BACKGROUND: The consequences of cystic fibrosis (CF) transmembrane conductance regulator (CFTR) protein dysfunction or absence begin during fetal development, with pancreatic, intestinal, hepatobiliary, and reproductive manifestations evident at birth. The advent of CFTR modulator therapies (CFTR modulator)-most notably elexacaftor/tezacaftor/ivacaftor (ETI), approved for children and adults with CF who carry responsive variants, and the recently approved vanzacaftor/tezacaftor/deutivacaftor (VTD), currently indicated for older pediatric and adult populations-has transformed long-term health o

We reviewed the contemporary use of cystic fibrosis transmembrane conductance regulator modulator therapy (CFTRm) during pregnancy, including animal data and published human cases involving unaffected pregnant CF (heterozygote) carriers with a prenatal diagnosis of fetal cystic fibrosis (CF). In pregnant individuals with CF, continuation of CFTRm is associated with preserved maternal pulmonary function comparable to nonpregnant peers and favorable obstetrical outcomes. CFTRm crosses the placenta readily, appears at low levels in breastmilk, and concerns for cataracts from animal models have no

Over the past 50 years, therapeutic advances have significantly improved the management of cystic fibrosis (CF). Recently, cystic fibrosis transmembrane conductance regulator (CFTR) modulators have been widely regarded as a therapeutic revolution, and their use is associated with a marked increase in pregnancies among affected women. However, pregnant women were excluded from clinical trials, limiting what is known about maternal and fetal safety. Maternal and fetal outcome tend to be poorer when the disease is severe, but there is limited data on such pregnancies exposed to CFTR modulators. W

Small molecule therapies that restore function to the cystic fibrosis (CF) transmembrane conductance regulator (CFTR) protein, referred to as CFTR modulators, have emerged as a key therapeutic approach for people with CF who carry responsive genetic variants. Their effects have been transformative to the lives of many of those living with CF. As the reach of CFTR modulators expands, new questions arise about their role beyond approved clinical indications for people with CF. One such area of growing interest is the use of CFTR modulator therapy during pregnancy to treat suspected meconium ileu

Highly effective modulator therapy (HEMT) has been life-changing for many living with cystic fibrosis (CF), a genetic disease affecting all races and ethnicities. With diagnoses now occurring in utero for many patients, prenatal treatment of the pregnant person with HEMT to mitigate the effects of CF on the growing fetus is a hot topic of discussion. This review summarizes recent literature for fetal transplacental therapy with HEMT for providers caring for parent/fetal dyads and offers expert opinion regarding monitoring and future areas of research.

BACKGROUND: Although highly effective modulator therapy may reduce future gastrostomy tube (g-tube) placements, a better understanding of the benefits of g-tube placement is needed.
OBJECTIVES: To describe the characteristics of children with cystic fibrosis (CF) who underwent g-tube placement and determine if earlier placement is associated with improved clinical outcomes.
DESIGN: Retrospective cohort analysis of children with CF born 2003-2019 and followed in the US CF Foundation Patient Registry.
RESULTS: There were 2337 children with CF (22.9% of the study cohort) who underwent g-tube p

BACKGROUND: Meconium ileus complicated by prenatal perforation and pseudocyst formation is a severe variant requiring prompt surgical management.
CASE PRESENTATION: A female newborn presented with severe abdominal distention, with a prenatal ultrasound showing ascites. Postnatal imaging suggested complex meconium ileus with meconium peritonitis. Laparotomy on day 2 revealed a large pseudocyst and a single perforation. The cyst was drained, adhesions released, inspissated meconium evacuated via appendiceal catheterization, and the perforation repaired. The infant recovered well, tolerated feed

We sought to determine whether a cystic fibrosis transmembrane conductance regulator (CFTR) mRNA could be administered to the fetus via the transamniotic route as a potential strategy for the perinatal management of cystic fibrosis-associated meconium ileus. Nine pregnant Sprague Dawley dams underwent volume-matched intra-amniotic injections in all their fetuses (n = 109) of either a suspension of a human CFTR mRNA encapsulated by a semi-synthetic composite, lipopolyplex (mRNA group; n = 98), or of a suspension of the same composite free of mRNA (control group; n = 11), on gestational day 17 (

Introduction: Cystic fibrosis (CF) frequently presents prenatally with meconium ileus (MI), a condition associated with significant neonatal morbidity and long-term gastrointestinal complications. The advent of highly effective CFTR modulators, particularly elexacaftor/tezacaftor/ivacaftor (ETI), during pregnancy remains off-label, and their role as in utero therapy for affected fetuses of carrier mothers is still emerging. Methods: We conducted a narrative literature review using PubMed, Embase, and Scopus to identify published reports of in utero CFTR modulator therapy for MI between 2022 an

Advances in genomic technologies, particularly massively parallel sequencing of cell-free fetal DNA, have transformed prenatal screening and created opportunities for fetal therapies. First-tier screening for trisomy 21 (T21) now enables early identification of affected fetuses, raising the possibility of prenatal intervention. Down syndrome (DS) is increasingly recognized not only as a neurodevelopmental condition but also as an interferon-driven disorder of immune dysregulation. Preclinical studies using human cellular models and trisomic mice demonstrated abnormalities in oxidative stress a

INTRODUCTION: Cystic fibrosis (CF) is a life-limiting autosomal recessive disorder caused by mutations in the CFTR gene. In Turkiye, high genetic diversity and consanguinity contribute to a heterogeneous CFTR variant spectrum; however, genotype-phenotype data remain limited.
METHODS: This retrospective study analyzed 92 Turkish CF patients (36 homozygous, 56 compound heterozygous) to characterize CFTR variant diversity and clinical outcomes. Next-generation sequencing (NGS) was performed and confirmed by Sanger sequencing. Clinical data were analyzed using chi-square and nonparametric tests (

INTRODUCTION: The management of meconium ileus (MI) has yet to reach a unified consensus among pediatric surgeons, particularly regarding diagnostics and variants, conservative treatment strategies, surgical approaches, and clinical outcomes. This topic was therefore selected for the 2024 Consensus Session of the Annual Congress European Paediatric Surgeons' Association (EUPSA).
METHODS: A structured literature review was conducted across Cochrane, Medline, and EMBASE databases up to April 2024. Nine pediatric surgeons appointed by the European Paediatric Surgeons' Association (EUPSA) Consens

BACKGROUND: Fetal meconium ileus (MI) is a clinically relevant manifestation of cystic fibrosis (CF). We conducted a systematic review with meta-analysis to evaluate the efficacy of prenatal CFTRm therapy, administered to carrier mothers or mothers with CF, for the prevention or treatment of fetal MI. The primary outcome was the occurrence of MI requiring surgical intervention.
METHODS: A systematic search was conducted in PubMed/MEDLINE, Embase, SCOPUS and the Cochrane Library. In-vitro and animal studies were excluded. Reporting quality and the risk of bias of the cohort studies was assesse

INTRODUCTION: Cystic fibrosis (CF) can present prenatally as fetal hyperechogenic bowel (FEB) or meconium ileus (MI), indicating early CFTR dysfunction. Elexacaftor/tezacaftor/ivacaftor (ETI) partially restores CFTR function and limited reports suggest it may benefit fetuses when administered maternally.
CASE PRESENTATION: We report four pregnancies in CF carrier mothers with confirmed or presumed fetal CF after FEB detection, treated off-label with ETI. Fetal bowel status was monitored via ultrasound and MRI, and maternal safety was assessed. In case 1, ETI from 26 to 36 weeks normalized bow

UNLABELLED: Neonatal presentation of cystic fibrosis (CF), particularly meconium ileus, has historically been associated with severe disease and poor prognosis. However, its impact on long-term outcomes in the era of newborn screening and CFTR modulators remains unclear. We aimed to compare long-term clinical outcomes between patients with and without neonatal presentation of CF. We conducted a retrospective observational study including all patients with CF followed at a tertiary reference center in Lisbon, Portugal, between 2013 and March 2025. Neonatal presentation was defined by meconium i

Background/Objectives: Meconium ileus (MI) related to cystic fibrosis (CF) presents unique nutrition challenges for newborns. The aim of this study was to present a descriptive case series reviewing the management practice and clinical outcomes of infants with CF and MI at a large, quaternary care center. Methods: A retrospective analysis examined patients born with CF and MI over fifteen years at a large pediatric hospital in the Midwest. Patient demographics, prenatal imaging data, newborn screening results, stool studies, sweat chloride and details of the clinical course were obtained throu

The introduction of cystic fibrosis transmembrane conductance regulator modulators (CFTRms) in the treatment of cystic fibrosis (CF) has significantly improved both the life expectancy and quality of life for patients affected by the disease. While this new therapy shows promising results in CF patients, data about the use of CFTRms during pregnancy in women who are carriers with fetuses affected by CF are limited. We present a new case report and literature review of fetal CF treated by the maternal assumption of CFTRms to understand the safety and the effectiveness of the treatment in resolv

BACKGROUND: Prenatal therapy with elexacaftor/tezacaftor/ivacaftor (ETI) has recently emerged as a potential strategy to modify the early natural history of cystic fibrosis (CF). While case reports have described prevention of meconium ileus and pancreatic insufficiency, data on male reproductive outcomes remain extremely limited.
CASE PRESENTATION: We report a male infant with CF (homozygous F508del) whose heterozygous carrier mother initiated ETI at 27+4 weeks of gestation after prenatal diagnosis. Pregnancy was uneventful until preterm delivery at 35+2 weeks. The neonate presented with nor

Acute lung injury (ALI) affects over 10% of patients hospitalised in critical care, with acute respiratory distress syndrome (ARDS) being the most severe form of ALI and having a mortality rate in the region of 40%. There has been slow but incremental progress in identification of biomarkers that contribute to the pathophysiology of ARDS, have utility in diagnosis and monitoring, and that are potential therapeutic targets (Calfee CS, Delucchi K, Parsons PE, Thompson BT, Ware LB, Matthay MA, Thompson T, Ware LB, Matthay MA, Lancet Respir Med 2014, 2:611--620). However, a major issue is that ARD
