Establishing and Optimizing Kangaroo Mother Care Practices Among Very-Low-Birth-Weight Neonates in a Tertiary Neonatal Intensive Care Unit (NICU): A Quality Improvement Approach.

Background Kangaroo Mother Care (KMC) is an evidence-based practice that improves outcomes in very-low-birth-weight (VLBW) neonates by lowering neonatal mortality, hypothermia, sepsis, and hospital stay duration while enhancing breastfeeding, physiological stability, and neurodevelopmental outcomes. Despite recommendations for initiating KMC within the early postnatal period (within the first 24-72 hours of life) and maintaining prolonged (six to eight hours/day) daily skin-to-skin contact, implementation and continuation of KMC remain inconsistent in neonatal intensive care units (NICUs). The role of targeted next-generation sequencing and ultrasound in diagnosing fetal cytomegalovirus infection: a case report.

BACKGROUND: Cytomegalovirus (CMV) infection is a leading cause of congenital infection and neonatal morbidity. Conventional diagnostic methods, such as polymerase chain reaction (PCR) and amniocentesis, remain important in the diagnosis of congenital CMV infection, although each method has its own limitations in clinical practice. CASE PRESENTATION: A 31-year-old woman, gravida 3 para 1, presented for routine prenatal evaluation. At 18 weeks of gestation, ultrasound revealed echogenic bowel and fetal ascites. Amniocentesis at 19 weeks showed normal chromosomal results, but targeted next-gener

A regional quality-improvement collaborative to reduce early neonatal hypothermia in critically ill newborns.

BACKGROUND: Early postnatal hypothermia (EPH) remains a significant clinical concern in critically ill transported neonates. A previous regional study reported an alarming EPH incidence rate of 82.1%, with wide variation across delivery facilities, underscoring the urgent need for quality improvement (QI) in early body temperature management. This study aimed to reduce the percentage of EPH in critically ill transported newborns by 20% to 25% within 1 year. METHODS: A standardized, evidence-based thermal care bundle and comprehensive staff training were implemented using multiple Plan-Do-Stud

Early postnatal hypothermia in critically transported newborns at a Chinese tertiary care neonatal centre: a retrospective study.

BACKGROUND: Neonatal hypothermia, especially occurring in the early postnatal period, is a critical condition associated with increased morbidity and mortality. Effective thermal management is essential, yet regional data on early postnatal hypothermia (EPH) incidence among critically ill neonates requiring inter-hospital transport are limited. METHODS: This retrospective, multicentre, observational cohort study involved critically ill newborns transported from six delivery hospitals, including two women's hospitals and four general hospitals, to a tertiary care neonatal centre between Januar

Establishment of an immortalized erythroid model for Hb Bart's hydrops fetalis with homozygous α0-thalassemia Southeast Asian deletion.

Hemoglobin Bart's hydrops fetalis syndrome (BHFS) is the most severe form of α-thalassemia and is highly prevalent in Southeast Asia. It is most commonly caused by homozygosity for the α0-thalassemia Southeast Asian deletion (--SEA/--SEA), which eliminates α-globin production and results in profound fetal anemia. Current management is limited to intrauterine transfusions with only transient benefit, and no pharmacological or gene-based therapies are available. Progress toward therapy has been hampered by the lack of disease-relevant in vitro models. Here, we established Chery-Bart's, an immort

Neonatal Hydrops and Biliary Atresia as an Early Presentation of Mevalonate Kinase Deficiency: A Case Report.

Mevalonate kinase deficiency (MKD) is a rare autosomal recessive autoinflammatory disorder with a broad clinical spectrum. Neonatal presentation is uncommon and may mimic severe infection, leading to diagnostic delay. We report a preterm female infant who presented with non-immune hydrops fetalis and persistent systemic inflammation from the first week of life, despite repeated negative blood and CSF cultures and multiple courses of broad-spectrum antibiotics. The infant subsequently developed progressive cholestasis with pale stools, and biliary atresia was confirmed by hepatobiliary imaging

Non-immune hydrops fetalis (NIHF): diagnostic evaluation and lessons learned from a case series.

BACKGROUND: Fetal hydrops, characterized by abnormal fluid accumulation in two or more fetal compartments, is a critical condition with a poor prognosis, often resulting in significant fetal mortality. The condition itself is not a diagnosis but a clinical sign with many underlying causes, ranging from infections and chromosomal abnormalities to rare maternal conditions such as mirror syndrome. CASE PRESENTATION: This review examines three cases of fetal hydrops, highlighting common etiologies, such as infections and chromosomal disorders, and more unusual causes like maternal complications.

Application of next-generation sequencing in nonimmune hydrops fetalis and its impact on pregnancy decisions.

BACKGROUND: Nonimmune hydrops fetalis (NIHF) has a highly heterogeneous etiology, with genetic factors contributing substantially to its pathogenesis. This study aimed to investigate the chromosomal and monogenic etiologies of NIHF. METHODS: This single-center retrospective cohort study included 121 pregnancies complicated by NIHF evaluated at a tertiary referral center between August 2020 and August 2023. All cases underwent copy number variation sequencing (CNV-seq), and 95 additionally underwent whole-exome sequencing (WES), both assays were based on next-generation sequencing (NGS) techno

Impact analysis and evaluation of the Ethiopian Neonatal Network.

The Ethiopian Neonatal Network (ENN) represents one of the first low-income country neonatal quality improvement (QI) communities. To determine if changes in structure, process and outcome measures were associated with formation of the ENN and to learn from teams, quantitative and qualitative analyses were completed. All infants discharged during 2018-2022 from 11 hospitals with consistent data collection over the 5-year period were included in infant-level analyses. Trends by year were evaluated using Cochran-Armitage tests. Annual surveys captured facility-level data. Nurse and physician lea

Prenatal Diagnosis and Monitoring of Type III Congenital Pulmonary Airway Malformation: A Case Report.

Congenital pulmonary airway malformation (CPAM) is a rare congenital lung anomaly resulting from abnormal bronchial development and disorganized respiratory structures, producing nonfunctional pulmonary lesions. Although uncommon, CPAM is the most frequently diagnosed congenital lung malformation. Prenatal identification allows for risk stratification and anticipatory management. Here we present a case of a fetus prenatally diagnosed with a type III CPAM during routine obstetric ultrasonography. Weekly ultrasonography was performed to monitor lesion progression and evaluate for complications,

Perinatal survival and long-term neurodevelopmental outcomes of congenital chylothorax.

BACKGROUND: Congenital chylothorax carries a poor prognosis, particularly with delivery at <32 weeks. Survival outcomes under consistent management prioritizing pregnancy prolongation despite worsening hydrops, as well as long-term neurodevelopmental data, remain limited. METHODS: In this single-center retrospective study, we analyzed fetuses with congenital chylothorax requiring perinatal drainage managed between 2005 and 2021. The protocol prioritized prolonging pregnancy to ≥32 weeks. Postnatally, octreotide and prednisolone were administered stepwise for persistent drainage. RESULTS: Of

Enhancing Hemoglobin Bart's hydrops fetalis syndrome prevention: a single-tube multiplex real-time PCR assay for the comprehensive detection of four significant α0-thalassemia deletions (--SEA, --THAI, --CR, and --SA) found in Thailand.

BACKGROUND: Hemoglobin (Hb) Bart's hydrops fetalis is a major public health concern in Southeast Asia, particularly in Thailand. Current screening strategies target the two most common α0 -thalassemia deletions (--SEA and --THAI). METHOD: In this study, we developed a single-tube multiplex real-time PCR assay for the simultaneous detection of four clinically relevant α0-thalassemia deletions (--SEA, --THAI, --CR, and --SA). The assay was validated using 538 clinical samples with diverse thalassemia genotypes and compared against conventional gap-PCR as the reference method. Analytical perform

False negativity for trisomy 18 in non-invasive prenatal testing: A case report.

False-negative results in non-invasive prenatal testing are rare but clinically significant, particularly in the presence of biological factors such as feto-placental mosaicism. This report describes a pregnancy affected by fetal trisomy 18 in which three consecutive non-invasive prenatal tests reported low risk despite abnormal first-trimester ultrasound findings (increased nuchal translucency, hydrops fetalis, and tricuspid regurgitation). Amniocentesis subsequently confirmed trisomy 18 in the fetus. Post-termination placental analysis revealed mosaicism, with high trisomy 18 levels on the f

Severe Antenatal Presentation of a Novel Dnase2 Mutation in a Preterm Omani Neonate: Expanding the Clinical Spectrum of an Ultra-Rare Interferonopathy.

Dnase2 deficiency is an ultra-rare type I interferonopathy resulting from impaired intracellular DNA degradation, leading to neonatal cytopenias and systemic inflammation. Whole-exome sequencing enables definitive diagnosis, and Janus kinase inhibitors have emerged as targeted immunomodulatory therapy. This report describes a preterm infant with an early and severe presentation of DNASE2 deficiency, manifesting antenatally with fetal hydrops, intrauterine growth restriction, and cardiomyopathy. Postnatally, the infant developed persistent pancytopenia, hepatosplenomegaly, cholestatic jaundice,

High Diagnosis Rate for Nonimmune Hydrops Fetalis With Prenatal Clinical Genome: Expanded Results From the Hydrops-Yielding Diagnostic Results of Prenatal Sequencing (HYDROPS) Study.

Nonimmune hydrops fetalis (NIHF) is characterized by abnormal fluid accumulation in ≥ 2 fetal compartments and may be genetic. The incremental diagnostic yield of prenatal exome sequencing (ES) for NIHF following a negative standard workup was previously explored on 22 cases. We report ES findings in the remaining cohort and investigate the incremental yield of genome sequencing (GS) in cases without a definitive diagnosis from exome. Enrollment criteria and exome methodology for the Hydrops-Yielding Diagnostic Results of Prenatal Sequencing (HYDROPS) study were previously reported. Exomes wit

Enhancing Diagnostic Precision in Non-Immune Hydrops Fetalis: The Incremental Value of Whole Exome Sequencing-A Prospective Cohort Study.

Non-immune hydrops fetalis (NIHF) is a phenotypically heterogeneous condition associated with substantial perinatal morbidity and mortality and its underlying etiology often remains unresolved despite comprehensive conventional investigations. We evaluated the etiological spectrum of NIHF and the additional diagnostic contribution of whole-exome sequencing (WES) in a prospective cohort from Western Rajasthan, India. Eighty consecutive fetuses with NIHF were enrolled between January 2020 and March 2026. Fetuses with immune hydrops or twin-to-twin transfusion syndrome were excluded. All cases un
