The development of a multiplex recombinase polymerase amplification reaction to detect the most common causative agents of eumycetoma.

PURPOSE: Mycetoma is a neglected tropical disease that affects the subcutaneous tissue. The disease can be caused by over 90 different pathogens, including both bacteria (actinomycetoma) and fungi (eumycetoma). While diagnostic tools for eumycetoma causative agents are available, these are generally not well suited for use in endemic regions. This study aims to develop an isothermal based multiplex recombinase polymerase amplification reaction (RPA), that can be integrated in the diagnostic workflow of endemic regions. METHODS: The RPA was designed targeting the Internal Transcribed Spacer (I

Pharmacokinetics and Pharmacodynamics of Fosravuconazole, Itraconazole, and Hydroxyitraconazole in Sudanese Patients With Eumycetoma.

BACKGROUND: The first clinical trial on eumycetoma was recently conducted in Sudan, comparing oral fosravuconazole, prodrug of active ravuconazole, with the standard-of-care oral itraconazole. Building on this trial, the present study aimed to characterize the pharmacokinetics-pharmacodynamics (PK-PD) of ravuconazole, itraconazole, and hydroxyitraconazole in patients with eumycetoma and guide selection of either a 200-mg or 300-mg dose of fosravuconazole. METHODS: Nonlinear mixed-effects modeling was used to develop population PK models in 52 patients receiving 3 daily loading doses followed

A Draft Genome Assembly for the Neglected Tropical Fungal Disease Pathogen Emarellia grisea and Comparison to Other Pleosporalean Agents Causing Eumycetoma in Humans.

Mycetoma is a neglected tropical disease, characterized by slowly evolving tumorous subcutaneous masses containing either bacteria (e.g., Actinomycetoma) or fungi (e.g., Eumycetoma; Madura foot). The fungal agents of mycetoma, including several genera belonging to the Pleosporales order of Ascomycetes, were designated high-priority pathogens in the WHO Fungal Priority Pathogens List of 2023 to signal the need for additional research and development and public health action. Despite this, many of the fungal causative agents are poorly described, especially in terms of the genomic basis for dise

Amphotericin B intercalated in layered clays as effective antifungal systems against eumycetoma causative agents.

Eumycetoma and other subcutaneous mycosis are severe chronic infections with a high incidence in tropical and subtropical regions, whose current treatment has a low curation rate. In the search for more effective and affordable therapies, hybrid materials based on amphotericin B (AmB), an antifungal drug with a broad spectrum of action, appear promising for localized treatment at the site of infection. In this context, the immobilization of AmB in suitable nanocarriers would be helpful to minimize its adverse side effects and ensure a controlled release over time. For this purpose, montmorillo

Detecting and Classifying Mycetoma in Histopathological Images Using DenseNet and U-Net.

Mycetoma, recognised by the WHO as a Neglected Tropical Disease, has significant diagnostic hurdles which lead to severe health consequences. Mycetoma can be caused by certain types of bacteria (actinomycetoma) or fungi (eumycetoma). Identifying whether mycetoma is bacterial or fungal plays a significant role in treatment planning, and an incorrect diagnosis can seriously affect patient prognosis and outcome. This paper explored the feasibility of using state-of-the-art artificial intelligence (AI) computer vision methods to effectively detect and diagnose mycetoma grains from histopathologica

Global sociodemographic, clinical, and epidemiological profiling of patients with mycetoma: A systematic review.

BACKGROUND: Mycetoma is a neglected tropical disease that affects subcutaneous tissues. This chronic granulomatous inflammatory disease often leads to high morbidity rates, including amputation, disability and social stigma. Despite its substantial impact, the epidemiology of mycetoma remains largely unknown. This systematic review aimed to establish a global sociodemographic and clinical profile of affected patients and characterise the geographic patterns of the causative organisms of mycetoma. METHODS: The study followed the 'Preferred Reporting Items for Systematic Review and Meta-Analysi

Eumycetoma: pathogenesis, current treatments, and the search for new drugs.

Eumycetoma is a neglected tropical disease which can be caused by 69 different fungal species. In 2024, eumycetoma was listed as a high priority fungal infection on the fungal priority list of the World Health Organization. Eumycetoma is characterized by chronic subcutaneous swelling, the formation of grains, and progressive tissue destruction. If left untreated, it can lead to severe complications such as bone destruction and permanent disability. Current antifungal treatments, such as itraconazole and terbinafine, are lengthy and often ineffective without surgical intervention. The search fo

Concurrent chromoblastomycosis and eumycetoma: a unique case of dual neglected tropical fungal diseases in Asia.

Chromoblastomycosis (CBM) and mycetoma, as implantation mycoses, have been listed as neglected tropical diseases (NTDs) by the World Health Organization. The concurrent occurrence of these two NTDs in a single patient is extremely rare. A 69-year-old female patient presented with papules on the dorsum of her left hand for over 5 months and nodules on the left lower limb accompanied by ulceration and pain for 20 days. Histopathological examination of the papule on the dorsum of the left hand revealed muriform cells and fungal culture of the tissue identified Fonsecaea monophora. Microscopic exa

Kinome analysis of Madurella mycetomatis identified kinases in the cell wall integrity pathway as novel potential therapeutic drug targets in eumycetoma caused by Madurella mycetomatis.

Eumycetoma is a neglected tropical subcutaneous disease most commonly caused by the fungus Madurella mycetomatis. Currently, eumycetoma is treated by a combination of antifungal therapy and surgery, with limited success rates. To identify novel drug targets we used an in silico approach to determine the kinases present in M. mycetomatis genome and rank them as potential drug targets. In total 132 predicted kinases were identified in M. mycetomatis, of which 21 were predicted to be essential for fungal viability and 4 of these had no human orthologues. Two were linked to the Cell Wall Integrity

A drug-repurposing screen of FDA- and EMA-approved drugs identifies two NF-κB inhibitors active against eumycetoma.

BACKGROUND: Mycetoma caused by Madurella mycetomatis (eumycetoma) is a neglected tropical disease that forms tumorous lesions in the subcutaneous tissue. The current standard treatment for eumycetoma consists of antifungal treatment combined with surgery, with limited success rates. Due to lack of investment, there are currently no large drug discovery programmes for mycetoma. Repurposing screening can therefore offer an effective and economical way to identify drugs that can be effective in eumycetoma treatment. Therefore, in this study we determined the in vitro activity and in vivo efficacy

The development of an Actinomadura madurae grain model in Galleria mellonella larvae.

Mycetoma is a neglected tropical disease characterized by large tumorous lesions. It can be caused by fungi (eumycetoma) or bacteria (actinomycetoma). The hallmark of mycetoma is the formation of grains by the causative agent. Grains can only be formed in vivo; therefore, in vivo models are crucial to studying mycetoma. In vivo, grain models have been developed in the invertebrate Galleria mellonella for eumycetoma, but not for actinomycetoma. Here, we aimed to develop the first actinomycetoma grain model in G. mellonella larvae. Actinomadura madurae strains DSM43236 and DSM44005 were used to

Simultaneous Detection of Four Madurella Species Using Loop-Mediated Isothermal Amplification (LAMP) for Eumycetoma Diagnosis.

Eumycetoma is a neglected tropical disease caused primarily by a Madurella mycetomatis infection, besides other related species. In this study, we designed a novel loop-mediated isothermal amplification (LAMP) primer set capable of simultaneously detecting four Madurella species (M. mycetomatis, M. pseudomycetomatis, M. tropicana, and M. fahalii). Genomic sequencing of M. pseudomycetomatis strain and comparative genome analysis revealed the candidate genes that were common among and specific to Madurella species. The 3 LAMP primer sets targeting these genes detected up to 1 pg of the genomic D

Sandalwood Sesquiterpene (Z)-α-Santalol Exhibits In Vivo Efficacy Against Madurella mycetomatis in Galleria mellonella Larvae.

In continuation of our recent report on the in vitro activity of sandalwood essential oils and their major components, the sesquiterpenes (Z)-α- and -β-santalols, against the pathogenic fungus Madurella mycetomatis, causative agent of the neglected tropical disease eumycetoma, we have now tested these isolated constituents, as well as native Royal Hawaiian sandalwood essential oil obtained from Santalum paniculatum, for in vivo activity in a model system using infected Galleria mellonella larvae. Besides confirming the superior activity of (Z)-α-santalol over the (Z)-β-isomer and the crude ess

Mycetoma of the upper back: an unusual presentation of a neglected tropical disease.

Mycetoma is a chronic, granulomatous infection typically affecting the lower limbs. We report a rare case of eumycetoma involving the upper back in a young man with a 10-year history of painless swelling that became symptomatic only recently. After initially diagnosed as an abscess, intraoperative discovery of black granules prompted further investigation. Histopathology confirmed Madurella mycetomatis infection. Imaging revealed a large heterogeneous mass without bone involvement. The patient underwent complete surgical excision followed by oral itraconazole therapy. At 1 month follow-up, pos

Successful Management of Progressive Mycetoma Affecting the Head and Neck.

Mycetoma is a chronic, destructive subcutaneous infection of fungal (eumycetoma) or bacterial (actinomycetoma) origin; predominantly affecting the extremities. Head and neck involvement is exceedingly rare, posing significant diagnostic and therapeutic challenges. Intractable cases are more difficult to manage, requiring alternative treatment strategies. We report the case of a 14-year-old girl with a 3-year history of ulcerating sinuses and nodules progressively spreading from the scalp and neck; to the upper back. Prior treatments with various antibiotics and antifungals provided no improvem

Mycotic popliteal aneurysm in an HIV-positive patient: a case report.

Mycotic popliteal aneurysm is rare. It is observed less rarely in patients immunocompromised by HIV. Its pathogenesis is linked to the weakening of the artery's angiotropism by HIV and to easy systemic mycobacterial colonisation. The diagnosis is based on clinical observations and computed tomography (CT) angiography of the limbs, which shows a mycotic pseudoaneurysm in the form of a cup-shaped eumycetoma and perianeurysmal abscesses. Surgery remains the treatment of choice, associated with intensive etiological treatment. We report the case of a 49-year-old HIV-positive patient admitted to Bo

Update on actinomycetoma treatment: linezolid in the treatment of actinomycetomas due to Nocardia spp and Actinomadura madurae resistant to conventional treatments.

INTRODUCTION: Mycetoma is a chronic granulomatous infection, common throughout tropical regions, and is considered a neglected disease that mostly affects impoverished populations. Mycetoma is divided into eumycetoma, caused by fungi, and actinomycetoma, caused by filamentous bacteria. Clinical presentation is distinctive, and making the diagnosis is usually not difficult; however, access to safe and effective treatments is a major challenge. There is not a single best treatment, with the choice of treatment depending on etiology, severity and extent of disease, and patient comorbidities. ARE

The WHO fungal priority pathogens list: a crucial reappraisal to review the prioritisation.

In October, 2022, WHO published the first fungal priority pathogen list, which categorised 19 fungal entities into three priority groups (critical, high, and medium), for prioritisation of research efforts. The final ranking was determined via multiple criteria decision analysis, considering both research and development needs and perceived public health importance. In this Personal View, we discuss the positioning of the fungal pathogens, namely, Mucorales, Candida spp, Histoplasma spp, Coccidioides and Paracoccidioides spp, Fusarium spp, eumycetoma causative agents, Talaromyces marneffei, an

Reconstructive surgery for mycetoma: Preliminary algorithm and a systematic review.

BACKGROUND: Surgical treatments of mycetoma are a cornerstone in management. However, while doing a wide surgical excision of mycetoma lesion, surgeons think about how to close the skin defect, which can be closed primarily, left to heal by secondary intension, by skin grafts or local flaps. In this review, we demonstrate the various applications and changes of mycetoma reconstruction after surgical excision. METHODS: This is a systematic literature search and review conducted to determine articles presenting mycetoma reconstruction options. Articles were identified, and the time of publicati
