Development of an evidence based pediatric subcutaneous rehydration "hypodermoclysis" protocol for mildly dehydrated children with a history of difficult venous access.

BACKGROUND: Establishing peripheral intravenous access in children who present to a hospital setting with mild to moderate dehydration is often challenging. These children may not require hospitalization especially when unable to tolerate rehydration orally. An alternate access option for rehydration therapy is hypodermoclysis, commonly referred to as subcutaneous rehydration (SCR), where fluids can be infused and absorbed through the subcutaneous tissue. Unlike achieving peripheral vascular access, subcutaneous needle insertion is not complex and results in minimal venous attempts and discomf

A newborn with type 1 Bartter syndrome: challenges in the treatment and development during 30 months follow-up-a case report.

Type I Bartter syndrome is a rare autosomal recessive tubulopathy resulting from mutations in the SLC12A1 gene, leading to defective sodium-chloride reabsorption in the thick ascending limb of the loop of Henle. Affected neonates typically present with profound fluid and electrolyte disturbances, polyuria, and metabolic derangements. Early diagnosis and individualized management are crucial to prevent life-threatening complications and support appropriate growth and development. We report the case of a male preterm infant born at 33 weeks' gestation following a pregnancy complicated by severe

Incidence and Predictors of Necrotizing Enterocolitis Among Low-Birth-Weight Neonates Admitted to Southern Ethiopian Public Hospitals.

Necrotizing enterocolitis (NEC) has emerged over the past 25 years as the most common gastrointestinal emergency in neonatal intensive care units (NICUs) worldwide. Despite significant improvements in survival among premature and low-birth-weight (LBW) neonates due to advances in neonatal intensive care, the incidence of NEC remains persistently high, largely because of the physiological vulnerability associated with LBW and prematurity. The purpose of this study was to assess the incidence and identify predictors of NEC among LBW newborns in South Ethiopia. A hospital-based prospective cohort

Time to diagnosis of acute kidney injury and its predictors among outborn neonates in India: a competing-risk analysis of tele-ICU network data (2022-2024).

BACKGROUND: There is limited data on neonatal acute kidney injury (AKI) from resource-constrained settings. In this study, we describe time to AKI diagnosis and its predictors among neonates born outside the treating hospital (outborn) in India, accounting for competing events like death and discharge against medical advice (DAMA). METHODS: This multicentre retrospective study leveraged data from a tele-NICU network across 27 sites in India from May 2022-June 2024. It included neonates 28 days or younger at admission with two or more serum creatinine values measured within a seven-day span, b

Congenital aldosterone deficiency and its resistance.

Aldosterone synthase deficiency (ASD), which is caused by a genetic defect in CYP11B2, involves a deficiency in aldosterone alone. Newborn and early childhood ASD patients can present with salt-wasting symptoms. In severe cases, this can lead to shock and be life-threatening. ASD must also be differentiated from another disease, pseudohypoaldosteronism type 1 (PHA1), which involves resistance to aldosterone. PHA1 can be classified into PHA1a, PHA1b, and secondary PHA1. PHA1a is caused by a heterozygous defect in NR3C2, which encodes the mineralocorticoid receptor. PHA1b is an autosomal recessi

[Acid-base and potassium balance disorders in neonatal calf diarrhea - Part 1: Pathophysiologic aspects and clinical signs].

ABSTRACT: Neonatal diarrhea remains one of the most frequent causes of morbidity, mortality, and economic loss in calves worldwide. The clinical consequences in affected animals are primarily attributable to fluid losses and consecutive dehydration, but also to the occurrence of metabolic acidosis and associated imbalances in potassium homeostasis. In calves with neonatal diarrhea, metabolic acidosis mainly results from the accumulation of strong anions, in particular D-lactate, which arises from bacterial fermentation processes of undigested nutrients within the gastrointestinal tract. Increa

A growing crisis of neonatal hyperosmolar-hypernatremic dehydration during heat waves: clinical insights from a semi-arid region.

Hyperosmolar-hypernatremic dehydration (HHND) is a life-threatening yet preventable neonatal condition, often due to inadequate breastfeeding. The recent North Indian heat wave heightened dehydration risks, necessitating an evaluation of extreme temperatures' impact on neonatal hydration. This retrospective study analysed neonates admitted to a tertiary care level 3 neonatal intensive care unit (NICU) at AIIMS Jodhpur between April and June 2024. Case records were reviewed, and details on maternal age, feeding practices, presenting complaints, biochemical profile, and outcome were studied. The

Epidermolysis bullosa accompanied by long-segment gastric outlet obstruction in a newborn: a report of a rare case.

Epidermolysis bullosa (EB) is a rare congenital disorder caused by mutations affecting structural proteins of the epidermis and dermo-epidermal junction, resulting in extreme skin and mucosal fragility, blistering with minimal trauma, and impaired wound healing. Gastric outlet obstruction (GOO) in newborns arises from disruption of normal gastric emptying and may cause poor feeding, weight loss, severe vomiting, dehydration, and electrolyte imbalance. Long-segment gastroduodenal atresia is a rare subtype of GOO in which the blockage spans a continuous region from the distal stomach into the pr

Harnessing Sunlight Safely for Newborn Care: Balancing Risks and Benefits in Resource-Limited Settings.

Growing global interest in solar energy has renewed attention to sunlight exposure and its health effects. Although sunlight contains harmful ultraviolet radiation (UVR), controlled exposure has preventive and therapeutic benefits. In many low- and middle-income countries, particularly in sub-Saharan Africa and parts of Asia, newborns are traditionally exposed to early-morning sunlight to prevent vitamin D deficiency and help treat neonatal jaundice. Vitamin D deficiency can lead to nutritional rickets, while untreated severe jaundice may cause serious complications such as brain damage, deafn

Correlation between the barrier function of the intestinal epithelium and susceptibility to PEDV infection in piglets at different ages.

Porcine epidemic diarrhoea (PED) results in vomiting, diarrhoea, dehydration, and death in pigs. To analyse the relationship between intestinal development and susceptibility to porcine epidemic diarrhoea virus (PEDV), six new-born, 1-, 2-, 3-, 4-, and 8-week-old piglets were orally inoculated with 2 × 107 copies of the PEDV strain LJX01/2014 and sacrificed 48 h after infection. The results demonstrated that PEDV infection elicits diarrhoea of diverse severity across piglets of distinct ages. Moreover, viral shedding and villous atrophy were most pronounced in neonatal piglets. With increasing

Clinical characteristics and predictors of disease progression in parkinson's disease in a cohort of patients in the ashanti region, Ghana.

BACKGROUND: Parkinson's disease (PD) is a progressive neurodegenerative disorder with motor and non-motor symptoms. Sub-Saharan Africa (SSA) with its rapidly rising ageing population is experiencing a surge in the burden of PD in clinical care. Delayed diagnosis and treatment initiation, coupled with socioeconomic and healthcare barriers, may uniquely contribute to the population of PD with advanced clinical disease in SSA. This study aimed to clinically characterize Ghanaian PD patients, and identify predictors of mid-to-advanced disease. METHODS: A hospital-based cross-sectional study was c

Case Report: Challenges in the diagnosis of pseudohypoaldosteronism: insights from a resource-limited setting.

BACKGROUND: Pseudo-hypoaldosteronism type 1 (PHA1) is a hereditary condition characterized by end-organ resistance to aldosterone, resulting in electrolyte imbalance, metabolic acidosis, and hyperaldosteronism. It is inherited in an autosomal dominant renal or an autosomal recessive systemic form. Here, we report a case series of PHA1, highlighting its challenging diagnosis and underscoring the complexities involved in reaching an accurate diagnosis in a resource-limited setting. CASE REPORT: Seven patients from seven unrelated Sudanese families were admitted to pediatric endocrinology units

HERPUD1 suppresses porcine epidemic diarrhea virus replication by recruiting HRD1 to degrade viral ORF3 protein.

UNLABELLED: The open reading frame 3 (ORF3) gene of porcine epidemic diarrhea virus (PEDV) is related to virus pathogenicity. A previous study found that PEDV ORF3 protein localizes in the endoplasmic reticulum (ER) to trigger the unfolded protein response (UPR). Nevertheless, the precise interactions and underlying mechanisms between ORF3 and UPR pathway-associated proteins remain largely unexplored. In this study, we demonstrate that ORF3 protein is capable of inducing ER stress through activating the transcription factor-6 (ATF6) pathway and PKR-like ER kinase (PERK) pathway. Through yeast

Development and implementation of "Mazboot Bunyad": a digital health intervention for the screening and management of undernutrition among children aged 6-59 months, at private provider clinics in urban slums of Karachi, Pakistan.

BACKGROUND: Although factor Xa inhibitors have become a popular choice for chronic oral anticoagulation, effective drug reversal remains difficult due to a lack of specific antidote. Currently, 4-factor prothrombin complex concentrate (4F-PCC) is considered the treatment of choice for factor Xa inhibitor-related major bleeding. However, safety of 4F-PCC and its risk of thrombosis when used for this off-label purpose remain unclear. The purpose of this retrospective study is to determine the rate of thromboembolism when 4F-PCC is used for the emergent reversal of factor Xa inhibitors. METHODS:

Serum Electrolyte Changes in Foals Receiving Rhodococcus equi-Specific Hyperimmune Plasma.

BACKGROUND: Rhodococcus equi-specific hyperimmune plasma (Re-HIP) is routinely administered to neonatal foals to help prevent rhodococcal pneumonia. In addition to pathogen-specific immunoglobulins, frozen plasma products (FPPs) contain electrolytes that may exceed concentrations found in neonatal foal plasma. Because FPPs are also used to treat failure of passive transfer and sepsis, understanding their effects on neonate homeostasis is crucial. This study evaluated the effects of administration of Re-HIP on the serum concentrations of Na+, Cl-, K+, albumin, and globulin of healthy neonatal f

Multi-omics analysis reveals gut microbiota-host transcriptomic remodeling associated with the protective effects of Lacticaseibacillus rhamnosus GG against rotavirus enteritis.

OBJECTIVE: Rotavirus enteritis is closely associated with intestinal epithelial injury, gut microbial dysbiosis, and mucosal immune perturbation; however, the microbiota-host mechanisms underlying the protective effects of Lacticaseibacillus rhamnosus GG (LGG) remain incompletely defined. This study aimed to characterize LGG-induced microbiota-host transcriptomic remodeling in neonatal mice with rotavirus enteritis using an integrated multi-omics framework. METHODS: BALB/c suckling mice were assigned to Control, Model, and LGG groups. Rotavirus enteritis was induced by oral gavage with SA11 r

Neonatal Suppurative Sialadenitis With Bilateral Gland Involvement in a Preterm Infant: A Rare Complication of Bacteraemia.

INTRODUCTION: Neonatal suppurative sialadenitis (NSS) is an uncommon but serious complication of salivary gland infection, with few cases reported in literature, most often affecting preterm infants. Typical cases involve the parotid gland and are caused by Staphylococcus aureus infection. Contributing factors include prematurity, dehydration, prolonged hospitalisation, and immature immunity. Sequential bilateral gland involvement through haematogenous spread is rare, and presents significant management challenges in prematurity. PRESENTATION: We report the case of a preterm infant, born at 3

Cross-cultural beliefs and practices of caregivers regarding sunlight exposure for newborn jaundice: a scoping review.

BACKGROUND: Neonatal jaundice affects most newborns worldwide and remains a major cause of preventable bilirubin-induced neurological injury in low-resource settings. Although most clinical guidelines recommend standardised electrically powered phototherapy and discourage sunlight exposure for newborns with jaundice due to safety concerns, uncontrolled exposure to direct sunlight remains widespread in many communities. We aimed to chart cross-cultural beliefs and practices regarding sunlight exposure for newborn jaundice to inform safer newborn care. METHODS: We conducted a scoping review usi
