Of a consecutive series of 91 babies aged less than 2 years, operated on for closure of a ventricular septal defect during the last four years, the 61 cases with a minimum post operative follow up period of 18 months were retained for review. Closure of the ventricular septal defect was carried out directly 41 times, and after pulmonary artery banding in the other 20 cases, using deep hypothermia with a short period of circulatory arrest and cardiopulmonary bypass.

A 58-day-old girl with hypoplastic left heart syndrome underwent bypass grafting from the pulmonary artery to the descending aorta, ligation of the ductus arteriosus, and banding of the main pulmonary artery distal to the graft. Blood glucose, blood lactate, plasma free fatty acids, plasma alpha-amino nitrogen, plasma free amino acids and blood urea nitrogen were measured in 15 hypothermic preterm infants (mean rectal temperature: 34.5 +/- 0.28 degrees C) during a 2-hour 'warming-up' period following admission to our neonatal intensive care unit. Hypothermia was associated with lactacidemia and hyperalaninemia. The increase in body temperature of the infants resulted in the decrease of lactacidemia and hyperalaninemia. Normal levels of other metabolites remained unchanged.

The classic modes of heat loss affecting the newborn are conduction, convection, radiation and evaporation. The disproportionate ratio of surface area to body weight (SA/BW) of the premature infant is the basis of inefficient heat conservation, combined with a blunted metabolic response. Hypothermia leads to hypoxemia, acidosis, hypoglycemia, kernicterus and other lethal complications. Mistakes are common. Attention to minute details is required for stabilization of body temperature at the "neutral temperature zone" in delivery room, nursery and in transport.

The clinical course and biochemical findings in a case of carbamyl-phosphate-synthetase deficiency are described. The patient, a boy, presented 48 h after birth with rapidly developing hypotonia and hypothermia. Pulmonary haemorrhage, melaena and haematemesis ensued and despite ventilatory assistance and peritoneal dialysis the patient died on the fifth day. A virtual absence of carbamyl phosphate synthetase I (N-acetylglutamate dependent) was proved by analysis of tissue samples removed post mortem. Other urea cycle enzymes were normal.

Twenty-two patients with congenital valvular aortic stenosis were surgically treated between 1967 and July 1975. Five (23%) were under 1 year of age (group I) and 17 (77%) were between 2 and 24 years (group II).
