Intraosseous Schwannoma of the Spine: A Report of Three Cases.

BACKGROUND: Asparagus (Asparagus officinalis) is an extensively grown and consumed vegetable. To a lesser extent than other Liliaceae vegetables, allergic contact dermatitis (ACD) due to asparagus has been reported. However, only a few case reports of asparagus IgE-mediated allergy have been published. In a previous study, we demonstrated that two lipid transfer proteins (LTPs) (Aspa o 1.01 and Aspa o 1.02) were relevant allergens of asparagus. OBJECTIVE: We retrospectively analysed the 27 patients diagnosed with asparagus allergy during the last 5 years. All of them reported adverse symptoms

The Use of Biologics for Thyroid Eye Disease.

Thyroid eye disease (TED) is the most common extrathyroidal manifestation of Graves' disease. It is an autoimmune disorder that may present with signs of inflammation and extracellular matrix modification, leading to the characteristic features of swelling within the orbit, proptosis, diplopia, and vision loss. Recently, a growing body of work has focused on novel therapies for the treatment of TED. We review novel therapeutic agents aimed at treating TED. A review of the literature was performed through search of PubMed, Scopus, and Web of Science databases for terms related to the treatment

The evolving landscape of thyroid eye disease: present and future.

Thyroid eye disease (TED) is a complex ocular autoimmune disorder primarily associated with Graves' disease. It leads to significant morbidity due to orbital inflammation, fibrosis, and tissue expansion. While corticosteroids have been the traditional mainstay of therapy, recent advancements in understanding TED pathophysiology have driven the development of targeted treatments. Notably, inhibition of the insulin-like growth factor-1 receptor with teprotumumab has revolutionized TED management, demonstrating efficacy in reducing proptosis and disease severity. Additional emerging therapies, in

Longitudinal phage-bacteria dynamics in the early life gut microbiome.

Microbial colonization of the human gut occurs soon after birth, proceeds through well-studied phases and is affected by lifestyle and other factors. Less is known about phage community dynamics during infant gut colonization due to small study sizes, an inability to leverage large databases and a lack of appropriate bioinformatics tools. Here we reanalysed whole microbial community shotgun sequencing data of 12,262 longitudinal samples from 887 children from four countries across four years of life as part of the The Environmental Determinants of Diabetes in the Young (TEDDY) study. We develo

Self-care interventions among women with gestational diabetes mellitus in low and middle-income countries: a scoping review.

BACKGROUND: Gestational diabetes mellitus (GDM) is a transitory form of diabetes occurring in pregnancy with maternal and neonatal health consequences if left untreated. GDM can, in most instances, be managed non-medically through self-care practices, such as eating healthy or engaging in physical activity. This is especially relevant in a global health context with scarce resources. There is no official definition of "GDM self-care"; hence, the content and delivery modes of such interventions may vary greatly. Therefore, this study aimed to landscape GDM self-care interventions in low- and mi

[Congenital hearing loss in children].

The identification of congenital hearing loss using Transient Evoked Otoacoustic Emissions and Automatic Auditory Brainstem Response in a newborn hearing screening program is crucial for initiating early rehabilitation with hearing aids or cochlear implants. Specific genetic causes, such as Pendred syndrome, connexin-26, stereocilin, and otoferlin-associated deafness, can be identified today using gene panels. Specifically, for otoferlin-associated deafness, it may be possible to offer gene therapy as a novel treatment for this specific genetic type of hearing loss.

High maternal serum thyroid-stimulating hormone receptor antibodies with fetal and neonatal thyrotoxicosis after total thyroidectomy for Graves' disease.

High thyroid-stimulating hormone receptor antibodies (TSHRAbs) during pregnancy pose clinical challenges, especially for women with a history of thyroidectomy or radioactive iodine therapy. We report a pregnant woman in her 30s with a history of Graves' disease, managed by total thyroidectomy and thyroid replacement therapy 10 years before presentation. Despite lacking a thyroid gland, she had elevated levels of TSHRAb early in her pregnancy. The fetus experienced early-onset intrauterine growth restriction; at 31 weeks gestation, demonstrated signs of thyrotoxicosis, including persistent abno

Thyroid disease as a teratogen.

Maternal thyroid hormones are critical for normal fetal development, particularly brain development. The increase in metabolic demand of pregnancy results in critical physiologic thyroid changes which ultimately lead to an increase in thyroid hormone production. Placental iodothyronine deiodinases tightly regulate the transfer of thyroid hormone to the fetus. As such, too little or excess thyroid hormone can have a teratogenic effect. Therefore, diagnosis and management of thyroid disorders in pregnancy is necessary to prevent adverse maternal, fetal, and neonatal outcomes; however, it remains

Prediction of fetal Graves' disease among pregnant women with Graves' disease who have undergone thyroidectomy or radioactive iodine therapy: A retrospective observational study.

Pregnant women with Graves' disease (GD) who have undergone thyroidectomy or radioactive iodine therapy can have high levels of thyroid-stimulating hormone (TSH) receptor antibodies, which are transferred to the fetus via the placenta, posing a risk for fetal GD. This retrospective observational study, conducted at two high-level perinatal medical centers in Tokyo and Osaka, Japan, aimed to identify predictors of fetal GD in pregnant women with GD who had undergone thyroidectomy or radioactive iodine therapy. In total, 65 women were included, and 79 singleton pregnancies and fetuses were analy

Targeted immunotherapies for Graves' thyroidal & orbital diseases.

BACKGROUND: Graves' hyperthyroidism and its associated Graves' orbitopathy are common autoimmune disorders associated with significant adverse health impact. Current standard treatments have limitations regarding efficacy and safety, and most do not specifically target the pathogenic mechanisms. We aim to review the latest development of targeted immunotherapies in these two closely related disorders. SUMMARY: Targeted immunotherapies of Graves' hyperthyroidism have recently demonstrated clinical efficacy in early phase clinical studies. They include rituximab, an anti-CD20 monoclonal antibod

Airway compression in a neonate with thyroid dyshormonogenesis due to SLC26A7 defect.

SLC26A7 encodes an apical iodide transporter in thyroid follicular cells and causes, when mutated, congenital hypothyroidism (CH). We report a patient diagnosed with goitrous CH, who initially presented with neck enlargement and inspiratory stridor on day 3 of life. Levothyroxine therapy resulted in prompt improvement in upper airway compression, and surgical interventions were avoided. The levothyroxine dosage per body weight was gradually reduced to 0.5 μg/kg/day by age 11 months. Genomic sequencing of CH-related genes identified compound heterozygous protein-truncating SLC26A7 variants: c.6

Frequency of Congenital Hypothyroidism and Radiographic Assessment of Knee Epiphysis in Newborn.

Congenital hypothyroidism (CH) is the commonest preventable cause of mental retardation in newborn. It is more prevalent in endemic goiter regions like Bangladesh. The clinical manifestations are often subtle or absent at birth. As early clinical diagnosis is difficult without newborn screening, most cases of congenital hypothyroidism remain undetected before three months of age. Before the appearance of clinical manifestation, CH can be identified by a simple knee X-ray. This cross-sectional observational study was conducted in the department of pediatrics, Rajshahi Medical College Hospital,

Concomitant Mutations in the Thyroglobulin and SLC26A4 Genes Leading to Fetal Goiter and Congenital Hypothyroidism in a Patient With Pendred Syndrome.

We described new forms of thyroglobulin gene (TG) mutation resulting in fetal goiter and congenital hypothyroidism in a pendred syndrome (PS) patient. Fetal hypothyroidism was diagnosed, based on ultrasonographic evidence of goiter alongside with fetal blood measurement of TSH (>100 mIU/L); levothyroxine intrauterine treatment was performed. At birth, ultrasound thyroid enlargement and primary hypothyroidism were confirmed. On Day 11, hearing screening using a combination of otoacoustic emissions and auditory brainstem response revealed profound bilateral sensorineural deafness. Genetic analys
