[A case report of primary hypomagnesemia with secondary hypocalcemia caused by TRPM6 gene variants].

A 26-day-old male infant presented with recurrent convulsions from 18 days of life. Laboratory investigations revealed severe hypomagnesemia (0.07 mmol/L) and hypocalcemia (1.65 mmol/L). Whole-exome sequencing was performed and identified compound heterozygous pathogenic variants in the TRPM6 gene, comprising c.5616G>A (p.Trp1872Ter) and a deletion of exons 20-23. The c.5616G>A variant was inherited from the father, and the exon 20-23 deletion was inherited from the mother; neither variant has been previously reported. Based on these findings, the diagnosis of primary hypomagnesemia with secon

Perinatal and Neonatal Outcomes in Infants of Diabetic Mothers: A Prospective Descriptive Study in a Tertiary Care Center in South India.

Background Maternal diabetes is a well-recognized contributor to neonatal morbidity. Infants born to mothers with gestational diabetes mellitus (GDM) and pregestational diabetes are predisposed to metabolic, respiratory, and hematological disturbances. Understanding the distribution of these outcomes is important for optimizing neonatal care. Objectives To describe neonatal outcomes, including metabolic, respiratory, hematological, and anthropometric parameters, along with perinatal factors and maternal demographic and clinical characteristics among infants of diabetic mothers in a tertiary ca

Persistent Neonatal Hypocalcemia as an Early Clue to TCIRG1-Related Infantile Osteopetrosis in an Infant of a Diabetic Mother: A Case Report.

Infantile malignant osteopetrosis is a rare autosomal recessive skeletal disorder caused by impaired osteoclast-mediated bone resorption, resulting in diffuse osteosclerosis, bone marrow failure, neurologic complications, and metabolic disturbances. Early diagnosis remains challenging, particularly when initial manifestations overlap with common neonatal conditions. Persistent neonatal hypocalcemia may be an early but underrecognized clue. We report a Saudi male infant with genetically confirmed TCIRG1-associated osteopetrosis who presented with persistent neonatal hypocalcemia and secondary h

Targeted removal of soluble Fms-like tyrosine kinase 1 in very preterm preeclampsia: a pilot trial.

Soluble Fms-like tyrosine kinase 1 (sFlt-1), a protein secreted by the placenta, plays a central role in the pathogenesis of preeclampsia-a life-threatening pregnancy complication for which no disease-specific treatment currently exists. We developed a strategy to selectively deplete circulating sFlt-1 and then conducted a single-arm, open-label trial to reduce circulating sFlt-1 in women with very preterm preeclampsia. The primary endpoints were safety and tolerability. Extracorporeal apheresis with an adsorber containing high-affinity IgG1 antibodies against sFlt-1 resulted in an approximate

Clinical and genetic characterization of patients with Digeorge syndrome: a single-center, first report from Sudan.

BACKGROUND: DiGeorge syndrome (DGS; OMIM #188400), also known as 22q11.2 deletion syndrome, is characterized by cardiac defects, abnormal facial features, thymic hypoplasia, cleft palate, and hypocalcemia. The syndrome is ranked as the second most common chromosomal change after Down’s syndrome, accounting for 1 in 2000 newborns. DGS syndrome is typically diagnosed through CGH and/or FISH analysis in developed countries. However, in low-resource healthcare settings, diagnosis often relies primarily on clinical manifestations due to limited access to genetic testing. The present study, the firs

Non-oliguric hyperkalemia in neonates less than 30 weeks of gestation: clinical characteristics and outcomes.

Hyperkalemia is a frequent electrolyte imbalance in very preterm infants, often occurring in the first 72 hours of life. A unique entity, non-oliguric hyperkalemia (NOHK), is characterized by elevated serum potassium despite adequate urine output. While frequently reported, its risk factors, biochemical profile, and outcomes remain inadequately understood, particularly in low- and middle-income countries. To determine the incidence, maternal and neonatal risk factors, biochemical profile, and outcomes associated with NOHK in preterm neonates between 26+0 and 30+0 weeks' gestation, we conducted

Factors associated with early onset hypocalcemia: A retrospective cohort study.

BACKGROUND: Early onset hypocalcemia, occurring within 3 days of birth, is prevalent among preterm infants. A central line is required to deliver calcium (Ca). The prediction of hypocalcemia is therefore clinically important when the requirement for initial intravascular calcium administration is anticipated. METHODS: This retrospective study included preterm infants born between 2019 and 2023 with a birthweight <1500 g or a gestational age (GA) <32 weeks in a single tertiary center. Hypocalcemia was defined as serum Ca <7 mg/dL or ionized calcium (iCa) <0.8 mmol/L within 72 h after birth. Pe

Neonatal Non-metabolic Seizure Distribution in Relation to Biochemical Abnormalities.

Introduction Neonatal seizures are a significant neurological manifestation in the early days of life. They can cause long-term developmental disabilities and increase mortality rates if not properly managed. They can also be a sign of underlying neurological disorders, metabolic disturbances, or hypoxic-ischemic injury. Among the numerous etiologies, birth asphyxia, neonatal meningitis, and biochemical abnormalities are widespread. Biochemical abnormalities occur either as an underlying cause or as an associated abnormality. So biochemical anomalies should be excluded in every case of neonata

Tracheal Compression on Chest X-ray Leading to the Diagnosis of Right Aortic Arch in 22q11.2 Deletion Syndrome.

22q11.2 deletion syndrome (22q11.2DS) is a relatively common disorder in pediatrics, frequently presenting with immune dysfunction, cardiac malformations, and hypoparathyroidism, all stemming from the development of the fourth branchial arch. While intracardiac malformations, such as tetralogy of Fallot, are highly prevalent, aortic abnormalities are also common. We report the case of a male diagnosed with 22q11.2DS at 12 years of age. His medical history included neonatal hypocalcemia, developmental delays, and later-onset tetany due to hypoparathyroidism. Despite a normal echocardiogram, a c

Diluted Regional Citrate Anticoagulation for Continuous Renal Replacement Therapy in Pediatric Patients: A Systematic Review and Suggested Practice Points.

INTRODUCTION: Continuous renal replacement therapy (CRRT) is increasingly used in critical pediatric patients with acute kidney injury (AKI). The choice of anticoagulant is vital to minimize circuit clotting and bleeding complications. Regional citrate anticoagulation (RCA) is preferred for its safety profile, particularly in critically ill pediatric patients who are susceptible to bleeding. METHODS: A comprehensive literature search was conducted using PubMed, Google Scholar, and Cochrane databases following PRISMA guidelines. Keywords included "diluted citrate," "regional citrate anticoagul

Hyperventilation Syndrome in a Child: Electrolyte Disturbances and Cardiac Involvement in Anxiety-Related Presentations.

Background: Hyperventilation Syndrome (HVS) is a well-recognized physiological consequence of acute anxiety, often resulting in respiratory alkalosis and subsequent electrolyte imbalances. Among these, a reduction in ionized calcium levels can lead to neuromuscular irritability and electrocardiographic abnormalities such as QTc prolongation. Although well-documented in specific settings, including autism spectrum disorders and drug-induced crises, such complications are rarely described in otherwise healthy pediatric patients presenting with isolated anxiety episodes. This report aims to raise

Pseudohypoparathyroidism: Challenges in Early Recognition and Diagnosis of a Rare Hereditary Disorder.

Pseudohypoparathyroidism (PHP) is an uncommon endocrine condition marked by an impaired response to parathyroid hormone (PTH), which results in biochemical abnormalities. Clinical manifestations can vary significantly, occasionally resembling other endocrine disorders. Genetic testing plays a critical role in distinguishing PHP from other conditions, as it enables precise diagnosis even when classical features are not initially present. We report the case of a male infant initially diagnosed with congenital hypothyroidism (CH) through neonatal screening and treated with levothyroxine. While gr
