Isolated Cerebral Mucormycosis Mimicking Meningioma in an Uncontrolled Diabetic Patient: A Case Report.

In a poorly controlled diabetic patient, isolated cerebral mucormycosis can mimic a meningioma on magnetic resonance imaging. The absence of a dural attachment at surgery was the first clue against meningioma, and histopathology was diagnostic. Early resection, antifungal therapy, and glycemic control were associated with a favorable outcome.

Reconsidering Granulocyte Infusions in Prolonged Neutropenia: A Case Report.

A 39-year-old man with therapy-related acute myeloid leukemia (t-AML) had refractory disease after induction with 7+3 (cytarabine for seven days and daunorubicin for three days). Reinduction with FLAG-Ida (fludarabine, cytarabine, G-CSF, and idarubicin) resulted in a hypocellular bone marrow (<5%) and prolonged pancytopenia complicated by mucormycosis. He received granulocyte colony-stimulating factor (G-CSF) and granulocyte-macrophage colony-stimulating factor (GM-CSF) without improvement, leading to the decision to implement granulocyte infusions. There was temporary and mild improvement in

Adjunctive palivizumab with ribavirin for respiratory syncytial virus infection during the early peri-transplant period in adult allogenic hematopoietic stem cell transplant recipients: Case series.

BACKGROUND: Respiratory syncytial virus (RSV) is associated with significant morbidity and mortality in hematopoietic stem cell transplant (HSCT) recipients, particularly during the peri-transplant period. Therapeutic options remain limited, and the role of adjunctive RSV-specific immunotherapy is unclear. We evaluated the outcomes of palivizumab combined with ribavirin in high-risk adult HSCT recipients with RSV infection. METHODS: We retrospectively reviewed adult HSCT recipients with RT-PCR-confirmed RSV infection who received palivizumab and ribavirin. Clinical characteristics, treatment

Clinical and Economic Trade-Offs in Antifungal Therapy for Invasive Aspergillosis and Mucormycosis: A Markov Model-Based Analysis.

Invasive aspergillosis and mucormycosis are life-threatening fungal infections characterized by high early mortality, substantial toxicity, and intensive healthcare resource utilization. While multiple antifungal therapies are recommended in international guidelines, their comparative clinical and economic value remains uncertain, particularly in resource-limited settings. We developed a hybrid decision tree and Markov model to evaluate antifungal strategies, including liposomal amphotericin B, isavuconazole, voriconazole, posaconazole, and caspofungin, from the perspective of the Colombian he

Dual Invasive Fungal Infections After Kidney Transplantation: A Case Report.

Donor-derived infections (DDIs), particularly fungal DDIs, are uncommon but serious complications of solid organ transplantation. We report a case of a 52-year-old woman who underwent deceased donor kidney transplantation complicated by probable donor-derived Candida albicans candidemia with native aortic valve endocarditis, managed medically with prolonged echinocandin therapy followed by suppressive fluconazole. Approximately 14 months post-transplant, she developed progressive rhino-orbital mucormycosis due to Rhizopus oryzae, requiring extensive surgical debridement and prolonged antifunga

Angioinvasion severity and admission GCS predict outcomes in invasive central nervous system fungal infections.

BACKGROUND: Central nervous system (CNS) invasive fungal infections caused by Aspergillus spp. and Mucorales are associated with high mortality and severe neurological disability, but predictors of functional outcome remain poorly defined. We compared clinical features, neuroimaging patterns, and outcomes among rhino-orbital-cerebral mucormycosis (ROCM), rhino-cerebral aspergillosis (RA), and cerebral aspergillosis (CA), and identified prognostic factors for mortality and functional outcome. METHODS: We performed a retrospective cohort study of 57 adults with proven or probable invasive CNS f

Delayed Diagnosis of Rhino-Orbito-Facial Mucormycosis in a Patient With Newly Diagnosed Diabetes and Diabetic Ketoacidosis: A Successful Case From a Resource-Limited Setting.

BACKGROUND: Rhino-orbito-facial mucormycosis is a rapidly progressive, life-threatening fungal infection that predominantly affects patients with uncontrolled diabetes mellitus, particularly those with diabetic ketoacidosis (DKA). Early diagnosis remains challenging in resource-limited settings because initial manifestations often mimic bacterial sinusitis. CASE PRESENTATION: We report a 38-year-old woman with newly diagnosed diabetes mellitus presenting with left facial pain, swelling, and nasal discharge accompanied by mild DKA. She was initially treated for bacterial maxillary sinusitis wi

Three hydrophobic surface binding a proteins link surface hydrophobicity to sporulation, biofilm formation, and host interaction in Mucor lusitanicus.

INTRODUCTION: Mucor lusitanicus is a model organism for studying fungal development and physiology, as well as pathogenicity of Mucorales fungi. Hydrophobic surface-binding proteins (HsbA family) have been described in filamentous fungi as interface-associated factors involved in adhesion, enzymatic recruitment, and surface interactions; however, their functional diversification in Mucorales remains poorly understood. METHODS: Here, we present a comprehensive characterization of three HsbA proteins in M. lusitanicus. RESULTS: All examined HsbA proteins share a conserved α-helical fold with a

Recent Therapeutic Advances and Emerging Repurposing Strategies of Posaconazole: A Comprehensive Review.

Globally, fungal diseases exert a considerable toll on human health, exhibiting wideranging epidemiological patterns across geographic locales. In the post-COVID era, the prevalence and risk of fungal infections have grown at an unprecedented pace, posing a colossal challenge to people with impaired immune systems owing to indiscriminate use of immunosuppressants and anti-fungal agents. In recent times, posaconazole (PCZ), a triazole with broad-spectrum activity, has emerged as a promising therapeutic option for the efficacious management of invasive fungal infections caused by Candida spp., A

Microvascular Osteocutaneous Free-Flap Reconstruction of Complex Maxillary Defects: A Two-Year Single-Center Experience.

Maxillary defects resulting from oncologic resection, severe trauma, or invasive infections such as mucormycosis present significant reconstructive challenges due to the need to restore midfacial support, oral competence, speech, mastication, and facial aesthetics. Microvascular osteocutaneous free flaps have expanded reconstructive options by providing vascularized bone and soft tissue for simultaneous restoration of structural support and intraoral lining. This report describes our reconstructive experience in three patients who underwent microvascular reconstruction of complex maxillary def

Computer-Aided Design and Computer-Aided Manufacturing (CAD-CAM)-Assisted Telescopic Overdenture for a Post-mucormycosis Maxillary Defect: A Case Report.

Post-COVID-19 mucormycosis may result in acquired maxillary defects that compromise mastication, speech, facial support, and separation between the oral cavity and sinonasal spaces. Rehabilitation becomes particularly challenging when the defect is associated with persistent oroantral communication and nasal regurgitation. This clinical report describes the prosthodontic rehabilitation of a 66-year-old man with an Aramany Class II right posterior maxillary defect following mucormycosis. Teeth 11, 12, and 17 were selected as strategic abutments. A staged approach was followed, beginning with a

Clinically Important Emerging and Uncommon Fungal Pathogens in Asia: Epidemiology, Antifungal Resistance and Challenges in Diagnosis and Management.

Fungal infections in Asia are influenced by warm, humid climates, high population density and expanding healthcare systems, which together facilitate the emergence, transmission and detection of both common and rare pathogens. While Candida and Aspergillus species remain the leading causes of invasive fungal disease, increasing attention has focused on uncommon yeasts and rare moulds, many of which show intrinsic or acquired antifungal resistance. Advances in molecular diagnostics and susceptibility testing have improved identification of emerging pathogens, including uncommon Candida species

Experimental assessment of first-generation HsbA-derived peptide antibodies and independent computational prioritization of candidate epitopes with predicted discriminatory potential for Lichtheimia corymbifera.

Hydrophobic surface-binding protein A (HsbA), a secreted fungal protein, was evaluated as a promising diagnostic target for Mucormycosis, a serious fungal infection. We tested (i) whether HsbA-derived antibodies show preferential binding to Lichtheimia corymbifera and (ii) identify HsbA-derived peptide regions as candidates for discriminating L. corymbifera from other species. Experimental assays against a panel of five Mucorales comparators, A. fumigatus, C. neoformans, five Candida species and three bacterial species showed that five of seven rabbit polyclonal antibody preparations raised ag

Kidney transplant outcomes in patients with complement dysregulation.

BACKGROUND: Complement dysregulation is a key mechanism underlying atypical hemolytic uremic syndrome and complement-mediated thrombotic microangiopathy in kidney transplantation, and is associated with post-transplant recurrence, graft dysfunction, and graft loss. Although Western cohort data are well established, integrated transplant outcome data combining genetic analysis, anti-factor H (AFH) antibody profiling, and individualized therapy from Indian and other South Asian populations remain limited, despite a distinct biologic profile in this region. AIM: To evaluate clinical characterist

Development of an isavuconazole physiologically-based pharmacokinetic model for adult and pediatric populations.

Isavuconazonium sulfate is the water-soluble prodrug of the active triazole antifungal agent isavuconazole. Isavuconazonium sulfate is approved for the treatment of invasive aspergillosis and invasive mucormycosis in pediatric and adult populations. In the present analysis, an isavuconazole physiologically-based pharmacokinetic (PBPK) model was built and verified in adult and pediatric populations using the Simcyp® simulator. The verified pediatric model was then used to predict the magnitude of DDI risk in pediatric populations vs. adult populations. The simulations conducted were designed to
