Primary testicular diffuse large B-cell lymphoma with cutaneous metastasis to the chest: a case report.

We present the case of primary testicular diffuse large B-cell lymphoma diagnosed at the stage of pectoral skin metastasis in a 40-year-old male patient. He presented with a painless mass in the left scrotum that was complicated by ulcerated gynecomastia. An orchiectomy combined with a pectoral tumorectomy led to this diagnosis. The tumor was of the non-centro-germinative B subtype and classified as pT3NxM1b, Ann-Arbor stage IV E. The patient died while undergoing chemotherapy. This case underscores the poor prognosis associated with this histological type and the value of immunohistochemistry

Successful perinatal management of a large placental chorio-angioma: a case report demonstrating the effectiveness of a public-private partnership model.

Placental chorio-angiomas are rare benign vascular tumours of the placenta which are usually small and clinically insignificant, but large lesions can cause serious fetal complications such as anaemia, hydrops fetalis and an adverse perinatal outcome. We report the case of a 29-year-old multigravida diagnosed at 28 weeks' gestation with a large placental chorioangioma accompanied by sonographic evidence of fetal anaemia and hydrops fetalis. Despite two intra-uterine transfusions which led to partial improvement, the hydrops persisted, necessitating a preterm caesarean delivery at 31 weeks. The

Multi-omic analysis identifies a multi-step pathology in a case of multiple chorangioma syndrome in monochorionic twins.

BACKGROUND: Chorangiomas, benign capillary lesions of the placenta, occur in ~1% of births, typically as solitary nodules. Multiple chorangioma syndrome is rare and increases risk of fetal heart failure, hydrops fetalis, and intrauterine death due to placental hemodynamic disruption. While genetic and hypoxic factors have been suggested in chorangioma development, direct molecular evidence is lacking. Here, we present a unique case of multiple chorangiomas confined to half of a shared placenta in monozygotic monochorionic diamniotic twins, providing a rare opportunity to dissect molecular driv

Finerenone in Treating a 12-Year-Old Boy Suffering Gitelman Syndrome Without Causing Gynecomastia.

Gitelman syndrome (GS) is an autosomal recessive disorder caused by mutations in the SLC12A3 gene, characterized by hypomagnesemia, hypokalemia, alkalosis, and hypocalciuria. Finerenone, a non-steroidal mineralocorticoid receptor antagonist (MRA), has shown cardiorenal protective effects and improved prognoses in type 2 diabetes (T2DM) and chronic kidney disease (CKD). However, its use in non-diabetic nephrology has been infrequently reported. This report presents a case of a male child with GS who was treated with finerenone in combination with potassium and magnesium supplements to elevate p

Cytogenetic and Molecular Findings in Hydrops-Related Mirror Syndrome.

OBJECTIVE: Mirror syndrome is a rare, life-threatening condition in which maternal fluid overload mirrors fetal hydrops. Data on genetic findings in affected pregnancies are limited. We compared genetic diagnoses in hydrops cases with and without mirror syndrome. METHODS: We conducted a retrospective cohort study (2012-2023) of pregnancies with non-immune hydrops at a single fetal center. Clinical data along with prenatal and postnatal genetic testing results were analyzed. Mirror syndrome was defined as fetal hydrops with new or worsening hypertension and significant maternal edema. Genetic

[A novel CYP19A1 variant causing aromatase deficiency in a family: case report and literature review].

Aromatase deficiency is a rare autosomal recessive disorder of estrogen biosynthesis caused by pathogenic variants in the CYP19A1 gene, located on chromosome 15q21.2. Maternal virilization during pregnancy serves as an important diagnostic clue. Affected females may present with virilized external genitalia at birth, and later in life may develop primary amenorrhea, ovarian cysts or ovarian dysplasia, delayed epiphyseal closure, and reduced bone mineral density. Herein, a family with aromatase deficiency admitted to the Department of Endocrinology and Metabolism at the Affiliated Hospital of Q

Unexpected Hair Regrowth Following Oral Bicalutamide Exposure in a Male Patient With Androgenetic Alopecia and a Suboptimal Response to Conventional Therapies: A Case Report.

Androgenetic alopecia (AGA) is the most common cause of non-scarring hair loss in men. Although topical minoxidil and oral finasteride are established treatments, some patients show limited clinical response despite prolonged therapy. Evidence on the use of oral bicalutamide in male AGA remains scarce because of concerns about systemic adverse effects. We report the case of a 45-year-old man with treatment-resistant AGA who experienced clinically apparent hair regrowth during oral bicalutamide exposure after an inadequate response to conventional therapies. Treatment was accompanied by gynecom

Bardet-Biedl syndrome in two sibling pairs: a case series.

BACKGROUND: Bardet-Biedl syndrome is a rare multisystem ciliopathy defined by retinal dystrophy, obesity, polydactyly, hypogonadism, and renal anomalies, often accompanied by neurodevelopmental and behavioral issues. Diagnosis relies heavily on clinical features, particularly in resource-limited settings where genetic testing is unavailable. CASE PRESENTATION: We describe two sibling pairs, all of South Asian, Pakistani ethnicity, clinically diagnosed with Bardet-Biedl syndrome using contemporary diagnostic frameworks. The first pair, two male brothers aged 17 and 21 years, came from a consan

Diverse clinical spectrum of Niemann-Pick C: insights from a single center.

OBJECTIVES: Niemann-Pick disease type C (NPC) is a lysosomal lipid trafficking disorder characterized by impaired intracellular cholesterol transport, resulting in progressive neurovisceral involvement. While neurological progression has been extensively described, metabolic biomarkers and lipid abnormalities may provide earlier diagnostic clues, particularly in high-consanguinity populations. METHODS: We retrospectively evaluated 14 genetically confirmed NPC patients followed at a tertiary pediatric metabolic center between 2018 and 2025. Clinical presentation, biochemical parameters includi

Discordance of 99mTc-PYP Uptake in the Myocardium and Breast Both Before and After Treatment in Transthyretin Cardiac Amyloidosis.

Scintigraphy using 99mTc-PYP as a bone-avid radiotracer reveals physiological and pathologic accumulation of the tracer in the extraosseous tissues and organs-including the breasts-in addition to the bones. 99mTc-PYP scintigraphy also reveals deposits of wild-type amyloid transthyretin (ATTRwt) in the myocardium and extracardiac sites, such as subcutaneous fat, skeletal trunk muscles, and alimentary tract, of patients with ATTRwt cardiomyopathy (CM). We describe an 87-year-old man with ATTRwt-CM in whom myocardial 99mTc-PYP uptake was reduced and mammary tracer uptake was increased due to gyne

A case of osteoporotic fracture leading to diagnosis of Klinefelter syndrome in a 76-year-old man.

Klinefelter syndrome (KS) is the most prevalent sex chromosome disorder in men but often remains clinically silent until later life. We describe an elderly man whose evaluation for a rib fracture uncovered the diagnosis. A 76-year-old male presented after a low-impact fracture during a coughing episode. Clinical history revealed delayed sexual maturation, small testes, infertility, and mild gynecomastia. Hormonal testing demonstrated markedly reduced testosterone with elevated luteinizing and follicle-stimulating hormones, suggesting primary testicular failure. Karyotype analysis confirmed 47,

Randomized trial of occlusive wrap for heat loss prevention in preterm infants: neurodevelopmental outcome.

BACKGROUND: Heat loss in the delivery room in very preterm infants is associated with increased mortality, hypothermia and other adverse outcomes. The Heat Loss Prevention (HeLP) trial resulted in a significant reduction of hypothermia but not a reduction in mortality. The HeLP follow-up trial was designed to determine whether application of occlusive wrap applied immediately after birth reduces adverse long-term neurodevelopmental outcomes at 18-24 months corrected gestational age in very preterm infants. METHOD: The original HeLP trial was a randomized controlled trial in infants < 28 weeks

Repair of Severe Tricuspid Regurgitation in a Neonate With Intrauterine Tricuspid Valve Chordae Tendinae Rupture.

Congenital tricuspid valve regurgitation (TR) is rare, and chordae tendinae rupture is an even more uncommon finding in neonates. We describe a case with intrauterine tricuspid valve chordae tendinae rupture leading to severe TR and hydrops fetalis. Successful tricuspid valve repair using artificial chordae, leaflet approximation, and annular reduction were used as rescue therapy for the neonate.

Primary cardiac hemangioma: a report of two cases and literature review.

Cardiac hemangiomas (CHs) are extremely rare tumors. Despite their benign nature, CHs can lead to serious complications, including cyanosis, arrhythmia, heart failure, hydrops fetalis, or fetal death. While echocardiography provides real-time assessment, CHs may mimic other cardiac abnormalities, necessitating further imaging modalities such as CT or MRI for accurate diagnosis. Treatment typically involves radical resection, even in asymptomatic cases. We describe two cases of cardiac hemangiomas in the atrium, and both patients exhibited an uneventful recovery after a radical resection.

Rare β-human chorionic gonadotrophin-secreting urothelial carcinoma initially presenting as gynecomastia.

Gynecomastia is a relatively common condition arising from disproportionate concentrations of estrogen and androgen. Most cases of bilateral gynecomastia in men are idiopathic or drug-induced but can rarely arise secondary to malignancy. We present the case of a 57-year-old male who presented with new-onset gynecomastia and recurrent episodes of hematuria. Laboratory testing revealed elevated estradiol, free testosterone, and β-human chorionic gonadotropin (β-hCG) with suppressed luteinizing hormone (LH) and follicle stimulating hormone (FSH), concerning for ectopic β-hCG secretion.
