The contribution of the antenatal period in the development of necrotising enterocolitis.

Necrotising enterocolitis (NEC) is a disorder characterised by intestinal ischaemia and inflammation. It predominantly impacts preterm and low birthweight infants and carries a high burden of mortality and both short and long-term morbidity for survivors. Postnatal factors such as abnormal bacterial colonisation of the gut (dysbiosis) and formula feeding are known to predispose to the development of NEC. The contribution of the antenatal period to the subsequent development of NEC is poorly understood. Although it is known that fetuses with growth restriction, particularly those with abnormal

Turk J Pediatr 2019; 61: 392-398. Necrotizing Enterocolitis (NEC) is a common devastating gastrointestinal disease, which usually develops in premature infants. Erythropoietin (EPO) as a hematopoietic hormone produced by the kidney can also be naturally found in amniotic fluid and breast milk. There is some evidence that supports the contribution of EPO in the prevention of inflammation and intestinal tissue r

Intrathoracic Gastric Perforation Due to Late-Presenting Bochdalek Hernia in an Adolescent: A Rare Case of Panvisceral Herniation.

Congenital diaphragmatic hernias (CDHs), particularly those of the Bochdalek type, arise when the pleuroperitoneal canal fails to close during fetal development, most often affecting the left posterolateral aspect of the diaphragm. These hernias are typically identified in the neonatal period, making their appearance in adolescence quite rare. When they do occur later in life, they may present with a range of GI or respiratory symptoms, which can complicate diagnosis. A missed or delayed diagnosis, especially in cases involving visceral perforation, can significantly increase the risk of compl

Neisseria mucosa Urinary Tract Infection in a Neonate With Posterior Urethral Valves: A Case Report.

Neisseria mucosa is a Gram-negative diplococcus that usually colonizes the upper respiratory tract and is generally regarded as a commensal organism. It has, nevertheless, been implicated as an opportunistic pathogen in bacteremia, endocarditis, septic arthritis, meningitis, and peritoneal dialysis-associated peritonitis; urinary tract infection (UTI) remains exceptionally rare. We report a UTI caused by N. mucosa in a 24-day-old male neonate with posterior urethral valves (PUVs), who presented with frank pyuria for 10 days. Direct Gram staining of urine revealed Gram-negative diplococci with

European Paediatric Surgeons' Association Consensus Statement on the Management of Meconium Ileus.

INTRODUCTION: The management of meconium ileus (MI) has yet to reach a unified consensus among pediatric surgeons, particularly regarding diagnostics and variants, conservative treatment strategies, surgical approaches, and clinical outcomes. This topic was therefore selected for the 2024 Consensus Session of the Annual Congress European Paediatric Surgeons' Association (EUPSA). METHODS: A structured literature review was conducted across Cochrane, Medline, and EMBASE databases up to April 2024. Nine pediatric surgeons appointed by the European Paediatric Surgeons' Association (EUPSA) Consens

Peritoneal Dialysis in Infant Methylmalonic Acidemia: Effective Management Despite Sterile Peritonitis.

Methylmalonic acidemia (MMA) is one of the rare inherited metabolic disorders, and the affected patients usually suffer from repeated life-threatening metabolic crises in which hyperammonemia and acidosis are observed. Although continuous venovenous hemodialysis is considered the first choice for extracorporeal detoxification in neonates and infants, the available experience regarding peritoneal dialysis in this group of patients is still limited, and how to optimally manage the complications of dialysis has not been clearly described. We report the case of a three-month-old female infant with

Prenatal CFTR modulator therapy and fetal meconium ileus in cystic fibrosis: a systematic review and individual patient data meta-analysis.

BACKGROUND: Fetal meconium ileus (MI) is a clinically relevant manifestation of cystic fibrosis (CF). We conducted a systematic review with meta-analysis to evaluate the efficacy of prenatal CFTRm therapy, administered to carrier mothers or mothers with CF, for the prevention or treatment of fetal MI. The primary outcome was the occurrence of MI requiring surgical intervention. METHODS: A systematic search was conducted in PubMed/MEDLINE, Embase, SCOPUS and the Cochrane Library. In-vitro and animal studies were excluded. Reporting quality and the risk of bias of the cohort studies was assesse

Prenatal Ultrasound and MRI of Bowel Abnormalities in Cystic Fibrosis with in utero Cystic Fibrosis Transmembrane Conductance Regulator Modulator Exposure: A Case Series.

INTRODUCTION: Cystic fibrosis (CF) can present prenatally as fetal hyperechogenic bowel (FEB) or meconium ileus (MI), indicating early CFTR dysfunction. Elexacaftor/tezacaftor/ivacaftor (ETI) partially restores CFTR function and limited reports suggest it may benefit fetuses when administered maternally. CASE PRESENTATION: We report four pregnancies in CF carrier mothers with confirmed or presumed fetal CF after FEB detection, treated off-label with ETI. Fetal bowel status was monitored via ultrasound and MRI, and maternal safety was assessed. In case 1, ETI from 26 to 36 weeks normalized bow

Long-term clinical outcomes of neonatal cystic fibrosis presentation: a single-center retrospective study.

UNLABELLED: Neonatal presentation of cystic fibrosis (CF), particularly meconium ileus, has historically been associated with severe disease and poor prognosis. However, its impact on long-term outcomes in the era of newborn screening and CFTR modulators remains unclear. We aimed to compare long-term clinical outcomes between patients with and without neonatal presentation of CF. We conducted a retrospective observational study including all patients with CF followed at a tertiary reference center in Lisbon, Portugal, between 2013 and March 2025. Neonatal presentation was defined by meconium i

Gastrointestinal Bleeding as an Atypical Presentation of Neonatal Gastric Perforation in a Preterm Infant: A Case Report.

Neonatal gastric perforation is a rare but life-threatening surgical emergency that typically occurs during the first week of life. Early diagnosis remains challenging because the initial clinical and radiographic findings may be nonspecific, particularly in preterm infants, yet prompt recognition and surgical intervention are essential for improving outcomes. We report the case of a four-day-old preterm, low birth weight female infant admitted to the neonatal intensive care unit with prematurity and respiratory distress. During hospitalization, she developed hematemesis and melena. Initial ab

Subacute Small Bowel Obstruction in a 50-Year-Old Woman With Complete Intestinal Malrotation: A Case Report.

Intestinal malrotation is a congenital anomaly resulting from the failure of the midgut to undergo its normal 270° counterclockwise rotation during embryological development. While the vast majority of cases are seen in neonates, adult presentation is exceptionally rare and often presents with vague abdominal complaints, leading to significant diagnostic delays. A 50-year-old woman presented with diffuse, colicky abdominal pain and recurrent bilious vomiting for three days. Vital signs revealed hypertension (170/100 mmHg) and tachycardia (107 bpm). The physical examination revealed a soft, but

Antenatal CFTR Modulators to Treat a Healthy Pregnant Woman with a Fetus Affected by Cystic Fibrosis Complicated by Meconium Ileus and Intestinal Volvulus: From a Suspicion of the Disease to a Targeted Treatment in Utero: Case Report and Narrative Review.

The introduction of cystic fibrosis transmembrane conductance regulator modulators (CFTRms) in the treatment of cystic fibrosis (CF) has significantly improved both the life expectancy and quality of life for patients affected by the disease. While this new therapy shows promising results in CF patients, data about the use of CFTRms during pregnancy in women who are carriers with fetuses affected by CF are limited. We present a new case report and literature review of fetal CF treated by the maternal assumption of CFTRms to understand the safety and the effectiveness of the treatment in resolv

Research at the Paris Foundling Hospital, Part 2: After the Revolution.

BACKGROUND: We know little about how neonatal research changed at the Paris Foundling Hospital following the revolution. SUMMARY: The number of unwanted children rose, and 5,392 infants were admitted in 1826 - a quarter of all infants born in Paris; 26% of them died in the infirmary before transport. The infants' appalling mortality was associated with artificial nutrition, and transfer to mercenary nurses in the countryside was organized. In the 1830s sedentary nurses began to run short, and nearly all babies were fed artificially at some time. François Chaussier was director at the Maternit

Prenatal initiation of elexacaftor/tezacaftor/ivacaftor via carrier mother prevents congenital bilateral absence of vas deferens in a male infant with cystic fibrosis.

BACKGROUND: Prenatal therapy with elexacaftor/tezacaftor/ivacaftor (ETI) has recently emerged as a potential strategy to modify the early natural history of cystic fibrosis (CF). While case reports have described prevention of meconium ileus and pancreatic insufficiency, data on male reproductive outcomes remain extremely limited. CASE PRESENTATION: We report a male infant with CF (homozygous F508del) whose heterozygous carrier mother initiated ETI at 27+4 weeks of gestation after prenatal diagnosis. Pregnancy was uneventful until preterm delivery at 35+2 weeks. The neonate presented with nor

Impact of timing from last dose of dexamethasone administration to delivery, different steroid courses, and fetal number on preterm neonatal outcomes.

OBJECTIVES: To evaluate the impact of the timing from the last dose of dexamethasone to delivery, different steroid courses, and the number of fetuses (singleton vs twin) on preterm neonatal outcomes. This study focused on respiratory complications and associated conditions. METHODS: A retrospective analysis was conducted on 1800 pregnancies, comprising 1585 singleton pregnancies and 215 twin pregnancies, resulting in a total of 2015 neonates. The timing of dexamethasone administration relative to delivery was categorized into intervals: less than 6 hours, between 2 and 7 days, between 8 and

Not all that drains is gut - umbilical venous catheter extravasation masquerading as intestinal perforation: A case report.

BACKGROUND: Umbilical venous catheterisation (UVC) is frequently used in neonates but may be complicated by malposition or migration. Extravasation is rare and can mimic other abdominal pathologies. CASE SUMMARY: We report a preterm infant, born at 27 weeks' gestation, who developed bilateral pneumothorax and spontaneous intestinal perforation requiring drainage. On day 8, the infant clinically deteriorated with persistent high-volume clear peritoneal drain output and worsening dehydration despite fluid escalation. Isolated Multiple Perforations in the Descending Colon of a Term Baby Who Presented With Vomiting: A Case Report and Literature Review.

Neonatal intestinal perforation is a life-threatening surgical emergency that is more commonly described in preterm or low-birth-weight infants. Isolated descending colonic perforation in a term neonate is rare and may be difficult to recognize when symptoms are non-specific. We report a male newborn delivered at 37+2 weeks, weighing 2.6 kg, with Apgar scores of 9 and 9 at 1 and 5 minutes. Breastfeeding was started at two hours and was initially tolerated. At 12 hours, he developed poor feeding and brownish vomiting, while stool passage was present from day one. Initial laboratory tests and ch
