Diagnostic Challenges in mNGS for Pulmonary Mucormycosis Presenting with Imaging Features Resembling Those of Lung Cancer.

BACKGROUND: Pulmonary mucormycosis is a rare fungal infection with an increasing incidence in clinical reports. Its non-specific presentation often mimics lung cancer, posing significant diagnostic challenges. This report aims to improve clinician recognition of this condition. CASE PRESENTATION: A 67-year-old woman with diabetes presented with cough and sputum production. Imaging revealed a right hilar mass with bronchial obstruction, mediastinal lymphadenopathy, and cystic cavities (Figure 1A). While initial suspicion favored lung cancer, metagenomic next-generation sequencing (mNGS) of bro

Diagnosis of Rhinoentomophthoromycosis in a 55 Years Old Male: A Case Report.

Entomophthoromycosis is a chronic fungal infection characterized by a diverse range of manifestations, including subcutaneous, mucocutaneous and visceral infections. The majority of subcutaneous infections caused by entomophthoralean fungi are attributed to Basidiobolus spp, C. coronatus or C. incongruous. In this report, we present a case of rhinoentomophthoromycosis in a middle-aged, immunocompetent male, primarily affecting the nasopharynx. Initially, the diagnosis posed a dilemma, but it was ultimately confirmed through meticulous examination of microscopy and histopathology of the aspirat

A bimodal analysis of clinical phenotypes and eosinophilic responses as prognostic indicators in gastrointestinal basidiobolomycosis.

OBJECTIVES: Gastrointestinal basidiobolomycosis (GIB) is an emerging infection caused by Basidiobolus ranarum (Entomophthorales), typically producing chronic granulomatous inflammation in immunocompetent hosts and remains under-recognized in endemic regions. We compared pediatric and adult disease trajectories and assessed whether absolute eosinophil count (AEC) is associated with outcome. METHODS: We conducted a 10-year (2015-2024) single-center retrospective observational study at The Royal Hospital, Muscat, Oman, including 13 patients with GIB diagnosed primarily by characteristic histopat

Piloting an AI Introduction Program for Incoming Medical Students: A Novel Approach to Medical Education.

Traumatic Brain Injury (TBI) could lead to intracranial hemorrhage (ICH), which has now been identified as a major cause of death after trauma if it is not adequately diagnosed and properly treated within the first 24 hours. CT examination is widely preferred for urgent ICH diagnosis, which enables the fast identification and detection of ICH regions. However, the use of it requires the clinical interpretation by experts to identify the subtypes of ICH. Besides, it is unable to provide the details needed to conduct quantitative assessment, such as the volume and thickness of hemorrhagic lesion

Clinical and health economic impact of isavuconazole for treatment of invasive aspergillosis and mucormycosis: a retrospective, matched multicentre cohort study in Germany.

PURPOSE: Isavuconazole is effective against invasive aspergillosis (IA) and mucormycosis (IM) and may improve clinical outcomes compared to alternative antifungal treatments. However, real-world evidence regarding its clinical use and the health economic burden of inpatient treatment for IA and IM of patients with haematological malignancies remains limited. METHODS: A retrospective, matched, multicentre cohort study was conducted in six German tertiary care centres. The study included adults with haematological or oncological diseases who were diagnosed with proven, probable, or possible IA

Stable mammalian expression of His-tagged prestin in Chinese hamster ovary cells.

INTRODUCTION: The demographic development in Germany, especially in Saxony-Anhalt (SA), also poses challenges for vascular surgery, as the incidence of vascular diseases has increased following demographic change. For example, the prevalence of peripheral arterial occlusive disease (PAOD) in industrialised countries is estimated at around 10-20% in people over 60 years of age; thus, the number of people affected will also increase here with demographic change. Especially in rural areas, it seems to be more difficult for patients to reach appropriate specialist treatment. MATERIAL AND METHODS:

A BRCT domain-containing protein induced in early phagocytosis plays a crucial role in the pathogenesis of the mucoralean Rhizopus microsporus.

Mucormycosis, caused by mucoralean fungi, is among the most lethal fungal diseases, and a deeper understanding of its pathogenesis is urgently needed. Transcriptomic profiling of virulent strain (WT) and an RNAi-deficient strain (r3b2Δ) of M. lusitanicus during phagocytosis uncovered thousands of differentially expressed genes (DEGs), highlighting early metabolic activation as a key survival strategy inside the phagosome. Enriched pathways included amino acid transport, nucleotide metabolism, and translation, reflecting an adaptive fungal response to nutrient deprivation and host immune stress

Albumin orchestrates a natural host defence mechanism against mucormycosis.

Mucormycosis is an emerging, life-threatening human infection caused by Mucorales fungi1-3. Metabolic disorders uniquely predispose an ever-expanding group of patients to mucormycosis through poorly understood mechanisms1,2,4,5, suggesting that uncharacterized host metabolic effectors may confer protective immunity against this infection. Here we uncover a master regulatory role of albumin in host defence against Mucorales through the modulation of fungal pathogenicity. Our initial studies identified severe hypoalb uminaemia as a prominent metabolic abnormality and an independent biomarker of

Genomic analyses of globally distributed Rhizopus microsporus populations indicate clinical isolates derived from environmental diversity reservoirs.

Mucormycosis is a group of diseases that is increasing in frequency. A common opportunistic human fungal pathogen in this group is Rhizopus microsporus, which is a globally distributed species present in soil-associated environments. A subset of isolates in this species host endobacteria that are hypothesized to influence fungal pathogenicity in both clinical and environmental settings. We have limited understanding of how clinically and environmentally derived isolates are related or how physiological attributes, including thermotolerance and endosymbiosis, are correlated with population stru

Airborne Mucorales in the Netherlands largely mirror Dutch clinical isolates causing mucormycosis.

We determined the optimal method to culture Mucorales species from air samples. Subsequently, we investigated the diversity of Mucorales species in Dutch air and compared these species with those causing mucormycosis in patients. We optimized the Mucorales culturing protocol by testing different growth conditions with samples from a newly developed air sampling approach. We used 120 air samples taken throughout the Netherlands in the project called Schimmelradar (September-October 2023). These samples were supplemented with additional air samples taken in the Netherlands (February 2024). The M

Conidiobolomycosis: apparent yet overlooked. A report of two cases.

BACKGROUND: Conidiobolomycosis is a rare, chronic subcutaneous fungal infection caused primarily by Conidiobolus coronatus and Conidiobolus incongruus. It predominantly affects immunocompetent individuals and presents as slowly progressive rhino-facial swelling. Due to its indolent nature and rarity, it is often misdiagnosed, leading to delayed treatment and potentially disfiguring outcomes. We report two successfully treated cases of conidiobolomycosis from Sri Lanka. CASE PRESENTATION: The first patient, a 27-year-old immunocompetent male, presented with a six-month history of left-sided na

Repurposing antiviral agents against mucormycosis.

Mucormycosis is a life-threatening fungal infection with limited treatment options and high mortality rates among immunocompromised individuals. To identify new therapeutic strategies, we screened a library of 618 antiviral compounds against Rhizopus delemar both alone and in combination with amphotericin B (AmB) to search for agents with intrinsic antifungal activity or the ability to enhance AmB's efficacy. Four candidates, IMB-301, U18666A, BLT-1, and obefazimod, showed potent in vitro effects, with three sustaining growth suppression comparable to AmB for up to 48 h in time-kill assays. Th

Real-World Performance of Aspergillus and Mucorales Polymerase Chain Reaction Assays on Bronchoalveolar Lavage Fluid in Immunocompromised Children and Young Adults.

We describe use of Aspergillus and Mucorales polymerase chain reaction (PCRs) in 54 immunocompromised children and young adults undergoing 66 bronchoscopies. Test positivity was 2/54 (3.2%) and 1/59 (1.7%) for Aspergillus and Mucorales. For Aspergillus PCRs, 4/54 (7.4%) positively impacted management, compared to 4/59 (6.8%) for Mucorales. Fungal PCRs may influence care of immunocompromised patients, though overall impact was limited.

Survival and renal outcomes of mucormycosis infection and its treatment regimens in resource-limited settings: a retrospective case series.

Mucormycosis is a rapidly progressive opportunistic fungal infection associated with high morbidity and mortality, especially in immunocompromised patients. In resource-limited settings, management is further hindered by delayed diagnosis and restricted access to safer antifungal formulations. To evaluate survival and renal outcomes among patients with mucormycosis treated with different antifungal regimens. A retrospective review was conducted on 19 patients diagnosed with mucormycosis. Clinical features, comorbidities, antifungal therapy, complications, and outcomes were analyzed. Patients w

The challenges of Candidozyma auris: updates on the laboratory investigation, infection prevention and management.

Candidozyma (also known as Candida) auris, now comprising six clades, has emerged globally as an important cause of nosocomial infection and outbreaks. Unique and concerning characteristics include its ability to persist in the environment and resist disinfectants and antifungals, as well as a propensity to cause invasive infection with mortality from bloodstream infections estimated at >40%. Other challenges have included isolation and identification in the laboratory, though newer enrichment protocols, chromogenic media, and Matrix-Assisted Laser Desorption/Ionization Time-of-Flight mass spe

Rapid and reliable diagnosis of mucormycosis using colorimetric loop-mediated isothermal amplification.

Current diagnostic approaches for mucormycosis are often limited by low sensitivity and prolonged turnaround times, which result in delayed treatment and poor clinical outcomes. We developed a novel diagnostic method utilizing a colorimetric loop-mediated isothermal amplification (LAMP) assay for the rapid and sensitive detection of mucormycosis. The assay incorporates specifically designed primers capable of detecting as low as 0.001 picograms (pg) of spiked genomic DNA from Mucorales fungi. This LAMP assay demonstrated a high sensitivity of 98% and a 100% specificity of detecting fungal ribo

Protoellisomyces batrachophilus, gen. et sp. nov., a novel member of the Mucor circinelloides complex causing mucormycosis in a toad.

Mucormycosis is an opportunistic infection that affects humans and animals, being caused by moulds of the order Mucorales. In amphibians, mucormycosis has been well documented in association with Mucor amphibiorum. In this study, histopathological examination of cutaneous nodules in a Japanese common toad (Bufo japonicus) revealed fungal granulomas and sporangia. Phylogenetic analyses of five partial gene fragments, namely the internal transcribed spacer, mini chromosome maintenance complex component 7 (mcm7), largest subunit of RNA polymerase II (rpb1), 20S ribosomal RNA accumulation protein
