Management of pregnancy in patients with Graves' disease secondary to alemtuzumab: a case series and literature review.

INTRODUCTION: Alemtuzumab treatment for multiple sclerosis frequently induces Graves' disease (GD), characterized by common coexistence of stimulating and blocking anti-thyrotropin receptor autoantibodies (TRAb), causing fluctuations of thyroid function. Alemtuzumab-induced TRAb-positive hypothyroidism is also common. These conditions are challenging to manage during pregnancy, considering the imperative necessity to maintain euthyroidism to minimize risks for both pregnancy and fetal/neonatal outcomes. METHODS: We reviewed the clinical course and therapeutic management of four pregnant women

Euthyroidism and Long-Term Remission of Graves' Hyperthyroidism Following Treatment With an FcRn Blocker: A Case Report.

BACKGROUND: Neonatal fragment crystallizable receptor (FcRn) blockers selectively inhibit FcRn-mediated IgG recycling, resulting in degradation of IgG autoantibodies, including thyrotropin receptor autoantibodies (TSH-R-Ab). METHODS: We describe a case involving a 55-year-old woman with a 17-month history of Graves' disease (GD) who was hyperthyroid despite 15 months on methimazole (MMI) 15-25 mg/day. She received the investigational FcRn blocker batoclimab in a clinical study and was subsequently followed in clinic. RESULTS: Within 1 week of starting weekly subcutaneous batoclimab, serum fr

Neonatal hyperthyroidism with myocardial injury: a case report.

Neonatal hyperthyroidism (NH) is a rarely reported condition, but inappropriate treatment can have serious consequences. Early diagnosis, treatment, and follow-up are, therefore, essential. We present the case of a male newborn who was admitted to the Pediatrics Department 24 min after birth due to prematurity, presenting with tachypnea and grunting for 20 min. Subsequently, the infant developed increased respiratory rate, tachycardia, crying, and irritability. Bilateral periorbital edema and hyperdynamic heart sounds were also noted. Myocardial injury markers were significantly elevated, indi

Maternal TSH-receptor antibodies predict fetal/neonatal hyperthyroidism in pregnancies with a history of Graves disease.

CONTEXT: Transplacental transfer of thyrotropin receptor antibodies (TRAbs) during pregnancy may cause fetal/neonatal thyroid dysfunction. Current guidelines recommend a universal TRAb titer threshold, irrespective of gestational age or assay platform. However, evidence supporting trimester-specific and assay-specific thresholds is lacking. OBJECTIVE: This work aimed to determine whether trimester-specific TRAb levels predict fetal/neonatal thyroid dysfunction and to establish assay-specific cutoff values for clinical risk stratification. METHODS: In this prospective, multicenter, observatio

Early predictors of 6-month thyroid-stimulating antibody levels after radioactive iodine therapy for Graves' disease: implications for pregnancy planning.

Current guidelines recommend a 6-month contraception period following radioactive iodine therapy (RAIT) for Graves' disease (GD), based primarily on radiation safety considerations; however, they do not account for the post-treatment surge in thyroid-stimulating antibodies (TSAb), which poses risks for fetal and neonatal GD. This retrospective study aimed to identify factors influencing TSAb trajectories and construct a predictive model for TSAb levels at 6 months after RAIT. We analyzed 82 patients who underwent RAIT, with TSAb values standardized to the upper limit of normal (ULN) to account

Neonatal Surveillance Gaps in Maternal Graves' Disease: Findings From a Queensland Maternity Hospital.

BACKGROUND: Graves' disease presents unique challenges in pregnancy due to the risk of neonatal morbidity and mortality from maternal TSH Receptor antibodies (TRAb). A maternity hospital in Queensland has implemented a policy to identify at-risk neonates and facilitate follow-up, including cord blood TRAb, thyroid function tests (TFTs), and clinic appointments. AIMS: To evaluate current neonatal follow-up practices for mothers with Graves' disease, assess missed opportunities for biochemical testing and propose recommendations to improve clinical care. MATERIALS AND METHODS: This retrospecti

Effects of lactoferrin-enriched dairy product on iron status in women with gestational diabetes mellitus.

Lactoferrin (LF) is an iron-binding glycoprotein with anti-inflammatory and microbiota-modulating properties, but its effects in gestational diabetes mellitus (GDM) remain unclear. In this double-blind randomized pilot trial (NCT07287384, registered on 11 December 2025), 50 women with GDM (24-30 weeks' gestation) were assigned to receive either an LF-enriched yogurt (100 mg LF/125 g, two servings/day; n = 25) or a control yogurt (n = 25) for eight weeks, alongside 30 mg/day elemental iron. The primary outcome was ferritin change; secondary outcomes included hematologic, metabolic, microbiota,

Maternal Third-Trimester TRAb Is Associated with Neonatal Thyroid Dysfunction and 24-Month Offspring Neurodevelopment: A Bidirectional Cohort Study.

BACKGROUND: Maternal Graves' disease (GD) has been linked to neonatal thyroid dysfunction, but its relationship with offspring neurodevelopment remains unclear. We examined the associations of maternal GD-related thyroid factors with neonatal thyroid function and neurodevelopment at 24 months. We also explored whether neonatal thyroid function might partly explain this association. METHODS: This single-center bidirectional cohort study included pregnant women with GD and their offspring delivered between January 1, 2019, and December 31, 2023. Maternal thyroid-related variables, including thy

Neonatal thyrotoxicosis secondary to maternal graves' disease: a case report.

INTRODUCTION: Neonatal thyrotoxicosis is a rare condition that often results from maternal antibody transfer, particularly in mothers who have had Graves’ disease. This disorder causes excessive thyroid hormone production in newborns. It can present with symptoms like tachycardia, poor weight gain, irritability, and, in severe cases, heart failure. Maintaining a high index of suspicion, making a prompt diagnosis, and intervening immediately are crucial. These steps help to mitigate the risk of long-term or serious complications. CASE HISTORY: We present a neonate with late-onset neonatal thyro

Neonatal Hyperthyroidism: Case Report and Literature Review.

Fetal and neonatal hyperthyroidism are rare, but severe, conditions caused by transplacental passage of thyrotropin receptor antibodies from a pregnant patient with current or past Graves disease. We present the case of a late preterm infant who developed thyrotoxicosis secondary to maternal Graves disease treated with radioactive iodine, followed by a review of the literature, clinical implications, and best practices on the diagnosis and treatment of neonatal hyperthyroidism. We highlight the importance of finding the underlying cause for hypothyroidism during pregnancy, ensuring those with

Graves' disease: a new era of pathophysiology-guided therapeutics.

Graves' disease (GD) is an autoimmune entity that had an unchanged treatment paradigm for more than half a century with radioactive iodine, antithyroid drugs (ATDs), and surgery representing the mainstay of therapy, with variation in their use over time and geographic location. Hypothyroidism has been an accepted tradeoff in many cases, though recently there are increased concerns about altered quality of life in these patients. Fortunately, the last decade has seen an intense pursuit of GD's immunological mechanisms and a recalibration of classic treatment modalities focusing on thyroid prese

Thyroid dyshormonogenesis caused by iodotyrosine deiodinase pathogenic variant: three cases presenting in adolescence.

We describe three patients from three unrelated consanguineous families, all homozygous for the c.301C>T (p.Arg101Trp) variant in the iodotyrosine deiodinase (IYD) gene. These patients presented in late childhood around puberty with diffuse goiter, primary hypothyroidism, and negative thyroid autoantibodies, consistent with thyroid dyshormonogenesis (DH). Initiation of levothyroxine therapy led to a significant reduction in goiter size and clinical improvement. Notably, all patients had normal thyroid function at neonatal screening, suggesting a delayed clinical manifestation of congenital hyp

A novel homozygous thyroglobulin gene variant presenting with massive congenital goiter and neonatal airway obstruction.

Congenital hypothyroidism due to thyroid dyshormonogenesis is a rare inherited disorder that may present with goiter of variable severity. Massive congenital goiter causing life-threatening respiratory distress at birth is exceptionally uncommon. We report a late-preterm male neonate who presented immediately after delivery with severe respiratory failure requiring endotracheal intubation due to critical tracheal compression caused by a massive congenital goiter. Postnatal imaging revealed marked thyroid hyperplasia with airway narrowing. Thyroid function tests demonstrated isolated low free t

New horizons for autoimmune hyperthyroidism with FcRn blockers.

PURPOSE OF REVIEW: Advances in understanding of autoimmune hyperthyroidism and neonatal Fc receptor (FcRn) biology have created an opportunity for a novel immunomodulatory treatment approach. This review is timely in evaluating newly emerged literature for FcRn blockers Batoclimab (IMVT-1401), Imeroprubart (IMVT-1402) and Efgartigimod (ARGX-113) in Graves' disease and thyroid eye disease. RECENT FINDINGS: Preliminary phase 2 clinical trial data of Batoclimab therapy in Graves' disease demonstrated rapid normalisation of thyroid hormone levels with the majority maintaining this after the treat

Clinical Trial Updates in Biological Treatment of Thyroid Eye Disease.

Thyroid eye disease (TED) is the most common extrathyroidal manifestation of Graves' disease, causing proptosis, diplopia, and, in severe cases, vision loss. The 2020 approval of the first insulin-like growth factor 1 receptor (IGF-1R) inhibitor for TED introduced disease-specific pharmacotherapy and prompted accelerated development of additional biological agents across multiple mechanistic classes. This review summarizes the clinical trial evidence for biological agents currently approved or in late-stage development for TED, drawing on peer-reviewed publications, conference presentations, a

Independent Association Between Lumbar Intervertebral Disc Degeneration and Fatty Degeneration of Paravertebral Muscles.

Malaria is an infectious disease caused by parasitic protozoans of the genus Plasmodium. Malaria control efforts on a global scale are in danger due to the emergence and spread of drug-resistant malaria. Despite stakeholders' dedication to the prevention and treatment of malaria, the current state of global health does not offer an effective answer to the issue of drug resistance. Furthermore, there is an information gap about the molecular mechanisms of Plasmodium falciparum's drug resistance, which makes it difficult to develop monitoring systems. Most countries lack adequate and comprehensi
