Two decades of mucormycosis: molecular epidemiology and antifungal susceptibility trends in Türkiye.

BACKGROUND: Mucormycosis is a life-threatening invasive fungal infection that mainly affects individuals with weakened immune systems. Despite its clinical importance, data from Türkiye on the epidemiology and antifungal susceptibility of fungi belonging to the order Mucorales are scarce. This study aimed to describe the clinical, epidemiological, and mycological features of mucormycosis over a twenty-year period at a tertiary care hospital. METHODS: All cases of mucormycosis diagnosed between 2003 and 2022 at Bursa Uludağ University Hospital were retrospectively evaluated. Cases were classif

Unveiling mucormycosis in Brazil: Analysis of 298 cases using amphotericin B distribution data.

BACKGROUND: Since mucormycosis is not a notifiable disease in Brazil, its true incidence is likely underestimated; this study aims to assess its prevalence and characterize its epidemiological, clinical, and laboratory features. METHODS: We conducted a retrospective observational study of patients with mucormycosis treated with lipid amphotericin B (AmB) across 156 public healthcare centers in Brazil from 2017 to 2023, using Ministry of Health data. RESULTS: There were 305 AmB distributions for 298 patients: 230 (75 %) lipid complex (ABLC) and 75 (25 %) liposomal formulations (LAmB). The hig

Extranodal Natural Killer/T-Cell Lymphoma Mimicking Chronic Mucormycosis.

We describe a rare case of a 34-year-old woman who presented with progressive, destructive midline facial lesions initially concerning for chronic rhinofacial mucormycosis. However, subsequent tissue biopsy and pathologic evaluation established the diagnosis of extranodal NK/T-cell lymphoma. The patient demonstrated rapid disease progression despite prompt initiation of asparaginase-based chemotherapy, highlighting the diagnostic challenges and aggressive course of this malignancy that may resemble invasive fungal infection.

Systematic surgical reassessment combined with liposomal Amphotericin B: the MICA protocol.

BACKGROUND AND OBJECTIVE: Rhino-orbito-cerebral mucormycosis (ROCM) is a life-threatening fungal infection in immunocompromised or diabetic patients, with mortality between 56 and 73% when the brain is involved. Standard management often fails to halt progression. The MICA (French to summarize the therapeutic strategy combining Mucormycosis management with Invasiveness and Cranial involvement treated with surgery and high-dose Amphotericin B therapy), protocol evaluated whether systematic surgical reassessment combined with liposomal amphotericin B could improve outcomes. METHODS: Patients un

Light restores sporulation in Rhizopus microsporus cured of its endosymbionts, unveiling their role in fitness and virulence.

Rhizopus microsporus is a major cause of mucormycosis, an infection caused by Mucorales that increasingly affects immunocompromised individuals. Certain isolates of R. microsporus harbor bacterial endosymbionts that regulate key fungal functions, particularly asexual and sexual reproduction, but these effects have been explored exclusively in environmental isolates. Although some clinical isolates contain Mycetohabitans endosymbionts, their influence on fungal reproduction remains unknown. This dependence on endosymbionts for asexual spore formation in environmental isolates has established th

Novel infection by Mucor hiemalis kills Caenorhabditis hosts through intestinal perforation.

The nematode Caenorhabditis elegans has emerged as a popular model system to investigate cell biology and host-pathogen interactions. Presently, C. elegans is studied as a natural host of intracellular pathogens such as microsporidia and Orsay virus, along with extracellular bacterial and fungal pathogens. The use of C. elegans as a model in host-pathogen research is limited by the number of naturally occurring pathogens that infect this organism. Through a sampling project to identify new pathogens of C. elegans, we identified the fungus Mucor hiemalis as a pathogen of Caenorhabditis species.

Phosphatidylcholine biosynthesis via ChoC is crucial for cellular integrity and virulence in Rhizopus microsporus.

Rhizopus microsporus is an opportunistic fungal pathogen and a prominent etiological agent of mucormycosis, a life-threatening invasive fungal infection. In this study, we investigated the role of choC, a gene encoding a putative phospholipid methyltransferase involved in phosphatidylcholine (PC) biosynthesis through the Bremer-Greenberg pathway. Deletion of choC using CRISPR-Cas9 led to severe defects in growth and asexual development, which persisted even at 37°C, a temperature relevant to host infection. Supplementation with choline or its precursor PDEA restored growth, confirming the impo

Rhino-orbito-cerebral Rhizopus delemar infection in a patient with anti-melanoma differentiation-associated-5-positive dermatomyositis diagnosed by metagenomic next-generation sequencing: a case report.

BACKGROUND: Rhino-orbito-cerebral mucormycosis, caused by pathogens such as Rhizopus delemar, is a life-threatening opportunistic infection primarily affecting immunosuppressed individuals. Anti-melanoma differentiation-associated protein 5-positive dermatomyositis (MDA5+ DM) is a distinct subtype of DM associated with interstitial lung disease (ILD) and high mortality. Fungal co-infections in anti-MDA5+ DM, particularly mucormycosis, are rarely reported. CASE PRESENTATION: We report a case of rhino-orbito-cerebral mucormycosis caused by Rhizopus delemar in a patient with anti-MDA5+ DM. The pa

Renal mucormycosis caused by Apophysomyces species: case report and literature review.

INTRODUCTION: Mucormycosis represents an uncommon yet aggressive and life-threatening fungal infection, typically occurring in immunocompromised individuals. Unlike most Mucorales infections, Apophysomyces frequently infects otherwise healthy hosts, raising significant clinical concern. Alarmingly its atypical manifestation, necrotizing fasciitis, is often misdiagnosed as a bacterial infection due to overlapping clinical features. CLINICAL CASE: Herein we present a case of Apophysomyces infection in kidney in an immunocompetent 52-year-old man, whose condition deteriorated swiftly, leading to

Emergency staged Whipple procedure for gastroduodenal mucormycosis: a rare life-saving intervention.

Gastrointestinal (GI) mucormycosis is a rare, angioinvasive disease with a high mortality rate. To prevent dissemination of infection, early surgical resections of affected areas and initiation of antifungals are necessary. Unfortunately, due to its rarity, diagnosis, and treatment are often delayed. Here, we have described a case of a 22-year-old with gastroduodenal mucormycosis. Initially radiological findings were thought to be a sequela of changes for gastric volvulus or pancreatitis. GI mucormucosis was later confirmed with tissue histopathology. He underwent staged emergency Whipple's pr

Unveiling the hidden link: diabetes mellitus as a catalyst for orbital apex syndrome.

BACKGROUND: Orbital Apex Syndrome (OAS) is a disease of multiple brain nerves at the orbital apex leading to vision loss and neurological impairments. Diabetes Mellitus (DM), a metabolic disorder with cardiovascular, immunological and neurological effects, is involved in OAS pathogenesis. However, the association between DM and OAS is not well studied. DM and OAS are poorly understood and may not be diagnosed correctly, especially when outbreaks such as COVID-19 are being investigated. METHODS: A systematic review of 33 studies published between 2000 and 2025 was conducted to analyze DM-relat

Case Report: Intra-abdominal Mucormycosis Diagnosed at the Time of Liver Transplantation for Surgically Induced Acute Liver Failure.

Mucormycosis is an aggressive fungal infection associated with high mortality, particularly among recipients of solid organ transplants. Gastrointestinal involvement is rare but can be fatal due to delayed diagnosis and limited therapeutic options. We report on a 59-year-old female who developed intra-abdominal mucormycosis that was diagnosed during orthotopic liver transplantation. She initially presented with a perforated duodenal ulcer requiring emergent surgical repair and subsequently developed acute liver failure due to an iatrogenic transection of the portal triad. She underwent urgent

[Mucormycosis].

Mucormycosis has a poor prognosis in immunocompromised individuals. Rhino-orbital cerebral mucormycosis spreads directly from the nasal cavity to the paranasal sinuses and then to the adjacent organs, resulting in a wide range of symptoms and differential diagnoses. For patients with risk factors, it is important to consider this disease and proceed with early testing and diagnosis. Treatment involves three key elements: surgical debridement, administration of antifungal drugs, and control of the underlying disease. Amphotericin B liposomal preparations are the first choice of antifungal drugs

Limb Mucormycosis in the Early Postoperative Period Following Kidney Transplantation: A Case Report.

Post-transplant mucormycosis is a rare but life-threatening complication with high mortality. We report a severe case of upper limb mucormycosis in a 58-year-old diabetic kidney transplant recipient. Despite standard immunosuppression, progressive skin lesions developed on the left forearm. Initial debridement failed to arrest progression, revealing extensive muscle necrosis and vascular thrombosis. Rhizopus oryzae was confirmed by histopathology and culture. Surgical management required mid-humeral amputation followed by shoulder disarticulation due to persistent fungal invasion. Antifungal t

Perineural Spread in COVID-Associated Rhino-Orbito-Cerebral Mucormycosis: A Single Tertiary Care Center Study.

BACKGROUND: Infection with rhino-orbito-cerebral mucormycosis (ROCM) has increased in the coronavirus disease (COVID) era, especially in diabetic patients. Rapid extension from the nose and paranasal sinuses to the orbit and brain is noted by osseous erosion, vascular invasion, and infrequently documented perineural spread. OBJECTIVE: To determine the incidence of perineural spread in ROCM cases and evaluate the common neural pathways involved, which help in prognosticating and triaging patients. METHODS AND MATERIALS: We evaluated demographic, clinical, and radiological findings of 270 pati

Therapeutic potential of a polyherbal extract against COVID-19 and associated inflammation and mucormycosis: In vitro characterization and in vivo validation in a human coronavirus 229E-infected mouse model.

The rise of viral infections has been accompanied by an increased incidence of secondary opportunistic infections, such as mucormycosis, highlighting the urgent need for broad-spectrum therapeutics capable of targeting multiple pathological pathways. This study evaluates the antiviral, antioxidant, anti-inflammatory, and antifungal potential of a polyherbal extract (PHE) and its optimized self-nanoemulsifying intravenous formulation (PH-SNEDDS). PHE displayed strong free-radical-scavenging activity (DPPH, IC50: 21.99μg/mL; ABTS, IC50: 58.97μg/mL) and significantly downregulated pro-inflammator
