[Anatomical correction of transposition of the great arteries in 7 newborn infants].

In the presented article, the course and results of anatomical correction of transposition of great arteries (TGA) in 7 neonates (2 females and 5 males) with mean body mass of 3250 g and 2 to 5 days old (mean 3 days) are reviewed. Surgery was performed in moderate hypothermia. St. Thomas cold cardioplegia was used. Mean aortic clamping time was 70 min (55-115), and the time of extracorporeal circulation was 165 min (117-210). Low cardiac output in all patients in the postoperative period required prolonged mechanical ventilation and positive inotropic drugs. Out of 7 patients operated, two die

Current status of cardiac surgery in childhood.

In the 50 years since Gross (1938) obliterated a patent ductus arteriosus, congenital cardiac surgery has come of age, synchronized with the world explosion in microtechnology and space age materials. The late 1960s and early 1970s saw Barratt-Boyes pioneering complete intracardiac repairs on infants with congenital heart disease employing modifications of the Kyoto technique (Shirotani) for profound hypothermia and circulatory arrest. The past 10-15 years have been marked by the more widespread dissemination of increasingly safe techniques, and the application of progressive incremental refin

Room temperature ADP-induced first-stage hyperaggregation of human platelets: the cause of rewarming deaths by thrombocytopenia in neonatal cold injury.

No satisfactory explanation has been proposed to explain the rewarming deaths common in neonatal cold injury (NCI). Since thrombocytopenia exists in NCI, aggregation studies were performed on human platelets at the relevant temperatures to search for a mechanism that would link thrombocytopenia with this complication. At the temperatures found in infants suffering from NCI, an increased sensitivity to ADP was noted: room-temperature ADP-induced first-stage hyperaggregation. Evidence exists to suggest that adequate release of ADP from erythrocytes occurs in vivo to cause such aggregation. Hypot

[Immediate and mid-term results of complete repair of truncus arteriosus during the first year of life].

Twenty five infants with truncus arteriosus underwent complete surgical correction in the first year of life between January 1984 and June 1990 at Marie Lannelongue Hospital. All had cardiac failure and pulmonary hypertension. Another severe cardiac malformation was present in 6 cases. Complete repair was carried out under cardiopulmonary bypass with moderate hypothermia. After closing the ventricular septal defect the continuity of the right ventricle and pulmonary artery was reestablished by a valved Dacron conduit with a bioprosthesis (13 patients), by an autologous pericardial conduit with

Cerebral perfusion and metabolism during profound hypothermia in children. A study of middle cerebral artery ultrasonic variables and cerebral extraction of oxygen.

Flow velocity of the right middle cerebral artery was studied in eight children during cardiac operations performed with profound hypothermia. Cerebral oxygen consumption was estimated by relating the difference in oxygen content between arterial and venous blood (jugular bulb) to flow velocity. In another six children, also during profound hypothermic procedures, the diameter of the middle cerebral artery was studied with an electronic echo-tracking instrument connected to a real-time ultrasound scanner. Flow velocity and estimated oxygen consumption decreased during cooling in proportion to

Moderate hypothermia in the management of resistant automatic tachycardias in children.

BACKGROUND: Automatic focus tachycardias are often resistant to electrical and pharmacological treatment. Moderate systemic hypothermia (32-34 degrees C) may reduce the tachycardia rate in children with His bundle tachycardia after cardiac surgery. METHODS: The case notes of seven children with automatic focus tachycardias treated with hypothermia were reviewed. Six had His bundle tachycardia after cardiac surgery and one had ectopic atrial tachycardia; all had signs of low cardiac output. RESULTS: Hypothermia led to a reduction in heart rate in all patients (from 211 (28) (mean (SD] to 146

Temperature-response studies of the detrimental effects of multidose versus single-dose cardioplegic solution in the rabbit heart.

Both single-dose and multidose cardioplegia are protective in the ischemic adult heart under normothermic and hypothermic conditions, but in the hypothermic neonatal rabbit heart single-dose cardioplegia only is protective, whereas multidose cardioplegia is damaging. The present studies in the isolated perfused working heart from neonatal rabbits (aged 7 to 10 days) were designed to characterize the interrelationships between temperature, frequency of cardioplegic infusion, and tissue protection. Hearts (n = 8/group) were subjected to 1, 1.5, 1.5, 3, 10, 12, or 18 hours of ischemia at 37.0 deg

Bacteremia during diarrhea: incidence, etiology, risk factors, and outcome.

To determine the importance of bacteremia in hospitalized patients with diarrhea in Bangladesh, from September 1982 through August 1983 the authors obtained blood for culture from 1,824 patients who were suspected of having sepsis (44% of all admissions). Nontyphoid bacteremia occurred in 243 patients. The most common pathogens were the Enterobacteriaceae (n = 66 episodes), Staphylococcus aureus (n = 65), Pseudomonas aeruginosa and other non-glucose-fermenting bacilli (n = 50), Streptococcus pneumoniae (n = 40), and Haemophilus influenzae (n = 16). When compared with an equal number of control

Intrauterine growth and spastic cerebral palsy. I. Association with birth weight for gestational age.

Birth weight, gestational age at delivery, and other factors were collected for 171 white children with spastic cerebral palsy. Their birth weights were compared with the birth weight distribution expected for a population of the same race, gestation, sex, maternal height, and parity, born in the same geographic area, and during the same time period. Birth weights of children with spastic cerebral palsy tended to be significantly lower than the median birth weight of their comparison population. Analysis stratified by gestation at delivery suggested that if the reduced birth weight were causal

Resuscitation at birth.

Birth asphyxia is frequent and often severe, occurring in about 10% and 1% respectively of all births; in a third it is unexpected. Delivery rooms must be organised and equipped and trained staff readily available so as to provide appropriate and timely resuscitation of the newborn. Simple procedures designed to prevent hypothermia, maintain a patent airway, improve oxygenation and ventilation are sufficient for the majority of babies. Circulatory support and biochemical resuscitation will be needed in a few. In the absence of other abnormalities, the long term prognosis for newborns who respo

Fentanyl dosage is associated with reduced blood glucose in pediatric patients after hypothermic cardiopulmonary bypass.

The authors retrospectively reviewed the charts of 36 pediatric patients who had undergone cardiac surgery with hypothermic cardiopulmonary bypass (CPB) (n = 24) or profound hypothermia with circulatory arrest (PHCA) (n = 12), none of whom had received dextrose in the clear CPB pump prime, maintenance iv fluids, or cardioplegia solution. The authors studied whether the doses of fentanyl or methylprednisolone, or rates of dextrose infusion from blood products during CPB or from vasoactive infusions in 5% dextrose in water, were correlated with the blood glucose concentrations at the termination

Prenatal risk factors for cot death in very preterm and small for gestational age infants.

In a nationwide prospective survey on very preterm and very-low-birthweight infants in The Netherlands, the incidence of cot death in infants discharged alive was 15%. The postnatal age at death in these infants did not differ significantly from age at death in other cot death infants. Using a case-control design, possible risk factors associated with cot death were identified: lower maternal age at first pregnancy; maternal smoking during pregnancy; hypothermia of the infant immediately after birth; decreased number of white blood cells and thrombocytes in the infant on the first day of life.

Physiologic and neuropathologic aspects of hypothermic circulatory arrest in newborn dogs.

A model of hypothermic circulatory arrest has been developed in the newborn dog. Ten puppies were anesthetized with halothane, paralyzed, and artificially ventilated with 70% nitrous oxide 30% oxygen to arterial oxygen pressure greater than 8.0 kPa (60 mm Hg), arterial carbon dioxide pressure of 4.4-5.6 kPa (33-42 mm Hg), and arterial pH of 7.35-7.42. Animals were surface cooled to 20 degrees C, after which cardiac arrest was produced with i.v. KCl. Dogs remained asystolic without ventilation for 1.0 (n = 4), 1.5 (n = 3), or 2.0 (n = 3) h. Resuscitation was accomplished with closed-chest compr

Hypothermia. Safe and efficient methods of rewarming the patient.

Hypothermia, a relatively common problem in the winter months, can cause significant morbidity. It presents in a variety of situations and affects a wide age range. Diagnosis requires a high index of suspicion, because the symptoms, which are primarily related to the central nervous system, are not distinctive. Appropriate management requires accurate measurement of core body temperature. Treatment is centered on rewarming the patient safely and efficiently while providing other supportive measures. Care should be taken to avoid arrhythmias. Simple precautions greatly reduce the risk of hypoth

[Neonatal septicemia: differences in full-term and pre-term newborn infants].

Neonatal septicemia was assessed by blood cultures in 115 newborns (NB) during a two years study in a pediatric hospital of reference in Mexico City. The studied patients were divided in two groups of gestational age, and the differences of etiologic agents, clinical signs, laboratory findings and clinical outcome were compared at term and preterm neonates. We observed Staphylococcus epidermidis became the first cause of septicemia in at term NB (P less than 0.001), while Escherichia coli and Klebsiella pneumoniae (P less than 0.01) were more frequent in the preterm neonates. The clinical mani

Anesthetic considerations in premature birth.

Premature birth still accounts for about 75% of perinatal mortality. Although great strides have been made in the care of premature babies over the past two decades, markedly decreasing mortality, the prevention of premature birth has not been greatly improved. Although tocolysis, particularly with the beta-2 agonists and magnesium sulfate, may delay birth and allow fetal maturation, it poses several risks which, if not recognized, can cause serious morbidity and even mortality. The use of these drugs and other less widely used tocolytics has important implications for the anesthesiologist. Th

Intestinal post-ischemic reperfusion injury: studies with neonatal necrotizing enterocolitis.

In the feline intestine studies have implicated superoxide (O.-) and other oxygen derived free radicals as initiators of injury as measured by increased capillary permeability during the reperfusion period. Biochemical mechanisms of this free radical generation include: xanthine oxidase dependent O.- production, hydrogen peroxide (H2O2) formation by superoxide dismutase (SOD), hydroxyl radical (OH-) production via the Haber-Weiss reaction, and lipid radical formation from membrane peroxidation. Pathological consequences of these events include inflammatory neutrophil infiltration, damage to th

Surgical management of patent ductus arteriosus. A review of 413 cases.

Ligation under controlled hypotension is a simple, quick and effective surgical management of patent ductus arteriosus. In this study 413 cases are reviewed: clinical presentations, cardiac catheterization and operative findings are studied in relation to the surgical procedures. Ligation of the ductus was the treatment of choice in 357 cases irrespective of the age and size of the ductus with only four instances of recanalization which needed religation. Other procedures included division and suture, patch aortoplasty, transaortic patch and transpulmonary closure under profound hypothermia an

Hypoglycemia in infants and children.

Although the conditions that cause hypoglycemia in adults may also be present in infants and children, there are many entities unique to the pediatric age group. This reflects the delicate balance that exists in the newborn and young child between glucose production and utilization. During fasting in infants and children, hepatic glucose production is normally two to three times that of adults when expressed on the basis of weight. In the newborn and young infants, hypoglycemia usually presents with irritability, feeding difficulties, lethargy, cyanosis, tachypnea, and/or hypothermia rather th
