Expanding the Estimated Fetal Weight Definition of Growth Restriction by Adding Small Abdominal Circumference: Prediction of Neonatal Morbidity.

OBJECTIVE: The Society for Maternal-Fetal Medicine's (SMFM) diagnostic criteria for fetal growth restriction (FGR) recently added abdominal circumference (AC) <10th percentile to estimated fetal weight (EFW) <10th percentile; however, its prediction of neonatal morbidity is unknown. Our objective was to compare the two definitions for their prediction of composite neonatal morbidity. METHODS: Secondary analysis of the Fetal Growth Study-Singletons, 2009-2013. The last ultrasound (mean 36.9 ± 2.3 weeks) was included from non-anomalous fetuses. Composite neonatal morbidity was the primary outco

Growth in neonates with congenital kidney failure requiring continuous kidney replacement therapy.

BACKGROUND: With advanced technology, survival of neonates with congenital kidney failure (CKF) requiring continuous kidney replacement therapy (CKRT) has improved. Nutrition is essential but difficult to attain as CKRT removes proteins and micronutrients, and many patients have multiple co-morbidities. Scant data exist to guide clinicians on appropriate energy requirements for growth. METHODS: We performed a retrospective study of neonates with CKF admitted to Children's of Alabama between 2016 and 2022 who required KRT within 10 days. We evaluated risk factors and growth in the 18/24 (75%)

PRenatal mOdulator treatment to PrEvent CF complicaTions (PROTECT) workshop report.

BACKGROUND: Data from cystic fibrosis (CF) animal models and case studies suggests that in utero administration of CF transmembrane conductance regulator (CFTR) modulators (variant specific therapies, VST) can rescue CFTR-related pathophysiology in the fetus. Use of VST during pregnancy to prevent disease in infants has not been systematically studied. Through stakeholder engagement, we sought to determine if formal research evaluation is warranted. METHODS: We surveyed CF care center directors to assess their awareness of the potential off-label use of VST for in utero treatment of a fetus w

Long-Term Peritoneal Dialysis Using a Tenckhoff Catheter in a Premature Infant With Homozygous Autosomal Recessive Polycystic Kidney Disease: A Case Report.

Polycystic kidney disease (PKD) is a clinical condition characterized by the presence of renal cysts and kidney enlargement. When it presents during the neonatal period, it can lead to progressive renal damage and eventually result in end-stage renal failure. Peritoneal dialysis (PD) can be particularly challenging in the neonatal intensive care unit due to the infant's thin abdominal wall and the often unsuitable size and shape of available catheters. As a result, the use of such catheters in infants is associated with high mortality and significant complications, including leakage, peritonit

Initiation of enteral feeding as per current protocol (at 24 h) versus as per clinical status (from 6 h onwards) in neonates born with antenatal reversed end diastolic flow (REDF) in the umbilical artery Doppler: a pilot randomized controlled trial.

OBJECTIVE: To compare the effects of initiating feeds at 24 h versus at 6 h in neonates with reversed end-diastolic flow (REDF) and determine the incidence of feed intolerance (FI) and necrotizing enterocolitis (NEC). DESIGN: Pilot randomized controlled trial. METHODS: Neonates with REDF (n = 52) were randomized to start feeds at 6 h (n = 26) or 24 h (n = 26) to assess FI, NEC, time to full feeds, sepsis, mortality, weight and gestation at discharge, and hospital stay. RESULTS: FI was observed in 14 (54%) neonates in 6 h group and 17 (65%) in 24 h group, with comparable NEC rates. Neonates

Acute Peritoneal Dialysis in Critical Preterm Infants: A Case Series and Review of the Literature.

Background: Acute kidney injury (AKI) in critically ill neonates is usually of pre-renal origin and, often, pharmacological treatment is not sufficient for resolution, requiring kidney replacement therapy (KRT). Due to the small body size and the unavailability of adequate devices for these patients, peritoneal dialysis (PD) appears to be the most easily achievable procedure. However, guidelines for PD management are lacking in this population. Objective: We aimed to report a single-center experience with preterm infants who underwent PD, describing the technical issues and the outcomes, and t

Association of specific microbiota taxa in the amniotic fluid at birth with severe acute and longer-term outcomes of very preterm infants: a prospective observational study.

BACKGROUND: Dysbiotic microbial colonization predisposes to severe outcomes of prematurity, including mortality and severe morbidities like necrotizing enterocolitis (NEC), late-onset infection (LOI) and bronchopulmonary dysplasia (BPD). Here, we studied the variations in the bacterial signatures in the amniotic fluid (AF) of very preterm deliveries < 32 weeks with severe acute and longer-term outcomes within a prospective cohort study. METHODS: One hundred twenty-six AF samples were available for 16S rRNA gene metabarcoding to describe bacterial community structure and diversity in connectio

Pediatric cardiac surgery in Sudan before the 2023 armed conflict: patterns, outcomes, and quality indicators from a single-center experience in a low-resource setting.

BACKGROUND: Sudan's pediatric cardiac surgery services improved over the past decade, especially after the 2022 national free program. However, conflict in April 2023 halted all services. There is a scarcity of scholarly literature concerning pediatric cardiac surgery in low-income African countries such as Sudan. This study documents the achievements made before the conflict. METHODS: This was a prospective observational study conducted at the Sudan Heart Center. Data were collected from hospital medical records between January and December 2022. All patients who underwent surgical intervent

Interconnected Organs: Exploring the Impact of Kidney-Lung Crosstalk in Critically Ill Neonates and Children.

Acute kidney injury (AKI) and acute lung injury (ALI) are common in critically ill neonates, children, and those with severe cardiac disease. Although kidney and lung dysfunction can occur independently, molecular signaling and organ crosstalk significantly influence the function of each organ. Additionally, there is a link between AKI and fluid balance disorders. A bidirectional and synergistic relationship exists between AKI and fluid imbalance, with fluid management potentially becoming compromised before or after AKI. Fluid accumulation can further worsen ALI by impairing gas exchange. Org

Doppler abnormality predisposes preterm infants with fetal growth restriction to postnatal intestinal disorder.

OBJECTIVE: Intestinal disorders (ID) impose a significant burden on preterm infants. Although previous studies have examined individual risk factors for types of ID such as necrotizing enterocolitis (NEC), meconium-related ileus (MRI), and focal intestinal perforation (FIP), the overarching etiology of ID as a whole remains underexplored. Therefore, this study aimed to identify obstetric risk factors for ID. METHODS: We retrospectively investigated singletons without congenital anomalies born between 22 weeks 0 days and 28 weeks 6 days of gestation between January 2013 and December 2022. We c

Development and application of a focused ultrasound protocol in neonatal foals.

This study aimed to develop a standardized ultrasound (US) protocol using selected thoracic and abdominal acoustic windows, to assess its feasibility and applicability in healthy foals, and to evaluate its usefulness for detecting common thoracic and abdominal conditions in sick foals. The Focused US (FUS) protocol included 12 thoraco-abdominal windows (4 right-side, 3 ventral, 5 left-side) using a curved-array probe. It was applied to cohorts of healthy (n = 17) and sick (n = 23) foals by a veterinary clinician with limited US experience. The FUS showed good feasibility and applicability, wit

Silent messengers in the nano-world: Harnessing extracellular vesicles as theranostic tools for neonatal surgical conditions.

Extracellular vesicles (EVs) are lipid bilayer-enclosed nanoparticles secreted by all cells that mediate intercellular communication by transferring proteins, lipids, and nucleic acids. Among their cargo, microRNAs are key post-transcriptional regulators of organogenesis. In fetal lung development, EV-mediated signaling is essential for the coordination of epithelial, mesenchymal, endothelial, and immune cells. In congenital diaphragmatic hernia (CDH), disruption of these interactions leads to pulmonary hypoplasia and pulmonary hypertension, the primary causes of morbidity and mortality. Amnio
