Enteric duplication cysts (EDCs) are rare congenital anomalies that may occur anywhere along the gastrointestinal tract. They are typically located adjacent to the alimentary canal, sharing a common muscular wall and sometimes a lumen. The conundrum arises in the diagnosis due to difficulty in identifying the anatomical abnormality via simple radiological modalities, thereby delaying intervention. Their clinical presentation varies with location and complications. While enteric duplication cysts are uncommon, they can present with non-specific and potentially life-threatening symptoms in infan

Intestinal malrotation is described as an abnormal positioning of intestinal loops within the peritoneal cavity, caused by defective rotation around the superior mesenteric artery. This results in a short mesenteric root, which predisposes to midgut volvulus when the intestine twists on its axis. We present a case of a five-day-old female with bilious vomiting, abdominal distension, and radiographic signs of obstruction. Surgical intervention was decided, revealing midgut volvulus associated with intestinal malrotation and questionable intestinal viability. A second-look surgery was scheduled

BACKGROUND/AIM: Cystic fibrosis (CF) is an autosomal recessive disease caused by mutations in the cystic fibrosis transmembrane conductance regulator (CFTR) gene. CF is characterized by respiratory tract infections, pancreatic insufficiency, meconium ileus, intestinal obstruction, and male infertility. A genotype to phenotype correlation is difficult to establish because of the heterogeneity of disease severity. Even patients with the same CFTR mutation can have varying clinical severities. In recent years, studies have explored the role of microRNA (miRNA) expression in the regulation of resp

We present a case series describing the management and genetic findings in five neonates with meconium ileus, presenting within 48 h of birth. Whole-exome sequencing identified cystic fibrosis transmembrane conductance regulator mutations in three neonates, confirming cystic fibrosis, with one heterozygous variant of unknown significance. Two ultimately required surgical intervention after failed conservative management. One died of septic shock.

INTRODUCTION: Cystic fibrosis (CF) is an autosomal recessive disease associated to low survival disease into adulthood. The appearance of the cystic fibrosis transmembrane conductance regulator (CFTR) therapy has revolutionized the management and the prognosis of these patients with an improvement in the pancreatic and lung function with a much better quality of life. More women with CF are considering being mothers. Other clinical scenario is the prenatal diagnosis of fetus with CF. Three case reports have been published, suggesting that CFTR therapy during pregnancy in fetuses affected by CF

Pneumoperitoneum in a mechanically ventilated neonate often raises the suspicion of an acute surgical abdomen. However, barotrauma-related pneumoperitoneum resulting from alveolar rupture and air dissection into the peritoneal cavity can mimic gastrointestinal perforation. Differentiating this rare complication of positive-pressure ventilation from a true viscus perforation is essential to prevent unnecessary surgical intervention. We report a two-month-old infant born prematurely with a history of intraventricular hemorrhage and patent ductus arteriosus who presented with frequent apneic epis

BACKGROUND: Autophagy is a self-degradative process that plays a role in removing misfolded or aggregated proteins located in the cytosol. Autophagy induction is initiated by the mammalian target of rapamycin (mTOR). Beclin-1, and microtubule-associated protein 1A/1B-light chain 3 (LC3) are proteins involved in the regulation of autophagy. Cystic fibrosis (CF) cells suffer from defective autophagy, related to altered proteostasis and chronic inflammation. The study aimed to assess the genetic expression of mTOR, Beclin-1, and LC3b in CF children and to correlate them with genotypes, phenotypes

PURPOSE: Neonatal bowel obstruction secondary to inspissated meconium has been historically associated with cystic fibrosis. Increasingly, meconium-related obstruction (MRO) has been observed in preterm infants. We conducted a multicenter mixed-methods study to better characterize the contemporary experience with MRO.
METHODS: A retrospective cohort study of infants with MRO was performed at seven children's hospitals from 2018 to 2022. Chi-squared tests, Kruskal-Wallis tests, and logistic regression were used to assess the association of cystic fibrosis, Hirschsprung disease, and prematurity

Cystic fibrosis (CF) is an autosomal recessive disorder caused by pathogenic variants in the CFTR gene. Over 2000 CFTR variants have been identified, and ongoing genetic characterization remains critical for guiding eligibility for CFTR modulator therapies and contributing to variant databases that support diagnosis and research. We describe a 20-year-old Caucasian male who is a compound heterozygous for the common F508del mutation and a novel variant, p.Ser1255del (c.3763_3765delTCA; delS1255). He was diagnosed with CF in the neonatal period following meconium ileus, confirmed by elevated swe

Background/Objectives: Cystic fibrosis (CF) is a rare autosomal recessive genetic disease that has a progressive and multisystemic course. The spectrum and frequency of mutations in the gene encoding the cystic fibrosis transmembrane conductance regulator (CFTR) vary both in European countries and in other geographical regions. The aim of our retrospective study was to present the genetic variants identified in a group of 48 CF patients from the Moldova region (Romania), as well as to establish genotype-phenotype correlations. Methods: Genetic testing was initially performed for 38 CFTR mutati

BACKGROUND: Meconium ileus (MI) is one of the earliest manifestations of cystic fibrosis (CF), affecting 15-20% of individuals. The impact of MI on health outcomes has yet to be elucidated and may vary based on the amount of health care resources. The aim of this study was to investigate the clinical impact of MI on outcomes among Brazilian CF individuals using data from the Brazilian Cystic Fibrosis Patient Registry.
METHODS: This retrospective cohort study included data from individuals with CF from 53 reference centres in Brazil. Data from individuals with a history of MI during the neonat

UNLABELLED: The diagnosis of extrapulmonary tuberculosis (EPTB) in children is challenging due to the variable and nonspecific manifestations and the low rate of M. tuberculosis isolations which delayed diagnosis, leading to an increase in cases with unfavorable outcomes.
OBJECTIVE: To describe the use of traditional and confirmatory diagnostic methods for EPTB at the point of care (POC) in pediatric patients at a referral hospital.
PATIENTS AND METHOD: Retrospective study in both male and female patients aged 0-17 years hospitalized from January 2012 to January 2022 in a pediatric referral

BACKGROUND: Gastrointestinal (GI) diseases are significant health problems worldwide. Due to the limited number of national reports among upper-middle-income countries, we aimed to study burdens and patterns of GI diseases in Thai children. METHODS: The study retrospectively analyzed data on inpatients aged <18 years provided by Thailand’s National Health Security Office (NHSO) from 2015 to 2023. RESULTS: The primary diagnosis of GI diseases during the study period ranged from 134,000 to 296,040 admissions per year (10.2-16% of all inpatient diagnoses). The most common diagnosis group was inte

Castrate-Resistant Prostate Cancer (CRPC) is characterized by persistent androgen receptor-driven tumor growth in the apparent absence of systemic androgens. Current evidence suggests that CRPC cells can produce their own androgens from endogenous sterol precursors that act in an intracrine manner to stimulate tumor growth. The mechanisms by which CRPC cells become steroidogenic during tumor progression are not well defined. Herein we describe a novel link between the elevated cholesterol phenotype of CRPC and the TERE1 tumor suppressor protein, a prenyltransferase that synthesizes vitamin K-2

The nitroimidazole antibiotic, metronidazole, is frequently prescribed to infants with serious intra-abdominal infections, and multiple dosing recommendations exist. We sought to evaluate the extent to which metronidazole doses and associated exposures achieved desired efficacy and safety in infants enrolled in the Antibiotic Safety in Infants with Complicated Intra-abdominal Infections (SCAMP) trial (NCT01994993). SCAMP participants received intravenous metronidazole as part of multimodal antimicrobial therapy. Participants received a 15 mg/kg loading dose and a 7.5 mg/kg maintenance dose at

Gastrointestinal infections represent a significant global health burden, ranking as the second leading cause of mortality among infants and children. Identifying of pathogens causing neonatal gastrointestinal infections has presented tough challenges. This study aimed to summarize the prevalence of common pathogens associated with neonatal gastrointestinal infections through a comprehensive systematic review and meta-analysis of published literature. The last search was performed on January 08, 2025, from databases including EMBASE, PubMed, Cochrane Libary, and Web of Science. The outcome var

BACKGROUND: The West Midlands Newborn Bloodspot Screening Laboratory is one of 16 in the UK and serves two tertiary paediatric cystic fibrosis (CF) centres (Staffordshire Children's Hospital at Royal Stoke and Birmingham Children's Hospital). CF newborn bloodspot screening (NBS) in this region started in November 2006 prior to the UK national roll-out in 2007. It uses an immunoreactive trypsinogen (IRT)/DNA/IRT protocol. We report the outcomes from 15 years of CF screening.
METHODS: The West Midlands CF NBS outcomes from 1 November 2006 to 31 October 2021 were reviewed. Clinical data were als

Acute thymic atrophy occurs following type 1 inflammatory conditions such as viral infection and sepsis, resulting in cell death and disruption of T cell development. However, the impact type 1 immunity has on thymic-resident innate lymphoid cells (ILCs) remains unclear. Single-cell RNA sequencing revealed neonatal thymic-resident type 1 ILCs (ILC1s) as a unique and immature subset compared to ILC1s in other primary lymphoid organs. Culturing murine neonatal thymic lobes with the type 1 cytokines interleukin-12 (IL-12) and IL-18 resulted in a rapid expansion and thymic egress of KLRG1+CXCR6+ c
