Backbone resonance assignment of mucoricin: a step towards structural insights into mucor pathogenesis.

Mucoricin is a key virulence factor in mucormycosis, as it targets host endothelial cells and inhibits protein translation through its N-glycosylase activity, leading to vascular leak and increased vascular permeability, thereby contributing to further disease progression. Its structure is quite unique, being structurally homologous to the Ricin B chain as confirmed by cross-reactivity with monoclonal anti-Ricin B chain antibodies, while exhibiting enzymatic activity similar to the Ricin A chain. Here, we report near-complete backbone (Soliman in Nat Microbiol 6(3):313–326, 2021) 1H, 15N, and

Upper Jaw Mucormycosis in an Immunocompetent Patient: A Clinical Report.

Mucormycosis is a rare but severe opportunistic infection, whose incidence has substantially increased since the coronavirus disease 2019 (COVID-19) pandemic. It predominantly affects immunocompromised patients, individuals with diabetes, and those receiving corticosteroids. Standard treatment involves extensive debridement of affected tissue combined with prolonged antifungal therapy. We present a case of an immunocompetent patient with no identifiable risk factors who developed maxillary mucormycosis associated with COVID-19 illness. The condition responded favorably to minimal debridement a

Integrin ß4 is a receptor for emerging fungal pathogens from the genera Lomentospora and Scedosporium.

Lomentospora prolificans is an environmental fungus that can cause life-threatening infections when airborne conidia are inhaled. Airway epithelial cells are likely to be the first host cells to interact with L. prolificans during pulmonary infection; however, the fungal and host factors that govern this interaction are completely unknown. Herein, we combined whole fungal cell pulldowns of surface proteins from airway epithelial cells and liquid chromatography-mass spectrometry (LC-MS) to identify host proteins that could potentially serve as host receptors for the fungus. We provide evidence

Chronic Invasive Fungal Rhinosinusitis Managed With Surgical Debridement and Topical Amphotericin B Without Systemic Antifungal Therapy: A Report of Two Cases.

Chronic invasive fungal rhinosinusitis (CIFR) is a rare subtype of invasive fungal rhinosinusitis characterized by a slow clinical course and histopathologic evidence of tissue invasion. It is distinct from noninvasive fungal ball rhinosinusitis and acute invasive fungal rhinosinusitis. The standard treatment for CIFR consists of surgical debridement combined with systemic antifungal therapy. However, systemic antifungal agents may not be feasible in certain patients because of drug-related toxicity, impaired renal function, or socioeconomic constraints. In addition, the optimal role of topica

A BRCT domain-containing protein induced in early phagocytosis plays a crucial role in the pathogenesis of the mucoralean Rhizopus microsporus.

Mucormycosis, caused by mucoralean fungi, is among the most lethal fungal diseases, and a deeper understanding of its pathogenesis is urgently needed. Transcriptomic profiling of virulent strain (WT) and an RNAi-deficient strain (r3b2Δ) of M. lusitanicus during phagocytosis uncovered thousands of differentially expressed genes (DEGs), highlighting early metabolic activation as a key survival strategy inside the phagosome. Enriched pathways included amino acid transport, nucleotide metabolism, and translation, reflecting an adaptive fungal response to nutrient deprivation and host immune stress

Lineage-specific endosymbiosis in Mucorales: restriction of Mycetohabitans to the genus Rhizopus.

Endosymbiotic bacteria have been reported in mucoralean fungi, yet their taxonomic distribution, range of ecological niches, and host specificity remain incompletely understood. Clarifying the occurrence of these bacterial partners across clinical and environmental Mucorales is essential for understanding their evolutionary and biological significance. In this study, we screened 578 isolates of Mucorales from both clinical and fermented food sources, including 360 from mucormycosis patients in The Netherlands, 40 from COVID-19-associated mucormycosis (CAM) patients in India, and 178 foodborne

Albumin orchestrates a natural host defence mechanism against mucormycosis.

Mucormycosis is an emerging, life-threatening human infection caused by Mucorales fungi1-3. Metabolic disorders uniquely predispose an ever-expanding group of patients to mucormycosis through poorly understood mechanisms1,2,4,5, suggesting that uncharacterized host metabolic effectors may confer protective immunity against this infection. Here we uncover a master regulatory role of albumin in host defence against Mucorales through the modulation of fungal pathogenicity. Our initial studies identified severe hypoalb uminaemia as a prominent metabolic abnormality and an independent biomarker of

Indian Fungal Infection National Declaration (I-FIND): India's Response to Fungal Diseases.

BACKGROUND: Serious fungal diseases are significant yet long neglected public health problem in India, affecting an estimated 4.1% of the population and with the mortality from invasive fungal diseases approaching 50%, surpassing several nationally prioritised conditions. The emergence and spread of drug resistant pathogens such as Candidozyma auris formerly Candida auris, azole resistant Aspergillus fumigatus, and Trichophyton indotineae together with COVID-19 associated mucormycosis epidemic, exposes critical gaps in preparedness, prevention, and care. Despite the presence of a WHO collabora

Managing Breakthrough Fungal Infections in Hematologic Patients: Determinants and Practical Management from a Latin American Perspective on Behalf of INFOCUS LATAM-ISHAM Working Group.

Breakthrough invasive fungal infections (bIFIs) are a challenging serious complication in high-risk hematologic patients and allogeneic hematopoietic stem cell transplantation recipients that may negatively impact their outcome. Despite advances in antifungal prophylaxis, diagnostics, and supportive care, bIFI occurrence reflects a complex interaction between host immunosuppression, emergence of resistant pathogens and pharmacological variables, including subtherapeutic drug exposure. Candida spp. have shifted towards non-albicans yeasts, whereas breakthrough mold infections more frequently in

Characterization of isavuconazole pharmacokinetics and pharmacodynamics in a real-life cohort.

OBJECTIVES: Isavuconazole (ISA) is a triazole drug approved for the treatment of invasive aspergillosis and mucormycosis. Due to the relatively low interindividual variability of ISA pharmacokinetics and the lack of defined cutoffs for efficacy and toxicity, therapeutic drug monitoring (TDM) is rarely performed. This study aimed to characterize ISA pharmacokinetics and pharmacodynamics in real-life clinical settings and assess the potential role of TDM. METHODS: We developed and validated a classical population pharmacokinetic (popPK) model using ISA concentrations obtained from patients unde

Poor platelet engraftment combined with persistent pulmonary infection after haploidentical allogeneic hematopoietic stem cell transplantation: a case report.

Poor graft function (PGF) with persistent thrombocytopenia is a serious post-transplant complication linked to increased mortality. Active infections can worsen outcomes. We report a case of secondary poor platelet engraftment with refractory pulmonary infection after haploidentical HSCT. A 52-year-old woman developed Rhizopus microsporus infection on day +1. Neutrophil engraftment occurred by day +13, but platelet engraftment was delayed until day +40. She later developed Pseudomonas aeruginosa septic shock (day +106) and multidrug-resistant Klebsiella pneumoniae superinfection (day +190). Th

Genomic analyses of globally distributed Rhizopus microsporus populations indicate clinical isolates derived from environmental diversity reservoirs.

Mucormycosis is a group of diseases that is increasing in frequency. A common opportunistic human fungal pathogen in this group is Rhizopus microsporus, which is a globally distributed species present in soil-associated environments. A subset of isolates in this species host endobacteria that are hypothesized to influence fungal pathogenicity in both clinical and environmental settings. We have limited understanding of how clinically and environmentally derived isolates are related or how physiological attributes, including thermotolerance and endosymbiosis, are correlated with population stru
