrate, and other NAD+-linked respiratory substrates. The capacity to oxidize the flavin adenine dinucleotide-linked substrate, succinate, was normal. The most pronounced deficiency was in skeletal muscle, the least in kidney mitochondria. Enzymatic assays on isolated mitochondria ruled out defects in complexes II

Despite having implemented rigorous glucose control for women with gestational diabetes early in the third trimester, we found excessive morbidity among the neonates of these women. To accurately assess the risk of newborn complications, we did a five-year review (1977 to 1981) of infants of class A diabetic mothers to determine the incidence and scope of morbidity in these infants. Fifty-one infants of class A mothers were identified (group 1) and randomly matched with 102 infants of nondiabetic mothers (group 2). The distribution of morbidity between the two groups was as follows: hypoglycem

We have reviewed ten children who underwent surgical therapy for persistent neonatal hypoglycemia over a 5-year period. All had inappropriately high insulin levels in the face of hypoglycemia, and all failed medical management with intravenous glucose, frequent feeds, diazoxide and glucagon. Two groups of five patients each were analysed retrospectively. Group 1 underwent 95% pancreatectomy, leaving a small amount of pancreatic tissue on the duodenum and common bile duct. The only major complication in this group was in one patient with common duct obstruction requiring choledochoduodenostomy.

In 56 strictly controlled diabetic pregnancies, altogether 60 amniotic fluid samples were analysed for their content of lipid-carrying cells and the presence of phosphatidylglycerol. The outcome of the amniotic fluid tests was examined for its association to macrosomia and to minor neonatal complications such as hypoglycaemia, hypocalcaemia, respiratory disturbances and hyperbilirubinaemia. The predictive value of these tests was nil, since all cases of neonatal complications occurred in the group showing maturity in tests at week 37. At term, phosphatidylglycerol was detected in 80% of the pr
