Clinical and clinicopathological characteristics of the septicaemic neonatal foal: review of 38 cases.

In an effort to identify improved methods for diagnosis of infection in the neonatal foal, clinical records from 38 septicaemic foals of less than one week of age were examined for trends in history, physical examination and clinicopathological findings. The survival rate of septicaemic foals, 26 per cent, was markedly less than the rate for all other foal admissions. Blood cultures were valuable in diagnosis and treatment of septicaemia and identified a preponderance of Gram-negative infection. Zinc sulphate turbidity test results were abnormally low in all septicaemic foals tested. The clini

[Are the theoretical drawbacks of beta-blocker treatment in pregnancy being confirmed? A review of the literature].

The use of beta-blocking drugs in the treatment of hypertension during pregnancy has been the subject of controversies on the basis of theoretical hazards due to the pharmacology and pharmacokinetic characteristics of these drugs. A review of the literature on the subject shows that: The danger of premature contractions, abortion or premature delivery does not seem to increase with the use of beta-blockers. The blood supply is not more impaired with beta-blockers than with other antihypertensive drugs according to fetal growth, birth-weight, frequency of perinatal deaths or APGAR score. Althou

Insulin receptors in normal and disease states.

The binding of insulin to its receptor has been studied under various physiological and pathological conditions. Quantitative studies have involved human circulating cells such as monocytes and erythrocytes, adipocytes, placental cells, and cultured cells such as fibroblasts and transformed lymphocytes. In animals, other target tissues such as liver and muscle have been studied and correlated with the human studies. Various physiological conditions such as diurnal rhythm, diet, age, exercise and the menstrual cycle affect insulin binding; in addition, many drugs perturb the receptor interactio

Clinical experience with one hundred seven diabetic pregnancies.

Clinical experience with 45 pregnancies in women with Class A diabetes and 62 pregnancies in women with insulin-requiring diabetes is described. The perinatal mortality rates were 0, 16.1, and 9.3, respectively, among the Class A, insulin-requiring, and total diabetic populations. Diabetic mothers experienced significantly higher prevalences of cesarean section and ketoacidosis than did the overall population. There were no other significant differences in maternal complications. Diabetic mothers demonstrated high rates of abnormal estriol levels and relatively low rates of positive contractio

Echocardiographic profile of infants of diabetic mothers.

Echocardiographic M-mode examinations were performed in 30 infants of diabetic mothers (IDM). Twelve of them had severe symptoms with hyperbilirubinemia, respiratory distress syndrome, septicemia and hypoglycemia in the neonatal period. The gestational age ranged from the 34th week to the 40th week, the birth weight ranging between 2280 g and 5820 g (mean 3455 g). Fifteen patients were restudied at different ages. In group A (age 1-14 days) out of 23, in group B (age 14 days-6 months) 5 from 13 IDM and in group C (older than 6 months, (n=10) no IDM had an increased ratio of intraventricular se

Follow-up studies on preterm infants after maternal beta-receptor agonist treatment.

A review of the existing literature regarding the outcome of infants delivered preterm after antenatal beta-receptor agonist treatment of the mother is presented. Some investigators have reported an increased rate of neonatal and long-term hazards, while others have not found a higher incidence of such complications after antenatal beta-receptor treatment. In our own material of 55 preterm survivors with follow-up up to 2 years of age, we found a modest rise in postnatal hypoglycemia rate but no other short-term or long-term sequelae. Postnatal monitoring of blood glucose and cardiovascular fu

A long-term follow-up study of 214 children with the syndrome of infantile spasms.

The long-term prognosis of 192 surviving children with the syndrome of infantile spasms was evaluated. The children had been admitted to three paediatric hospitals in Helsinki at the time of initial diagnosis. The aetiological factors of the syndrome were carefully studied in each case. ACTH therapy was employed in 162, usually for about six weeks. The follow-up study 3-19 (mean 10.4) years later was made at the Children's Hospital, University of Helsinki. The rate of mortality was 19.6 per cent. Normal development was seen in 12 per cent and slightly subnormal in 10.4 per cent of the survivin

[Pancreatic resection in the treatment of infantile hypoglycaemia].

Authors report four patients with hyperinsulinemic unremitting hypoglycaemia due to pancreatic nesidioblastosis. Onset was neonatal in three of them and at the end of the first year in the remaining one. After variable periods of only partially successful medical therapy, the four patients were operated and subtotal pancreatectomy was carried out. This alone was sufficient in one case, and in another one diazoxide made control possible. The other two children had a total pancreatectomy, and one of them has needed insulin ever since. Hypoglycaemia is under control in all children and only one h

Malformations in infants of diabetic mothers.

Maternal insulin-dependent diabetes has long been associated with congenital malformations. As other causes of mortality and morbidity have been eliminated or reduced, malformations have become increasingly prominent. Although there is not universal agreement, the great majority of investigators find a two- to threefold increase in malformations in infants of insulin-dependent diabetic mothers. This increase is not seen in infants of gestational diabetics. It probably is not present in women whose diabetes can be controlled by diet or oral hypoglycemic agents. The risk does not appear to be pr

Umbilical cord glycosylated hemoglobin in infants of diabetic mothers: relationships to neonatal hypoglycemia, macrosomia, and cord serum C-peptide.

Relationships of neonatal glycemia and birthweight to antecedent fetal glycemia and insulinemia have been examined in the offspring of 63 insulin-dependent diabetic and 29 nondiabetic mothers. Glycosylated hemoglobin levels in maternal and cord blood were measured by the thiobarbituric acid (TBA) colorimetric technique to estimate antecedent fetal and maternal glycemia; cord serum C-peptide was assayed to estimate fetal insulinemia. Glycosylated hemoglobin levels were significantly elevated in the diabetic mothers and their offspring as compared with controls (P less than 0.001), and maternal

Diabetes and pregnancy in South Carolina: prevalence, perinatal mortality, and neonatal morbidity in 1978.

Through retrospective analysis of vital records and hospital discharge data, this study examines the prevalence of diabetes in pregnancy and diabetes-related perinatal mortality and neonatal morbidity in South Carolina in 1978. Diabetes was observed in 5.9/1000 deliveries. Diabetic natality rates for whites and nonwhites were 5.1/1000 and 6.9/1000, respectively. In comparison with comprehensive screening programs, these rates suggest that more than half of all diabetic pregnancies in South Carolina are unrecognized. The overall diabetic perinatal mortality rate was 102 deaths/1000 deliveries,

Investigation on serum C-peptide concentrations in pregnant diabetic women and in newborns of diabetic mothers.

Serum C-peptide concentrations at delivery and neonatal complications were investigated in a group of diabetic mothers and in their infants (IDM, n = 29). Furthermore, the changes in B-cell function and in daily insulin demand was followed up in 12 pregnant diabetics (of them 8 with long-term and 4 with short-term and "mild" diabetes) during pregnancy. All these diabetic mothers were under an intensive metabolic control with the aim to achieve normoglycaemia mainly in the second part of pregnancy. Mean cord blood C-peptide level of 29 IDM was 0,58 +/- 0,43 nmol/l, being not significantly highe

Serial blood T4 and TSH determinations in low birth weight infants. Influence of gestational age, birth weight and neonatal pathology on thyroid function.

Serial blood T4 and TSH determinations were performed on a total number of 352 low birth weight (LBW) infants: 210 healthy newborns and 142 infants affected by respiratory distress syndrome (RDS), sepsis, hyperbilirubinaemia, hypocalcemia, or hypoglycemia. The healthy infants have been divided into three groups in relation to gestational age (less than or equal to 33 wk, 34-36 wk, greater than or equal to 37 wk). Statistical evaluation among groups shows that reduced T4 concentration in LBW infants is strictly related to gestational age, while TSH failed to demonstrate any significant correlat

The autonomic nervous system and perinatal metabolism.

The development of the autonomic nervous system in relation to perinatal metabolism is reviewed with particular attention given to the adipocyte, hepatocyte and the A and B cells of the islets of Langerhans. Adrenergic receptors develop in the B cell independently of normal innervation and by the time of birth, in most species studied, the pancreas, liver and adipose tissue respond appropriately to autonomic signals. Birth is associated with a huge surge in circulating catecholamines which is probably responsible for the early postnatal rise in free fatty acids and glucagon concentrations in p

Metabolic events associated with the preparation of the fetus for independent life.

The metabolic changes late in fetal development that are essential for neonatal survival are discussed. In many species gluconeogenesis develops before birth but provides substrate for intracellular biosynthesis and not for glucose production because of low activities of glucose 6-phosphate translocase. At the time of glycogen deposition in species with a relatively mature brain at birth the translocase develops and glucagon and adrenaline can stimulate glucose production and synthesis to elevate blood glucose concentrations both pre- and postnatally. The other metabolic fuel accumulated befor

The glucose tolerance test as a means of identifying intrauterine growth retardation.

The purpose of this prospective, nonconcurrent study was to determine whether maternal hypoglycemia, as measured by a 3-hour glucose tolerance test, is associated with pregnancies resulting in growth-retarded infants. Our results demonstrate that maternal hypoglycemia is significantly associated with intrauterine growth retardation other than the low birth weight type and that the 3-hour glucose tolerance test is an effective early screening test in detecting pregnancies at risk of fetal growth retardation.

Pregnancy in insulin-dependent diabetics. A 5 1/2-year study at Groote Schuur Hospital.

During a 5 1/2-year period we have seen only 39 pregnant women with insulin-dependent diabetes, as opposed to 171 with established insulin-independent diabetes. Tight control with two injections of mixed insulins per day was attempted, but satisfactory blood glucose values were obtained in only 16 cases. Nevertheless the overall perinatal mortality rate was 77/1000; of the 3 infants which died 2 had lethal congenital abnormalities and 1 was born to a mother whom we had been seeing for only 4 weeks. Perinatal morbidity was similar to that in other series, except that few of our infants were ove

Haemoglobin AIc predicts the perinatal outcome in insulin-dependent diabetic pregnancies.

Haemoglobin AIc (Hb AIc) was measured in 112 insulin-dependent diabetic pregnancies on an average 4.7 times. Hb AIc was high in early pregnancy in all three patients with severe fetal malformations. There were six pregnancies with a perinatal death. The maximum Hb AIc values were significantly higher in the second trimester (p less than 0.001) in these pregnancies than in the other diabetic pregnancies. When neonatal hypoglycaemia was present, the mean maternal Hb AIc was significantly higher in the second (p less than 0.005) and third (p less than 0.02) trimesters of pregnancy than in the gro

Effect of somatostatin infusion on intermediary metabolism and entero-insular hormone release in infants with hyperinsulinaemic hypoglycaemia.

The hypoglycaemia of infantile hyperinsulinism is often exceedingly difficult to control. The use of somatostatin has been advocated recently in such infants because of its effect on inhibiting insulin release, but nothing is known of the wider effects of this potent hormone in the young child. Two infants presenting at 9 weeks and 5 days of age with severe hyperinsulinaemic hypoglycaemia were studied during an infusion of somatostatin. In both infants normoglycaemia was restored with suppression of insulin secretion. An increase in blood ketone bodies occurred, but no change was seen in blood

Effect of cooling on the glucoregulation of anaesthetized and non-anaesthetized newborn dogs.

UNLABELLED: When newborn dogs 2-14 days of age, anaesthetized with Nembutal, were cooled to 30 degrees C body core temperature, their rate of CO2 production (QCO2) decreased as predicted by van't Hoff's law, but the rates of hepatic glucose production and overall glucose utilization were decreased to a lesser degree, indicating an increase in the percentage contribution of plasma glucose to the overall energy turnover. In different litters of pups cooled to 32 degrees C, QCO2 and the rate of glucose production and utilization were decreased as predicted by van't Hoff's law. The metabolic clear
