CYP19A1 variation in healthy children: associations with breast development and pubertal gynecomastia.

BACKGROUND: CYP19A1 encodes aromatase, the enzyme converting androgens to estrogens, thereby influencing breast tissue development in healthy children. OBJECTIVE: To examine associations between CYP19A1 single nucleotide polymorphisms (SNPs) and circulating estradiol (E2) and testosterone (T), the E2/T ratio and age at thelarche in girls, and the presence of gynecomastia in boys. METHODS: A total of 1,023 healthy participants (aged 5.9-20.0 years) from the Copenhagen Puberty Study were assessed for pubertal status, and serum sex steroid concentrations were measured by LC-MS/MS. Genotyping of

The Impact of Antenatal Lactation Counseling on Breastfeeding Practices and Newborn Feeding Behavior Among Primipara Mothers Attending a Tertiary Care Hospital in Puducherry, India: A Randomized Controlled Trial.

Introduction Although human milk provides unparalleled immunobiological and nutritional advantages essential for optimal neonatal development, the establishment of successful lactogenesis and sustained breastfeeding remains a complex psychophysiological challenge, particularly for primiparous mothers. First-time parturients frequently encounter biomechanical difficulties, such as suboptimal infant positioning and latch, which are compounded by postnatal anxiety. These factors can disrupt oxytocin-mediated milk let-down and precipitate disorganized neurobehavioral feeding reflexes in the newbor

Melatonin treatment for neonatal encephalopathy.

RATIONALE: Neonatal encephalopathy (NE) affects 1.15 million newborns annually and is associated with a high incidence of mortality and long-term neurodevelopmental disability (NDD). Therapeutic hypothermia (TH) is the only proven therapeutic intervention available. However, it is only partially effective, and evidence suggests it is not safe in low-income countries. Melatonin is a promising neuroprotective treatment in NE as it provides anti-inflammatory properties. Pre-clinical models have demonstrated improved outcomes with melatonin. However, its effect on human newborns is unclear. OBJEC

Placental Expression of CD15 in Fetal Anemia From Hydrops Fetalis.

INTRODUCTION: CD15 expression has been used as a marker of endothelial immaturity in the placenta and is linked to the setting of antenatal hypoxia. If so, expression should be increased in fetal anemia, especially when severe enough to cause hydrops fetalis. This has not been previously reported and the current study tested this hypothesis. MATERIALS AND METHODS: The study group consisted of 10 cases of anemic hydrops in second to early third trimester of gestation and 10 controls matched for gestational age. The majority (70%) of hydrops cases were due to hemoglobin (Hb) Bart. Placentas und

Identification of Human Transferrin Receptor as an Entry Co-receptor for Parvovirus B19 Infection of Human Erythroid Progenitor Cells.

UNLABELLED: Parvovirus B19 (B19V), a member of the genus Erythroparvovirus within the Parvoviridae family, infects human erythroid progenitor cells (EPCs) of bone marrow and fetal liver, and causes various hematological disorders. The minor capsid protein VP1 of B19V contains a unique N-terminal region (VP1u) that facilitates virus binding and internalization into EPCs via its receptor-binding domain (RBD). We previously identified tyrosine protein kinase receptor UFO (AXL) as a proteinaceous receptor for B19V infection of EPCs. In this study, we employed an ascorbate peroxidase 2 (APEX2)-base

Neonatal PBPK model of midazolam and its metabolites for perinatal asphyxia and therapeutic hypothermia.

BACKGROUND AND PURPOSE: Therapeutic hypothermia (TH) decreases morbidity and mortality in neonates with perinatal asphyxia (PA) and moderate-to-severe hypoxic-ischaemic encephalopathy. Midazolam is an antiepileptic and substrate of CYP3A4. Drug metabolism during PA/TH remains partially characterized, and disentangling effects of PA and TH is difficult, making precision dosing challenging. The I-PREDICT project aims to develop a framework for drug disposition in PA/TH neonates, using physiologically based pharmacokinetic (PBPK) modelling. The predictive performance of the model was explored for

Pregnancy induced hypertension and mirror syndrome associated with fetal hemoglobin Bart's disease in Thailand: a retrospective cohort study.

BACKGROUND: Pregnancy-induced hypertension (PIH) and mirror syndrome associated with fetal hemoglobin (Hb) Bart's disease have not been well characterized. This study primarily compared the incidence of these conditions in late gestation between affected and low-risk pregnancies, and secondarily assessed differences in other maternal complications. METHODS: A large retrospective cohort study (between 1992 and 2022) was conducted at a single tertiary center. The consecutive singleton pregnancies with fetal Hb Bart's disease diagnosed at ≥24 weeks, considered as delayed diagnosis were recruited

The Role of Sex Steroids, Sex Hormone-binding Globulin and Leptin in the Development of Pubertal Gynecomastia and its Persistence into Adulthood.

OBJECTIVE: Pubertal gynecomastia (PG) is a benign condition considered to be caused by imbalance between estrogen and testosterone, though this is not frequently evident from serum analysis. This study aimed to determine possible alteration in sex hormone levels, as well as to investigate the role of sex hormone-binding globulin (SHBG) and leptin in PG development and persistence. METHODS: A total of 95 boys with PG were included. They were divided into 3 groups considering stage. Anthropometric measurements, hormonal investigations, SHBG and leptin were collected and compared group of 64 con

Refined classification and phenotype-driven analysis of PIEZO1 variants in hereditary red blood cell and iron disorders.

Interpreting genetic variants in complex genes such as PIEZO1 remains challenging because of marked allelic heterogeneity, relative tolerance to missense variation, and overlapping clinical phenotypes. Gain-of-function variants in PIEZO1 cause dehydrated hereditary stomatocytosis (DHS1, or hereditary xerocytosis), a pleiotropic syndrome characterized by anemia of variable severity and iron overload. In this study, we provided and applied an integrative framework combining the American College of Medical Genetics and Genomics guidelines, quantitative in silico predictions, structural domain ann

Prenatal stress improves the ability of neonatal lambs to maintain body surface temperature during periods of cold exposure.

Exposure to cold and hypothermia is a significant threat to newborn lamb survival in extensive systems, and the ability to thermoregulate is essential for improved survivability. Lambs exhibit enhanced thermogenic abilities and increased brown fat deposition when ewes experience cold during late-pregnancy. This study evaluated the effects of various prenatal stressors on newborn lamb thermogenesis through three experiments. Experiment 1 examined the impact of mid-pregnancy shearing and wetting on lamb thermogenesis. Experiment 2 involved transporting and cold-exposing late-pregnancy ewes. Expe

Fetal ascites caused by ovarian incarceration within an inguinal hernia and development of maternal mirror syndrome.

Fetal ascites is a non-specific finding which can be associated with fetal hydrops or an isolated intra-abdominal cause. We describe a case of fetal ascites associated with an intra-abdominal mass suspected to be ovarian in nature and diagnosed in the third trimester. The mother developed maternal mirror syndrome, previously reported to be associated with fetal hydrops but not reported in cases of isolated fetal ascites. Neonatal laparoscopy revealed the left ovary was oedematous and enlarged next to an inflamed and oedematous deep ring suggesting a previously incarcerated ovarian inguinal her

Antenatal Presentation of MRPS22-Related Mitochondrial Disease Confirmed With Rapid Proteomics.

MRPS22-related mitochondrial disease (MIM#611719) is a rare autosomal recessive disorder caused by defects in the mitochondrial ribosomal protein S22, a component of the small mitoribosomal subunit essential for mitochondrial translation. Of the few reported cases, most present antenatally with a severe phenotype, conveying a poor prognosis. We describe a fetus with severe antenatal-onset MRPS22-related mitochondrial disease and the use of multi-omics in the molecular diagnosis. A primigravida underwent termination of pregnancy following identification of multiple congenital anomalies (hydrops

Feasibility and reproducibility of resting-state functional magnetic resonance imaging in asphyxiated neonates.

BACKGROUND: In the ongoing search for prognostic tools in neonates suffering perinatal asphyxia, we assessed the feasibility of resting-state functional magnetic resonance imaging (RS-fMRI). The aim of our study was to determine both the intra-session and inter-subject reproducibility of resting-state networks (RSNs), as well as factors affecting these. NEW METHOD: We conducted a prospective multicenter study in 21 asphyxiated term newborns (mean gestational age = 39.2 weeks, mean 5-minute Apgar score = 3) after treatment with controlled hypothermia at a level III neonatal intensive care unit
