Clinical practice guidelines for neonatal therapeutic hypothermia across European Society for Paediatric and Neonatal Intensive Care (ESPNIC) member countries: a multidisciplinary blind comparison.

BACKGROUND: Therapeutic hypothermia (TH) decreases mortality and neuro-developmental disability in neonates greater or equal than 36 weeks' gestation with moderate-to-severe hypoxic-ischaemic encephalopathy (HIE) due to perinatal asphyxia. TH is often applied in different ways, as national guidelines and target populations may significantly differ. We aimed to compare TH guidelines from various European countries as we hypothesised that guidelines are significantly different and often of insufficient quality. METHODS: Multidisciplinary, blind, critical appraisal of clinical practice guideline

Adjunctive mild controlled hypothermia for moderate necrotizing enterocolitis in very low birth weight infants: a prospective cohort study with external contemporaneous controls.

UNLABELLED: Necrotizing enterocolitis (NEC) is a severe condition in preterm infants. Few therapies are available to slow disease progression or reduce the need for surgery. This study aims to assess if mild controlled hypothermia as an adjunctive treatment reduces surgical intervention in preterm infants with NEC. A single-center intervention was conducted from 2019 to 2023 and compared with an external, contemporaneous prospective cohort study. Infants weighing less than 1500 g with moderate NEC were enrolled. One center administered mild hypothermia and served as the intervention cohort. Th

Neonatal surgical mortality in East Africa: A systematic review and meta-analysis.

INTRODUCTION: Neonatal surgical conditions contribute significantly to under-five mortality, particularly in low- and middle-income countries (LMICs). However, comprehensive data on neonatal surgical mortality (NSM) and its determinants in East Africa remain scarce. This systematic review and meta-analysis aimed to estimate the pooled mortality rate among neonates with surgical conditions and identify key predictors of mortality in the region. METHODS: Following PRISMA guidelines, we searched PubMed, Scopus, EMBASE, and Google Scholar from inception to 30 June 2025. Observational studies from

Expert Consensus Approach to Developing Inpatient Common Data Elements for Neonatal Encephalopathy Research.

BACKGROUND AND OBJECTIVES: Neonatal encephalopathy (NE)-including hypoxic-ischemic encephalopathy (HIE)-is associated with significant morbidity and mortality worldwide. While data registries provide hypothesis-generating data, aid in quality improvement, and track management/outcomes over time, we recently demonstrated that only 4 of 1,281 (0.3%) variables were collected by all 22 international NE/HIE registries: birth weight, gestational age, and 1- and 5-minute Apgar scores. In response, our team set out to develop a set of common inpatient data elements to enable future harmonization of NE

Hypoxic-Ischemic Encephalopathy in Newborns: Pathophysiology, Early Identification, and Management.

Hypoxic-ischemic encephalopathy (HIE) is a severe neurological condition resulting from impaired oxygen delivery and reduced cerebral blood flow to the neonatal brain during the perinatal period. It remains one of the leading causes of neonatal mortality and long-term neurodevelopmental disability worldwide, despite significant advances in obstetric and neonatal care. The pathophysiology of HIE is complex and evolves through a cascade of events beginning with primary energy failure, followed by secondary energy failure characterized by mitochondrial dysfunction, oxidative stress, inflammation,

Neonatal Outcomes in Hypoxic Ischemic Encephalopathy Exposed to Chorioamnionitis, a Retrospective Cohort Study.

BACKGROUND: Hypoxic ischemic encephalopathy (HIE) leads to neonatal mortality and long-term morbidities. Clinical and histological chorioamnionitis is an additional risk factor for neonates with HIE, which could increase morbidity and brain injury. We aimed to assess the short-term outcomes in neonates with HIE exposed to chorioamnionitis. METHODS: We conducted a retrospective cohort study using the Canadian Neonatal Network database. We included neonates with ≥35 weeks gestational age with HIE treated with therapeutic hypothermia (TH) born between January 2011 and December 2021. Chorioamnion

Melatonin treatment for neonatal encephalopathy.

RATIONALE: Neonatal encephalopathy (NE) affects 1.15 million newborns annually and is associated with a high incidence of mortality and long-term neurodevelopmental disability (NDD). Therapeutic hypothermia (TH) is the only proven therapeutic intervention available. However, it is only partially effective, and evidence suggests it is not safe in low-income countries. Melatonin is a promising neuroprotective treatment in NE as it provides anti-inflammatory properties. Pre-clinical models have demonstrated improved outcomes with melatonin. However, its effect on human newborns is unclear. OBJEC

Neonatal PBPK model of midazolam and its metabolites for perinatal asphyxia and therapeutic hypothermia.

BACKGROUND AND PURPOSE: Therapeutic hypothermia (TH) decreases morbidity and mortality in neonates with perinatal asphyxia (PA) and moderate-to-severe hypoxic-ischaemic encephalopathy. Midazolam is an antiepileptic and substrate of CYP3A4. Drug metabolism during PA/TH remains partially characterized, and disentangling effects of PA and TH is difficult, making precision dosing challenging. The I-PREDICT project aims to develop a framework for drug disposition in PA/TH neonates, using physiologically based pharmacokinetic (PBPK) modelling. The predictive performance of the model was explored for

Prenatal stress improves the ability of neonatal lambs to maintain body surface temperature during periods of cold exposure.

Exposure to cold and hypothermia is a significant threat to newborn lamb survival in extensive systems, and the ability to thermoregulate is essential for improved survivability. Lambs exhibit enhanced thermogenic abilities and increased brown fat deposition when ewes experience cold during late-pregnancy. This study evaluated the effects of various prenatal stressors on newborn lamb thermogenesis through three experiments. Experiment 1 examined the impact of mid-pregnancy shearing and wetting on lamb thermogenesis. Experiment 2 involved transporting and cold-exposing late-pregnancy ewes. Expe

Feasibility and reproducibility of resting-state functional magnetic resonance imaging in asphyxiated neonates.

BACKGROUND: In the ongoing search for prognostic tools in neonates suffering perinatal asphyxia, we assessed the feasibility of resting-state functional magnetic resonance imaging (RS-fMRI). The aim of our study was to determine both the intra-session and inter-subject reproducibility of resting-state networks (RSNs), as well as factors affecting these. NEW METHOD: We conducted a prospective multicenter study in 21 asphyxiated term newborns (mean gestational age = 39.2 weeks, mean 5-minute Apgar score = 3) after treatment with controlled hypothermia at a level III neonatal intensive care unit

Establishing and Optimizing Kangaroo Mother Care Practices Among Very-Low-Birth-Weight Neonates in a Tertiary Neonatal Intensive Care Unit (NICU): A Quality Improvement Approach.

Background Kangaroo Mother Care (KMC) is an evidence-based practice that improves outcomes in very-low-birth-weight (VLBW) neonates by lowering neonatal mortality, hypothermia, sepsis, and hospital stay duration while enhancing breastfeeding, physiological stability, and neurodevelopmental outcomes. Despite recommendations for initiating KMC within the early postnatal period (within the first 24-72 hours of life) and maintaining prolonged (six to eight hours/day) daily skin-to-skin contact, implementation and continuation of KMC remain inconsistent in neonatal intensive care units (NICUs). A regional quality-improvement collaborative to reduce early neonatal hypothermia in critically ill newborns.

BACKGROUND: Early postnatal hypothermia (EPH) remains a significant clinical concern in critically ill transported neonates. A previous regional study reported an alarming EPH incidence rate of 82.1%, with wide variation across delivery facilities, underscoring the urgent need for quality improvement (QI) in early body temperature management. This study aimed to reduce the percentage of EPH in critically ill transported newborns by 20% to 25% within 1 year. METHODS: A standardized, evidence-based thermal care bundle and comprehensive staff training were implemented using multiple Plan-Do-Stud

Early postnatal hypothermia in critically transported newborns at a Chinese tertiary care neonatal centre: a retrospective study.

BACKGROUND: Neonatal hypothermia, especially occurring in the early postnatal period, is a critical condition associated with increased morbidity and mortality. Effective thermal management is essential, yet regional data on early postnatal hypothermia (EPH) incidence among critically ill neonates requiring inter-hospital transport are limited. METHODS: This retrospective, multicentre, observational cohort study involved critically ill newborns transported from six delivery hospitals, including two women's hospitals and four general hospitals, to a tertiary care neonatal centre between Januar
