A comparison was made between 123 infants with pathologic antepartal cardiotocograms (CTG) during late pregnancy and a corresponding control group of matched individuals with regard to obstetric and neonatal data as well as the subsequent neuromotor development of the infants. After Norwood's initial report of successful first-stage palliation of hypoplastic left heart syndrome in neonates, an improved understanding of both the pathologic anatomy and physiology of this common condition has led to improving surgical results. Twenty-five neonates have been treated by modifications of the Norwood procedure, and the early mortality between January, 1984, and July, 1985, has been 24% (six deaths). Two premature neonates weighing less than 2 kg died, as did two neonates who could not be resuscitated with prostaglandin E1 and had cardiac arrests immediately preoperatively.

Between 1975 and 1985, 125 infants 2 to 365 days old (majority, 30 days old or less) with coarctation of the aorta underwent surgical repair. Forty-seven patients (38%) had severe congestive heart failure (CHF), metabolic acidosis, and poor systemic perfusion. From 1962 to mid-1984, 63 infants underwent coarctation repair. Cardiac defects were present in 46 (73%). Infants with D-loop transposition of the great arteries (D-TGA) may have unacceptable results after the balloon septostomy while awaiting surgery. It has been our policy to repair defects in infants with D-TGA and intact ventricular septum or small ventricular septal defect at the time of diagnosis. In spite of an adequate balloon atrial septostomy, some neonates (less than 28 days of age) with transposition of the great arteries and intact ventricular septum (TGA, IVS) remain hypoxemic and acidotic and require operative treatment. Fetal heart rate (FHR) and fetal acid-base status were studied prospectively in 61 patients in preterm labor of unknown etiology. Neonatal data were analyzed, and all surviving infants participated in a follow-up study for at least two years. Ominous FHR patterns correlated well with fetal acidosis.
