Infant Renal Replacement Therapy Using Carpediem: A Multicenter Observational Cohort Study from the ICONIIC Learning Network.

OBJECTIVE: To evaluate indications and outcomes of Carpediem as the first infant-specific continuous renal replacement therapy (CRRT) platform available for clinical use in the US. STUDY DESIGN: A multicenter, retrospective, and prospective observational study was conducted through the "Improving CRRT Outcomes in Neonates and Infants through Interdisciplinary Collaboration" Learning Network. Data were collected from the first four US centers utilizing Carpediem. A treatment course was defined as sequential CRRT procedures separated by ≤72 hours. Infant cohorts were categorized by CRRT indicat

Enteric Duplication Cysts as a Diagnostic and Therapeutic Challenge: Three Unique Case Reports With Review of Literature.

Enteric duplication cysts (EDCs) are rare congenital anomalies that may occur anywhere along the gastrointestinal tract. They are typically located adjacent to the alimentary canal, sharing a common muscular wall and sometimes a lumen. The conundrum arises in the diagnosis due to difficulty in identifying the anatomical abnormality via simple radiological modalities, thereby delaying intervention. Their clinical presentation varies with location and complications. While enteric duplication cysts are uncommon, they can present with non-specific and potentially life-threatening symptoms in infan

Second-Look Surgery for Intestinal Salvage in a Neonate With Midgut Volvulus and Intestinal Malrotation: A Case Report.

Intestinal malrotation is described as an abnormal positioning of intestinal loops within the peritoneal cavity, caused by defective rotation around the superior mesenteric artery. This results in a short mesenteric root, which predisposes to midgut volvulus when the intestine twists on its axis. We present a case of a five-day-old female with bilious vomiting, abdominal distension, and radiographic signs of obstruction. Surgical intervention was decided, revealing midgut volvulus associated with intestinal malrotation and questionable intestinal viability. A second-look surgery was scheduled

Barotrauma-Related Pneumoperitoneum in a Ventilated Child: Distinguishing Air Leak From Bowel Perforation.

Pneumoperitoneum in a mechanically ventilated neonate often raises the suspicion of an acute surgical abdomen. However, barotrauma-related pneumoperitoneum resulting from alveolar rupture and air dissection into the peritoneal cavity can mimic gastrointestinal perforation. Differentiating this rare complication of positive-pressure ventilation from a true viscus perforation is essential to prevent unnecessary surgical intervention. We report a two-month-old infant born prematurely with a history of intraventricular hemorrhage and patent ductus arteriosus who presented with frequent apneic epis

Diagnostic methods of extrapulmonary tuberculosis in a pediatric referral hospital.

UNLABELLED: The diagnosis of extrapulmonary tuberculosis (EPTB) in children is challenging due to the variable and nonspecific manifestations and the low rate of M. tuberculosis isolations which delayed diagnosis, leading to an increase in cases with unfavorable outcomes. OBJECTIVE: To describe the use of traditional and confirmatory diagnostic methods for EPTB at the point of care (POC) in pediatric patients at a referral hospital. PATIENTS AND METHOD: Retrospective study in both male and female patients aged 0-17 years hospitalized from January 2012 to January 2022 in a pediatric referral

Burdens and patterns of gastrointestinal diseases in Thai children: an analysis based on Thai national health coverage data.

BACKGROUND: Gastrointestinal (GI) diseases are significant health problems worldwide. Due to the limited number of national reports among upper-middle-income countries, we aimed to study burdens and patterns of GI diseases in Thai children. METHODS: The study retrospectively analyzed data on inpatients aged <18 years provided by Thailand’s National Health Security Office (NHSO) from 2015 to 2023. RESULTS: The primary diagnosis of GI diseases during the study period ranged from 134,000 to 296,040 admissions per year (10.2-16% of all inpatient diagnoses). The most common diagnosis group was inte

Estimating Network Adequacy of Autism Service Providers in Affordable Care Act Markets.

Castrate-Resistant Prostate Cancer (CRPC) is characterized by persistent androgen receptor-driven tumor growth in the apparent absence of systemic androgens. Current evidence suggests that CRPC cells can produce their own androgens from endogenous sterol precursors that act in an intracrine manner to stimulate tumor growth. The mechanisms by which CRPC cells become steroidogenic during tumor progression are not well defined. Herein we describe a novel link between the elevated cholesterol phenotype of CRPC and the TERE1 tumor suppressor protein, a prenyltransferase that synthesizes vitamin K-2

Metronidazole exposure-response and safety in infants.

The nitroimidazole antibiotic, metronidazole, is frequently prescribed to infants with serious intra-abdominal infections, and multiple dosing recommendations exist. We sought to evaluate the extent to which metronidazole doses and associated exposures achieved desired efficacy and safety in infants enrolled in the Antibiotic Safety in Infants with Complicated Intra-abdominal Infections (SCAMP) trial (NCT01994993). SCAMP participants received intravenous metronidazole as part of multimodal antimicrobial therapy. Participants received a 15 mg/kg loading dose and a 7.5 mg/kg maintenance dose at

Prevalence of pathogens associated with neonatal gastrointestinal infections: a systematic review and meta-analysis.

Gastrointestinal infections represent a significant global health burden, ranking as the second leading cause of mortality among infants and children. Identifying of pathogens causing neonatal gastrointestinal infections has presented tough challenges. This study aimed to summarize the prevalence of common pathogens associated with neonatal gastrointestinal infections through a comprehensive systematic review and meta-analysis of published literature. The last search was performed on January 08, 2025, from databases including EMBASE, PubMed, Cochrane Libary, and Web of Science. The outcome var

Type 1 immunity enables neonatal thymic ILC1 production.

Acute thymic atrophy occurs following type 1 inflammatory conditions such as viral infection and sepsis, resulting in cell death and disruption of T cell development. However, the impact type 1 immunity has on thymic-resident innate lymphoid cells (ILCs) remains unclear. Single-cell RNA sequencing revealed neonatal thymic-resident type 1 ILCs (ILC1s) as a unique and immature subset compared to ILC1s in other primary lymphoid organs. Culturing murine neonatal thymic lobes with the type 1 cytokines interleukin-12 (IL-12) and IL-18 resulted in a rapid expansion and thymic egress of KLRG1+CXCR6+ c

Early treatment with fluvoxamine, bromhexine, cyproheptadine, and niclosamide to prevent clinical deterioration in patients with symptomatic COVID-19: a randomized clinical trial.

BACKGROUND: Repurposed drugs with host-directed antiviral and immunomodulatory properties have shown promise in the treatment of COVID-19, but few trials have studied combinations of these agents. The aim of this trial was to assess the effectiveness of affordable, widely available, repurposed drugs used in combination for treatment of COVID-19, which may be particularly relevant to low-resource countries. METHODS: We conducted an open-label, randomized, outpatient, controlled trial in Thailand from October 1, 2021, to June 21, 2022, to assess whether early treatment within 48-h of symptoms o

Diuretic response after neonatal cardiac surgery: a report from the NEPHRON collaborative.

BACKGROUND: Multicenter early diuretic response (DR) analysis of single furosemide dosing following neonatal cardiac surgery is lacking to inform whether early DR predicts adverse clinical outcomes. METHODS: We performed a retrospective cohort study utilizing data from the NEPHRON registry. Random forest machine learning generated receiver operating characteristic-area under the curve (ROC-AUC) and odds ratios for mechanical ventilation (MV) and respiratory support (RS). Prolonged MV and RS were defined using ≥ 90th percentile of observed/expected ratios. Secondary outcomes were prolonged CIC

Misdiagnosis and Clinical Insights into Acral Amelanotic Melanoma-A Systematic Review.

BACKGROUND: Acral amelanotic melanomas (AAMs), a rare subset of melanomas located on acral sites such as the palms, soles, and subungual areas, are diagnostically challenging due to their lack of typical pigmentation and often benign clinical appearance. Misdiagnosis is common, leading to delays in treatment and potentially worse outcomes. This systematic review aims to synthesise evidence on cases of AAM initially misdiagnosed as other conditions, to better understand their clinical and epidemiological characteristics, diagnostic pitfalls, and management strategies. METHODS: A comprehensive

A Rare and Unusual Case of Hypernatremic Dehydration in a Newborn Presenting With Adrenal Haemorrhage and Leading to Acute Kidney Injury.

Adrenal haemorrhage, although a rare entity in the neonatal period, is a known complication of birth asphyxia. Adrenal haemorrhage progresses differently depending on the type and extent of the glands involved. Adrenal haemorrhage can cause persistent jaundice, fever, dehydration, scrotal swelling, abdominal wall discolouration, septicemia, and a shock-like state. Here, we report the case of a four-day-old male infant who presented with jaundice, poor feeding, and hypernatremic dehydration. The patient developed acute kidney injury and, eventually, renal failure due to adrenal haemorrhage. He

Safety and efficacy of PfSPZ Vaccine against malaria in healthy adults and women anticipating pregnancy in Mali: two randomised, double-blind, placebo-controlled, phase 1 and 2 trials.

BACKGROUND: Plasmodium falciparum parasitaemia during pregnancy causes maternal, fetal, and infant mortality. Poor pregnancy outcomes are related to blood-stage parasite sequestration and the ensuing inflammatory response in the placenta, which decreases over successive pregnancies. A radiation-attenuated, non-replicating, whole-organism vaccine based on P falciparum sporozoites (PfSPZ Vaccine) has shown efficacy at preventing infection in African adults. Here, we aimed to examine vaccine safety and efficacy of the PfSPZ Vaccine in adults and women who anticipated conception. METHODS: Two ran

A rare case of extensive pneumatosis cystoides intestinalis with intestinal malrotation: Case report.

Intramural bowel gas (Pneumatosis intestinalis) refers to the radiological or clinical evidence of gas within the wall of the bowel lumen. While intramural gas could be secondary to life-threatening pathologies such as mesenteric ischemia in adults and necrotizing enterocolitis in neonates, it could also occur as a rare benign sub-type called Pneumatosis cystoides intestinalis, which is characterized by multiple gas-filled cysts in the submucosa and/or subserosal of the gastrointestinal tract. Distinguishing between life-threatening Pneumatosis intestinalis and its benign subtypes requires car

Incidence of infection rate for shunt implantation: the zero % rate is always a myth.

INTRODUCTION: Paediatric CSF shunt infection rate remains a well-known complication that is not only responsible of potentially severe sequels for patients but also for economical expenses. In that study, we questioned if it is possible to attain the zero percent rate of infection that should be the goal of every paediatric neurosurgeon. METHODS: We report our series of patients treated with a CSF device from January the first 2016 to December 31 2018. RESULTS: In all 147 patients treated for hydrocephalus, the follow-up was of at least of 2 years from the implantation. Antibiotic-coated tub

Case Report of Meconium Peritonitis: A Rare Cause of Non-immune Hydrops Fetalis.

Meconium peritonitis (MP) as a cause of non-immune hydrops in neonates is rarely reported. We present a case of a 35-week gestational-age male neonate diagnosed with hydrops secondary to MP. Antenatal scan at 34 weeks and five days revealed features of fetal hydrops, and the prenatal workup did not reveal the etiology. Postnatal imaging confirmed MP with abdominal calcifications, and the neonate developed pneumoperitoneum due to intestinal perforation. Emergency laparotomy revealed diffuse MP without a definitive site of perforation. An ileostomy was created and closed before discharge. Follow

Bedside transfontanelle drain placement reduces the need for operative intervention in infants with subdural fluid collections.

OBJECTIVE: Subdural fluid collection is a common neurosurgical condition in the pediatric population. Patients requiring surgical intervention have historically been managed with subdural drains, subdural-subgaleal shunting, subdural-peritoneal shunting, and mini-craniotomies. An alternative procedure for patients with an open anterior fontanelle is bedside transfontanelle drainage. This study aims to evaluate the efficacy of the transfontanelle angiocatheter drain placement including complications and the need for any further surgery. METHODS: We performed a single-center retrospective revie
