Delayed Rewarming Thrombocytopenia Not Disseminated Intravascular Coagulation, is the Cause of Rewarming Deaths in Neonatal Cold Injury.

Disseminated intravascular coagulation (DIC) was suspected of being the cause of fatal rewarming deaths in neonatal cold injury (NCI) in the absence of any other known explanation for this complication. It was associated with thrombocytopenia, bleeding, and abnormal clotting studies. Subsequently, this suspicion was questioned because of the reversibility of thrombocytopenia and more recently because of the delineation of a new entity DRT (delayed rewarming thrombocytopenia) that better explained the clinical and laboratory features of this condition. The sudden bleeding that occurs after 24 h

Novel purine derivatives mitigate hypoxia ischemia related brain injury through agrin, zyxin and synaptotagmin proteins.

Hypoxic-ischemic encephalopathy (HIE) is a major cause of morbidity and mortality in newborns resulting in motor and cognitive impairment. Therapeutic hypothermia is the only treatment approved for HIE. Consequently, there is a critical requirement for additional treatments for hypoxic-ischemic (HI) brain injury because hypothermia is only partially protective. Pharmacological therapeutics are as yet not available to treat HIE. Therefore, we developed a novel trisubstituted purine-derivative drug (BRT_002) to attenuate HI related brain injury. The safety of BRT_002 was confirmed by treating ad

Testing a Liquid Crystal Visual Thermometer Device in Newborns and Young Infants.

Objective Severe neonatal jaundice is a major cause of death and disability among newborns in low- and middle-income countries (LMICs). Filtered sunlight phototherapy (FSPT) is safe and effective but requires hourly temperature monitoring to detect hyperthermia/hypothermia. This need for close temperature monitoring by family members and community health extension workers is impeding FSPT scale-up in LMICs, where healthcare providers are scarce and nurseries are understaffed. The ability of a caregiver to accurately measure temperature can affect infant health outcomes in many other illnesses

Surgery on the aortic arch and feasibility of electroencephalography (SAFE) monitoring in neonates: protocol for a prospective observational cohort study.

INTRODUCTION: While survival rates following neonatal surgery for congenital heart disease (CHD) have improved over the years, neurodevelopmental delays are still highly prevalent in these patients. After correcting for the CHD subtype, the severity of developmental impairment is dependent on multiple factors, including intraoperative brain injury, which is more frequent and more severe in those undergoing aortic arch repair with deep hypothermic circulatory arrest (DHCA). It is proposed that brain injury may be reduced if cooling is stopped at the point of electrocerebral inactivity (ECI) on

Factors contributing to neonatal mortality in a tertiary center in south West Bank: A single-center retrospective cohort study.

Neonatal mortality remains a critical public health issue, particularly in low- and middle-income countries, where factors such as preterm birth, low birth weight, congenital anomalies, infections, and limited access to quality healthcare contribute significantly to neonatal deaths. This study examines neonatal mortality outcomes in the Neonatal Intensive Care Unit (NICU) at the Palestine Red Crescent Society (PRCS) Hospital in Hebron, Palestine for newborns that was transferred to PRCS from other hospitals. An institutional-based cohort study was conducted on 606 neonates admitted "transferre

Strategy to Identify Infants with Hypoxic Ischemic Encephalopathy for Therapeutic Hypothermia-A Retrospective Audit.

BACKGROUND/OBJECTIVES: Although there is a critical need for timely, accurate recognition of infants with hypoxic ischemic encephalopathy (HIE) eligible for therapeutic hypothermia (TH), there is little published literature that comprehensively validates strategies to achieve this. For the Mater Mothers' Hospital, a screening protocol combining use of umbilical cord gases according to obstetric criteria and other evidence of depression at birth with a decision aid (the HIE Trigger Tool (TT)) for at-risk infants was developed. We audited whether the protocol supported appropriate clinical decis

Topiramate for the treatment of neonatal seizures and beyond.

Acute symptomatic neonatal seizures are one of the most common neurological disorders in newborns admitted to neonatal intensive care units and require prompt treatment. Up to 50% of neonatal seizures are refractory to first-line medications such as phenobarbital (PB), and another 30% fail second-line therapy. Furthermore, antiseizure medications (ASMs) such as PB have short-term adverse effects and may exert long-term detrimental effects on neurodevelopment. Thus, the development of more effective and safer ASMs is an urgent medical need. Because of its multimodal mechanisms of action and neu

Refractory Bradycardia and Unreactive Mydriasis Revealing Severe Intraoperative Neonatal Hypothermia in a Resource-Limited Setting.

Perioperative hypothermia represents a major complication in pediatric anesthesia, particularly in neonates. We report the case of a newborn who underwent general anesthesia for esophageal fistula repair (following primary repair of esophageal atresia) and developed severe hypothermia manifested by the triad of refractory severe bradycardia, bilateral areflexic mydriasis, and chest wall rigidity. Management combining active rewarming, alkalinization, and cardiovascular support resulted in a complete recovery with a progressive normalization of clinical parameters. We discuss the clinical signs

The advantages and limitations of animal models for understanding acute neonatal brain injury.

Neonatal hypoxic-ischemic encephalopathy (HIE) remains a major cause of death and disability around the world. Therapeutic hypothermia is now established to improve outcomes in term and near-term infants in high-income countries, but even in this setting, many infants still survive with disability. To further improve outcomes, experimental models are needed to test new interventions before clinical translation. In the present review, we discuss the strengths and weaknesses of a range of near-term and term models and their utility for understanding the pathophysiology of HIE and to test potenti

Significant Pathology in Young Infants Presenting With Hypothermia: A Multicenter Study.

OBJECTIVE: Hypothermia in young infants is often attributed to immature thermoregulation but may be the harbinger of significant pathology. We aimed to determine the prevalence and type of significant pathology in young infants aged 90 days or younger presenting with hypothermia (≤36.0 °C) and explore associations between this outcome and presenting characteristics and evaluation. METHODS: We conducted a multicenter, retrospective cohort study of young infants evaluated in the emergency department (ED) or hospital setting for hypothermia over a 5-year period. Patients aged 90 days or younger

Dexmedetomidine: Shifting Paradigms in Neonatal Sedation and Pain Control.

BACKGROUND: Newborns, including preterm infants, are capable of responding to pain. Recurrent pain exposure is associated with suboptimal motor development, cognitive impairments, abnormal brain growth, and maladapted nociceptive reactions. PROBLEM: Current agents, primarily opioids and benzodiazepines, raise major concerns due to their adverse effects, including insufficient sedation or analgesia, withdrawal, depressed respiratory effort, tolerance, and occasional paradoxical agitation. Commonly used drugs such as midazolam and morphine have been shown to induce neuroapoptosis and neurodevel

Immersion Swaddle Bathing Compared to Traditional Sponge Baths for the First Newborn Bath in the Hospital and Thermoregulation.

PURPOSE: The purpose of this quality improvement project was to promote newborn thermoregulation. BACKGROUND: Thermoregulation is a primary concern in the newborn population because poor thermoregulation contributes to impaired metabolic functioning, specifically glucose and respiratory regulation. In severe cases, if untreated, impaired thermoregulation can lead to increased risk of morbidity and mortality. METHODS: Two types of newborn bathing methods were evaluated for their effects on thermoregulation in a sample of healthy newborns: traditional sponge baths and immersion swaddle baths.

Neurophysiological changes associated with dysglycemia in term neonates with neonatal encephalopathy.

OBJECTIVE: To investigate the relationship between hypo/hyperglycemia and EEG measures in term neonates with neonatal encephalopathy (NE) treated with therapeutic hypothermia (TH). METHODS: Ninety-six neonates underwent concurrent continuous glucose monitoring (CGM) and EEG for 48 h within the first 72 h of life. Five-minute epochs of hypoglycemia (≤2.6 mmol/L; ≤47 mg/dL) and hyperglycemia (>10.1 mmol/L; >182 mg/dL) were identified. Continuous EEG data was segmented into 5-min epochs corresponding to 5-min CGM data. Visual EEG background score and 22 computational EEG measures were estimated

Respiratory management in neonates with hypoxic-ischemic encephalopathy during therapeutic hypothermia: a binational analysis of current practice.

UNLABELLED: The respiratory management of neonates with hypoxic-ischemic encephalopathy (HIE) undergoing therapeutic hypothermia (TH) is a challenging issue, but remains poorly standardized. The aim of this study was to describe current practice regarding respiratory management, particularly tracheal intubation, in neonates undergoing TH in France and French-speaking Belgium. In this binational cross-sectional study, current practices about respiratory management were collected by an online-based survey sent to all tertiary neonatal intensive care units providing TH in France and French-speaki

Towards goal-directed perfusion - Part II: Temperature dependence of Q10, oxygen-demand dependence on age and weight, further clinical insights.

BackgroundGoal-directed perfusion (GDP) during cardiopulmonary bypass (CPB) seeks to match oxygen delivery with patient-specific metabolic demand, yet determinants of oxygen demand and dynamics of oxygen extraction ratio (OER) are not well characterised in paediatric populations. Building on our GARIX (Global AutoRegressive Integrated model with eXogenous variables and an equilibrium force) model from Part 1, we analysed minute-level intraoperative data to derive clinical insights.MethodsGARIX integrates autoregressive memory, exogenous clinical variables (cardiac index, haemoglobin, arterial
