Clinical presentation, etiology, and treatment outcomes of mycetoma: A 25-year retrospective study in Southern Thailand.

BACKGROUND: Mycetoma is a chronic subcutaneous infection caused by fungi (eumycetoma) or filamentous bacteria (actinomycetoma). Although recently recognized by the World Health Organization as a neglected tropical disease, data from Southeast Asia are scarce. Previous reports from Thailand were limited and outdated. METHODOLOGY/PRINCIPAL FINDINGS: We conducted a 25-year retrospective study (2000-2025) at a tertiary referral hospital in southern Thailand. Patients were identified from hospital records and confirmed by histopathology and/or culture. Fifty patients met inclusion criteria: 31 (62

Cutaneous and subcutaneous nodular scedosporiosis in a cat.

This article reports the clinical, diagnostic, and therapeutic aspects of a rare case of cutaneous and subcutaneous scedosporiosis in a 16-year-old domestic cat. The animal presented with a painful, ulcerated nodule at the base of the tail. Histopathology and fungal culture confirmed a pyogranulomatous dermatitis with hyaline hyphae, and molecular analysis identified Scedosporium sphaerospermum, a soil-dwelling fungus newly described. Despite targeted antifungal treatment with itraconazole and topical ciclopirox olamine, the cat developed systemic complications and died. This case emphasizes t

Environmental and structural risk factors for mycetoma in the White Nile State, Sudan: an ethnographic study.

BACKGROUND: For decades, poor Sudanese communities in rural areas have silently battled mycetoma, a stigmatizing, disabling, neglected tropical disease. Its linkage to specific, ecology-dependent livelihoods such as farming, woodcutting and collecting and animal herding has obscured its cause and environmental and structural dimensions at both the local and global levels. This ethnographic study challenges the prevailing singular focus on the injuries that have dominated biomedical reasoning for years. METHODS: We deployed an ethnographic approach utilizing participant observation, in-depth in

Molecular docking approaches in mycetoma: Toward improved patient management.

Mycetoma is a neglected tropical disease characterised by chronic, granulomatous inflammation of the subcutaneous tissues, often leading to disfigurement, disability, and significant socioeconomic burdens. Caused by a diverse array of bacterial and fungal pathogens, eumycetoma is predominantly driven by Madurella mycetomatis, and current treatment strategies are limited and often ineffective. Conventional antifungal therapies, such as itraconazole, require prolonged administration, frequently combined with surgical interventions, yet cure rates remain suboptimal, and recurrence is common. The

A critical anthropological evaluation of digital media based interventions for mycetoma in Sudan; the case of "Hope for a Devastating Neglected Disease" documentary film.

Mycetoma, a disfiguring and debilitating neglected tropical disease (NTD), long languished in obscurity. Prior to 2013, limited research, ineffective treatments, and inadequate global recognition plagued the disease. However, a pivotal 2013 meeting in Geneva marked a turning point. Recognizing advocacy as a critical missing link, the Drugs for Neglected Diseases Initiative (DNDi) spearheaded efforts to raise awareness and address knowledge gaps. Scientific symposia, media coverage, and research collaborations, including the DNDi trial, paved the way for mycetoma inclusion in the 2016 WHO NTD l

Case Report: diagnosis of mixed Scedosporium apiospermum infection assisted by bronchoalveolar lavage fluid morphology.

BACKGROUND: Scedosporium apiospermum is a highly aggressive opportunistic pathogen, widely distributed in natural environments. Infections predominantly occur in immunocompromised individuals but may also affect immunocompetent individuals with predisposing factors such as trauma, drowning, exposure to contaminated water, diabetes, or malnutrition. CASE PRESENTATION: In the reported case, morphological abnormalities were initially identified in bronchoalveolar lavage fluid (BALF). Subsequent microbial culture identification and metagenomic next-generation sequencing (mNGS) confirmed a mixed in

Advances in the treatment of mycetoma: the emerging role of nanomedicine.

Mycetoma is a chronic, debilitating, neglected tropical disease characterized by a progressive subcutaneous infection caused by either fungi (eumycetoma) or bacteria (actinomycetoma), typically affecting the limbs, but no body part is immune. Current treatments remain limited, relying on long-term antifungal or antibiotic regimens that offer only modest efficacy, carry significant toxicity; and often surgical intervention is required. Drug-repurposing efforts, including computational screening and phenotypic assays, have identified several FDA-approved compounds with potential activity against

Implantation mycoses: A nationwide survey on diagnostic and treatment modalities in Nepal.

Implantation mycoses (IM) are a group of fungal diseases which occur after a transcutaneous trauma. The World Health Organization has listed some of the IM, namely sporotrichosis, chromoblastomycosis and eumycetoma as one of the Skin Neglected Tropical Diseases targeted for control by 2030. There are no robust data on IM from Nepal since these diseases are not a part of routine disease surveillance. In this study, an online and in-person survey was conducted using a standard set of questionnaires among the registered dermatologists of Nepal. For this survey sporotrichosis, chromoblastomycosis

Mycetoma as a major cause of limb amputation in Northeastern Ethiopia: A facility-based retrospective study.

Background Mycetoma is a chronic, progressively destructive infection that can result in severe disability and limb loss. In Ethiopia, diagnostic capacity and access to effective treatment remain limited, and the burden of mycetoma is poorly characterized. Recent clinical observations from the Afar Region suggest a high frequency of advanced disease and amputation, yet systematic evidence on the burden is lacking. This study aimed to describe the clinical burden of mycetoma, diagnostic and treatment practices, care-seeking patterns, and the extent of limb amputation at Dubti General Hospital i

Mycetoma.

The Neglected Tropical Disease mycetoma is a chronic, progressive infectious disease characterized by large tumor-like masses in subcutaneous tissues. It is endemic in rural areas of low- and middle-income countries, where it can lead to extensive health and socioeconomic burdens for the patient and their family. It is especially prevalent among male farmers. The disease can be caused by both bacteria and fungi, referred to as actinomycetoma and eumycetoma, respectively. Inoculation occurs when a skin breach allows the pathogen to enter the subcutaneous tissue. The pathogen can form grains, wh

Vertebral osteomyelitis caused by Chaetomium Globosum in a young adult: A case report and literature review.

RATIONALE: Chaetomium globosum (C. globosum) is a saprophytic fungus widely distributed in nature, characterized by low clinical pathogenicity but capable of causing severe infections, particularly in immunocompromised hosts or under conducive environmental conditions. Spinal infections attributable to C. globosum are exceedingly rare and often present diagnostic dilemmas due to atypical presentations. This case underscores the importance of considering fungal etiologies in spinal pathologies. Furthermore, the strain mentioned in this study has been appropriately preserved and is available for

Operationalizing One Health Surveillance for Tropical Fungal Diseases: Practical Implications of Bioinformatics for Clinical and Epidemiological Practice.

Fungal infections are an increasing global public health concern yet remain underprioritized because of persistent challenges in diagnosis, treatment, and surveillance. The WHO Fungal Priority Pathogens List underscores the growing burden of antifungal resistance and includes agents responsible for neglected tropical mycoses, such as Paracoccidioides spp. and eumycetoma. Despite recent advances, fungal diagnostics remain limited by poor standardization, high costs, and restricted applicability in endemic, resource-limited settings. In addition, mounting evidence shows that fungal pathogens evo

Madura foot in a non-endemic setting: surgical management and reconstruction with skin graft.

Mycetoma is an uncommon, chronic, granulomatous infectious condition that affects the skin, dermis and subcutaneous tissues. It is prevalent in tropical and subtropical areas. The patient presented to our hospital with a left foot lesion that he first noticed 2 years ago. The lesion consisted of multiple pits discharging black granules and pus. Culture revealed that the grains comprise a mass of fungal hyphae organised within a deeply eosinophilic amorphous matrix (Splendore-Hoeppli material). The patient received multidrug therapy, radical excision of mycetoma and a skin graft. He responded w

Actinomycetoma of the wrist: diagnostic insights from a pathology perspective (case report).

Actinomycetoma is a chronic, progressive subcutaneous infection caused by filamentous bacteria of the Actinomycetales order, characterized by tumefaction, sinuses, and grains. We report a rare case of actinomycetoma affecting the wrist of a 45-year-old male, which presented as a chronic, slowly enlarging swelling with multiple discharging sinuses. Initial clinical suspicion was broad, encompassing various chronic infections or soft tissue tumors. However, definitive diagnosis was established through meticulous histopathological examination of a biopsy specimen, revealing characteristic "sulphu

A qPCR identification scheme to detect the most common causative agents of actinomycetoma in Africa.

Mycetoma is a neglected tropical disease characterized by mutilating tumorous lesions in the subcutaneous tissue. The causative agents are found embedded in granules called grains. Mycetoma is either caused by bacteria (actinomycetoma) or fungi (eumycetoma). To initiate the appropriate treatment, it is important to identify the causative agent rapidly and molecular identification for eumycetoma revolutionized the time to identification. For actinomycetoma this was not possible yet. Here we developed a multiplex qPCR identification scheme for the most common causative agents of actinomycetoma i
